Abstract:We report a case of a 63-year-old man with cerebral sparganosis due to infection with a sparganum larva and discuss the characteristic magnetic resonance imaging and magnetic resonance spectroscopy features of cerebral sparganosis. A 63-year-old man, who lived in a rural community, presented to our center for numbness and paresthesia on his right limbs, and a mass lesion on his left parietal lobe, initially suspected to be vascular malformations by local clinicans. He consulted to our service for a definite diagnosis. He underwent a cranial magnetic resonance imaging on admission, which revealed a mass lesion with prominent perifocal oedema in the left parietal lobe. The lesion was presented as slight hypointensity on T1-weighted images but slight hyperintensity on T2-weighted images, and demonstrated abnormal enhancement with the pattern of a peripheral ring shape on enhanced magnetic resonance scans. Further tests, magnetic resonance spectroscopy and enzyme-linked immunosorbent assay for parasites were performed. A single-voxel magnetic resonance spectroscopy showed a dramatically increased choline level and no significantly decreased N-acetylaspartate level in this lesion. The result of enzyme-linked immunosorbent assay was strongly positive for anti-sparganum antibody in blood. With these findings the patient was finally diagnosed with cerebral sparganosis. Following treatment, the patient's neurological deficits markedly improved. A peripheral ring shape enhancement was characteristic of cerebral sparganosis on enhanced magnetic resonance scans. The result of magnetic resonance spectroscopy was thought to be a new finding according to the literature, however, more cases should be studied to confirm the result.
Analysis of magnetic resonance imaging findings of fungal spondylitis

Abstract:The clinical and magnetic resonance imaging(MRI) data of 8 cases of fungal spondylitis were retrospectively enrolled in the study, and the typical MRI findings were mainly discussed. The results released that fungal spondylitis should be suspected when the infection spreads along paraspinal ligament, and vertebral lesions show jumping discontinuous distribution. A blurred and incomplete outline of endplate on T1 weighted imaging sequence, loss of hyperintensity of the vertebral body, and banded like hypointensity under endplate on T weighted imaging sequence, indicate fungal spondylitis. These features contribute to the diagnosis and differential diagnosis of fungal spondylitis.
A case report on transient global ventricular wall thickening secondary to acute myocarditis Focus on the unique role of cardiac MRI

Abstract:Introduction: Transient left ventricular wall thickening is known to develop in the acute phase of myocarditis, with several reports documenting this unusual mode of myocarditis. Diagnosing myocarditis can be challenging because symptoms, clinical exam findings, electrocardiogram results, biomarkers, and echocardiogram results are often non-specific. Therefore, cardiac magnetic resonance imaging has become the primary non-invasive imaging tool in patients with suspected myocarditis. Patient concerns and diagnosis: A 51-year-old male was referred to our hospital with a 20-day history of fever. Initial echocardiogram demonstrated diffuse concentric left ventricular hypertrophy with depressed left ventricular diastolic function, previously misdiagnosed as restrictive cardiomyopathy. Cardiac magnetic resonance imaging (MRI) showed global ventricular wall thickening, and the negative delayed enhancement made hypertrophic cardiomyopathy and myocardial amyloidosis less likely. This information, along with laboratory analyses, led to a diagnosis of acute myocarditis. Interventions and outcomes: The patient underwent a treatment regimen, including a prescription of levofloxacin and other supporting treatments. During the period following, the patient experienced a few minor episodes of atypical chest pain with spontaneous remission. The patient was discharged after 8 days of hospitalization. A cardiac MRI evaluation was repeated after 17 months, this time showing that the wall thickness had returned to normal; the myocarditis resolved without sequela. Conclusions: In summary, we report on a case of transient global ventricular wall thickening secondary to acute myocarditis, which rarely has been described previously. Our study demonstrates that transient ventricular wall thickening related to myocardial interstitial edema also can involve the right ventricular wall, a fact that is important in diagnosis and differential diagnosis. Cardiovascular magnetic resonance currently is considered the most comprehensive and accurate diagnostic tool in patients with suspected myocarditis.

Abstract:The ILAE Neuroimaging Task Force aimed to publish educational case reports highlighting basic aspects related to neuroimaging in epilepsy consistent with the educational mission of the ILAE. Neurocysticercosis (NCC) is highly endemic in resource-limited countries and increasingly more often seen in non-endemic regions due to migration. Cysts with larva of the tapeworm Taenia solium lodge in the brain and cause several neurological conditions, of which seizures are the most common. There is great heterogeneity in the clinical presentation of neurocysticercosis because cysts vary in number, larval stage, and location among patients. We here present two illustrative cases with different clinical features to highlight the varying severity of symptoms secondary to this parasitic infestation. We also present several examples of imaging characteristics of the disease at various stages, which emphasize the central role of neuroimaging in the diagnosis of neurocysticercosis.

