Preliminary Research on Acute Kidney Injury after Cardiac Surgery in Elderly People

Abstract:Background The aim of the present study was to analyze the current situation of acute kidney injury(AKI) after cardiac surgery in adults of different age,and to investigate the possibility of prophylaxis and treatment.Methods We prospectively collected data from patients who underwent cardiac surgery from April 2009 to May 2011,and analyzed the incidence and risk factors of AKI after surgery and short-term clinical outcomes in patients of different age.Results A total of 3896 patients were included,of whom 3086 patients were in young and middle-aged group(18~65),772 patients in elderly group(65~80),and 38 patients in senile group(=80).The male proportion,preoperative serum creatinine,NYHA III grade,and proportions of diabetes mellitus,hypertension,coronary angiography,and coronary artery bypass graft(CABG) surgery,postoperative use of adrenaline and noradrenaline for hypotension were significantly higher in elderly group than in young and middle-aged group(all P0.05).The preoperative serum creatinine,NYHA III-IV grade and proportion of(CABG) surgery were significantly higher in senile group than in elderly aged group(all P0.05).The incidence of AKI after cardiac surgery was significantly lower in young and middle-aged group than in elderly and senile groups(23.1% vs.39.9%,P0.05;23.1% vs.42.1%,P0.05).There was no significant difference in hospital mortality among the three groups.Multivariate logistic regression analysis showed that male gender,age,preoperative SCr 1.2mg/dl,postoperative use of adrenaline for hypotension were the independent risk factors for AKI after cardiac surgery in patients over 65 years old.Conclusions The incidence of AKI after cardiac surgery in elderly people increased with age and with higher mortality.The incidence of AKI was related to a number of perioperative risk factors.Elderly people complicated with AKI after cardiac surgery should be managed with positive treatment to improve their outcome.
