Abstract:Preclinical and clinical studies have shown that prior receipt of radiotherapy enhances antitumor immune responses, a phenomenon we call the "radio-memory effect." However, all of the evidence regarding this effect to date comes from work with PD1/PDL1 inhibitors. Here we explored whether this effect also occurs with other forms of immune therapy, specifically interleukin-2 (IL-2). We retrospectively assessed outcomes in patients with malignant pleural effusion (MPE) who had previously received radiotherapy for non-small-cell lung cancer (NSCLC) within 18 months before the intrapleural infusion of IL-2 or cisplatin. Radiotherapy sites included lungs, thoracic lymph nodes, and intracranial. All patients received intrapleural infusion of IL-2 or cisplatin, and most had had several cycles of standard chemotherapy for NSCLC. We identified 3,747 patients with MPE (median age 64 years [range 29-88)) treated at one of several institutions from August 2009 through February 2015; 642 patients had been treated with IL-2 and 1102 with cisplatin and had survived for at least 6 months afterward. Among those who received IL-2, 288 had no radiotherapy, 324 had extracranial (i. e., thoracic) radiotherapy, and 36 had intracranial radiotherapy. The median follow-up time for surviving patients was 38 months. Patients who had received extracranial radiotherapy followed by IL-2 had significantly longer PFS than patients who had not received extracranial radiotherapy (i. e., either no radiotherapy or intracranial radiotherapy). Patients who had received intracranial or extracranial radiotherapy followed by IL-2 had significantly longer OS than did other patients. No survival advantage was noted for prior radiotherapy among patients who received intrapleural cisplatin. We speculate that previous radiotherapy could enhance the efficacy of subsequent intrapleural infusion of IL-2, a " radio-memory" effect that could be beneficial in future studies.

Clinical work: we retrospectively assessed outcomes in patients with malignant peritoneal effusion (MPE) who had previously received radiotherapy for colon or rectum cancer within 18 months before the intrapleural infusion of IL-2 or cisplatin. All patients received intrapleural infusion of IL-2 or cisplatin, and most had had several cycles of standard chemotherapy for colon or rectum cancer. 2. Translational work: MC38 cell line (5×106) was subcutaneously inoculated into the right lower extremity of C57/BL mice to construct a primary tumor model. Six days later, MC38 cell line (1×106) was subcutaneously inoculated into the left lower limb to construct a second tumor model. The main research is as follows: To explore the optimal radiation dose and fraction for radiotherapy; Whether the addition of IL-2 will further enhance the efficacy of radiotherapy combined with anti-PD-1; To explore the optimal combination model of radiation, anti-PD-1 and IL-2; Whether the addition of IL-2 will increase the abscopal effect produced by radiotherapy and anti-PD-1. Results: 1. We identified 3,747 patients with MPE (median age 66 years [range 24–81)) treated at one of several institutions from August 2009 through May 2016; 412 patients had been treated with IL-2 and 592 with cisplatin and had survived for at least 3 months afterward. 2. Hypofractionated radiotherapy (12Gy × 2) combined with IL-2 is better; IL-2 will enhance the efficacy of radiotherapy combined with anti-PD-1; IL-2 and anti-PD-1 followed by radiotherapy might maximize the anti-tumor effect; IL-2 might increase the abscopal effect produced by radiotherapy combined with anti-PD-1. Conclusions: We speculate that previous radiotherapy could enhance the efficacy of IL-2 and the anti-tumor effect produced by radiotherapy combined with anti-PD-1 could be enhanced by IL-2. The “radio-memory” effect could be beneficial in future studies.
Combined Radiotherapy and Anti–PD-L1 Antibody Synergistically Enhances Antitumor Effect in Non–Small Cell Lung Cancer

