Abstract:Radiation-induced lung toxicity (RILT) is an important dose-limiting toxicity during thoracic radiation therapy. The purpose of this study is to investigate perfusion (Q) single-photon emission computerized tomography (SPECT)-weighted functional dose-volume histogram (FDVH) during the course of radiation therapy (RT) for predicting RILT in patients with non-small-cell lung cancer (NSCLC). (This study is supported by NSFC81172133 and NSFC81372413) Fifteen locally advanced NSCLC patients were enrolled prospectively. All patients were treated with definitive RT 60Gy and concurrent cisplatin based chemotherapy. The simulation computerized tomography (CT), Q-SPECT-CT were performed prior to and during RT at approximately 45 Gy. The RT plans really performed were generated only from the simulation CT 3 days before treatment and three dimensional conformal radiation therapy (3D CRT) or intensity modulated radiation therapy techniques (IMRT) were used according to clinical need. Functional lung was weighted by the 99m-technetium (99m-Tc)-labeled macro aggregated albumin (MAA) SPECT lung perfusion beyond a threshold of 30% of the maximum radioactivity. The baseline dose-volume histogram (baseline-DVH) based on baseline simulation CT, during-DVH based on during-RT, baseline functional lung DVH (baseline-FDVH) and during-FDVH were calculated respectively. The RILT grade was defined according to the National Cancer Institute Common Toxicity Criteria, version 4.0. The development of RILT was considered as a binary variable: “no-RILT” (Grade≥1) and “RILT” (Grade≥2). The predictive abilities of baseline-DVH, during-DVH, baseline-FDVH, and during-FDVH were calculated and compared based on the area under receiver operating characteristic (ROC) curve (AUC). The accumulative incidence of ≥ 2 grade RILT was 46.7% with a median follow-up of 6 months. Univariate analysis showed that the standard (V5–V20) and functional (FV5–FV20) parameters, the mean lung dose (MLD) and functional MLD (FMLD) were associated with RILT (all value of P < 0.05). By ROC curve analysis, the AUCs of baseline-DVH metrics were 0.821-0.857, the AUCs of during-DVH metrics were 0.857-0.893, the AUCs of baseline-FDVH metrics were 0.821-0.911, and the AUCs of during-FDVH metrics were 0.839-0.964. These results indicated that the FDVH parameters seemed to add more predictive value than the DVH parameters; the parameters during RT seemed to add more predictive value than those based on baseline simulation CT or Q-SPECT-CT. The during-FDVH parameters showed the best predictive value. For example, during-RT FV10’s AUCs in predicting RILT was 0.964 that of V10 prior to RT was 0.857. During-FDVH parameters based on the Q-SPECT-CT are identified as reliable predictors for RILT, and may guide RT plan modification in patients with NSCLC.
