Abstract:Dyke-Davidoff-Masson Syndrome (DDMS) is a rare neurological disease with an unknown incidence. The manifestations of DDMS are variable, while typical symptoms are seizures, hemiparesis, and mental retardation. Here, we present a case involving a 19-year-old male patient who presents with headaches, mood changes, and a history of seizures during childhood. Based on the neuroimages, a diagnosis of DDMS was established. The application of sertraline hydrochloride as a therapeutic intervention has alleviated the symptoms. This case report illustrates the importance of understanding the clinical features of DDMS based on imaging.
Clinical Characteristics and Neuroimaging Findings of Seven Patients with Dyke Davidoff Masson Syndrome

Abstract:Background DDMS is a rare disease diagnosed by clinical and radiological characteristics. But the complexity of radiological and clinical manifestations of DDMS has become a challenge diagnostically. To date, the reported cases with DDMS had highly varied clinical manifestations including seizures, contralateral hemiplegia/hemiparesis, facial asymmetry, mental retardation, etc. In addition to typical clinical findings, some new characteristics have been recently added to the spectrum of DDMS. However, few cases have been reported to be associated with neuropsychiatric symptoms according to the literature. This study aimed to investigate the neuropsychiatric manifestations associated with Dyke-Davidoff-Masson syndrome (DDMS) and related imaging findings. Methods This study included 7 patients diagnosed with DDMS between 2014 and 2020. The clinical characteristics, neuropsychiatric manifestations, and radiological results were retrospectively evaluated. Results Seven patients (five males and two females) with a mean age of 28.0 ± 9.73 (range 15.0–41.0) years were included. Five patients were admitted to the psychiatric unit due to psychological and behavioral disorders. Two patients were referred to the neurology unit mainly due to epilepsy. Six patients had epileptic seizures, 4 had hemiplegia, 3 had mental retardation, 2 patients had external ear deformities, and 2 had facial asymmetry. Neuropsychiatric symptoms were presented in 6 (85.7 %) cases. Cases 2–6 developed affective disorders. Deficits in verbal communication, impairment of social interaction, lack of insight, adulia and hypobulia appeared in cases 1–4. Schizophrenia with apathy, and epileptic schizoid psychosis were observed in cases 4 and 5 respectively. Case 6 had behavioral disorders, hyperactivity, tic disorder, mental retardation, anxiety, catatonic symptoms and suicidal tendency. Case 7 had seizures and mental retardation, and no psychiatric symptoms were presented. Radiological examinations showed unilateral cerebral atrophy, enlarged lateral ventricles, and various compensatory hypertrophy of the skull in all cases. The midline structure has shifted to the affected side in 5(71.4 %) cases. Atrophy of the basal ganglia or brain stem was observed in 4(57.1 %) cases. Conclusions The hallmark imaging manifestations of DDMS facilitated the diagnosis in most cases. This study illustrated that a variety of psychoneurotic disorders and ear abnormalities were correlated with DDMS.
Management and Molecular Diagnosis of Persistent Müllerian Duct Syndrome in Chinese Patients

Abstract:Abstract Background: To report the management and molecular findings of patients with persistent Müllerian duct syndrome (PMDS).Materials and Methods: The hospital records of PMDS patients seen in our center between 2012 and 2020 were retrospectively reviewed. Exome sequencing and Sanger verification were performed for accurate diagnosis. We also reviewed reported clinical and molecular data on PMDS.Results: A total of 12 PMDS patients with a median age of 13 months and a median follow-up time of 25 months were included in the study. Preoperative diagnoses were obtained on the basis of ultrasound (7/12) and exome sequencing (1/12). Twelve different AMH variants in 9 patients (2 frameshift/nonsense, 1 deletion, 8 missense, 1 in-frame) and 6 different AMHR2 variants in 3 patients (5 missense, 1 splicing) were identified. Not all patients with AMH defects showed low serum AMH concentrations (2/9), but all patients with AMHR2 defects showed elevated AMH levels (3/3). During surgery, an abnormal vas deferens was observed in half of the patients. Patients with orchidopexy with uterine preservation presented complications including irreducible cryptorchidism (2/8) and Müllerian remnant cysts (3/8). Patients with subtotal hysterectomy had complications of injury to the vas deferens (1/3) and hemorrhage (1/3).Conclusions: This is the first report involving a large Chinese population. The present study not only expands the variation spectrum but also provides new insights into the management of the uterus.
Preliminary Investigation of Gender Assignment in 46,XY Disorders of Sex Development with Severe Male Undermasculinisation

