Abstract:In recent years, the term 'chronic alcoholism' has had a meaning that is more descriptive than diagnostic. Several subtypes of alcoholism have been established and are now a necessary tool for studying therapy outcome. Alcohol-dependent patients can be subtyped based on clearly assigned dimensions (e.g. biological, sociological and psychological disturbances). Craving and the underlying disturbance must be treated. The number of pharmacological agents that may reduce alcohol intake has increased recently. We conducted a prospective long-term study based on four subtypes of alcohol-dependent patients to assess the efficacy of acamprosate. Our findings demonstrate that these patient subtypes are relevant to outcome in trials of pharmacological agents. We strongly recommend subtyping alcohol-dependent subjects in future trials, because the usefulness of effective drugs could be overlooked when they are tested in a heterogeneous population.

Abstract:This chapter is concerned with a way of thinking about the relationship between alcohol and human beings. It is not a report of specific research, nor does it attempt systematically to review the literature. It offers some conceptual observations based on a perspective of change as perceived by a social-behavioral scientist. During the last 35 years, there have been significant changes in patterns of alcohol consumption, in the social and the chemical environments in which alcohol is used, in the functions and liabilities of alcohol, in our knowledge about alcohol and the human body, and in our perceptions of the meaning of this knowledge. Not all of these changes are recognized, understood or reflected in currently prevailing definitions of alcohol problems nor in social responses to these problems.

Abstract:Science Progress, Volume 107, Issue 2, April-June 2024. In a recent publication, we applied a novel model to address phenotypic heterogeneity in genetic research on alcohol misuse by stratifying individuals based on their patterns of alcohol use behaviours and comorbid psychopathology. In this Commentary, we provide further descriptions of the subtypes of alcohol misuse that emerged from the empirical mixture modelling approach and present new results comparing these groups on sociodemographic characteristics and additional alcohol use outcomes. We take a broad perspective to discuss how these results fit with existing typologies of alcohol misuse and how the results inform future genetic research. Our findings add further evidence that conceptualisations of a binary distinction between 'internalising' (relief-seeking) versus 'externalising' (reward-seeking) subtypes does not fully capture the complexity of alcohol misuse. However, accounting for individual differences in these dimensions is a promising means to reduce heterogeneity and thereby improve power for gene discovery and, eventually, personalised medicine applications. We argue that more detailed, person-specific assessment of alcohol misuse measures, particularly with attention to longitudinal trajectories, is needed to further advance this important line of research.

Abstract:Introduction Identifying subgroups of Temporary (alcohol) Abstinence Challenge (TAC) participants may offer opportunities to enhance intervention effectiveness. However, knowledge about such subgroups is missing. This study aimed to (i) describe a TAC population; (ii) identify subgroups of participants based on determinants of changes in drinking behaviour; and (iii) characterise subgroups in terms of sociodemographic and other characteristics. Methods Data from 3803 Dutch TAC participants were analysed to identify subgroups using three‐step Latent Class Analysis. Classes were based on determinants of changes in drinking behaviour (i.e., drinking refusal self‐efficacy, craving and behavioural automaticity) and were characterised by sociodemographic characteristics, drinking behaviour, previous participation in TACs, self‐reported health and life satisfaction. Results The majority of TAC participants were female, highly educated, employed, 53 years old on average, participated in previous TACs and reported relatively high alcohol use. Four classes of participants were identified: (i) 'ordinary drinkers' (49.0%); (ii) 'drinkers in control' (21.4%); (iii) 'habitual drinkers with perceived control to refuse' (18.4%); and (iv) 'drinkers not in control' (11.2%). Class 2 drank least often and non‐excessive volumes, while other classes typically drank 4 or more days per week and 3 to 4 glasses per drinking day, with the highest alcohol use found in class 4. Discussion and Conclusions Different configurations of determinants in this study's four subgroups may require different intervention approaches and might inform personalised support. Future research is needed to examine the predictive value of these subgroups on post‐challenge drinking behaviour to assess support needs and participation value.