Abstract:Introduction: Immune escape frequently occurs and restricts the durability of the antitumor immune response to radiotherapy. Programmed death 1 (PD-1) and programmed death ligand 1 (PD-L1) are important immune checkpoint molecules that could cause tumor cells to escape the host immune response. The aim of the study was to explore the role of PD-L1 in radioresistance and the antitumor effect of combined radiotherapy and anti-PD-L1 therapy in NSCLC.Methods: The role of the phosphoinositide 3-kinase/protein kinase B signal transducer and activator of transcription 3, epithelial-mesenchymal transition, and tripartite motif containing 21 in regulating PD-L1 expression after radiotherapy was investigated by small interfering-PD-L1 RNA transfection, immunohistochemistry, Western blot, and immunoprecipitation. The synergistic effect of radiotherapy and anti-PD-L1 antibody was evaluated in a mouse model. PD-L1 expression on tumor specimens was examined in a retrospective cohort of patients who received concurrent chemoradiotherapy.Results: PD-L1 expression was increased in vivo and in vitro after conventionally fractionated radiation. Radiotherapy in combination with anti-PD-L1 antibody synergistically enhanced antitumor immunity by promoting CD8-positive T cell infiltration and reducing the accumulation of myeloid derived suppressor cells and tumor-infiltrating regulatory T cells in a mouse model. Radiotherapy may up-regulate PD-L1 expression through the phosphoinositide 3-kinase/AKT and signal transducer and activator of transcription 3 pathways. PD-L1 may also stimulate cell migration and facilitate the epithelial-mesenchymal transition process to induce radioresistance. Moreover, down-regulating PD-L1 could alleviate radioresistance' by promoting apoptosis. Intriguingly, patients with negative PD-L1 expression had a significantly higher objective response rate (88% versus 43.1% [p < 0.001]) and disease control rate (100% versus 86.2% [p = 0.026]) than those with positive PD-L1 expression after delivery of radiotherapy.Conclusions: Conventionally fractionated radiotherapy in combination with anti-PD-L1 antibody shows a synergistic antitumor immunity in NSCLC. Abstract:Background: Radiation therapy is a common treatment for cancer patients, with nearly 50% of them undergoing this procedure. While radiation is effective at targeting tumor cells, it also has the unintended consequence of killing lymphocytes, which are among the most sensitive cells to radiation within the erythroid, myeloid, and lymphoid lineages. This phenomenon is known as radiation-induced lymphopenia (RIL) and is considered a negative prognostic factor in various malignant solid tumors. It occurs severely in over 40% of solid tumor patients who receive radiotherapy. Some previous studies have shown promising results with IL15 therapy in increasing circulating lymphocytes in cancer patients. However, until now, no studies have investigated the impact of IL15 in the context of radiation-induced lymphopenia. Our study aims to investigate whether IL15 can boost lymphocyte numbers and decrease tumor burden following radiation treatment in a mouse model. Methods: To conduct our study, we subcutaneously injected TSA tumor cells into the flank region of mice. Once the tumors reached a palpable size, approximately 50mm3, we divided the mice into four groups: Control (Con), Irradiation (IR), Control with IL15 (Con+IL15), and Irradiation with IL15 (IR+IL15). The irradiated mouse groups received a radiation dose of 2 Gray in the thorax and 1 Gray in the spleen for five consecutive days, simulating the severe radiation-induced lymphopenia observed in cancer patients. After the radiation treatment, the IL15-treated groups received IL15 in combination with recombinant IL15 in a 1:6 ratio weekly. Throughout the experiment, we monitored the circulating lymphocytes and tumor growth. After two weeks following the last radiation dose, we euthanized the mice, collected blood, spleen, and tumor samples, and identified immune cells using flow cytometry. Results: The radiation treatment significantly increased tumor growth while decreasing the number of lymphocytes, including B cells, T cells, and NK cells, in the blood. IL15 administration led to an increase in the number of lymphocytes both in the blood and within the tumor compared to the irradiated group. We observed a trend toward reduced tumor growth in the group that received both IL15 and radiation (IR+IL15) compared to the group that received radiation alone (IR). Conclusion: IL15 effectively boosted the number of lymphocytes in both the bloodstream and tumors, resulting in slower tumor growth in the IL15-treated group compared to the irradiated group. This study is of significant importance as it demonstrates the potential of IL15 to counteract radiation-induced lymphopenia in mice with TSA tumors. Efficacy and Safety of Immunoradiotherapy for Advanced Non-Small Cell Lung Cancer: a Retrospective Comparative Cohort Study

