Abstract:The frequent occurrence of persistent or relapsed disease after induction chemotherapy in AML necessitates a better understanding of the clonal relationship of AML in various disease phases. In this study, we used SNP 6.0 array-based genomic profiling of acquired copy number aberrations (aCNA) and copy neutral LOH (cnLOH) together with sequence analysis of recurrently mutated genes to characterize paired AML genomes. We analyzed 28 AML sample pairs from patients who achieved complete remission with chemotherapy and subsequently relapsed and 11 sample pairs from patients with persistent disease after induction chemotherapy. Through review of aCNA/cnLOH and gene mutation profiles in informative cases, we demonstrate that relapsed AML invariably represents re-emergence or evolution of a founder clone. Furthermore, all individual aCNA or cnLOH detected at presentation persisted at relapse indicating that this lesion type is proximally involved in AML evolution. Analysis of informative paired persistent AML disease samples uncovered cases with 2 coexisting dominant clones of which at least one was chemotherapy sensitive and one resistant, respectively. These data support the conclusion that incomplete eradication of AML founder clones rather than stochastic emergence of fully unrelated novel clones underlies AML relapse and persistence with direct implications for clinical AML research.

Abstract:Leukemia is a polyclonal and progressive disease with drastic intra-clone heterogeneity. During the early stages of disease development, it is mostly shaped by the deterministic effects of key initiating events, which could establish the roles of genomic instability and cell plasticity in clonal evolution. Later, preleukemic cells acquire successive genetic mutations, undergoing distinct evolutionary trajectories. In this review, we summarize the current understanding of how genetic lesions define distinct clonal architectures. We further highlight two classical evolutionary models and their relevant prognostic implications. Given that drug selection pressure remains a major driving force of relapse, we also discuss recurrent patterns of clonal evolution under chemotherapy and targeted therapy. Understanding the rationale for directing distinct clonal evolution patterns will be instrumental in the development of different therapeutic strategies to prevent or overcome drug resistance and relapse during disease progression.

Abstract:Abstract Clonal diversity is a consequence of cancer cell evolution driven by Darwinian selection. Precise characterization of clonal architecture is essential to understand the evolutionary history of tumor development and its association with treatment resistance. Here, using a single-cell DNA sequencing, we report the clonal architecture and mutational histories of 123 acute myeloid leukemia (AML) patients. The single-cell data reveals cell-level mutation co-occurrence and enables reconstruction of mutational histories characterized by linear and branching patterns of clonal evolution, with the latter including convergent evolution. Through xenotransplantion, we show leukemia initiating capabilities of individual subclones evolving in parallel. Also, by simultaneous single-cell DNA and cell surface protein analysis, we illustrate both genetic and phenotypic evolution in AML. Lastly, single-cell analysis of longitudinal samples reveals underlying evolutionary process of therapeutic resistance. Together, these data unravel clonal diversity and evolution patterns of AML, and highlight their clinical relevance in the era of precision medicine.

Meanwhile, later mutations in genes such as and , have been shown to contract at remission and in the case of often are absent at relapse. We sought to understand how early CH mutations influence subsequent evolutionary trajectories throughout remission and relapse in response to induction chemotherapy. Here, we assembled a retrospective cohort of patients diagnosed with AML at our institution that underwent genomic sequencing at diagnosis as well as at the time of remission and/or relapse (total n = 182 patients). Corroborating prior studies, and mutations were generally eliminated at the time of cytologic complete remission but subsequently reemerged upon relapse, whereas , and mutations often persisted through remission. Early CH-related mutations exhibited distinct constellations of co-occurring genetic alterations, with and mutations enriched in AML, while and mutations were enriched in and AML, respectively. In the case of and mutations, these differences vanished at the time of complete remission yet readily reemerged upon relapse, indicating the reproducible nature of these genetic interactions. Thus, early CH-associated mutations that precede malignant transformation subsequently shape the evolutionary trajectories of AML through diagnosis, therapy, and relapse.

