Abstract:Soft tissue sarcomas (STS) of the extremity and trunk are heterogeneous and rare tumors that require coordinated multidisciplinary management. Surgical resection remains the backbone of treatment for localized tumors, with the addition of radiotherapy to surgery to achieve high rates of local control. Despite this, overall survival is limited because of significant distant metastatic risk and a lack of efficacious systemic therapies. Clinical trials have produced conflicting results on the impact of systemic therapy in the neoadjuvant and adjuvant settings for patients with localized disease, leaving systemic treatment decisions largely guided by shared decision making and prognostic prediction tools such as nomograms. This article will review the foundational data as well as latest developments in surgical, radiotherapy, and systemic management supporting current practice guidelines for localized STS of the extremity and trunk.

Abstract:<p>Soft‐tissue sarcomas (STS) are rare tumors that account for 1% of all adult malignancies, with over 100 different histologic subtypes occurring predominately in the trunk, extremity, and retroperitoneum. This low incidence is further complicated by their variable presentation, behavior, and long‐term outcomes, which emphasize the importance of centralized care in specialized centers with a multidisciplinary team approach. In the last decade, there has been an effort to improve the quality of care for patients with STS based on anatomic site and histology, and multiple ongoing clinical trials are focusing on tailoring therapy to histologic subtype. This report summarizes the latest evidence guiding the histiotype‐specific management of extremity/truncal and retroperitoneal STS with regard to surgery, radiation, and chemotherapy.</p>

Advances of Systemic Treatment for Adult Soft-Tissue Sarcoma

Abstract:Soft-tissue sarcoma (STS) is a group of rare but highly heterogeneous neoplasms. Systemic treatment with cytotoxic chemotherapy and targeted agents is one of the main therapeutic modalities in patients with unresectable or metastatic disease, while adjuvant and neoadjuvant chemotherapy for adult-type sarcomas remain controversial. Although an anthracycline (doxorubicin) and ifosfamide remain the cornerstone for chemotherapy, advances have been made recently to exceed its limited efficacy, other agents such as trabectedin, eribulin have been approved. In a recent study, the addition of platelet-derived growth factor receptor (PDGFR) antibody-olaratumab to doxorubicin resulted in prolongation of progression-free survival and overall survival, which really means a breakthrough for STS. There is more emerging evidence of different sensitivity to treatment for different histological subtypes, second-line treatment for advanced sarcoma is being increasingly driven by histology. Cytotoxic drugs such as dacarbazine, gemcitabine, and taxanes have shown moderate activity in specific subtypes. Tyrosine kinase inhibitors (TKIs), including pazopanib and anlotinib, appear to be the promising targeted therapies. Other signal pathway inhibitors as CDK4/CDK6 inhibitor, imatinib, mTOR inhibitors, ALK inhibitor has shown some preliminary effect that need to be verified in the future trials. Checkpoint inhibitors as anti-PD-1 and CTLA-4 monoclonal antibodies have been used as a single agent or in combination in the early clinical trials, while further research needs to focus on better patient selection and new combinational strategies. In this review, we aim to summarize the advances of chemotherapy, targeted therapy and immunotherapy in the management of STS.

Abstract:Sarcomas consist of a group of rare solid tumors of mesenchymal cell origin with distinct clinical and pathological fea-tures. Generally, they are divided into two categories: sarcoma of soft tissue and sarcoma of bone. Sarcomas account for approximately 1% of all adult malignancies and 15% of pediatric malignancies. Soft tissue sarcomas (STS) include more than 50 different subtypes, and the most common subtypes are undifferentiated pleomorphic sarcoma, gastrointestinal stromal tumor, liposarcoma, leiomyosarco-ma, synovial sarcoma, and malignant peripheral nerve sheath tumor. Surgery is still the main therapy method for STS, but it leads to a high recurrence rate, poor prognosis, and unsatisfactory efficacy of chemotherapy. With deeper understanding of the biological behavior of tumors, several emerging targeted drugs have achieved good efficacy. Overall, considering the pathological subtypes, features of the molecular genetics, stage, and prognostic factors of STS patients, the individualized therapy model, including surgery, chemotherapy, ra-diotherapy, and targeted therapy, is recommended for STS patients.