Abstract:Objective To explore the feasibility of gender assignment in 46,XY disorders of sex development (DSD) with severe undermasculinisation mainly based on molecular diagnosis.Methods A retrospective study of 45 patients of 46,XY DSD with severe undermasculinisation were admitted between November 2015 and October 2018 at Children's Hospital,Zhejiang University School of Medicine.The initial social gender were all female,of whom the external genital manifestations were Prader 0 to 2;the degree of masculinity was scored using external masculinisation score (EMS);the position and development of the gonads were examined by ultrasound,cystoscopy and laparoscopy,also including assessing the development of the Wolffian tube and the Müllerian tube.The level and ratio of testosterone to dihydrotestosterone before and after hCG stimulation were evaluated for the function of Leydig cell and 5α-reductase-2.Gender role scales and sandbox games were used to assess gender role behavior.Genital sensitivity to androgen stimulation was assessed;A panel including 163 genes related to gender development were determined by second-generation sequencing in all 45 patients.Finally,a multidisciplinary team (MDT) makes a gender assignment after a comprehensive analysis mainly based on the molecular etiological diagnosis.Results Thirty-nine out of 45 patients (87％) had an identifiable genetic etiology,and the remaining 6 (13％) were negative for genetic testing.Forty-five patients had EMS less than or equal to 3 points.Sexual psychological assessment was performed in 39 patients,with male dominance in 24 (62％) and female dominance in 15 (38％).The gender assignment was 23 cases (51％) for male and 19 cases (42％) for female,and 3 cases (7％) were not completely determined.Conclusions Molecular diagnosis provides a strong basis for appropriate gender assignment of 46,XY DSD children with severe undermasculinisation.Based on molecular diagnosis,each DSD should be analyzed by professional MDT to analyze the clinical symptoms/signs,gonadal development,gonad tumor risk,external genital morphology,sexual psychological assessment,potential fertility opportunities,parental views,Social and cultural factors,etc.make appropriate gender assignment.
Surgical Management and Molecular Diagnosis of Persistent Müllerian Duct Syndrome in Chinese Patients.

Abstract:Persistent Müllerian duct syndrome (PMDS) is a rare clinically and genetically overlapping disorder caused by mutations in the anti-Müllerian hormone (AMH) gene or the anti-Müllerian hormone receptor type 2 (AMHR2) gene. Affected individuals present uterus and tubes in normally virilized males and are discovered unexpectedly during other surgeries. Since it is rare and complex, a definitive clinical diagnosis can be missed, and there are no guidelines regarding how to deal with the uterus. In the present study, exome sequencing and Sanger verification were performed for causal variants in 12 PMDS patients. Preoperative diagnoses were made by positive exome sequencing in 8 patients. Of them, 7 patients evoked on the basis of ultrasound indicating bilateral testes on the same side of the body. Twelve different AMH variants (2 frameshift/nonsense, 1 deletion, 8 missense, and 1 in-frame) in 9 patients and 6 different AMHR2 variants (5 missense and 1 splicing) in 3 patients were identified. Seven variants were classified as "pathogenic" or "likely pathogenic", and 4 of them were novel. All but two patients with AMH defects showed low serum AMH concentrations, but all patients with AMHR2 defects showed elevated AMH levels. During surgery, an abnormal vas deferens was observed in half of the patients. Eight patients underwent orchidopexy with uterine preservation. Of them, 2 patients presented complications including irreducible cryptorchidism, and 3 patients developed Müllerian remnant cysts. Three patients underwent subtotal hysterectomy. Of them, one patient had complication of injury to the vas deferens, and one had hemorrhage after operation. This is the first report of PMDS involving a large Chinese population. The present study not only expands the variation spectrum but also provides clinical experience about the management of the uterus.

