Abstract:Diabetic nephropathy (DN) is the leading cause of end-stage renal disease (ESRD) in the worldwide.Recently,with the increasing incidence of diabetes in China,prevalence of DN has also growth rapidly.Moreover,the proportion of ESRD patients caused by DN did not significantly decreased in the past twenty years,suggesting that the prevention and treatment of DN was still a challenge for public health.In this issue,we invited nephrologists and diabetologists to highlight the current advances from different aspects,including disease risk factors,diagnosis and prognosis,prevention and treatment,genetics studies,pathologic classification,and molecular biomarker researches.

Abstract:Diabetic nephropathy (DN) is the main cause of end-stage renal disease. DN is a complex disease mediated by genetic and environmental factors, and many cellular and molecular mechanisms are involved in renal damage in diabetes. There are no biomarkers that reflect the severity of the underlying renal histopathological changes and can effectively predict the progression of renal damage and stratify the risk of DN among individuals with diabetes mellitus. Current therapeutic strategies are based on the strict control of glucose and blood pressure levels and, although there are new anti-diabetic drugs, these treatments only retard renal damage progression, being necessary novel therapies. In this Special Issue, there are several comprehensive reviews and interesting original papers covering all these topics, which would be of interest to the growing number of readers of the Journal of Clinical Medicine.

Global diabetic kidney disease research from 2000 to 2017: A bibliometric analysis

Abstract:Background: This study aimed to analyze the scientific outputs of diabetic kidney disease (DKD) research and explore its hotspots and frontiers from 2000 to 2017, using bibliometric methods. Methods: Articles in DKD research between 2000 and 2017 were retrieved from the Web of Science Core Collection (WoSCC). We used the VOSviewer 1.6.8 and CiteSpace 5.2 to analyze publication years, journals, countries, institutions, authors, references, and keywords. Keywords with citation bursts were used to analyze the research hotspots and emerging trends. Results: We identified 27,577 publications in DKD research from 2000 to 2017. The annual publication number increased with time. Nephrology Dialysis Transplantation published the highest number of articles. The United States was the most influential country with most publications and collaborations with other countries. Harvard University was the leading institute. Parving had the most cited publications. Keywords analysis indicated that the renin-angiotensin system inhibition used to be the most prevalent research topic, while recent research hotspots were podocyte, inflammation, and biomarker. The biomarkers for DKD screening, diagnosis, and prognosis could be a research frontier. Conclusions: The number of DKD related publications rapidly increased over the past 2 decades. Our study revealed the structure, hotspots, and evolution trends of DKD research. Further studies and more collaborations are needed.

Abstract:Diabetic nephropathy (DN) is the main cause of end-stage kidney disease (ESKD). DN-related ESKD has the worst prognosis for survival compared with other causes. Due to the complex mechanisms of DN and the heterogeneous presentations, unmet needs exist for the renal outcome of diabetes mellitus. Clinical evidence for treating DN is rather solid. For example, the first Kidney Disease: Improving Global Outcomes (KDIGO) guideline was published in October 2020: KDIGO Clinical Practice Guideline for Diabetes Management in Chronic Kidney Disease. In December of 2020, the International Society of Nephrology published 60 (+1) breakthrough discoveries in nephrology. Among these breakthroughs, four important ones after 1980 were recognized, including glomerular hyperfiltration theory, renal protection by renin-angiotensin system inhibition, hypoxia-inducible factor, and sodium-glucose cotransporter 2 inhibitors. Here, we present a review on the pivotal and new mechanisms of DN from the implications of clinical studies and medications.

Abstract:As diabetes and diabetic kidney disease have reached epidemic proportions across the world, there has been rapid expansion in our knowledge on the immunopathogenesis, genetic and epigenetic mechanisms, and identification of novel therapeutic targets. In this chapter, we will review the major research topics and current developments in the field of diabetes and diabetic kidney disease, which shall provide a perspective on what is on the horizon. Briefly, we will discuss the roles of the immune system in diabetes development and the ongoing research on immunotherapies; we will review the advances in regenerative medicine and nanotechnology for diabetes treatment; we will examine current research on diabetic kidney disease, focusing on the roles of epithelial-to-mesenchymal transition in kidney fibrosis and novel therapeutic targets that may reverse fibrosis; and we will also review current findings in genetic and epigenetic studies, as well as in proteomics and metabolomics research related to diabetic kidney disease. These advancements clearly point to a future in diabetes and diabetic kidney disease with greatly expanded therapy options.

Abstract:China is the country with the largest burden of diabetes worldwide. Action is now taking place at different levels by the Chinese Diabetes Society, which has a major role in coordinating multi-level activities across the country. These include, among others, the implementation of local guidelines for therapy, campaigns for education, trials for type 2 diabetes prevention and the establishment of extensive international collaborations in basic as well as clinical research. In this Special Issue, we briefly report on such initiatives, among which is a scientifically sound approach to exploring Chinese traditional medicine as a therapeutic option for diabetes treatment.

Abstract:Diabetic foot wounds and infections pose a significant and evolving challenge in diabetes care. Diabetic wound healing has become a major global concern for a very long time. Continuous research has been conducted to increase the healing process in diabetic ulcers to the rate of amputation. Wound healing is prolonged in diabetic patients due to various conditions, such as high glucose levels, neuropathy, poor blood circulation, and prolonged inflammation around the limbs, which causes the healing to be delayed compared to normal patients. Understanding the complexity of chronic foot wounds and the management and proper treatment would lead to a decrease in the risk of amputation. The medical team all over the world is constantly researching to lower the risk. This review paper offers a compelling journey through the multifaceted world of diabetic foot wounds and infections. It underscores the urgency of understanding classification, tackling multidrug resistance, and harnessing microbial insights to revolutionize the treatment and management of diabetic foot complications. Furthermore, it unveils state-of-the-art diagnostics, heralding a brighter future in the battle against this debilitating complication of diabetes.