Abstract:Non-rhabdomyosarcoma soft tissue sarcoma (STS) comprises most STS in pediatric patients. It is a diverse set of over 30 histologic subtypes. Treatment is based on risk group determined by tumor size, grade, and the presence of metastases. Surgical resection is a cornerstone of therapy, as tumors are often resistant to chemotherapy or radiation. While patients with isolated tumors less than 5 cm may undergo upfront resection, strong consideration should be given to neoadjuvant chemoradiotherapy to ensure negative margins at surgical resection and optimal outcomes. Sentinel lymph node biopsy is strongly recommended for clear cell and epithelioid sarcomas. The most common metastatic site is the lung, and metastases should be resected at the end of therapy, when feasible. Unfortunately, many high-risk patients progress on therapy, and alternative strategies including earlier metastatic control require investigation.

Abstract:Soft tissue sarcomas (STSs), including gastrointestinal stromal tumors (GISTs), are mesenchymal neoplasms with heterogeneous clinical behavior and represent broad categories comprising multiple distinct biologic entities. Multidisciplinary management of these rare tumors is critical. To date, multiple studies have outlined the importance of biological characterization of mesenchymal tumors and have identified key molecular alterations which drive tumor biology. GIST has represented a flagship for targeted therapy in solid tumors with the advent of imatinib which has revolutionized the way we treat this malignancy. Herein, the authors discuss the importance of biological and molecular diagnostics in managing STS and GIST patients.

Abstract:Sarcomas are tumors of mesenchymal origin that account for approximately 1%of human cancers. There are at least 60 dif-ferent subtypes of soft tissue sarcoma (STS). However, most tumor types are highly malignant tumors that lack an optimal therapeutic agent. Although gastrointestinal stromal tumor (GIST) responds well to imatinib, other types of STSs remain challenging because of their heterogeneous genetic and clinical features and poor prognosis. In recent years, the field of molecular targeted therapy has pro-gressed rapidly. Patients with advanced disease can receive individualized treatment to improve their quality of life. This review focus-es on the targeted therapies that have been evaluated for advanced non-GIST STS. Other promising novel therapeutic agents that are currently in the early phases of investigation are also presented.

Abstract:Soft tissue sarcomas are a diverse and heterogeneous group of cancers of mesenchymal origin. Each histological type of soft tissue sarcoma has unique clinical particularities, which makes them challenging to diagnose and treat. Multidisciplinary management of these rare diseases is thus key for improved survival. The role of surgery has been well established, and it represents the cornerstone curative treatment for soft tissue sarcomas. To date, local recurrence is the leading cause of death in low-grade sarcomas located at critical sites, and distant metastasis in high-grade sarcomas, regardless of the site of origin. Management must be tailored to each individual histologic type. We describe the most common types of extremity, trunk, abdominal, and retroperitoneal soft tissue sarcoma along with characteristics to consider for optimized management.

Abstract:To analyze the clinical characteristics of soft tissue sarcomas (STS) diagnosed from different hospitals be-tween 2011 and 2015 to provide evidence-based information for further standardized diagnosis and treatment of STS. Meth-ods:Data were from 15 hospitals, including 14 tertiary hospitals and 1 level A secondary general hospital, from Beijing, Henan, Hebei, Hubei, Zhejiang, Guangdong provinces. Clinical information of patients who were newly diagnosed was col-lected from 2011 to 2015. The questionnaire was self-made and data were entered using Epidata 3. 0. Tumor sites, histologi-cal types were coded according to International Classification of Diseases for Oncology (third version). Chronological ages at first diagnosis were further classified into 18 age groups and 4 age brackets. Main therapeutics were analyzed. Results:A to-tal of 3,712 patients (1,895 males and 1,817 females) were enrolled from 15 hospitals. The most common histological sub-type of STS was gastrointestinal stromal tumor (543 cases, 14. 39% ), followed by sarcomas (not otherwise specified) (521 cases, 14. 04% ). STS could occur anywhere in the body. It could be found in connective and soft tissue (1,130 cases, 30. 44% ) in head, limbs, and pelvic and so on. New cases of STS peaked in females aged 55-59 (13. 54% ) and in males aged 60-64 (12. 98% ), respectively. The most common histological subtype of STS in 0-to-14-year-olds and 15-to-44-year-olds were rhabdomyoma and sarcoma (not otherwise specified), respectively. Gastrointestinal stromal tumor was the most common histologic subtype of STS in people aged 45-59 and over 60. The most common therapeutic was surgery, fol-lowed by chemotherapy and radiotherapy. Conclusion:Gastrointestinal stromal tumor is the most common histological sub-type of STS, especially in the elderly. Males have a later peak in age of onset. The most common therapeutic of STS was sur-gery. Standard should be further developed for the selection of therapeutics.