Abstract:Background Treatment of radiotherapy (RT) combined with immune checkpoint inhibitor (ICI) may remarkably improve the prognosis in patients with metastatic non-small cell lung cancer (NSCLC). However, the treatment time of RT, irradiated lesion and the optimum combined scheme, have not been fully determined. Methods Data regarding overall survival (OS), progression-free survival (PFS), treatment response, and adverse events of 357 patients with advanced NSCLC treated with ICI alone or in combination with RT prior to/during ICI treatment were retrospectively collected. Additionally, subgroup analyses for radiation dose, time interval between RT and immunotherapy, and number of irradiated lesions were performed. Results Median PFS of the ICI alone and ICI + RT groups was 6 and 12 months, respectively (P<0.0001). The objective response rate (ORR) and disease control rate (DCR) were significantly higher in the ICI + RT group than in the ICI alone group (P=0.014; P=0.015, respectively). However, OS, the distant response rate (DRR), and the distant control rate (DCRt) did not differ significantly between the groups. Out-of-field DRR and DCRt were defined in unirradiated lesions only. Compared with RT application prior to ICI, its application concomitant with ICI led to higher DRR (P=0.018) and DCRt (P=0.002). Subgroup analyses revealed that single-site, high biologically effective dose (BED) (≥72 Gy), planning target volume (PTV) size (<213.7 mL) RT groups had better PFS. In multivariate analysis, PTV volume [≥213.7 vs. <213.7 mL: hazard ratio (HR), 1.89; 95% confidence interval (CI): 1.04–3.42; P=0.035] was an independent predictor of immunotherapy PFS. Additionally, radio-immunotherapy increased the incidence rate of grade 1–2 immune-related pneumonitis compared with ICI alone. Conclusions Combination therapy using ICIs and radiation may improve PFS and tumor response rates in advanced NSCLC patients regardless of programmed cell death 1 ligand 1 (PD-L1) level or previous treatments. However, it may increase the incidence of immune-related pneumonitis.

Abstract:Radiotherapy (RT) is routinely used in cancer treatment, but expansion of its clinical indications remains challenging. The mechanism underlying the radiation-induced bystander effect (RIBE) is not understood and not therapeutically exploited. We suggest that the RIBE is predominantly mediated by irradiated tumor cell-released microparticles (RT-MPs), which induce broad antitumor effects and cause immunogenic death mainly through ferroptosis. Using a mouse model of malignant pleural effusion (MPE), we demonstrated that RT-MPs polarized microenvironmental M2 tumor-associated macrophages (M2-TAMs) to M1-TAMs and modulated antitumor interactions between TAMs and tumor cells. Following internalization of RT-MPs, TAMs displayed increased programmed cell death ligand 1 (PD-L1) expression, enhancing follow-up combined anti-PD-1 therapy that confers an ablative effect against MPE and cisplatin-resistant MPE mouse models. Immunological memory effects were induced.

Abstract:Numerous studies revealed that radiation can promote the immunogenic cell death, enhance the immune response of tu-mor-antigen-specific T cells, and induce systematic antitumor immunity, which may result in abscopal effect. Abscopal effect refers to the regression of non-irradiated metastatic lesions distant from the primary tumor site that is directly subjected to irradiation. Despite that abscopal effect has been clinically reported and that many preclinical studies have confirmed its biological basis, abscopal effect remains a subclinical phenomenon, in which objective efficacy is difficult to obtain. Currently, the anti-PD-1/PD-L1 therapy in clinical applications shows a clear and definite curative effect, thereby making local radiotherapy combined with immunotherapy a new treat-ment pattern. However, the clinical practice of this new mode should be based on appropriate dose and fraction and the most suitable temporal combination to achieve the synergic antitumor immune response.

