The protective mechanism of artery only occlusion is unknown, although one postulate is that retrograde venous perfusion improves renal oxygenation. A pilot study in a porcine model did in fact show that percentage oxyhemoglobin (%HbO2) is greater in AO kidneys between 16 and 24 minutes post occlusion. We assessed whether this finding applies to humans undergoing partial nephrectomy. METHODS We performed a prospective cohort study comparing percentage oxyhemoglobin in the renal cortex during Artery-Only occlusion vs. Artery and Vein occlusion in patients undergoing partial nephrectomy at our center between March 2009 and July 2011. Digital light projection hyperspectral imaging (HSI) was used to obtain real time ‘optical biopsies' representative of renal oxygenation status at several time points during occlusion. Mean %HbO2 was compared between cohorts at 5 minute grouped intervals using the paired t-test. RESULTS HSI data was collected on 37 patients undergoing open partial nephrectomy; 25 subjected to AV occlusion and 12 subjected to AO occlusion. Mean ischemic time was 37 +/- 7.02 minutes. Baseline %HbO2 was 71% and 73.6% in the AV and AO groups respectively (p= 0.226). Within 5 minutes of occlusion, the mean %HbO2 decreased 6.22% in the AV group and 13.3% in the AO group. Mean %HbO2 was higher in the AV group at all but two time points, although these differences were not statistically significant. In the AV group mean %HbO2 actually increased from 59.85% to 64.75%, a rise of 4.9% (p=0.09), between 5-10 minutes and 40-45 minutes post-occlusion. In contrast, %Hb02 in the AO group did not rebound after the initial post-occlusion decline. After reperfusion, %HbO2 returned to near baseline levels, 71.81% in the AV cohort and 70.49% in the AO cohort. CONCLUSIONS Lower %HbO2 in AO kidneys contradicts the hypothesis that artery only occlusion results in beneficial retrograde perfusion and oxygenation. Instead, AO occlusion may result in unilateral venous drainage from the renal capillaries resulting in lower O2 availability. On the other hand, lower %HbO2 in the AO group may simply reflect superior oxygen extraction.