Abstract:Soft tissue sarcomas (STSs) are relatively rare heterogeneous solid tumors of the mesenchymal origin. They account for approximately 1% of all malignant tumors in adults and have more than 70 histological subtypes. Consequently, the rarity and heterogeneity of STSs make their diagnosis and treatment very challenging. Nanotechnology has attracted increasing attention from researchers due to the unique physicochemical and biological properties of nanomaterials with potential medical applications as nanoprobes, drug delivery systems, photosensitizers, radioenhancers, antitumor agents, and their combinations for cancer diagnosis and treatment. This review discusses the progress made in the use of nanotechnology for the diagnosis and treatment of STSs and highlights future prospects of the STS multimodality therapy.

Abstract:Objective:To explore the outcome of soft tissue sarcoma (STS) on patients with soft tissue metastasis. Methods:We ana-lyzed 25 STS patients with soft tissue metastasis primarily localized on extremity and trunk. The study was conducted from June 2010 to June 2016 by retrospective analysis of the clinical and pathological characteristics of the patients. The assessed endpoints were overall survival. Results:Six patients (24%) had synchronous soft tissue metastasis, and 19 patients (76%) had metachronous metasta-sis. The average time for primary tumor recession of metastatic lesions was 45.3 months. Metastases were most common in parts of the trunk in 18 patients (72%), followed by the head and neck in 5 patients (20%). Eleven patients (44%) with lung metastasis had poor prognosis. Conclusion:STS occurred more rarely in soft tissue metastasis than in pulmonary metastasis. Neoadjuvant chemotherapy and surgical treatment were the major therapies employed. Targeted therapy as a new treatment rendered good results.

Abstract:Soft-tissue sarcomas (STSs) are a heterogeneous group of malignancies of mesenchymal origin with an incidence of 4-8 per 100,000 within Europe; they consist of various subtypes, each expressing distinct morphological, histopathological, and molecular features. According to the 2020 World Health Organization (WHO) classification, STSs are divided into 11 broad categories comprising approximately 80 entities. STS management guidelines are constantly evolving due to the development of newer diagnostic methods and treatments including the discovery of new chemotherapeutic agents and immunomodulators. Although the referral, diagnostic, and management pathways may vary in different healthcare systems, the core management principles remain the same. This article explores various established STS guidelines on imaging diagnosis, staging, and follow-up by international societies. It also describes radiologic utility in assessing recurrence and postoperative mimics. The objective is to provide radiologists and clinicians with imaging guidance useful at various stages of STS management.

Abstract:Soft tissue sarcomas (STS) are rare tumours arising in mesenchymal tissues, and can occur almost anywhere in the body. Their rarity, and the heterogeneity of subtype and location means that developing evidence-based guidelines is complicated by the limitations of the data available. However, this makes it more important that STS are managed by teams, expert in such cases, to ensure consistent and optimal treatment, as well as recruitment to clinical trials, and the ongoing accumulation of further data and knowledge. The development of appropriate guidance, by an experienced panel referring to the evidence available, is therefore a useful foundation on which to build progress in the field. These guidelines are an update of the previous version published in 2010 (Grimer et al. in Sarcoma 2010:506182, 2010). The original guidelines were drawn up following a consensus meeting of UK sarcoma specialists convened under the auspices of the British Sarcoma Group (BSG) and were intended to provide a framework for the multidisciplinary care of patients with soft tissue sarcomas. This current version has been updated and amended with reference to other European and US guidance. There are specific recommendations for the management of selected subtypes of disease including retroperitoneal and uterine sarcomas, as well as aggressive fibromatosis (desmoid tumours) and other borderline tumours commonly managed by sarcoma services. An important aim in sarcoma management is early diagnosis and prompt referral. In the UK, any patient with a suspected soft tissue sarcoma should be referred to one of the specialist regional soft tissues sarcoma services, to be managed by a specialist sarcoma multidisciplinary team. Once the diagnosis has been confirmed using appropriate imaging, plus a biopsy, the main modality of management is usually surgical excision performed by a specialist surgeon. In tumours at higher risk of recurrence or metastasis pre- or post-operative radiotherapy should be considered. Systemic anti-cancer therapy (SACT) may be utilized in some cases where the histological subtype is considered more sensitive to systemic treatment. Regular follow-up is recommended to assess local control, development of metastatic disease, and any late-effects of treatment. For local recurrence, and more rarely in selected cases of metastatic disease, surgical resection would be considered. Treatment for metastases may include radiotherapy, or systemic therapy guided by the sarcoma subtype. In some cases, symptom control and palliative care support alone will be appropriate.