Abstract:Background: Adults with acute myeloid leukemia with normal cytogenetics (AML-CN) have variable relapse risks. New mutations correlated with relapse risk are needed. Understanding clonal trajectory of relapse in this setting may improve understanding of leukaemia development. Aims: Interrogate genomic landscape and clone trajectory data in adults with CN-AML and identify new mutations associated with relapse risk. Methods: 333 consecutive, newly-diagnosed adults with CN-AML seen in Peking University Peoples Hospital March, 2006 and April, 2018 were studied. Whole-exome sequencing (WES) was done in 23 subjects with diagnosis, remission and relapse samples. Using these and published data we developed a customized panel of 322 AML-related genes. Diagnostic DNA samples of the remaining 310 subjects were studied and the coding regions or hotspot areas of the 322 genes were sequenced by deep target regional sequencing (TRS). Average effective sequencing depths were 200X (145–265X) for WES and 1500X (1000–1800X) for TRS. Median coverages were 99.9% (77.6, 99.9%) and 99.0% (98.0, 99.5%). Results: We identified 3672 non-synonymous somatic variants in 309 genes by TRS. In 305 subjects ≥5 mutations were identified (98%; 95% confidence interval [CI], 97, 100%), with a median of 13 mutations per subject (range, 2–27; Figure). We identified mutations at 4 new loci including DDX11 in 23 subjects (7%), CYP2F1(4%), NPIPA5 (3%) and MED12 (2%). We then interrogated the impact of DDX11 mutations by matching these subjects with 115 controls according to age (<60/≥60 y), NPM1(Y/N) and FLT3-ITD(Y/N) mutations and WBC (<100/≥100x10E+9/L). In multivariable analyses DDX11 mutation was independently-associated with cumulative incidence of relapse (CIR; Hazard Ratio [HR] = 2.15 [1.12, 4.13]; P = 0.021) and relapse-free survival (RFS; HR = 2.12 [1.03, 4.38];P = 0.042). Other mutations identified included driver genes in the RAS/RTK signaling pathway (27%), chromatin modifier pathway(10%), DNA-methylation pathway (8%), transcription factor pathway (8%), histone methylation pathway (5%), cohesin complex pathway (3%) and the spliceosome pathway (3%). NPM1 was the most frequent mutated gene in our cohort (N = 75; 24% [19, 29%]), followed by FLT3-ITD (N = 64; 21% [16, 25%]) and DNMT3A (N = 59; 19% [15, 23%]). Using triplicate samples from the 23 subjects we identified 3 clonal trajectory patterns: (1) Dominant clone at diagnosis and relapse without new mutations (N = 2); (2) Dominant clone at diagnosis with ≥1 new mutation (N = 13); and (3) evolution from a minor clone detected at diagnosis (N = 8).The clonal trajectory of relapse after chemotherapy was like that posttransplant but the latter was associated with more relapse-specific sub-clones possibly related to intensive pretransplant conditioning.Summary/Conclusion: We identified 4 new mutated genes in AML-CN and show DDX11 mutations correlate with a higher CIR and worse RFS in multivariable analyses. We also detected 3 distinct clonal trajectories which provide insight into mechanisms of relapse after chemotherapy and transplantation and the pathogenesis of AML-CN.

Abstract:Chronic lymphocytic leukaemia (CLL) is a frequent B-cell malignancy, characterized by recurrent somatic chromosome alterations and a low level of point mutations. Here we present single-nucleotide polymorphism microarray analyses of a single CLL patient over 29 years of observation and treatment, and transcriptome and whole-genome sequencing at selected time points. We identify chromosome alterations 13q14 -, 6q - and 12q+ in early cell clones, elimination of clonal populations following therapy, and subsequent appearance of a clone containing trisomy 12 and chromosome 10 copy-neutral loss of heterogeneity that marks a major population dominant at death. Serial single-cell RNA sequencing reveals an expression pattern with high FOS, JUN and KLF4 at disease acceleration, which resolves following therapy, but reoccurs following relapse and death. Transcriptome evolution indicates complex changes in expression occur over time. In conclusion, CLL can evolve gradually during indolent phases, and undergo rapid changes following therapy.

