Abstract:A thorough understanding of the spatiotemporal dynamics of blood supply and tissue viability is of great importance in stroke researches. In the current study, vascular and cellular responses to focal ischemia were monitored with optical coherence tomography on chronic rat photothrombotic stroke model. The 3D mapping of blood perfusion and cellular scattering were achieved by analyzing the temporal dynamics and depth attenuation of intrinsic backscattered light respectively. Optical coherence tomography revealed that vessels of different types presented various spatial and temporal dynamics during the photothrombotic occlusion and the later recovery period. The large distal middle cerebral arteries presented a spontaneous recanalization and the small pial microvessels presented a reperfusion along with newly appeared vessels from the peripheral into the core area. The cortical capillary perfusion presented a weak recovery. Compared to the male group, the female rats showed a faster vascular recovery after photothrombotic. Moreover, the dynamic changes of the cellular scattering signal showed a high spatial and temporal correlation with the cortical capillary perfusion. Combined with well-designed photothrombotic stroke model and chronic optical window, optical coherence tomography imaging offers a unique approach to improve the understanding of stroke procedure and evaluate the treatment outcomes.

Abstract:Functional imaging techniques have been continuously developed based on optical coherence tomography (OCT). OCT angiography (OCTA) technique employs the relative motion of red blood cells and surrounding tissues as the endogenous label for blood flow. By analyzing the dynamic optical scattering characteristics of spatially scattered signals in OCT, the blood flow motion information is extracted. Thus, OCTA enables an in vivo, label free, and 3D high-resolution blood flow imaging by distinguishing dynamic blood flow areas and surrounding static tissues in three-dimensional space. The optical attenuation coefficient ( OAC) algorithm evaluates the degree of tissue damage and accurately reveals tissue activity by analyzing the attenuation characteristics of spatially scattered signals in OCT with depth. OCTA technology and OAC algorithm enable in vivo, label-free, 3D high-resolution, and long-term monitoring of stroke progression in real time, including real-time assessment of ischemia and blood flow reperfusion, and tissue damage and its degree of recovery. A systematic review is made on the development of OCTA technology and OAC algorithm, and the progress of related stroke research is further introduced. The abovementioned OCT technology has important application value in the field of biomedicine.
Correlation of Optical Attenuation Coefficient Estimated Using Optical Coherence Tomography with Changes in Astrocytes and Neurons in a Chronic Photothrombosis Stroke Model

Abstract:The optical attenuation coefficient (OAC) estimated using optical coherence tomography (OAC-OCT) offers a label-free 3D mapping of tissue infarction, but the physiological origin of the OAC contrast remains unclear. For effectively suppressing OAC fluctuations, we propose a hybrid (wavelength/angle) division multiplexing (HDM) method, which improved the OAC contrast by 70.7% in tissue phantoms. To test the feasibility of OAC-based infarction detection, triphenyltetrazolium chloride (TTC) staining was performed on fresh ex vivo brain slices, and the TTC-defined infarction was used as the ground truth. Sharp OAC contrast was observed between the TTC-defined infarction (1.09 mm-1) and normal tissue (0.79 mm-1). The OAC infarction spatially matched well with the TTC-defined infarction. To further explore the physiological origin of OAC contrast in ischemic stroke at the cellular level, the dynamic changes in OAC were measured in the rat cortex in vivo over 3 weeks after photothrombosis (PT) occlusion and found significantly correlated with the changes in astrocytes and neurons acquired with ex vivo hematoxylin and eosin (HE), glial fibrillary acidic protein (GFAP), and NeuN staining. These results suggest that OAC imaging enables non-invasive infarction detection and its contrast might originate from the changes in astrocytes and neurons in the chronic PT stroke model. The cellular responses revealed by in vivo OAC imaging would be essential for evaluating treatments and even developing novel therapies.

Abstract:Visible-light optical coherence tomography (Vis-OCT) is an emerging technology that can image hemodynamic response in microvasculature. Vis-OCT can retrieve blood oxygen saturation (sO2) mapping using intrinsic optical absorption contrast while providing high-resolution anatomical vascular structures at the same time. To improve the accuracy of Vis-OCT oximetry on vessels embedded in highly scattering medium, i.e., brain cortex, we developed and formulated a novel dual-depth sampling and normalization strategy that allowed us to minimize the detrimental effect of ubiquitous tissue scattering. We applied our newly developed approach to monitor the hemodynamic response in mouse cortex after focal photothrombosis. We observed vessel dilatation, which was negatively correlated with the original vessel diameter, in the penumbra region. The sO2 of vessels in the penumbra region also dropped below normal range after focal ischemia.

