Abstract:Radiation-induced dermatitis (RID) is a class effect associated with radiotherapy for breast cancer. Anecdotal use of topical EGCG solution has been reported in patients with breast cancer. We aimed to assess topical EGCG solution for prevention of RID in patients with breast cancer. This study enrolled women aged 18 years or older with postmastectomy radiotherapy who had histologically confirmed breast cancer and completed adjuvant chemotherapy. Participants were randomly assigned (2:1) to receive either topical EGCG solution (660umol/L) or placebo (0.9% NaCl Normal Saline) until two weeks after radiation completion. The primary endpoint was incidence of grade 2 or worse RID between EGCG group and placebo group. The second endpoints included: the rate of grade 2 or worse radiation-induced discomforts; RID and symptom severity; RID index (RIDI, designed to account for not only the maximum grade, but also for the duration of RID) and symptoms scores (pain, burning-feeling, itching, pulling-feeling and tenderness) index throughout the treatment. RID was evaluated by investigator according to the Radiation Therapy Oncology Group (RTOG) scale weekly. Symptoms related to RID were measured by patients using the Skin Toxicity Assessment Tool. Safety assessments were done according to Common Terminology Criteria for Adverse Events, version 4.0. The objective was to demonstrate a 22.3% decrease in that incidence of grade ≥2 toxicity. Assuming a 10% attrition rate, to detect this difference with a power of 0.80 using a two-sided test at significance level 0.05, it was necessary to recruit 180 patients. This trial is registered at ClinicalTrials.gov, number NCT02580279. Between November 2014 and June 2019, 165 were evaluable for efficacy finally. Median radiation dose of both groups was 50Gy. The occurrence of grade≥2 RID was significantly lower (50.5% v 72.2%; P = 0.008) with the use of EGCG than with placebo. The maximum scores of RID was also significantly lower (P = 0.014). The maximum and grade≥2 discomforts in radiotherapy volumes of patients were significantly lower for the EGCG treatment versus placebo except pulling-feeling score (All P<0.05). The mean RIDI of patients in EGCG was 5.22 ± 1.60, compared with 6.21± 1.56 (T = -3,79, P<0.001) in placebo group. The differences were statistically apparent in all symptom index comparisons (All P<0.05). Overall, 118 (71.5%) of 165 patients had at least one adverse event. The most frequently reported grade 3 and 4 adverse events in the safety set were leukopenia (deemed prior chemotherapy or radiation related). 4 patients had adverse events definitely or suspected to be related to EGCG treatment, with pricking skin sensation (three [2.7%]) and application site pruritus (one [0.9%]). Prophylactic use of EGCG solution substantially reduced the incidence and severity of RID in patients receiving adjuvant radiotherapy and could be a new standard of skin care for patients receiving adjuvant radiotherapy.