Alcohol-Dependent Subjects Show Different Personality Traits Compared With Subjects With Multiple Substance Dependence: Preliminary Data

Abstract:Methods: We compared personality traits of 27 persons with multiple substance dependence with personality data of 52 alcohol-dependent persons regarding their personality traits and disorders (obtained by using SCID-II, TCI and NEO FFI). Both patient groups were free of any other mental disorder. Results: In SKD-II, we found significant differences in the male group in dependent and scizotypic personality disorder. There were no significant differences in the female group, but sample was very small. We also found significant differences between alcohol-dependent and multiple substance-dependent persons in extraversion and novelty seeking. Conclusions: We detected significant differences in personality disorders evaluated by SCID-II. Temperament and character items—as evaluated by NEO FFI and TCI—showed also significant differences in personality traits. Given the limited number of subjects, the data should be regarded as preliminary until replicated in a larger sample. Nevertheless, the findings may be of clinical relevance with respect to prognosis or individualized treatment. These findings should be treated with caution until replicated.

Abstract:In the Stockholm Adoption Study, two types of alcoholism, "Type I" and "Type II", have been identified on the basis of genetic predisposition. In the present study, this classification has been applied to a clinical sample. The two types of alcoholism were clinically clearly identifiable. Type I alcoholism was characterized by late onset and few social complications. Type II alcoholism was characterized by early onset, use and abuse not only of alcohol, but also of glue, cannabis, amphetamine and opioids, together with several social complications. The subjects with Type II alcoholism had also more alcoholism and depression among their first-degree relatives than the subjects with Type I alcoholism. Furthermore, the two types of alcoholism were separable by means of the biological marker - platelet MAO activity. While platelet MAO activity was normal in Type I alcoholics, as compared with healthy controls, it was clearly low in the Type II alcoholics. This subclassification of alcoholism seems to be of value in future studies concerning the etiology, epidemiology and treatment of alcoholism.

Abstract:This study examines the attitudes of 589 randomly chosen licensed California psychologists and marriage, family and child counselors toward alcoholism. Their anonymous replies revealed that the two professional groups were relatively homogeneous in nature. The majority of each subscribed to the disease concept of alcoholism, recognized the importance of abstinence in problem resolution and as a treatment goal, rejected insight as a prerequisite of sobriety, believed in a positive prognosis and were extremely willing to be involved in the treatment of alcoholics. Negative attitudes toward alcoholics were expressed with respect to attribution of responsibility and a persistence of stigmatization of the alcoholic.

Abstract:Bartholomew's (1990) four-category typology of adult attachment styles was compared with Hazan & Shaver's (1987) three-category typology in terms of three substantive issues. First, the same two dimensions were found to underlie both typologies, and the Bartholomew and Hazan & Shaver measures corresponded as predicted. Second, there were no gender differences on Hazan & Shaver's measure, in line with previous studies, but there were gender differences on Bartholomew's measure, especially in her two avoidant categories. More males than females were dismissing avoidants; more females than males were fearful avoidants. Third, a hypothesis advanced by Latty-Mann & Davis (1988) was confirmed. Adult children of alcoholics scored high on both avoidant and anxious-ambivalent scales of Hazan & Shaver's measure, and fell predominantly into Bartholomew's fearful-avoidant category, suggesting that at least some fearful adults are grown-up versions of the `disorganized, disoriented' children identified by Crittenden (1988) and by Main & Solomon (1990). These children are more common in families troubled by parental alcoholism, depression or abuse.

Abstract:Alcohol, the most consumed drug in the United States, is associated with various psychological disorders and abnormal personality traits. Despite extensive research on adolescent alcohol consumption, the impact of early alcohol sipping patterns on changes in personality and mental health over time remains unclear. There is also limited information on the latent trajectory of early alcohol sipping, beginning as young as 9-10 years old. The dorsal anterior cingulate cortex (dACC) is crucial for cognitive control and response inhibition. However, the role of dACC remains unclear in the relationship between early alcohol sipping and mental health outcomes and personality traits over time. Utilizing the large data from the Adolescent Brain and Cognitive Development study, we aim to comprehensively examine the longitudinal impact of early alcohol sipping patterns on psychopathological measures and personality traits in adolescents, filling crucial gaps in the literature. We identified three latent alcohol-sipping groups, which demonstrate distinct personality traits and depression score trajectories. Bilateral dACC activation during the stop-signal task moderated the effect of early alcohol sipping on personality and depression over time. Additionally, bidirectional effects were observed between alcohol sipping and personality traits. This study provides insights into the impact of early alcohol consumption on adolescent development.