Abstract:Monitoring hemodynamic responses in the rodent stroke model is important in the study of ischemic stroke. However, it is hard for a single imaging modality to comprehensively explore these changes. In this study, we implemented laser speckle imaging (LSI) and visible-light optical coherence tomography (Vis-OCT) to detect cerebral blood flow (CBF), angiography and oxygen saturation (sO 2 ) in the mouse model of distal middle cerebral artery occlusion (dMCAO). LSI provides full-field, real-time cerebral blood flow (CBF) map to guide the Vis-OCT imaging. Vis-OCT offers angiography and sO 2 measurements. The results showed the detailed CBF and vasculature changes before and after dMCAO. After dMCAO, sO 2 changed little in arteries and arterioles, while sO 2 decreased in veins and venules. We also showed that larger veins had a higher decrease in sO 2 value in consecutive vein branches after dMCAO. It is suggested that the combination of LSI/Vis-OCT provides a promising tool for the rodent stroke model study.

Abstract:Monitoring cortical hemodynamic response after ischemic stroke (IS) is essential for understanding the pathophysiological mechanisms behind IS-induced neuron loss. Functional optical coherence tomography (OCT) is an emerging technology that can fulfill the requirement, providing label-free, high-resolution 3D images of cerebral hemodynamics. Unfortunately, strong tissue scattering pose a significant challenge for existing OCT oximetry techniques, as they either ignore the effect or compensate it numerically. Here we developed a novel dual-depth sampling and normalization strategy using visible-light OCT (vis-OCT) angiograms that can provide robust and precise sO2 estimations within cerebral circulation. The related theoretical formulation were established, and its implication and limitations were discussed. We monitored mouse cortical hemodynamics using the newly-developed method. Focal ischemic stroke was induced through photothrombosis. The analysis on pre- and post-IS vis-OCT images revealed both vascular morphology and oxygenation altered substantially after the occlusion. First, the ischemic core could be clearly identified as angiographic intensity fell below the detection limit. In addition, vessel dilation presented universally in the penumbra region. Notably for pial arteriles, the percentage of increase demonstrated inverse relationship with their pre-occlusion, pre-dilation dimeter. Vis-OCT oxygenation maps on intact cortex revealed spatial sO2 variations within pial vessels. Specifically, sO2 in arterioles decreased as it bifurcated and plunged into deeper tissue. Similarly, venous sO2 was higher in the larger, more superficial pial brunches. However, such difference was no longer appreciable after photothrombosis. Averaged arteriole sO2 dropped to 64% – 67% in the penumbra region.

Abstract:The occurrence and development of ischemic stroke are closely related to cerebral blood flow. Real-time monitoring of cerebral perfusion level is very useful for understanding the mechanisms of the disease. A wide field of view (FOV) is conducive to capturing lesions and observing the progression of the disease. In this paper, we attempt to monitor the whole-brain microcirculation in middle cerebral artery occlusion (MCAO) rats over time using a wide FOV swept-source OCT (SS-OCT) system. A constrained image registration algorithm is used to remove motion artifacts that are prone to occur in a wide FOV angiography. During ischemia, cerebral perfusion levels in the left and right hemispheres, as well as in the whole brain were quantified and compared. Changes in the shape and location of blood vessels were also recorded. The results showed that the trend in cerebral perfusion levels of both hemispheres was highly consistent during MCAO, and the position of the blood vessels varied over time. This work will provide new insights of ischemic stroke and is helpful to assess the effectiveness of potential treatment strategies.
Visualizing cortical blood perfusion after photothrombotic stroke in vivo by needle-shaped beam optical coherence tomography angiography