Abstract:Cryptococcal meningitis (CM) is the most common type of fungal meningitis. The clinical symptoms of CM are nonspecific, and neuroimaging characteristics are variable. Herein, we present a case of a senile female with CM that was once misdiagnosed as cerebral infarction. Her condition worsened and she developed hydrocephalus. No apparent predisposing factors of CM were reported in this patient. The diagnosis of CM was definitely made after India ink staining of cerebrospinal fluid was positive. This case indicates that clinicians should bear cryptococcal infection in mind when the symptoms are nonspecific and neuroimaging findings are atypical.
An unusual presentation of infective endocarditis: Transient encephalopathy with reversible lesions in corpus callosum and white matter

Abstract:Clinical mild encephalitis/encephalopathy with a reversible splenial lesion (MERS) is a clinic-radiological syndrome, which has been reported to be associated with many conditions and the most common pathogens are virus. However, bacteria are rare pathogens for MERS. We report a 20-year-old man diagnosed with definite infective endocarditis, who presented with transient encephalopathy with reversible lesions in the entire corpus callosum and bilateral white matter on magnetic resonance imaging (MRI). The blood culture indicated a Staphylococcus aureus infection. His neurological manifestation improved and imaging abnormalities faded after receiving a combination of intravenous immunoglobulin, methylprednisolone, and antibiotics. Clinicians should be aware of transient encephalopathy with reversible callosal lesions as a potential unusual presentation of infective endocarditis.

Abstract:Background: Mild encephalitis/encephalopathy with a reversible splenial lesion (MERS) is a rare clinicoradiological syndrome characterized by transient mild encephalopathy and magnetic resonance imaging (MRI) findings of a reversible lesion in the splenium of the corpus callosum (SCC). Multiple causes have been proposed for the pathogenesis of MERS, with infection as the most pre-eminent.Case Presentation: We report the case of a 10-year-old girl with MERS due to scrub typhus. Her clinical manifestations of headache and drowsiness, together with lesions involving the SCC, as shown by MRI, and their complete resolution upon follow-up fulfilled the diagnosis of MERS. At the same time, the characteristic eschar of the skin and the positive Weil-Felix test result confirmed the existence of scrub typhus infection.Conclusions: To the best of our knowledge, we described the first pediatric case of MERS associated with scrub typhus. The case indicated that an MERS patient with fever should be considered as possibly having a scrub typhus infection. The characteristic black eschar of scrub typhus generally occurs after bite of mite that is important and useful to the doctor for making proper diagnosis.
Imaging and Clinical Characteristics of Marchiafava-Bignami Disease: Six Cases

Abstract:Objective To investigate the presentation and radiologic findings of MarchiafavaBignami disease (MBD).Methods Six cases of MBD who were diagnosed and treated in our hospital were retrospectively analyzed,including the imaging examination(such as cranial CT,magnetic resonance imaging (MRI)),laboratory tests,clinical symptoms and prognosis.Results Six cases of MBD disease were presented with swelled,thickened corpus callosum,iso-or hypo-intensity on T1WI,hyper-intensity on T2WI and fluid attenuated inversion recovery and restricted diffusion on diffusion weighted imaging.Coma of acute onset was the major clinical finding in case 1,case 4 and case 6,which showed the lesions in the entire corpus callosum and extended to the white matter,and the prognosis of these cases were worse.Slow reaction and memory decrease were the clinical findings in case 2 and case 3,which showed the genu and splenium of corpus callosum involvement on MRI and CT,and returned to normal with aggressive treatment.Numbness and weakness of the lower lambs were the major clinical finding in case 5,which showed that the lesion was limited in the splenium and body of corpus callosum on MRI.The patient recovered after treatment.Conclusions MBD may present with various clinical forms,but have characteristic imaging findings.Outset form and neuroimaging characteristics of MBD are critical for improvement of the clinical outcome.