Abstract:e16195 Background: Immune checkpoint inhibitor monotherapy has emerged as a breakthrough therapy in the treatment of various cancer. However, programmed cell death (PD-1) inhibitors alone as secondary line treatment yield uncertain response in liver cancer in different studies. Despite encouraging preclinical findings that radiation primes the immune system and produces a synergistic anti-tumor immunity for durable disease control, few clinical data is published so far. To investigate the efficacy and safety of the combination of radiation and PD-1 inhibitors in metastatic and recurrent liver cancer, we proposed this study. Methods: Patients diagnosed with stage IV (American Joint Committee on Cancer Staging System 8th) or recurrent liver cancer, received radiotherapy followed by PD-1 inhibitors or concurrent radiation and PD-1 inhibitors were enrolled. Data regarding clinicopathologic characteristics, treatment protocols, response rates, toxicities, and survival were collected. Survivals were calculated from the date of the first delivery of immunotherapy using Kaplan-Meier method. Results: From April 2017 to December 2020, 34 patients were eligible. Twenty-nine (85.3%) patients were male and 5(14.7%) were female. The median age was 58 years old. Twenty-three patients (67.6%) had hepatocellular carcinoma. Totally, radiotherapy was delivered to 92 lesions (28 liver, 28 venous tumor emboli, 13 lymph nodes, 18 bone, 5 others) of these patients. The radiation dose ranged from 40Gy to 60Gy (2-5Gy per fraction). Sixteen patients, 15 patients, 5 patients, 1 patient and 1 patient received toripalimab, sintilimab, camrelizumab, nivolumab and pembrolizumab, respectively (Four patients received multiple- drugs in different time ). PD-1 inhibitors were administered very 3 weeks until progression or limiting toxicities. After treatment, 3,18 and 11 patients achieved complete response, partial response and stable disease. The response rate and disease control rate were 61.8% and 94.1%, respectively. Grade 3 adverse events were observed in 11 patients (32.4%), including 6 patients with thrombocytopenia (2 with grade 4, 4 with grade 3), 2 patients with gastrointestinal events and 3 with others. No grade 5 adverse event was recorded. The median follow up time was 16.5 months. The median overall survival (OS) time and progression free survival (PFS) time were 15.4 months and 11.3 months. One-year OS and PFS were 62.9% and 46.5%, respectively. Two-year OS and PFS were 31.8% and 11.3%, respectively. Conclusions: The combination of immunotherapy and radiotherapy was safe and tolerable. The application of radiation before or during PD-1 inhibitors delivery fostered the immune response and enhance the efficacy of immunotherapy with prolonged PFS and OS in recurrent and metastatic liver cancer. A phase II prospective trial is ongoing.

Abstract:Radiation therapy is one of the standard therapeutic modalities for esophageal cancer, achieving its main antitumor efficacy through DNA damage. However, accumulating evidence shows that radiotherapy can substantially alter the tumor microenvironment, particularly with respect to its effects on immune cells. We hypothesized that the immune response elicited by radiotherapy may be as important as the radiation itself for successful treatment. More specifically, immunomodulatory cytokines may enhance the effectiveness of radiotherapy. To investigate this hypothesis, we measured changes in the serum interferon-gamma (IFN-gamma) and interleukin-2 (IL-2) concentrations during radiotherapy and compared these modifications with outcomes. We found that serum concentrations of IL-2 and IFN-gamma were positively associated with local response to radiotherapy in esophageal cancer. More generally, the intensity of the radiotherapy-elicited immune response was positively associated with local response to radiotherapy in esophageal cancer. Changes in serum IL-2 and IFN-gamma concentrations were further associated with increased risks of acute hematologic toxicity and acute organ toxicity of the esophagus, lung, and skin. These results suggest that deciphering the mechanisms of radiotherapy-elicited immune response may help in the development of therapeutic interventions that would enhance the efficacy of radiotherapy and convert some ineffective responses to effective responses.

