Abstract:Background: Based on CT image data, a computational fluid dynamics (CFD) model of the aortic arch was established. We aimed to investigate the hemodynamic features associated with end-to-side anastomosis (ESA) surgery for coarctation of the aorta (CoA) by CFD model.Methods: The data of enhanced CT two-dimensional medical images obtained through clinical practice were processed using medical image post-processing software. The three-dimensional model of the aortic arch was obtained through the geometric model and boundary condition. This was subsequently transformed into a CAD model, which can be used for simulation calculation.Results: The CFD model accurately reflected the shape of the aortic arch, and produced the hemodynamic results before and after ESA for CoA.Conclusions: The CFD model provides a virtual execution platform for the scientific research of aortic arch disease and will be helpful to evaluate the operation plan, even to determine the best surgical procedure. Hemodynamic analysis may be helpful to evaluate the therapeutic effects of other aortic diseases.

Abstract:A three-dimensional and three-component velocity measurement on the flow field in a 45° end-to-side anastomosis model is conducted to investigate the hemodynamics, which is an important factor to the intimal hyperplasia formation and graft failure after surgery. Thanks to the advanced volumetric measurement technology of tomographic particle image velocimetry, the recirculation zone, low-speed region, and the spiral flow structures can be visualized. As a result, the flow field of three cases with the local maximum velocity of 0.15, 0.8, and 1.4 m/s are visible and the inlet velocity profile tends to be skewed as the flow rate increases. The mean vorticity contours indicate that the positive vortex center rotates 6.47°, 50.23°, and 90.4° and the negative vortex center rotates 20.44°, 15.73°, and 68.47°, respectively, in three cases. The instantaneous vortex structures identified by the λci criterion demonstrate two large-scale vortex structures in the distal section. The two vortices have the tendency to intertwine while one of them decays earlier. The wall shear stress (WSS) distributions on the entire model with the local maximum of 0.8, 5.8, and 13.8 Pa in three cases have been quantitatively achieved. The abnormal WSS and WSS gradient can help localize risk areas and understand the intimal hyperplasia formation. A detailed illustration of hemodynamics inside the 45° end-to-side anastomosis model has been provided, which demonstrates more comprehensive large-scale flow structures and abnormal WSS regions. Combined with the information of flow structures and WSS distribution, the understanding of the hemodynamics in the anastomosis can be strengthened.

Abstract:This article has been retracted: please see Elsevier Policy on Article Withdrawal (https://www.elsevier.com/locate/withdrawalpolicy). This article has been retracted at the request of the authors, after they reported methodological errors that compromise the accuracy of the results and the conclusion drawn from their findings. Specifically, the authors have reported an improvement in the accuracy of fractional flow reserve (FFR) estimation by improving the accuracy of coronary blood flow calculation in the resting state. This was later identified by the authors to be due to an error in their methodology when setting the CTA image grayscale threshold to 100 in the software MIMICS, which was reported to the journal. This value is too large, resulting in the coronary branches model with sizes between 1 and 1.7 mm not being displayed, and thus, overestimating the accuracy of the FFR estimation. Further evaluation of the method with a more appropriate parameter value led to a decrease in the accuracy of their FFR predictions from 91.74 % to 77.69 %, which is lower than Taylor's method (82.64 %). This contradicted the conclusion drawn from the paper. The Editors understand the significance of retracting a published paper and its potential impact on the authors, the readership, and the scientific community. Abstract:Coarctation of the Aorta (CoA) is one of the most serious congenital heart defects in newborns. Approximately 30% of cases of CoA associated with aortic arch hypoplasia (AAH). The end-to-side aortic anastomosis (ESAA) is an effective surgical procedure to be performed for correction of CoA with AAH. In this study, the approach of computational fluid dynamics (CFD) was used to investigate the hemodynamics in patient-specific models of CoA with aortic arch hypoplasia before and after the procedure of ESAA. The results showed that the pressure drop and energy loss (EL) were significantly decreased after the surgical correction was done. Uniformed velocity profile and blood flow distribution were surgically created for patient's recovery of cardiac function. The approach of CFD can be used to disclose and evaluate the hemodynamics of CoA and its correction procedure.

Abstract:ObjectivesWe sought to develop a hemodynamic model of aortic and ductal arches using computational fluid dynamics (CFD) and 3D/4D spatio-temporal image correlation (STIC) fetal echocardiography and to investigate the hemodynamics of coarctation of aorta (CoA) in human fetuses using this approach. MethodsWe obtained 3D/4D STIC fetal echocardiographic images of the aortic and ductal arches (DA) in five normal fetuses. Based on these images, we simulated the hemodynamics in the two arches using CFD. Subsequently, we reduced the dimensions of aortic isthmus from 100% to 85%, 70%, 55%, 40%, and 25% of the original dimension digitally. Numerical simulation was repeated in each condition, and flow profile, velocity, pressure, and wall shear stress (WSS) were compared with those of the baseline normal aortic and ductal arches. ResultsWith the progressive narrowing in the aortic isthmus, there were alterations in the flow profile, velocity, pressure, and WSS. The downstream vortexes disappeared, and the double helix profile became single helix. When the aortic isthmus reduced by 55% in dimension, there was an exponential increase in velocity and WSS and decrease in pressure. ConclusionsThe aortic and ductal arch geometry and flow lead to the alterations in flow profile, velocity, pressure, and WSS in the aortic isthmus in normal and CoA models, which are conductive of ductal issue migration into these areas. A 55% reduction in the dimension of aortic isthmus is associated with exponential change in velocity, pressure, and WSS, a probable threshold for hemodynamically significant CoA.

Abstract:Coarctation of the aorta (CoA) is one of the most common congenital cardiac anomalies that could be corrected by surgery. However, the effects of surgical anastomosis on aortic arch flow are still unknown. In this study, a unique three-dimensional way was utilized to measure the growth of anastomotic site and computational fluid dynamics (CFD) was applied to investigate the hemodynamic effects of the growth of anastomosis site on patient-specific aortic arch flow before, 1 year and 2 years after surgery. The volume of anastomotic site, distribution of total pressure, wall shear stress (WSS), streamlines, energy efficiency and flow distribution ratio were calculated. The results indicated that the volume of anastomotic site has grown at 2-year follow-up. The total pressure and WSS were more uniformly distributed, whereas the three branches of aortic arch showed varying degrees of hypoperfusion. Energy efficiency increased obviously as the growth of anastomotic site. Our results implied that the numerical methods can be applied to evaluate the long-term effects of anastomosis site growth on aortic arch flow.