Abstract:Purpose To analyze magnetic resonance imaging (MRI) findings and clinical diagnosis and treatment data relating to Angiostrongylus cantonensis infection to gain insight into the disease. Materials and Methods We retrospectively analyzed the epidemiology, clinical manifestations, diagnosis and treatment data, imaging manifestations, and outcomes of 27 patients who were clinically diagnosed with angiostrongyliasis and who underwent contrast-enhanced brain MRI. Results Patients with A. cantonensis infection had a history of eating raw mollusks in the endemic area, and they mainly presented with dizziness and headache of varying degrees and vomiting (n = 7). Laboratory examinations revealed increased peripheral blood and cerebrospinal fluid (CSF) eosinophils, as well as increased CSF protein levels. Brain MRI findings mainly included eosinophilic meningitis, whereas linear or nodular enhancement of the pia mater was observed in enhanced T1-weighted and fluid-attenuated inversion recovery images, accompanied by encephalitis or vasculitis. Meningitis manifested as multiple, thickened flow voids around the meninges, and contrast-enhanced scans showed substantial enhancement in intracranial dilated and hyperplastic blood vessels. Conclusion The possibility of A. cantonensis infection should be considered in the effective use of albendazole or mebendazole as a treatment. Combining clinical history with laboratory examination is helpful in diagnosing A. cantonensis infection. A final definite diagnosis can be confirmed by detecting larvae in the CSF. The administration of corticosteroids during pathogen therapy can substantially reduce the therapeutic response.

Abstract:Objective The purpose of this study was to identify characteristic magnetic resonance imaging (MRI) features of cerebral sparganosis, a rare parasitic disease caused by the plerocercoid larva of Spirometra mansoni. Methods This retrospective study reviewed medical records, computed tomography (CT) and MRI scans and pathological specimens from patients with pathologically proven cerebral sparganosis. The location, signal intensity and contrast enhancement characteristics of the lesions were assessed. Results Records of 12 patients (seven male and five female; age range 8–35 years) were reviewed. A total of 13 lesions were identified: of the 10 patients with supratentorial lesions, nine had a single lesion and one had bilateral hemispheric lesions. Two patients had a single lesion in the ependyma of the 4th ventricle. All lesions were iso-hypointense on T1-weighted images, slightly hypointense on T2-weighted images and surrounded by extensive oedema. Ten of the 13 lesions demonstrated a ‘string-knots sign’, characterized by a tangled string in a knot-like shape on contrast-enhanced MRI. Conclusion A string-knots sign enhancement pattern in cortical–subcortical regions should suggest the diagnosis of cerebral sparganosis.
Magnetic resonance imaging and next-generation sequencing for the diagnosis of cystic echinococcosis in the intradural spine: a case report

Abstract:Background: Cystic echinococcosis (CE) is a parasitic zoonotic disease caused by the larval stage of Echinococcus granulosus. The liver and lungs are the most common sites for infection. Infection of the intradural spine is rare. Case presentation: A 45-year-old woman of Han ethnicity presented with a chronic history of recurrent lumbar pain. Magnetic resonance imaging of the lumbar spine revealed the classical characteristic of multiple cystic lesions of variable sizes, manifesting a "bunch of grapes" appearance, localized within the spinal canal at the L4-L5 vertebral level. In the meanwhile, metagenomic next-generation sequencing identified Echinococcosis granulosa. The patient underwent surgery to remove the cyst entirely and subsequently took albendazole 400 mg orally twice daily for 6 months. Conclusion: Spinal CE should be suspected in patients with multiple spinal cystic lesions and zoonotic exposure. metagenomic next-generation sequencing serves as a robust diagnostic tool for atypical pathogens, particularly when conventional tests are inconclusive. Prompt and aggressive treatment for spinal cystic echinococcosis is imperative, and further research is warranted for improved diagnostic and therapeutic strategies.

Abstract:Objective:To analyse the magnetic resonance imaging features of cystic meningiomas,to help avoid misdiagnosis and improve the diagnostic accuracy.Methods:Retrospectively evaluated the data of 17 patients with cystic meningiomas.There were 8 males and 9 females,and the age ranged from 36~70 years old with the average age of 49.7 years old.17 patients had unenhanced MRI or enhanced MRI.And 3 patients underwent magnetic resonance spectroscopy(MRS).We adopted Nauta''s classification and reviewed the literature with respect to their MR imaging features and histopathology results.Results:Cystic meningiomas were frequently located on the cerebral convexity,connected to the dura with broad base.MRI scans showed the tumors solid and mixed cystic lesions.The solid parts of the tumors demonstrated hypointense or isointense on T1-weighted images,isointense or hyperintense on T2-weighted images,with significant enhancement after gadolinium administration.The cystic wall showed enhancement or not.MRS showed the absence of N-Acetyl Aspartate,a prominent choline peak with a shallow creatine peak.Of the 17 cases,seven cases were Nauta I,eight cases were NautaⅡ,and two cases were NautaⅣ.Conclusion:Cystic meningiomas were particularly uncommon tumors that are often difficult to distinguish from other intra-axial solid and mixed cystic lesions.Conventional MRI and MRS could play a significant role in the diagnosis of cystic meningiomas,the preoperative clinical treatment and proper surgical program.
Infratentorial Cryptococcus neoformans meningoencephalitis with cerebellar infarction: A rare case report