Abstract:Optical imaging techniques provide low-cost, non-radiative images with high spatiotemporal resolution, making them advantageous for long-term dynamic observation of blood perfusion in stroke research and other brain studies compared to non-optical methods. However, high-resolution imaging in optical microscopy fundamentally requires a tight optical focus, and thus a limited depth of field (DOF). Consequently, large-scale, non-stitched, high-resolution images of curved surfaces, like brains, are difficult to acquire without z-axis scanning. To overcome this limitation, we developed a needle-shaped beam optical coherence tomography angiography (NB-OCTA) system, and for the first time, achieved a volumetric resolution of less than 8 μm in a non-stitched volume space of 6.4 mm × 4 mm × 620 μm in vivo. This system captures the distribution of blood vessels at 3.4-times larger depths than normal OCTA equipped with a Gaussian beam (GB-OCTA). We then employed NB-OCTA to perform long-term observation of cortical blood perfusion after stroke in vivo, and quantitatively analyzed the vessel area density (VAD) and the diameters of representative vessels in different regions over 10 days, revealing different spatiotemporal dynamics in the acute, sub-acute and chronic phase of post-ischemic revascularization. Benefiting from our NB-OCTA, we revealed that the recovery process is not only the result of spontaneous reperfusion, but also the formation of new vessels. This study provides visual and mechanistic insights into strokes and helps to deepen our understanding of the spontaneous response of brain after stroke.

Longitudinal monitoring of blood perfusion and brain tissue damage in photothrombotic ischemic stroke rat model

Abstract:The brain experiences alterations including cerebral ischemia and tissue damage after focal ischemic stroke. A thorough understanding of the spatiotemporal dynamics of blood perfusion and tissue damage is of great importance in stroke research. In chronic rat photothrombotic (PT) stroke model, a parallel study of both vascular and cellular responses to regional ischemia was performed with optical coherence tomography (OCT) in a label-free and depth-resolved manner. OCT revealed that vessels of different types and depth presented various spatial and temporal dynamics. In the ischemic core area, the distal middle cerebral arteries (dMCAs) were blocked gradually with laser irradiation and a spontaneous recanalization was observed at Day 5. In the chronic recovery period, the blocked small pial microvessels presented an apparent neovascularization progressing from the peripheral into the core area, with the final blood flow volume exceeding the baseline before PT. While the cortical capillary perfusion of the core area totally disappeared at Day 3 after PT and never recovered in the core area till the end of observation. The results demonstrated that blood reperfusion mainly occurred in the dMCAs vessels and pial microvessels of the superficial layer, but not in capillaries located deep in the cortex. The response of the cellular scattering and tissue damage showed a high spatial and temporal correlation with the capillary perfusion. On the whole, ischemic area and lesion area from attenuation coefficient are not exactly the same but complentary, with great help in understanding stroke mechanism comprehensively.
Wearable optical coherence tomography angiography probe for monitoring mouse brain in vivo

Abstract:Optical coherence tomography (OCT) has become a promising tool for studying anatomical and functional dynamics of the cerebral cortex, offering advantages such as label-free imaging, high resolution, and non-invasive optical biopsy. However, observing the brains of non-anesthetized and freely moving mice has been a long-standing challenge for OCT. In this study, we designed a wearable OCT probe to observe the vascular morphology of the mouse brain and track shortterm vascular changes after thrombosis. We utilized a microelectromechanical system (MEMS) scanning mirror for three-dimensional scanning. Compared to traditional OCT systems, this wearable imaging probe features miniaturization, low cost, portability, and stability, allowing for imaging of the mouse brain in a non-anesthetized and freely moving state. The entire probe weighs 8 g and achieves a lateral resolution of 5.5 mu m, a longitudinal resolution of 12 mu m, and an effective imaging area of 4 mm x 4 mm. We evaluated the performance of the probe through phantom experiments and imaging of the mouse brain's vascular network, and successfully monitored local vascular morphological changes in the mouse brain shortly after stroke under awake conditions. We believe that the wearable probe can be applied in various fields such as ophthalmology, dermatology, and dentistry, and due to its portability and non-invasiveness, the wearable OCT probe is expected to have wide clinical research applications.
Deep optoacoustic localization microangiography of ischemic stroke in mice