Abstract:Background Acute myeloid leukemia (AML) is characterized by clonal heterogeneity, leading to frequent relapses and drug resistance despite intensive clinical therapy. Although AML's clonal architecture has been addressed in many studies, practical monitoring of dynamic changes in those subclones during relapse and treatment is still understudied. Method Fifteen longitudinal bone marrow (BM) samples were collected from three relapsed and refractory (R/R) AML patients. Using droplet digital polymerase chain reaction (ddPCR), the frequencies of patient's leukemic variants were assessed in seven cell populations that were isolated from each BM sample based on cellular phenotypes. By quantifying mutant clones at the diagnosis, remission, and relapse stages, the distribution of AML subclones was sequentially monitored. Results Minimal residual (MR) leukemic subclones exhibit heterogeneous distribution among BM cell populations, including mature leukocyte populations. During AML progression, these subclones undergo active phenotypic transitions and repopulate into distinct cell population regardless of normal hematopoiesis hierarchic order. Of these, MR subclones in progenitor populations of patient BM predominantly carry MR leukemic properties, leading to more robust expansion and stubborn persistence than those in mature populations. Moreover, a minor subset of MR leukemic subclones could be sustained at an extremely low frequency without clonal expansion during relapse. Conclusions In this study, we observed treatment persistent MR leukemic subclones and their phenotypic changes during the treatment process of R/R AML patients. This underscores the importance of preemptive inhibition of clonal promiscuity in R/R AML, proposing a practical method for monitoring AML MR subclones.

Abstract:Despite improvements in therapeutic protocols and in risk stratification, acute myeloid leukemia (AML) remains the leading cause of childhood leukemic mortality. Indeed, the overall survival accounts for ~70% but still ~30% of pediatric patients experience relapse, with poor response to conventional chemotherapy. Thus, there is an urgent need to improve diagnosis and treatment efficacy prediction in the context of this disease. Nowadays, in the era of high throughput techniques, AML has emerged as an extremely heterogeneous disease from a genetic point of view. Different subclones characterized by specific molecular profiles display different degrees of susceptibility to conventional treatments. In this review, we describe in detail this genetic heterogeneity of pediatric AML and how it is linked to relapse in terms of clonal evolution. We highlight some innovative tools to characterize minor subclones that could help to enhance diagnosis and a preclinical model suitable for drugs screening. The final ambition of research is represented by targeted therapy, which could improve the prognosis of pediatric AML patients, as well as to limit the side toxicity of current treatments.