Abstract:Background: Cryptococcal meningoencephalitis (CM), is a highly fatal fungal infection of the central nervous system (CNS), affecting not only immunocompromised patients, but also apparently immunocompetent patients. CM is mainly caused by Cryptococcus neoformans (C. neoformans), while viral hepatitis B (HBV) tends to be a rare inducement. According to the literature, the most common cerebral area affected by CM was frontal lobe, while infratentorial lesions were rare, especially those complicated with cerebellar infarction. Methods: This study capitally analyzed the clinical data of an elderly female suffering from infratentorial CM complicated with cerebellar infarction, with a history of chronic HBV. Results: The patient suffered from the symptoms of dizziness, insanity, low-grade fever, and high cranial pressure throughout the course of the disease. Her MRI findings were hydrocephalus and infratentorial lesions, including bilateral cerebellums and meningeal enhancement. The pathogene was Cryptococcus revealed by both the cytology and ink stain of cerebrospinal fluid, and was confirmed to be C. neoformans by the Next generation sequencing (NGS). After 12 days of intravenous amphotericin B (AMB) treatment, the patient developed oliguria, and 3 days after the termination of AMB treatment, the renal function recovered. Brain MRI reexamination after the treatment showed that the diffused lesions in the cerebellum were significantly decreased, and acute infarction occurred on the left cerebellum although it was asymptomatic. The patient took fluconazole 400mg per day after discharge, without complaints during the follow-up two months later. Conclusion: C. neoformans infection may be a possible pathogeny in chronic HBV patients with meningoencephalitis. Cerebellar infarction might be a complication of CM, therefore MRI is supposed to be re-examined during antifungal therapy. Additionally, monitoring renal function plays a vital role after AMB treatment, and renal function may recover after termination.
Cerebral Sparganosis Presenting with Atypical Postcontrast Magnetic Resonance Imaging Findings: a Case Report and Literature Review

Abstract:BackgroundSparganosis, a rare and severe parasitic infection caused by the larvae of Spirometra species or simply sparganum, generally involves subcutaneous tissue or muscle. But occasionally, sparganum can also invade the human brain, resulting in cerebral sparganosis.Case presentationA 33-year-old woman presented with a 10-day history of headache. Postcontrast magnetic resonance imaging (MRI) revealed an irregular lesion with enhancement and the tunnel-shaped focus extending to the contralateral hemiphere. Cerebrospinal fluid (CSF) analysis disclosed pleocytosis (166 cells/mu L) and an elevated protein concentration (0.742g/L). Enzyme-linked immunosorbent assay (ELISA) revealed positive sparganum-specific antibody in both blood and CSF. Finally, the diagnosis of cerebral sparganosis was comfirmed. She received praziquantel treatment and got a favorable outcome during six-month follow-up.ConclusionsIrregular enhancement and the tunnel sign that extends to the contralateral hemisphere on postconstrast MRI are unusual presentations of cerebral sparganosis. ELISA for sparganum-specific antibody can help confirm the diagnosis. Although surgery is the preferred treatment for cerebral sparganosis, praziquantel might also achieve satisfying outcomes.

Abstract:Conventional magnetic resonance imaging (MRI) and computed tomography (CT) are used to diagnose central pontine myelinolysis (CPM), which is seen in the setting of osmotic changes, typically with the rapid correction of hyponatremia. However, they typically follow clinical symptoms and fail to detect myelinolytic lesions within the first two weeks, limiting their efficacy in early diagnosis. CPM can mimic brainstem ischaemic changes on CT head and a glioma on MRI. This case reviews the relationship between radiological changes seen with clinical symptoms and serum sodium levels, combined with reviewing pioneering advances in radiomic analysis, including diffusion-weighted MRI, CT brain perfusion and MR spectroscopy.
Magnetic Resonance Imaging Characteristics with Pathological Correlation of Cavernous Malformation in Cavernous Sinus