Abstract:Super-resolution optoacoustic imaging of microvascular structures deep in mammalian tissues has so far been impeded by strong absorption from densely-packed red blood cells. Here we devised 5 µm biocompatible dichloromethane-based microdroplets exhibiting several orders of magnitude higher optical absorption than red blood cells at near-infrared wavelengths, thus enabling single-particle detection in vivo. We demonstrate non-invasive three-dimensional microangiography of the mouse brain beyond the acoustic diffraction limit (<20 µm resolution). Blood flow velocity quantification in microvascular networks and light fluence mapping was also accomplished. In mice affected by acute ischemic stroke, the multi-parametric multi-scale observations enabled by super-resolution and spectroscopic optoacoustic imaging revealed significant differences in microvascular density, flow and oxygen saturation in ipsi- and contra-lateral brain hemispheres. Given the sensitivity of optoacoustics to functional, metabolic and molecular events in living tissues, the new approach paves the way for non-invasive microscopic observations with unrivaled resolution, contrast and speed.

Abstract:Cerebral edema is a severe complication of acute ischemic stroke with high mortality but limited treatment. Although parameters such as brain water content and intracranial pressure may represent the global assessment of edema, optical properties can appear heterogeneously throughout the cerebral tissue relative to the site of injury. In this study, we have monitored the edema formation and progression in both permanent and transient middle cerebral artery occlusion models in rats. Edema was reflected by the decrease of optical attenuation coefficient (OAC) value in OCT system. By utilizing swept-source optical coherence tomography (SS-OCT), we found that in photochemically induced permanent focal stroke model, both the edema size and edema index, steadily developed until the end of monitor (7h). Comparatively, when transient ischemia was introduced with endothelin-1 (ET-1), the edema was detected as early as 15min, and began to recover after 30min until monitor was finished (3h). Despite the majority of the edema being recovered to some extent, the condition of a small region within the edema kept deteriorating, presumably due to the reperfusion damage which might result in serious clinical outcomes. Our study has compared the edema characteristics from two different acute ischemic stroke situations. This work not only confirms the capability of OCT to temporal and spatial monitor of edema but is also able to locate focal conditions at some areas that might highly determine the prognosis and treatment decisions.
Real-time imaging of mouse lenticulostriate artery following brain ischemia.

Abstract:Detection of microvascular changes in experimental stroke models is limited by current technologies. Using state-of-the-art synchrotron radiation (SR), we explored the feasibility of detecting the normal morphological variations of lenticulostriate arteries (LSAs) and the changes to LSAs following middle cerebral artery occlusion (MCAO). Cerebral microvessels of ICR mice were imaged with synchrotron radiation microangiography using nonionic iodine and barium sulfate as contrast agents. Using SR we reproducibly observed the detailed cerebral microvasculature of LSAs arising from the origin of middle cerebral artery (MCA) with a resolution of approximately 5 micrometers, at least a 20-fold greater resolution compared to CT or MRI imaging. Notably, SR microangiography was able to reveal ischemia/reperfusion induced leakage in the lenticulostriate artery territory. To our knowledge this is the first time that the three-dimensional morphology of LSAs and real time visualization of LSA hemorrhage have been characterized in live mice. This work demonstrates that SR microangiography can provide a unique tool for furthering experimental stroke research to examine the efficacy of neuroprotective therapies on parameters such as angiogenesis and vascular integrity.
In vivo imaging of hemodynamics and oxygen metabolism in acute focal cerebral ischemic rats with laser speckle imaging and functional photoacoustic microscopy

Abstract:Stroke is a devastating disease. The changes in cerebral hemodynamics and oxygen metabolism associated with stroke play an important role in pathophysiology study. But the changes were difficult to describe with a single imaging modality. Here the changes in cerebral blood flow (CBF), cerebral blood volume (CBV), and oxygen saturation (SO2) were yielded with laser speckle imaging (LSI) and photoacoustic microscopy (PAM) during and after 3-h acute focal ischemic rats. These hemodynamic measures were further synthesized to deduce the changes in oxygen extraction fraction (OEF). The results indicate that all the hemodynamics except CBV had rapid declines within 40-min occlusion of middle cerebral artery (MCAO). CBV in arteries and veins first increased to the maximum value of 112.42 ± 36.69% and 130.58 ± 31.01% by 15 min MCAO; then all the hemodynamics had a persistent reduction with small fluctuations during the ischemic. When ischemia lasted for 3 h, CBF in arteries, veins decreased to 17 ± 14.65%, 24.52 ± 20.66%, respectively, CBV dropped to 62 ± 18.56% and 59 ± 18.48%. And the absolute SO2 decreased by 40.52 ± 22.42% and 54.24 ± 11.77%. After 180-min MCAO, the changes in hemodynamics and oxygen metabolism were also quantified. The study suggested that combining LSI and PAM provides an attractive approach for stroke detection in small animal studies.