Abstract:The interaction between radiotherapy and the host immune system has uncovered new mechanisms that can be exploited to improve the efficacy of radiotherapy. In this article, the authors highlight data providing new explanations for the success or failure of radiotherapy, and postulate, using radiation-induced tumour equilibrium (RITE) as an example, how the combination of immune-modulation and radiation could tip the balance of the host immune response to promote cure. Investigations into the interaction between radiotherapy and the host immune system have uncovered new mechanisms that can potentially be exploited to improve the efficacy of radiotherapy. Radiation promotes the release of danger signals and chemokines that recruit inflammatory cells into the tumour microenvironment, including antigen-presenting cells that activate cytotoxic T-cell function. By contrast, radiation can attract immunosuppressive cells into the tumour microenvironment. In rare circumstances, the antitumour effect of radiotherapy has been observed outside of the radiation field, known as the abscopal effect. This phenomenon is proposed to have an immune origin and indicates that local radiotherapy elicits systemic effects. Herein, we highlight data that provide new mechanistic explanations for the success or failure of radiotherapy, and postulate how the combination of immune-modulation and radiation could tip the balance of the host immune response to promote cure. We use the concept of radiation- induced tumour equilibrium (RITE) as a starting point to discuss the mechanistic influence of immune-checkpoint therapies on radiotherapy efficacy.
Impact of Radiotherapy Concurrent with Anti-PD-1 Therapy on the Lung Tissue of Tumor-Bearing Mice.

Abstract:Pneumonitis is a common adverse effect found in non-small cell lung cancer patients after radiotherapy or immune checkpoint inhibitor treatment. We investigated the effects of these two therapies, combined, in the lung tissue of an orthotopic tumor-bearing mouse model. The mice received an 8 Gy dose three times with or without 200 µg anti-programmed death-1 (anti-PD-1) antibody intraperitoneal injection every three days. Lung tissues were H&E stained to determine histological changes. The serum levels of cytokines, such as interferon-γ, tumor necrosis factor and interleukin-5, were detected by cytometric bead array. The neutrophil infiltration was evaluated by immunohistochemical staining for myeloperoxidase. The lung injury score was higher in the treated groups than the control group, especially in the combined treatment group, in which the proportion of neutrophils in lung tissues was significantly higher compared to any other groups. Similarly, the CD4/CD8 ratio of the lung tissues in the combined treatment group, as well as the serum levels of interferon-γ, tumor necrosis factor and interleukin-5, were significantly higher than the other groups. These findings indicate that radiation combined with anti-PD-1 treatment leads to more severe lung injury in the orthotopic tumor-bearing mouse model, accompanied by increased neutrophil infiltration and increased inflammatory response.

Abstract:Radiotherapy (RT), administered to roughly half of all cancer patients, occupies a crucial role in the landscape of cancer treatment. However, expanding the clinical indications of RT remains challenging. Inspired by the radiation-induced bystander effect (RIBE), we used the mediators of RIBE to mimic RT. Specifically, we discovered that irradiated tumor cell-released microparticles (RT-MPs) mediated the RIBE and had immune activation effects. To further boost the immune activation effect of RT-MPs to achieve cancer remission, even in advanced stages, we engineered RT-MPs with different cytokine and chemokine combinations by modifying their production method. After comparing the therapeutic effect of the engineered RT-MPs in vitro and in vivo, we demonstrated that tIL-15/tCCL19-RT-MPs effectively activated antitumor immune responses, significantly prolonged the survival of mice with malignant pleural effusion (MPE), and even achieved complete cancer remission. When tIL-15/tCCL19-RT-MPs were combined with PD-1 monoclonal antibody (mAb), a cure rate of up to 60% was achieved. This combination therapy relied on the activation of CD8+ T cells and macrophages, resulting in the inhibition of tumor growth and the establishment of immunological memory against tumor cells. Hence, our research may provide an alternative and promising strategy for cancers that are not amenable to conventional RT.