A case report of breast development as the first manifestation combined with 46, XY complete disorder of sex development

Abstract:To report the process of diagnosis and treatment of 1 case with SRY gene mutation of 46, XY complete gonadal dysplasia, and to discuss the clinical characteristics, diagnosis and treatment of the disease.Due to clitoral enlargement for 8 months, a 9 years old girl was admitted to the Children′s Hospital Affiliated to Zhejiang University School of Medicine.Previously, she had early breast development, and suffered from high gonadotropin expression when she was 6 years and 4 months old.Physical examination: breast B3 stage, female vulva, clitoris hypertrophy, normal urethra, normal vaginal opening, slightly thick hymen ring, the development of pubic hair was 2 stages, and Prader score level 1.Laboratory data showed elevated levels of estradiol, testosterone, and human chorionic gonadotrophin.Genetic examination revealed that the chromosome karyotype was 46, XY and SRY gene detection was positive.Therefore, the patient was diagnosed with 46, XY complete gonadal dysplasia.Bilateral gonadectomy was performed, and the posto-perative pathological diagnosis was bilateral gonadoblastoma with left dysgerminoma.The tumor did not recur after che-motherapy.The etiology of early breast development needs to be carefully identified.Patients with sexual characteristics dysplasia need to accept the chromosome karyotype analysis and gene detection, and surgical exploration should be performed when necessary for a correct diagnosis as soon as possible.
Recent advances in clinical management of androgen insensitive syndrome

Abstract:As one of the most common sexual dysplasia diseases, androgen insensitivity syndrome (AIS) is divided into complete, partial and mild types according to the degree of androgen receptor function impairment. Complete androgen insensitivity syndrome (CAIS) shows a normally feminized external genital phenotype while partial androgen insensitivity syndrome (PAIS) usually presents with vague external genitalia at birth. Mild androgen insensitivity syndrome (MAIS) is considered in adolescent males with feminized breasts and infertility. The endocrinology of AIS is characterized by persistent androgen resistance and the core of its clinical management lies in gender allocation. After assessments of a multidisciplinary team, CAIS is assigned as the female and most cases of PAIS belong to the males. And vigilance is needed for gender redistribution due to gender anxiety in adulthood. At the same time, plastic surgery of external genital after sex allocation, long-term hormone replacement, optimal timing of gonadectomy and fertility potential and genetic recommendations all require personalized management. This review summarized the latest researches in clinical management of androgen insensitivity syndrome.
Diagnosis of Dyke-Davidoff-Masson syndrome in an adult

Abstract:Dyke-Davidoff-Masson syndrome is a rare neurological condition characterized by intractable seizures, cerebral hemiatrophy with contralateral hemiparesis. Our patient, a 38-year-old female, presented following a left focal seizure with secondary generalization. She had a history of epilepsy, associated with left-sided hemiparesis, beginning at the age of 7. Physical examination showed increased left-sided tone and brisk reflexes, with an extensor plantar reflex on the left. The MRI brain showed features suggestive of Dyke-Davidoff-Masson syndrome: right-sided cortical atrophy, calvarial thickening and dilated frontal sinus. Additional MRI findings were of right cerebral peduncle atrophy and left cerebellar atrophy. This case report intends to emphasize the importance of Dyke-Davidoff-Masson syndrome as an unusual cause of seizures in an adult complicated by poor social determinants of health, leading to its delayed diagnosis.
Multisystemic smooth muscle dysfunction syndrome in children: a case report and literature review