Abstract:Abstract Abstract 3870 Introduction: The phenotypic characterization of dominant CLL clones at various disease stages using multiple experimental approaches has deepened our understanding of CLL biology and clinical behavior. However, much less is known about the phenotype and phenotypic changes that occur in CLL over time and with disease progression when measured longitudinally. Analysis of large CLL cohorts at various disease time points is needed to better capture the dynamics of this disease. Methods: A total of 143 CLL cases were surveyed at trial enrollment and longitudinally for changes in acquired genomic copy number aberrations (aCNA), acquired uniparental disomy (aUPD), and somatically acquired mutations in the genes TP53, SF3B1, and NOTCH1. On an unselected basis, longitudinal samples were studied at a minimum of 24 months after initial sample procurement in the case of no therapeutic intervention or at the time of relapse after chemotherapy. The median time span between enrollment and longitudinal samples was 40.5 months for samples without intervening therapy (range 9–79 mos.) and 42 months for relapsed cases (range 7–74 mos.). One hundred eight cases did not receive therapeutic interventions in-between sample procurements, while 35 were relapsed from therapy. In 13 cases, time spans less than 24 months were allowed if the patient carried a high-risk CLL genomic feature or if limited follow-up samples were available. Cells were purified by flow cytometry sorting of viable CD19+ and CD3+ cells. DNA samples isolated from paired tumor and normal samples were studied for aCNA using Affymetrix SNP 6.0 arrays and the software program dChip. Samples were also analyzed for LOH/aUPD via internally-developed software. aCNA were scored through visual interpretations of the dChip display of paired samples and corroborated through an algorithmic lesion calling method. Finally, the genes TP53 exons 2–10, SF3B1 exons 13–17, and NOTCH1 exons 34 were resequenced in all samples. Results: The dominant phenomenon observed in this large CLL cohort was genomic stability for aCNA and gene mutations, which was detected in 116 (81%) of cases analyzed. Importantly however, 27 (19%) of CLL cases differed between enrollment and longitudinal follow-up. Within this subgroup of cases, 21 cases harbored changes in aCNA/LOH/aUPD, while 7 cases carried a change in the mutation status of TP53, NOTCH1 or SF3B1 (one case had both aCNA and mutation changes). Specifically, 19 cases with aCNA/LOH/aUPD changes added lesions at longitudinal analysis and lost none: clear examples of clonal evolution. Four cases added TP53 mutations and two cases added a NOTCH1 mutation. Conversely, TP53 mutations were never lost, while one case with a novel TP53 mutation lost a pre-existing NOTCH1 mutation and one case lost a SF3B1 mutation. No case gained a SF3B1 mutation. Only one CLL case at relapse demonstrated an aCNA/LOH/aUPD pattern that was completely unrelated to the dominant CLL clone at enrollment. Conclusion: In this report, we describe genomic changes in a cohort of 143 CLL cases analyzed longitudinally. This study constitutes the largest analysis of such changes reported to date. Our overall estimates of genomic instability in CLL are likely underestimates of the true frequency, given the observed natural selection against aggressive CLL inherent in this type of analysis. From this data several conclusions can be reached that have implications for our understanding of CLL clonal dynamics and the effects of therapy on clonal evolution/emergence: i) the vast majority of genomic changes are acquisitions of new lesions/mutations combined with the retention of pre-existing lesions–therefore persistence of antecedent clones underlies CLL relapse and disease persistence; ii) CLL cases demonstrating clonal evolution were enriched for cases that received intervening chemotherapy (∼50% in this cohort); iii) the acquisition of TP53 mutations followed intervening chemotherapy, providing further impetus to carefully select patients for chemotherapy; iv) NOTCH1 mutations were rarely acquired over time and in one case lost; v) no SF3B1 mutations were acquired over time and in one case lost; vi) the latter two observations argue against a strong driver role for these mutations in CLL progression/relapse; vii) the acquisition of aCNA/LOH/aUPD and gene mutation was non-overlapping, indicative of distinct roles for each aberration type in CLL evolution. Abstract:Normal hematopoietic stem and progenitor cells (HSPCs) inherently accumulate somatic mutations and lose clonal diversity with age, processes implicated in the development of myeloid malignancies1. The impact of exogenous stressors, such as cancer chemotherapies, on the genomic integrity and clonal dynamics of normal HSPCs is not well defined. We conducted whole-genome sequencing on 1,032 single-cell-derived HSPC colonies from 10 patients with multiple myeloma (MM), who had undergone various chemotherapy regimens. Our findings reveal that melphalan treatment distinctly increases mutational burden with a unique mutation signature, whereas other MM chemotherapies do not significantly affect the normal mutation rate of HSPCs. Among these therapy-induced mutations were several oncogenic drivers such as TET2 and PPM1D. Phylogenetic analysis showed a clonal architecture in post-treatment HSPCs characterized by extensive convergent evolution of mutations in genes such as TP53 and PPM1D. Consequently, the clonal diversity and structure of post-treatment HSPCs mirror those observed in normal elderly individuals, suggesting an accelerated clonal aging due to chemotherapy. Furthermore, analysis of matched therapy-related myeloid neoplasm (t-MN) samples, which occurred 1-8 years later, enabled us to trace the clonal origin of t-MNs to a single HSPC clone among a group of clones with competing malignant potential, indicating the critical role of secondary mutations in dictating clonal dominance and malignant transformation. Our findings suggest that cancer chemotherapy promotes an oligoclonal architecture with multiple HSPC clones possessing competing leukemic potentials, setting the stage for the selective emergence of a singular clone that evolves into t-MNs after acquiring secondary mutations. These results underscore the importance of further systematic research to elucidate the long-term hematological consequences of cancer chemotherapy.