Abstract:Purpose: Radiotherapy modifies the tumor microenvironment and causes the release of tumor antigens, which can enhance the effect of immunotherapy. L19 targets the extra domain B (ED-B) of fibronectin, a marker for tumor neoangiogenesis, and can be used as immunocytokine when coupled to IL2. We hypothesize that radiotherapy in combination with L19-IL2 provides an enhanced antitumor effect, which is dependent on ED-B expression. Experimental design: Mice were injected with syngeneic C51 colon carcinoma, Lewis lung carcinoma (LLC), or 4T1 mammary carcinoma cells. Tumor growth delay, underlying immunologic parameters, and treatment toxicity were evaluated after single-dose local tumor irradiation and systemic administration of L19-IL2 or equimolar controls. Results: ED-B expression was high, intermediate, and low for C51, LLC, and 4T1, respectively. The combination therapy showed (i) a long-lasting synergistic effect for the C51 model with 75% of tumors being cured, (ii) an additive effect for the LLC model, and (iii) no effect for the 4T1 model. The combination treatment resulted in a significantly increased cytotoxic (CD8(+)) T-cell population for both C51 and LLC. Depletion of CD8(+) T cells abolished the benefit of the combination therapy. Conclusions: These data provide the first evidence for an increased therapeutic potential by combining radiotherapy with L19-IL2 in ED-B-positive tumors. This new opportunity in cancer treatment will be investigated in a phase I clinical study for patients with an oligometastatic solid tumor (NCT02086721). Observation on efficacy of radiotherapy combined with hyperthermia in treatment of non-small-cell lung cancer and significance of change of VEGF,sIL-2R and IL-6

Abstract:Objective To evaluate the efficacy of radiotherapy and hyperthermia in treatment of advanced non-small-cell lung cancer(NSCLC) and to analyse the change and clinic significance of serum VEGF,sIL-2R and IL-6 levels before and after radiotherapy and hyperthermia.Methods Sixty patients with advanced NSCLC were randomly divided into two groups,30 received radiotherapy and hyperthermia,30 received radiotherapy only.The efficacy and toxicity of radiotherapy and hyperthermia in treatment of advanced NSCLC and the changes of serum VEGF,sIL-2R,IL-6 levels before and after radiotherapy and hyperthermia were observed.Results The local response rate in the radiotherapy combined with hyperthermia group was higher than that in the radiotherapy only group.There was a significant difference in the response rates of chest pain and severe cough in radiotherapy combined with hyperthermia group compared with control group.There was no significant difference between the two groups in side-effects.The serum VEGF levels in the 2nd month after the treatment did not significantly change compared with the serum VEGF levels before treatment and the 2nd week after the treatment in radiotherapy combined with hyperthermia group,while there was a progressive increase in the serum VEGF levels in the radiotherapy only group,there was significant difference in the serum VEGF levels between the 2nd month after the treatment and before treatment.There was no significant difference between two groups.The level of sIL-2R was significantly progressively decreased in both group.The change of the level of sIL-2R in the radiotherapy combined with hyperthermia was more significant than that in the radiotherapy only group,the two groups had significant differences.The serum IL-6 level progressively increased in the radiotherapy combined with hyperthermia group.There was significant difference in the serum IL-6 levels between the 2nd month after the treatment and before treatment in the radiotherapy only group.Conclusions Radiotherapy combined with hyperthermia was better than radiotherapy alone in local response rate.Radiotherapy combined with hyperthermia not only can significantly elevate the quality of life in patients with advanced NSCLC,but also improves accompanying symptoms.

Abstract:Objective: To study the anti-tumor effect of repeated low dose pEgr-IFNγ-Endostatin gene-radiotherapy on mice bearing Lewis lung carcinoma. Methods: Mice bearing Lewis lung carcinoma were divided randomly into control 9 10 Gy, 4 × 2.5 Gy, 4 × P, P + 10 Gy and 4 × (P + 2.5 Gy) group. The pEgr-IFNγ-Endostatin plasmid packaged with liposome was injected into tumors which were irradiated by X-ray 36 h later. Tumor growth rates at different time were observed after gene-radiotherapy. Number of lung metastatic nodes and tumor weight of the mice were detected 18 d after gene-radiotherapy. Results: Tumor growth rate of the mice in gene-radiotherapy for four-time group was significantly lower than that for only once group 12-18 d after gene-radiotherapy. Number of lung metastatic nodes and tumor weight of the mice in gene-radiotherapy for four-time group were significantly lower than those for only once group 18 d after gene-radiotherapy. Conclusion: Anti-tumor effect of repeated low dose pEgr-IFNγ-Endostatin gene-radiotherapy is better than that of high dose for only once.
Improved outcomes of palliative radiotherapy combined with immune checkpoint inhibitors in recurrent or metastatic cervical cancers