Abstract:Objective To explore the diagnosis and standardized operation mode of extremity soft tissue sarcomas ( STS)in order to improve the treatment efficacy. Methods The clinical data of 709 patients with extremity STS underwent surgical treatment were analyzed retrospectively. Results Of all the 709 patients,709 patients were ex-amined with B ultrasound,and 135 patients with CT,587 patients with MRI. The 216 patients with supplementary wide resection received no preoperative biopsy,the remaining 493 patients received the preoperative biopsy(350 pa-tients fine needle biopsy,89 patients coarse needle biopsy,54 patients cutting biopsy). The operation mode:434 pa-tients underwent wide local excision,125 patients radical resection,113 patients compartment barrier resection,37 patients amputation. The pathological diagnosis result after operation:103 patients with malignant fibrous histiocyto-ma,89 patients with synovial sarcoma,78 patients with liposarcoma,74 patients with rhabdomyosarcoma,67 patients with dermatofibrosarcoma protuberan,62 patients with fibrosarcoma,46 patients with leiomyosarcoma,41 patients with alveolar soft part sarcoma,36 patients with clear cell sarcoma,31 patients with desmoid type fibromatosis,29 patients with epithelioid sarcoma,27 patients with malignant schwannoma,11 patients with extraskeletal Ewing sar-coma,10 patients with extraskeletal osteosarcoma,and 5 patients with angiosarcoma. The 531 patients had been fol-lowed up for 0. 3 ~4. 5 years,73 patients(13. 7%)had the local recurrence,the recurrence time was 11 ~32 months and the median recurrence time was 21 months;127 patients(23. 9%)appeared the distant metastases(86 patients with lung metastasis,31 patients with liver metastasis,10 patients with bone metastases). Conclusion MRI or CT should be taken as the routine preoperative examination,biopsy is helpful to the diagnosis of STS,opera-tion is the most important method in the treatment of STS,the standardized operation mode can decrease the recur-rence rate.

Abstract:Soft tissue sarcomas (STS) have long been a formidable challenge in oncology, partly because of their rarity and diversity, which complicates large-scale studies and slows the advent of new treatments. Traditionally anchored by anthracycline-based chemotherapy, the landscape of STS treatment hasn't shifted dramatically in the past twenty years. However, recent strides in research are starting to paint a more hopeful picture. Leveraging advanced molecular profiling, researchers are now tailoring treatments to the unique genetic makeup of tumors, with targeted therapies showing promise. Innovations such as NTRK inhibitors for NTRK-rearranged sarcomas and gamma-secretase inhibitors for desmoid tumors are changing clinical practices. The rise of immunotherapy, including novel agents like LAG-3 inhibitors and bifunctional proteins that target both TGF-β and PD-L1, offers new avenues for treatment, particularly when combined with traditional therapies like chemotherapy. Meanwhile, the approval of epigenetic treatments for specific sarcoma subtypes heralds a new wave of strategy based on histological specificity, which could lead to more personalized and effective care. While challenges remain, the field of STS treatment is evolving, driven by a deeper understanding of the disease's biological underpinnings and a commitment to innovative research approaches.

Abstract:Despite the low incidence of soft tissue sarcomas (STSs), hundreds of thousands of new STS cases are diagnosed annually worldwide, and approximately half of them eventually progress to advanced stages. Currently, chemotherapy is the first-line treatment for advanced STSs. There are difficulties in selecting appropriate drugs for multiline chemotherapy, or for combination treatment of different STS histological subtypes. In this study, we first comprehensively reviewed the efficacy of various chemotherapeutic drugs in the treatment of STSs, and then described the current status of sensitive drugs for different STS subtypes. anthracyclines are the most important systemic treatment for advanced STSs. Ifosfamide, trabectedin, gemcitabine, taxanes, dacarbazine, and eribulin exhibit certain activities in STSs. Vinca alkaloid agents (vindesine, vinblastine, vinorelbine, vincristine) have important therapeutic effects in specific STS subtypes, such as rhabdomyosarcoma and Ewing sarcoma family tumors, whereas their activity in other subtypes is weak. Other chemotherapeutic drugs (methotrexate, cisplatin, etoposide, pemetrexed) have weak efficacy in STSs and are rarely used. It is necessary to select specific second- or above-line chemotherapeutic drugs depending on the histological subtype. This review aims to provide a reference for the selection of chemotherapeutic drugs for multi-line therapy for patients with advanced STSs who have an increasingly long survival.