Abstract:OBJECTIVE:To analyze extracerebral cavernous malformation located in the cavernous sinus and correlate their magnetic resonance imaging (MRI) features to pathological findings.MATERIALS AND METHODS:Nineteen patients (5 men and 14 women; mean age, 50.6 years; range, 32-71 years) with surgically verified cavernous malformations in the cavernous sinus were reviewed. MRI including T1-weighted, T2-weighted, and postcontrasted T1-weighted imaging was carried out in all instances with a 1.5-T superconductive system (Signa; General Electric Medical Systems, Milwaukee, WI).RESULTS:All these lesions measured from 2.5 cm to 6 cm (average, 4.3 cm) with 13 lesions located to the right side and 6 to the left side. Magnetic resonance images showed that 18 cases were hypointense to white matter on T1-weighted images with only one case showed mixed hypointensity and hyperintensity. On T2-weighted images, 17 cases showed marked homogeneous hyperintensity, 1 case showed marked hyperintensity with some signal void structures and 1 with mixed intensity. Marked homogeneous enhancement after contrast material administration was found in 7 cases, and the remaining 12 showed marked heterogeneous enhancement. Pathologically, these lesions can be classified as type A, type B, and type C. Type A was sponge-like with intact pseudocapsule; type B was mulberry-like with the pseudocapsule incomplete or absent; and type C was composed of both mulberry-like composition and sponge-like composition. Lesions with homogeneous contrast enhancement on MRI correlated with type A pathological findings, whereas those with heterogeneous enhancement correlated with type B and type C pathological findings. Asymmetrical dumbbell-shaped masses of 13 cases involving the sellar and round masses of 6 cases with 2 protruding into the sellar were found. All of the cavernous malformations displaced the adjacent temporal lobe without adjacent brain edema. The internal carotid arteries were displaced or encased in all cases.CONCLUSIONS:If a well-demarcated, homogeneous high signal intensity lesion on T2-weighted images with a dumbbell configuration involving both parasellar and sellar regions with marked heterogeneous or homogeneous enhancement is found, the diagnosis of cavernous sinus cavernous malformation should be entertained.
Case Report: A Case of Sarocladium strictum Meningoencephalitis in an Immunocompetent Patient After Invasive Operation

Abstract:As an opportunistic phytopathogen, Sarocladium strictum has only been shown to cause neurological disease in immunocompromised patients, where antifungal therapy was not effective. We report a case of Sarocladium strictum meningoencephalitis in an apparently immunocompetent young woman who presented with severe headache and slight fever after undergoing transnasal endoscopic repair of cerebrospinal fluid rhinorrhea. Chronic sinusitis and suspicious intracranial fungal lesions were observed on enhanced magnetic resonance imaging (MRI). Both culture and metagenomic next-generation sequencing of her cerebrospinal fluid were positive for Sarocladium strictum . After local debridement, treatment with amphotericin B plus voriconazole and Ommaya reservoir implantation, the patient improved significantly. Unfortunately, her symptoms worsened again despite plenty of antifungal therapy for a month.

MRI features of megalencephalic leukoencephalopathy with subcortical cysts

Abstract:Objective: To investigate the MRI features of the megalencephalic leukoencephalopathy with subcortical cysts (MLC). Methods: MR images of 7 patients with clinical diagnosis of MLC were analyzed retrospectively. There were 4 patients underwent diffusion weighted imaging (DWI) and 4 patients underwent single voxel MR spectroscopy (MRS). Results: MRI demonstrated diffused high intensity in bilateral white matter on T2WI and subcortical cysts in bilateral temporal lobes. Six of them had abnormal signal in the corticospinal tract, 4 in the white matter of the cerebellum. No abnormality was found in grey matter. MRS showed mildly reduced or normal N-acetylaspartate (NAA) peak, without obvious lactate peak. DWI showed diffused low signal intensity in the bilateral white matter, with no abnormal high signal. Conclusion: MLC has typical features on MR imaging, and MRI may be helpful for the diagnosis of the disease.

Abstract:Cerebral malaria is associated with high mortality and morbidity in patients infected with Plasmodium Falciparum. The mechanisms of cerebral malaria include sequestration of parasitized red blood cells in brain capillaries, production of cytokines, immune cell/platelet accumulation, and release of microparticles, resulting in disruption of the blood-brain barrier, which caused brain injuries. The severity of this reflects on neurological findings ranging from simple delirium to profound coma. We herein present unique magnetic resonance imaging findings of a case of fulminant cerebral malaria as computed tomography studies usually underestimate the extent of cerebral involvement in malaria.