Abstract:Objective To analyze the clinical characteristics and diagnosis of multisystemic smooth muscle dysfunction syndrome (MSMDS).Method Clinical data of a case diagnosed as MSMDS and hospitalized in our hospital in July 2016 was retrospectively analyzed.Literature search was performed at databases of PubMed,Wanfang,China National Knowledge Infrastructure and VIP with the key words "multisystemic smooth muscle dysfunction syndrome ACTA2".The literature retrieval was confined from January 1980 to November 2016.The characteristics of MSMDS were summarized through review of literature.Result A girl aged 1.6 years had recurrent cough and wheeze for more than 1 year,complicated with congenital fixed dilated pupils,patent ductus arteriosus,pulmonary hypertension,chronic lung disease,and cerebrovascular abnormalities.We had done gene analysis for the patient and found ACTA2 c.536C ＞ T (p.R179H) heterozygous mutations,but her parents were normal.Totally 11 reports were retrieved from foreign language literature and no report from Chinese literature could be found;the retrieved articles reported a total of 25 cases of multiple system smooth muscle dysfunction syndrome.The minimum age was 11 months,17 cases were female,8 were male.The clinical common characteristic is congenital fixed dilated pupils,patent ductus arteriosus,cerebrovascular disease,pulmonary hypertension,chronic lung disease,and so on.Conclusion Genetic testing for ACTA2 gene mutations should be considered in infants presenting with congenital fixed dilated pupils and patent ductus arteriosus.

Abstract:Maffucci's syndrome is a rare,congenital mesodermal dysplasia combined with dyschondroplasia and haemangiomatosis,and there are only about 200 reported cases. We reported a case with Maffueei syndrome.

Abstract:Dyggve-Melchior-Clausen (DMC) disease, also known as DMC syndrome, is a rare, progressive genetic disorder that is characterized by skeletal and intellectual abnormalities. The case report involves a four-year-old male child presenting with marked short stature, intellectual disability, coarse facies, and microcephaly. Initial investigations, including blood tests and radiological evaluations, prompted further genetic testing via whole-exome sequencing. This identified a homozygous mutation in the Dymeclin (DYM) gene, implicating DMC disease. The condition usually poses diagnostic challenges due to overlapping clinical features with Morquio syndrome. This case highlights the importance of a comprehensive diagnostic approach and genetic testing in elucidating the underlying genetic etiology of complex presentations in pediatric patients.

Abstract:A disorder of sex development (DSD) is defined as a congenital condition in which the development of chromosomal, gonadal, or anatomical sex is atypical. [ 1–3 ] According to the Chicago classification, DSD is subdivided into 3 groups based on cytogenetic, hormonal, gonadal histology, and clinical findings: 46, XY (primary disorder of testicular development or disorders in androgen synthesis or action), 46, XX (disorders of ovarian development, congenital adrenal hyperplasia, aromatase deficiency), and sex chromosome disorders (Klinefelter syndrome, Turner syndrome, chimerism, mixed gonadal dysgenesis). [ 1 , 3 , 4 ]

One Case of Misdiagnosed 46,XX Male Syndrome and Literature Review

Abstract:Objective To investigate the cause of misdiagnosis and the clinical features of 46,XX male syndrome,and to analyze the male sex determination and molecular genetic basis.Method The clinical features,laboratory examinations,cytogenetic and molecular features of 46,XX(SRY positive)male syndrome were analyzed.Combined with literature review,the causes of misdiagnosis,the prevention and treatments of this disease were analyzed.Result The sociopsychology of the patient was male.He came to hospital for gynecomastia,and was misdiagnosed as idiopathic adolescent gynecomastia.And then serum sex hormones gave some clues of this disease.Definite diagnosis was made by karyotype analysis and FISH exanmination.Conclusion The clinical manifestations of 46,XX male syndrome are heterogenous.It also can cause gynecomastia,and be easily misdiagnosed.So comprehensive physical and laboratory tests,including karyotype analysis,are necessary.
