Effective Connectivity in the Limbic System in Parkinson's Disease with Mild Cognitive Impairment

Abstract:Objective: Since the mapping of cognitive deficiency in Parkinson’s disease (PD) is still unclear, it is necessary to find structural and functional changes in PD with cognitive impairment for early intervention.Method: We enrolled 17 PD patients with mild cognitive impairment (MCI), 18 PD patients without cognitive impairment, 14 Alzheimer disease (AD) patients, and 25 age-matched normal controls. Voxel-based morphology was used to explore structural changes in PD, and then we constructed a spectral dynamic causal model, including the structurally abnormal regions and their functionally related ones to compare their internal effective connectivity between groups.Result: Amygdala and anterior hippocampus (AMY-AH) gray matter density loss was found in PD with and without MCI. AMY-AH had significant functional connectivity with the medial prefrontal cortex (mPFC), and its volume was positively correlated with cognitive performance. Compared to PD, PD-MCI demonstrated inhibitory connections from the right AMY-AH to mPFC, as well as self-inhibitory in left AMY-AH and increased connection between left AMY-AH to mPFC.