Abstract:Abstract Chemotherapy/ targeted therapy are both known to trigger evolution of treatment resistant clones that can lead to relapse. Allogeneic stem cell transplant (alloSCT) for refractory Chronic Lymphocytic Leukemia (CLL) patients is associated with better outcomes. We hypothesized that allogeneic T-cell immunotherapies, including alloSCT and donor lymphocyte infusion (DLI) would impact tumor evolution through the application of selective immunologic pressure with reciprocal changes in the T-cell compartment. Here, we tested a cohort of 24 heavily pre-treated CLL patients treated. Treatments consisted of alloSCT alone, or with follow-up DLI, which are two established mediators of effective Graft versus Leukemia (GVL). Our cohort included 11 patients who relapsed (denoted as non-responder, NR) after alloSCT and 13 patients who had complete response (CR) after alloSCT, with 11/13 patients showing durable CR with a median post-transplant overall survival (OS) of 9.8 years. We mapped the evolutionary trajectories of tumor cells by whole exome sequencing (WES) of sort purified CLL in post-transplant relapsed patients. To investigate changes in immune repertoire and gene expression post-transplant, CD3 positive T-cells from peripheral blood and bone marrows of CLL patients at complete donor chimerism were analyzed both at bulk and at the single cell level. We found evidence of subclonal leukemic evolution in the majority of our CLL patient cohort after nonmyeloablative HLA-matched alloSCT. Different patterns of CLL evolution were observed, and these changes included putative CLL drivers in every case. In all of the 11 patients with longitudinal post-alloSCT samples available, we observed branched CLL evolution in 4 patients, linear evolution in 4 patients, and no evolution in 3 patients. These data suggest that differential sensitivity of leukemic subclones to allogeneic T cell killing may underlie the branched and linear evolution that we observed, and therefore can shape leukemic subclonal architecture after transplant. Of note, we found that clonal CLL was more responsive to alloSCT in comparison to CLL with subclonal disease architecture.To identify T-cells with GVL potential, we first cataloged potential neoantigens by screening mutated regions in CLL with in silico HLA binding prediction models. Neoantigen specific T-cells were then sorted from longitudinal peripheral blood samples using tetramers, followed by identification of GVL specific TCR in both bulk and single cell setting. We were able to identify T-cells that coevolved with specific tumorigenic lesions in a subset of CLL patients. Taken together, our results suggest that donor-derived antigen-specific T-cells mediate clonal selection of CLL with concurrent changes in allogeneic T-cells, and that these changes can be monitored in longitudinal patient samples. Abstract:Acute myeloid leukaemia (AML) is an uncontrolled clonal proliferation of abnormal myeloid progenitor cells in the bone marrow and blood. Advances in cancer genomics have revealed the spectrum of somatic mutations that give rise to human AML and drawn our attention to its molecular evolution and clonal architecture. It is now evident that most AML genomes harbour small numbers of mutations, which are acquired in a stepwise manner. This characteristic, combined with our ability to identify mutations in individual leukaemic cells and our detailed understanding of normal human and murine haematopoiesis, makes AML an excellent model for understanding the principles of cancer evolution. Furthermore, a better understanding of how AML evolves can help us devise strategies to improve the therapy and prognosis of AML patients. Here, we draw from recent advances in genomics, clinical studies and experimental models to describe the current knowledge of the clonal evolution of AML and its implications for the biology and treatment of leukaemias and other cancers.

Abstract:Abstract Background Multiple myeloma is a malignancy of clone plasma cells that were composed of various multisteps development and had different molecular genetical variation in different phases in the course of disease. Clonal evolution promotes the disease development from prophase to advanced stage. We investigated the paths of clonal evolution in a cohort myeloma patients by a unique multicolor FISH (mFISH) to identify the clonal architecture of different time points, and first reported the survival and prognostic value of various clonal evolution paths. Methods Totally 85 myeloma patients with CD138+ cells at diagnosis versous relapse or former relapse versous latter relapse were enrolled in this study between January 2004 and December 2014. Firstly, the clonal evolution were analyzed by conventional iFISH (cFISH) with a panel probes of 1q21, RB-1 and TP53. The clonal architecture of different time points were detected by mFISH in 62 samples from different time points of selected 25 patients among the 85 patients. The survival significance and prognostic value of 3 different clonal evolution paths identified by mFISH were analyzed. Results: 1). Clonal evolution were detected in 34/85(40%) cases by cFISH analysis. The survival analysis showed that patients with clonal evolution had significant shorter over survival (OS) than those with clonal stabilization. The meadian OS of these 2 groups were 42.67 (95% CI:32.15-53.19) and 71.46 (95%CI:58.99-83.93) months respectively, the difference between these 2 groups was significant (p=0.010). Comparing the survival since post relapse/clonal evolution, significant difference (p=0.003) also existed in these 2 groups with a median survival of 14.79 (95% CI:10.01-19.56) months and 37.61 (95%CI:27.30-47.92) months respectively. 2). Three clonal evolution paths were identified by mFISH in different phase samples from 25 myeloma patients. 36% (9/25) of them were clonal stabilization, 32% (8/25) were linear clonal evolution, 32% (8/25) were branching clonal evolution. mFISH had more powerful efficacy of clonal evolution detection than cFISH. Of these 25 cases, 14 cases were clonal stabilization and 11 were clonal evolution identified by cFISH. 7/14 (50%) with clonal stabilization by cFISH were identified into clonal evolution by mFISH, 1 with linear evolution and 6 with branching evolution. The 11 cases with clonal evolution identified by cFISH were further classified into 7 with linear evolution and 4 with branching evolution by mFISH detection. 3). Our results indicated that clonal evolution type had significant impact on survival of myeloma patients. The median OS of patients with clonal stabilization, linear evolution and branching evolution were 87.00 (95%CI:74.53-99.47), 24.99 (95% CI:16.58-33.39) and 35.70 (95% CI:27.10-44.30) months respectively. There were significant difference among these 3 groups (p=0.004). The median survival post relapse/clonal evolution in these 3 groups were 34.00 (95%CI:20.14-47.86), 8.06 (95% CI:4.91-11.22) and 22.75 (95% CI:13.49-32.01) months, the difference among them was significant (p=0.007). Conclusions mFISH is a simple and clinical practicable tool for clonal detection and analysis of myeloma. Our results firstly demonstrated that the clonal evolution type had significant impact on survival of patients with myeloma. According to the 3 different clonal evolution paths, individualized treatment strategy would be required for different patients with myeloma. Disclosures No relevant conflicts of interest to declare.