Abstract:Background Immunotherapy provides a remarkable survival advantage for patients with recurrent or metastatic cervical cancer (R/M CC). However, the role of immunotherapy in combination with radiotherapy in R/M CC remains unclear. Methods We retrospectively analyzed factors affecting immunotherapy effectiveness in patients with R/M CC. Clinical outcomes including tumor response and patient survival were assessed. Kaplan-Meier curves with the log-rank test were employed to compare survival data. Cox regression analysis was utilized to investigate prognostic factors. Results A total of 65 R/M CC patients treated with immune checkpoint inhibitors were eligible for analysis. We found that immunotherapy combined with palliative radiotherapy showed a significant positive correlation with complete response (OR = 6.31; 95 %CI: 1.74–22.91; p = 0.005). The 36-month progression-free survival (PFS) rate (73.7 % vs 33.8 %, p = 0.0048) and 36-month overall survival (OS) rate (85.7 % vs 38.7 %, p = 0.0043) were also prominently increased. We further demonstrated that patients prolonged 36-month PFS rate (69.9 % vs 15.2 %; p < 0.001) and 36-month OS rate (64.6 % vs 39.7 %; p = 0.032) when they had more than 4 cycles of immunotherapy. Meanwhile, our findings showed that patients with only recurrence had longer 36-month OS rate (77.7 % vs 44.4 % vs 40.1 %; p = 0.024) compared to those with only metastasis and both. We also observed that patients with squamous carcinoma had higher 2-year PFS rate (57.9 % vs 14.6 %; p = 0.042) than those with other pathological subtypes (adenocarcinoma, adenosquamous carcinoma and neuroendocrine carcinoma). Conclusions The combination of immunotherapy and palliative radiotherapy increased complete response rates and improved survivals in recurrent or metastatic cervical cancer patients.

Abstract:e16063 Background: The development of immunotherapy has changed treatment methods for advanced esophageal cancer (EC).However, the efficacy of ICIs in advanced EC is not ideal, and the clinical research on the combination of ICIs and Radiotherapy(RT) in advanced EC remains insufficient. This study aimed to evaluate the effectiveness and safety of radiation in first-line immunotherapy for advanced EC. Methods: We conducted 201 advanced EC patients who received first-line ICIs in West China Hospital. The enrolled patients were divided into 2 groups based on whether they received radiation therapy. Results: The analysis included data from 201 patients. The radiotherapy (RT) group had 103 patients (51.2%), whereas the non-radiotherapy (NRT) group included 98 individuals (48.8%). The RT sites include esophagus lesions (83, 80.6%), metastatic lymph nodes (11, 10.7%), liver metastases (4, 3.9%), bone metastases (2, 1.9%), brain metastases (2, 1.9%), lung metastases(1, 0.97%). The median radiation dose was 50.4 Gy (range, 12-68). The RT group had a median OS of 22.8 months (95% CI, 17.8-NA), which was longer than the NRT group's 14.6 months (95% CI, 12.8-26.5) (p = 0.038). And the median PFS was 12.9 months (95% CI, 11.2-14.0) in the RT group versus 7.3 months (95% CI, 6.3-9.7) in the NRT group (p = 0.006). Patients in the RT group had a higher disease control rate (DCR) (99.0% vs. 76.5%, P < 0.001). However, a higher proportion of patients in the RT group developed grade 3-4 myelosuppression (37.86% V.S. 20.20%, P = 0.006), and there was no significant difference in the other AEs. Conclusions: In the first-line immunotherapy for advanced esophageal cancer, radiotherapy has been attributed to improved survival with tolerable toxicity.