A Systems Nephrology Approach to Diabetic Kidney Disease Research and Practice

Abstract:Background: Diagnosis and staging of diabetic kidney disease (DKD) via the serial assessment of routine laboratory indices lacks the granularity required to resolve the heterogeneous disease mechanisms driving progression in the individual patient. A systems nephrology approach may help resolve mechanisms underlying this clinically apparent heterogeneity, paving a way for targeted treatment of DKD. Summary: Given the limited access to kidney tissue in routine clinical care of patients with DKD, data derived from renal tissue in preclinical model systems, including animal and in vitro models, can play a central role in the development of a targeted systems-based approach to DKD. Multi-centre prospective cohort studies, including the Kidney Precision Medicine Project (KPMP) and the European Nephrectomy Biobank (ENBiBA) project, will improve access to human diabetic kidney tissue for research purposes. Integration of diverse data domains from such initiatives including clinical phenotypic data, renal and retinal imaging biomarkers, histopathological and ultrastructural data, and an array of molecular omics (transcriptomics, proteomics, etc.) alongside multi-dimensional data from preclinical modelling offers exciting opportunities to unravel individual-level mechanisms underlying progressive DKD. The application of machine and deep learning approaches may particularly enhance insights derived from imaging and histopathological/ultrastructural data domains. Key messages: Integration of data from multiple model systems (in vitro, animal models, and patients) and from diverse domains (clinical phenotypic, imaging, histopathological/ultrastructural, and molecular omics) offers potential to create a precision medicine approach to DKD care wherein the right treatments are offered to the right patients at the right time.

Abstract:Diabetic nephropathy (DN) is a kidney disorder secondary to diabetes and is one of the main diabetic microvascular complications. As the number of diabetic patients grows, DN has become the leading cause of chronic kidney disease in China. Unfortunately, no definitive cure currently exists for DN. Cornus officinalis (CO), frequently utilized in clinical settings for diabetes mellitus treatment, has proven vital in both preventing and treating DN. This article explores the pathogenesis of DN and how CO and its active compounds regulate glucose and lipid metabolism, exhibit anti-inflammatory properties, inhibit oxidative stress, regulate podocytes, and manage autophagy. The mechanism and role of and its active compounds in the treatment of DN are discussed.

Abstract:Background: Diabetic nephropathy (DN) and diabetic retinopathy (DR) are microvascular complications of diabetes that share a similar pathogenesis and clinical relevance. The study aimed to visually analyze the research status and development trend of the relationship between DN and DR by means of bibliometrics and knowledge mapping. Methods: Publications were collected from the Science Citation Index-Expanded of the Web of Science Core Collection between 2000 and 2021. CiteSpace, Alluvial Generator, and Microsoft Excel were used to analyze and present the data. Results: A total of 3,348 publications were retrieved and 3,285 were included in the analysis after deduplication. The publications demonstrated an annually increasing trend. The results of the collaborative network analysis showed that the United States, Steno Diabetes Center, and Tien Y. Wong were the most influential country, institution and author, in this field of research, respectively. The analysis of references and keywords showed that the pathogenesis of DN and DR and their relationship with cardiovascular disease are research hotspots. The clinical relevance and drug therapy for DN and DR will become frontiers of future research in this field. Conclusion: This study is the first to visualize the correlation between DN and DR using a bibliometric approach. This study provides a reference of research trends for scholars.
Clinical efficacies, underlying mechanisms and molecular targets of Chinese medicines for diabetic nephropathy treatment and management

Abstract:Diabetic nephropathy (DN) has been recognized as a severe complication of diabetes mellitus and a dominant pathogeny of end-stage kidney disease, which causes serious health problems and great financial burden to human society worldwide. Conventional strategies, such as renin-angiotensin-aldosterone system blockade, blood glucose level control, and bodyweight reduction, may not achieve satisfactory outcomes in many clinical practices for DN management. Notably, due to the multi-target function, Chinese medicine possesses promising clinical benefits as primary or alternative therapies for DN treatment. Increasing studies have emphasized identifying bioactive compounds and molecular mechanisms of reno-protective effects of Chinese medicines. Signaling pathways involved in glucose/lipid metabolism regulation, antioxidation, anti-inflammation, anti-fibrosis, and podocyte protection have been identified as crucial mechanisms of action. Herein, we summarize the clinical efficacies of Chinese medicines and their bioactive components in treating and managing DN after reviewing the results demonstrated in clinical trials, systematic reviews, and meta-analyses, with a thorough discussion on the relative underlying mechanisms and molecular targets reported in animal and cellular experiments. We aim to provide comprehensive insights into the protective effects of Chinese medicines against DN.

Abstract:Diabetic Retinopathy is a critical health problem influences 100 million individuals worldwide, and these figures are expected to rise, particularly in Asia. Diabetic Retinopathy is a chronic eye disease which can lead to irreversible vision loss. Considering the visual complexity of retinal images, the early-stage diagnosis of Diabetic Retinopathy can be challenging for human experts. However, Early detection of Diabetic Retinopathy can significantly help to avoid permanent vision loss. The capability of computer-aided detection systems to accurately and efficiently detect the diabetic retinopathy had popularized them among researchers. In this review paper, the literature search was conducted on PubMed, Google Scholar, IEEE Explorer with a focus on the computer-aided detection of Diabetic Retinopathy using either of Machine Learning or Deep Learning algorithms. Moreover, this study also explores the typical methodology utilized for the computer-aided diagnosis of Diabetic Retinopathy. This review paper is aimed to direct the researchers about the limitations of current methods and identify the specific areas in the field to boost future research.