Abstract:The study of hemodynamic and vascular changes following ischemic stroke is of great importance in the understanding of physiological and pathological processes during the thrombus formation. The photothrombosis model is preferred by researchers in stroke study for its minimal invasiveness, controllable infarct volume and lesion location. Nevertheless, there is a lack in high spatiotemporal resolution techniques for real time monitoring of cerebral blood flow (CBF) changes in 2D-profile. In this study, we implemented a microscopic laser speckle imaging (LSI) system to detect CBF and other vascular changes in the rodent model of photothrombotic stroke. Using a high resolution and high speed CCD (640 x 480 pixels, 60 fps), online image registration technique, and automatic parabolic curve fitting, we obtained real time CBF and blood velocity profile (BVP) changes in cortical vessels. Real time CBF and BVP monitoring has been shown to reveal details of vascular disturbances and the stages of blood coagulation in photothrombotic stroke. Moreover, LSI also provides information on additional parameters including vessel morphologic size, blood flow centerline velocity and CBF spatiotemporal fluctuations, which are very important for understanding the physiology and neurovascular pathology in the photothrombosis model. Real-time cerebral blood flow (CBF) monitoring during stroke onset can provide important information for the prediction of brain injury; however, this is difficult to achieve in clinical practice due to various technical problems. We created a photothrombotic focal ischemic stroke model utilizing our self-developed miniature headstage in conscious and freely moving rats. In this model, a high spatiotemporal resolution imager using laser speckle contrast imaging technology was integrated to acquire real-time two-dimensional CBF information during thrombosis. The feasibility, stability, and reliability of the system were tested in terms of CBF, behavior, and T2-weighted magnetic resonance imaging (MRI) findings. After completion of occlusion, the CBF in the targeted cortex of the stroke group was reduced to 16 +/- 9% of the baseline value. The mean infarct volume measured by MRI 24 h postmodeling was 77 +/- 11 mm(3) and correlated well with CBF (R-2 = 0.74). This rodent model of focal cerebral ischemia and real-time blood flow imaging opens the possibility of performing various fundamental and translational studies on stroke without the influence of anesthetics. Abstract:Doppler optical coherence tomography (DOCT) provides a novel method to measure blood flow velocity in vessels with diameter at micrometer scale. In this study, a developed spectral domain DOCT system is applied to monitor cerebral blood flow velocity changes in a rat. An animal model with a cranial window is used, and by application of a drug, light, and electric stimulations, changes in blood flow velocity of the pial artery in sensory cortex are measured in real time. The results show significant differences in blood flow velocity before and after drug administration or light and electric stimulations, demonstrating the feasibility of DOCT in cerebral microcirculation study. Given its noninvasive nature, high spatial resolution, high velocity sensitivity, and high imaging speed, DOCT shows great promise in brain research by imaging blood flow changes at micrometer scale vessels, which helps to understand the pathogenesis of cerebral diseases and neurodegenerative diseases.

Abstract:Animal models of stroke are used extensively to study the mechanisms involved in the acute and chronic phases of recovery following stroke. A translatable animal model that closely mimics the mechanisms of a human stroke is essential in understanding recovery processes as well as developing therapies that improve functional outcomes. We describe a photo-thrombosis stroke model that is capable of targeting a single distal pial branch of the middle cerebral artery with minimal damage to the surrounding parenchyma in awake head-fixed mice. Mice are implanted with chronic cranial windows above one hemisphere of the brain that allow optical access to study recovery mechanisms for over a month following occlusion. Additionally, we study the effect of laser spot size used for occlusion and demonstrate that a spot size with small axial and lateral resolution has the advantage of minimizing unwanted photodamage while still monitoring macroscopic changes to cerebral blood flow during photothrombosis. We show that temporally guiding illumination using real-time feedback of blood flow dynamics also minimized unwanted photodamage to the vascular network. Finally, through quantifiable behavior deficits and chronic imaging we show that this model can be used to study recovery mechanisms or the effects of therapeutics longitudinally. (C) The Authors.