Abstract:Background Precision medicine approaches aim to improve treatment outcomes by identifying which treatments work best for specific individual phenotypes. In the treatment of alcohol use disorder (AUD), precision medicine approaches have been proposed based on phenotypes characterized by individuals who drink primarily to enhance rewarding experiences (i.e., reward drinking) or those who drink primarily to relieve negative states (i.e., relief drinking). This study examined these phenotypes across treatment‐ and nontreatment‐seeking individuals and the stability of the phenotypes over time. Methods We used latent profile and latent transition analyses to identify and assess longitudinal stability (over 3 or 4 months) of reward and relief drinking subgroups within a nontreatment‐seeking community sample that engaged in hazardous drinking (n = 189) and two treatment‐seeking samples of individuals with AUD enrolled in two large clinical trials (n = 1726, n = 1383). We examined prospective associations with alcohol consumption and consequences at long‐term follow‐up (15 or 18 months). Results Results supported four subgroups: low reward/low relief, low reward/high relief, high reward/low relief, and high reward/high relief. The community sample contained more individuals classified within the high reward/low relief subgroup than treatment‐seeking samples. Subgroups were generally more stable over time in the community sample than in the treatment‐seeking samples. Alcohol consumption and consequences decreased over time for the treatment‐seeking samples, with consequences and drinking frequency decreasing for the community sample. Participants classified within the high reward/high relief and low reward/high relief groups reported the most consequences and consumption at long‐term follow‐up. Conclusion Reward and relief drinking phenotypes can be identified within community and treatment‐seeking samples of individuals who drink heavily. The phenotypic subgroups appear to be stable over time in the absence of treatment, change somewhat during treatment, and provide utility in predicting alcohol consumption and consequences.

Abstract:The assessment, diagnosis, and classification of mental disorders are embedded in social and cultural norms. In view of their Anglo-Saxon origins, the prevailing diagnostic criteria and instruments for their assessment have a strong Western influence. Yet they are used internationally with the implied assumption of their cross-cultural applicability. The WHO Cross-Cultural Applicability Research (CAR) study was designed to test this assumption as it applies to disorders relating to the use of alcohol and drugs. This multi-disciplinary research project was conducted in nine countries having different patterns of alcohol and drug use. The results suggest that, even though some similarities exist with respect to the definition of problematic use of alcohol in these ethnically diverse societies, very substantial differences also exist. A number of core concepts underpinning diagnosis of disorders relating to the use of alcohol have no equivalence in the local languages of the various cultures, while some others lacked cultural applicability because of their relative 'distance' from cultural and ethnic norms of drinking. This distance often relates to the difficulties of adapting descriptors of drinking norms in a 'wet' culture to one that is decidedly 'dry'.

Abstract:Finding the vulnerable minority “Only” about 10 to 15% of people exposed to alcohol develop alcohol-related problems. The behavioral repertoire of people confronted with opportunities to consume alcohol involves numerous choices between this drug reward and healthy alternatives. Augier et al. established a choice procedure that begins to address alcohol addiction in rats (see the Perspective by Spanagel). They found that a minority of outbred rats continued to self-administer alcohol even when a high-value alternative (such as sugar) was available. That minority displayed a remarkable constellation of behavioral traits resembling the human clinical condition, including a high motivation to obtain alcohol and continued use despite adverse consequences. The cause was impaired GABA (γ-aminobutyric acid) clearance in the central amygdala. Postmortem tissue analysis supported the possibility of a similar pathology in human alcoholism. Cluster analysis of patients with alcohol use disorder featuring alexithymia, depression, and diverse drinking behavior

Abstract:Aim: This study aimed to identify subgroups of alcohol use disorder (AUD) based on a multidimensional combination of alexithymia, depression, and diverse drinking behavior. Method: We recruited 176 patients with AUD, which were initially divided into non-alexithymic (n = 130) and alexithymic (n = 46) groups using a cutoff score of 61 on the Toronto Alexithymia Scale (TAS-20). Subsequently, the profiles of the two groups were compared. Thereafter, a two-stage cluster analysis using hierarchical and K-means methods was performed with the Z-scores from the TAS-20, the Quick Inventory of Depressive Symptomatology Self-Report Japanese Version, the 12-item questionnaire for quantitative assessment of depressive mixed state, and the 20-item questionnaire for drinking behavior pattern. Results: In the first analysis, Alexithymic patients with AUD showed greater depressive symptoms and more pathological drinking behavior patterns than those without alexithymia. Cluster analysis featuring alexithymia, depression, and drinking behavior identified three subtypes: Cluster 1 (core AUD type) manifesting pathological drinking behavior highlighting automaticity; Cluster 2 (late-onset type) showing relatively late-onset alcohol use and fewer depressive symptoms or pathological drinking behavior; and Cluster 3 (alexithymic type) characterized by alexithymia, depression, and pathological drinking behavior featuring greater coping with negative affect. Conclusion: The multidimensional model with alexithymia, depression, and diverse drinking behavior provided possible practical classification of AUD. The alexithymic subtype may require more caution, and additional support for negative affect may be necessary due to accompanying mood problems and various maladaptive drinking behaviors.
A new perspective on European drinking cultures: A model-based approach to determine variations in drinking practices across 19 European countries.