Abstract:*These authors contributed equally to this work Background: The combination of radiotherapy and immunotherapy can bring benefits to patients, especially advanced patients. However, conventional radiotherapy brings about great adverse reactions. How about the hypofractionated low-dose radiotherapy? Materials and Methods: In this retrospective cohort study, we included 32 patients with metastatic solid tumors treated with hypofractionated radiotherapy combined with an immune checkpoint inhibitor. Patients underwent radiotherapy of 4Gy/Fx on day 1, 3, and 5, and received single-drug immunotherapy of PD-1 inhibitor on day 2. We evaluated the following outcomes: objective response rate (ORR), disease control rate (DCR), change of nonirradiated and irradiated lesions, quality of life, and symptom improvement. Results: Among the 32 patients, the ORR was 9.4% (3/32) and the DCR was 56.25% (18/ 32). Hypofractionated radiotherapy combined with immunotherapy showed a remarkable efficacy of local control on metastatic tumor patients. Local masses irradiated in two patients (6.25%) were complete remission, partial response rate was 37.5% (12 patients), and 56.25% was stability (18 patients). Out of those 18 patients, 15 patients had the local masses shrank more or less. The ORR of local control reached 43.75%, and its DCR was 100%. In addition, the intratumor necrosis rate was 44.4% in the SD patients. Median progression-free survival was 3.8 months (95%Cl: 2.2–5.4). By treating the local mass, the symptoms of most patients were alleviated, and the quality of life was improved. Conclusion: Our retrospective analysis revealed that hypofractionated radiotherapy combined with immunotherapy was effective in local control, it also relieved clinical symptoms and improved quality of life. The adverse effect rate was low. However, the incidence of abscopal effects was low either. This mode was suitable for the palliative treatment and expected to improve survival for patients with metastatic tumors.

Abstract:e14632 Background: Lung cancer continues to be the primary cause of cancer-related mortality on a global scale. The integration of radiotherapy and immunotherapy has yielded significant advancements in the treatment of locally advanced non-small cell lung cancer. However, the occurrence of treatment-associated pneumonia remains the predominant adverse event necessitating treatment cessation and permanent medication discontinuation. The immune-modulating effects of radiation therapy on the body contribute to a complex interplay between radiation and inflammation. Low-dose radiotherapy (LD-RT) has historically been utilized for the management of benign inflammatory conditions. Recently, LD-RT was found to be effective in treating severe pneumonia in patients with severe COVID-19 as well as acute respiratory distress syndrome (ARDS). This investigation seeks to examine the impact of LD-RT on lung tissue morphology and the inflammatory immune microenvironment in preclinical models of lung injury induced by high-dose radiotherapy combined with programmed death-1(PD-1) inhibitor therapy. Methods: 6-8 week old C57BL/6 mice were treated with a single high-dose thoracic radiotherapy (15Gy) and the PD-1 inhibitor (once a week), with or without the low-dose radiotherapy (1.0/1.5Gy) four weeks later. Lung tissue was collected one and a half months and three months respectively after the initial high-dose radiotherapy for evaluation using HE and Masson staining, Image J software for analysis of collagen volume fraction (CVF), and immunohistochemical staining for CD3+ and CD8+ T lymphocytes. Cytokine transforming growth factor (TGF)-β1、interleukin (IL)-6 and tumor necrosis factor (TNF)-α level in lung tissue was detected by ELISA. Statistical analysis was performed using GraphPad Prism 9.5.0 software. Results: The radiotherapy-immunotherapy combination (iRT) group exhibits notable lung injury characterized by the proliferation of alveolar type II epithelial cells, thickening of alveolar septa, interstitial edema, pulmonary congestion and bleeding, increased infiltration of inflammatory cells, and significant deposition of interstitial collagen fibers. In contrast, the iRT+ LD-RT group demonstrates a significant decrease in lung tissue damage, interstitial fibrosis, collagen volume fraction (CVF), levels of IL-6 and TGF-β, as well as the infiltration of CD3+ and CD8+T cells compared to the iRT group. Conclusions: Low-dose radiation has the potential to ameliorate lung injury induced by the combination of radiotherapy and PD-1 inhibitors by modulating the inflammatory milieu, thereby potentially mitigating the development of pulmonary fibrosis.
