Abstract:Objective To establish an ultrasonography (US) scoring system for screening compensated liver cirrhosis (CLC) in patients with CHB. Methods 432 patients with CHB were retrospectively analyzed. Patients had been accepted ultrasonographic scanning and liver biopsy. US indexes of liver surface, liver parenchyma, gallbladder wall, hepatic vessel, splenomegaly and diameter of main portal vein (MPV) were analyzed for the association with stages of fibrosis. US scoring system consisting of liver parenchyma, gallbladder wall, hepatic vessel and splenomegaly was used to detect CLC. Kappa statistics was used to measure agreement between US observes. Results Cirrhosis was found in 18.5% of patients. Kappa values were more than 0.8, which indicated well agreement between US observes. Liver surface and MPV in cirrhosis patients did not significantly differ from other stages of fibrosis.There were significantly differences between cirrhosis and other stages of fibrosis in liver parenchyma, gallbladder wall, hepatic vessel and splenomegaly (P < 0.01). Positive likelihood rate (PLR) of hepatic vessel profile for determining cirrhosis amounted to 11.06. The ROC curve showed that score of 5 should be cut-off point for screening cirrhotic patients with sensitivity of 81.3%, negative predictive value of 92.8%. Score of 8 achieved a PLR of 9.86 for determining CLC, with specificity of 98.6%. Conclusion Hepatic vessel profile can predict compensated cirrhosis well. Cirrhosis can be screened sensitively by US scoring system. Liver biopsy is still the confirming diagnosis for most CLC patients.