Abstract:BACKGROUND AND AIMS In recent decades, alcohol drinking in the European Union has been characterized by increasing homogenization of levels of drinking coupled with an overall decrease. This study examined whether we can still distinguish distinct practices of drinking by addressing two research questions: (1) Are drinking practices still characterised by the choice of a certain alcoholic beverage? (2) How do drinking practices vary across countries? DESIGN Cross-sectional study: Latent-class analyses of drinking variables and fractional response regression analyses of individual characteristics for individual-level class endorsement probabilities, respectively. SETTING 19 European countries and one autonomous community. PARTICIPANTS 27,170 past-year drinkers aged 18 to 65 years in 2015. MEASUREMENTS Data collected through the Standardized European Alcohol Survey included frequency of past-year drinking, pure alcohol intake per drink day, occurrence of monthly risky single-occasion drinking, and preferred beverage, along with sociodemographic data. FINDINGS Three latent classes were identified: 1) light to moderate drinking without risky single-occasion drinking (prevalence: 68.0%, 95% confidence interval [CI]: 66.7 - 69.3), 2) infrequent heavy drinking (prevalence: 12.6%, 95% CI: 11.5 - 13.7) and 3) regular drinking with at least monthly risky single-occasion drinking (prevalence: 19.4%, 95% CI: 18.1 - 20.9). Drinking classes differed considerably in beverage preference, with women reporting a generally higher share of wine and men of beer drinking. Light to moderate drinking without risky single-occasion drinking was the predominant drinking practice in all locations, except for Lithuania where infrequent heavy drinking (Class 2) was equally popular. Sociodemographic factors and individual alcohol harm experiences (Rapid Alcohol Online Screen) explained up to 20.5% of the variability in class endorsement. CONCLUSIONS Beverage preference appears to remain a decisive indicator for distinguishing Europeans' drinking practices. In most European countries, multiple drinking practices appear to be present.

Abstract:Background Impairments in executive function and social cognition are highly prevalent in individuals with an alcohol use disorder (AUD). Some studies show that similar difficulties are displayed by individuals with a positive family history of AUD (FH+) compared with individuals with a negative family history (FH‐). Yet, no studies have jointly investigated cognitive and affective theory of mind at the behavioral level. Moreover, some studies show preserved executive and socioemotional functioning in FH+ participants. One possible explanation for these divergent results is that FH+ individuals are cognitively heterogeneous. In this study, we examined the frequency and co‐occurrence of difficulties in executive function and social cognition among FH+ individuals at the individual level. Methods Sixty FH+ and 60 FH− participants matched on age, sex, and education level were included. They completed tasks assessing executive functions (Stroop, Trail Making Test) and affective and cognitive theory of mind (Movie for the Assessment of Social Cognition). They also completed self‐report questionnaires measuring impulsivity, alexithymia, and empathy. Single‐case analyses assessed the proportion of FH+ participants with difficulties in executive function and/or theory of mind. Results FH+ individuals exhibited difficulties in response inhibition and made more errors during theory of mind processing, indicating an absence of mental state representation, compared with FH− individuals. In the FH+ sample, 53.33% had executive function and/or theory of mind difficulties. Those with lower theory of mind scores reported higher alexithymia and lower empathy on self‐report measures. Conclusions FH+ individuals display heterogeneous executive function and theory of mind abilities. Given that they mostly occur independently of one another, executive function and theory of mind difficulties may be distinct vulnerability markers in AUD.

Abstract:This online survey study explored how Australian laypeople ( N = 147) define alcohol intoxication using language, standard drinks, blood alcohol concentration (BAC), and symptoms. Participants used an extensive vocabulary to describe intoxication and better understood intoxication in terms of standard drinks and symptoms compared to BAC. Lay intoxication definitions and perceived alcohol-induced impairment thresholds (memory and capacity for consent) were influenced by personal characteristics (e.g. age, personal alcohol consumption). Participants rated symptom-based evidence as most useful when evaluating a person's intoxication status in a legal setting and welcomed expert evidence. Findings can inform litigation and educational strategies that facilitate accurate engagement with alcohol intoxication in the courtroom.

Abstract:Background Although the link between alcohol involvement and behavioral phenotypes (e.g. impulsivity, negative affect, executive function [EF]) is well-established, the directionality of these associations, specificity to stages of alcohol involvement, and extent of shared genetic liability remain unclear. We estimate longitudinal associations between transitions among alcohol milestones, behavioral phenotypes, and indices of genetic risk. Methods Data came from the Collaborative Study on the Genetics of Alcoholism (n = 3681; ages 11–36). Alcohol transitions (first: drink, intoxication, alcohol use disorder [AUD] symptom, AUD diagnosis), internalizing, and externalizing phenotypes came from the Semi-Structured Assessment for the Genetics of Alcoholism. EF was measured with the Tower of London and Visual Span Tasks. Polygenic scores (PGS) were computed for alcohol-related and behavioral phenotypes. Cox models estimated associations among PGS, behavior, and alcohol milestones. Results Externalizing phenotypes (e.g. conduct disorder symptoms) were associated with future initiation and drinking problems (hazard ratio (HR)⩾1.16). Internalizing (e.g. social anxiety) was associated with hazards for progression from first drink to severe AUD (HR⩾1.55). Initiation and AUD were associated with increased hazards for later depressive symptoms and suicidal ideation (HR⩾1.38), and initiation was associated with increased hazards for future conduct symptoms (HR = 1.60). EF was not associated with alcohol transitions. Drinks per week PGS was linked with increased hazards for alcohol transitions (HR⩾1.06). Problematic alcohol use PGS increased hazards for suicidal ideation (HR = 1.20). Conclusions Behavioral markers of addiction vulnerability precede and follow alcohol transitions, highlighting dynamic, bidirectional relationships between behavior and emerging addiction.

Abstract:A comprehensive account of the causes of alcohol misuse must accommodate individual differences in biology, psychology and environment, and must disentangle cause and effect. Animal models can demonstrate the effects of neurotoxic substances; however, they provide limited insight into the psycho-social and higher cognitive factors involved in the initiation of substance use and progression to misuse. One can search for pre-existing risk factors by testing for endophenotypic biomarkers in non-using relatives; however, these relatives may have personality or neural resilience factors that protect them from developing dependence. A longitudinal study has potential to identify predictors of adolescent substance misuse, particularly if it can incorporate a wide range of potential causal factors, both proximal and distal, and their influence on numerous social, psychological and biological mechanisms. Here we apply machine learning to a wide range of data from a large sample of adolescents (n = 692) to generate models of current and future adolescent alcohol misuse that incorporate brain structure and function, individual personality and cognitive differences, environmental factors (including gestational cigarette and alcohol exposure), life experiences, and candidate genes. These models were accurate and generalized to novel data, and point to life experiences, neurobiological differences and personality as important antecedents of binge drinking. By identifying the vulnerability factors underlying individual differences in alcohol misuse, these models shed light on the aetiology of alcohol misuse and suggest targets for prevention.

Abstract:High adolescent alcohol consumption is predictive for alcohol problems later in life. To tailor interventions, early identification of risk groups for adolescent alcohol consumption is important. The IMAGEN dataset was utilized to investigate predictors for problematic alcohol consumption at age 18-20 years as a function self and parental personality and drug-related measures as well as life-events and cognitive variables all assessed at age 14 years (N = 1404). For this purpose the binary partitioning algorithm ctree was used in an explorative analysis. The algorithm recursively selects significant input variables and splits the outcome variable based on these, yielding a conditional inference tree. Four significant split variables, namely Place of residence, the Disorganization subscale of the Temperament and Character Inventory, Sex, and the Sexuality subscale of the life-events questionnaire were found to distinguish between adolescents scoring high or low on the Alcohol Use Disorders Identification Test about five years later (all p < 0.001). The analyis adds to the literature on predictors of adolescent drinking problems using a large European sample. The identified split variables could easily be collected in community samples. If their validity is proven in independent samples, they could facilitate intervention studies in the field of adolescent alcohol prevention.