Abstract:Abstract The impact of clonal heterogeneity on disease behavior or drug response in acute myeloid leukemia remains poorly understood. Using a cohort of 2,829 patients, we identify features of clonality associated with clinical features and drug sensitivities. High variant allele frequency for 7 mutations (including NRAS and TET2 ) associate with dismal prognosis; elevated GATA2 variant allele frequency correlates with better outcomes. Clinical features such as white blood cell count and blast percentage correlate with the subclonal abundance of mutations such as TP53 and IDH1 . Furthermore, patients with cohesin mutations occurring before NPM1 , or transcription factor mutations occurring before splicing factor mutations, show shorter survival. Surprisingly, a branched pattern of clonal evolution is associated with superior clinical outcomes. Finally, several mutations (including NRAS and IDH1 ) predict drug sensitivity based on their subclonal abundance. Together, these results demonstrate the importance of assessing clonal heterogeneity with implications for prognosis and actionable biomarkers for therapy.

Abstract:Relapsed acute lymphoblastic leukaemia (ALL) is associated with resistance to chemotherapy and poor prognosis. Gain-of-function mutations in the 5'-nucleotidase, cytosolic II (NT5C2) gene induce resistance to 6-mercaptopurine and are selectively present in relapsed ALL. Yet, the mechanisms involved in NT5C2 mutation-driven clonal evolution during the initiation of leukaemia, disease progression and relapse remain unknown. Here we use a conditional-and-inducible leukaemia model to demonstrate that expression of NT5C2(R367Q), a highly prevalent relapsed-ALL NT5C2 mutation, induces resistance to chemotherapy with 6-mercaptopurine at the cost of impaired leukaemia cell growth and leukaemia-initiating cell activity. The loss-of-fitness phenotype of NT5C2+/R367Q mutant cells is associated with excess export of purines to the extracellular space and depletion of the intracellular purine-nucleotide pool. Consequently, blocking guanosine synthesis by inhibition of inosine-5'-monophosphate dehydrogenase (IMPDH) induced increased cytotoxicity against NT5C2-mutant leukaemia lymphoblasts. These results identify the fitness cost of NT5C2 mutation and resistance to chemotherapy as key evolutionary drivers that shape clonal evolution in relapsed ALL and support a role for IMPDH inhibition in the treatment of ALL.

Abstract:Acute myeloid leukemia (AML) is a hematological malignancy characterized by cytogenetic and genomic alterations. Up to now, combination chemotherapy remains the standard treatment for leukemia. However, many individuals diagnosed with AML develop chemotherapeutic resistance and relapse. Recently, it has been pointed out that leukemic stem cells (LSCs) are the fundamental cause of drug resistance and AML relapse. LSCs only account for a small subpopulation of all leukemic cells, but possess stem cell properties, including a self-renewal capacity and a multi-directional differentiation potential. LSCs reside in a mostly quiescent state and are insensitive to chemotherapeutic agents. When LSCs reside in a bone marrow microenvironment (BMM) favorable to their survival, they engage into a steady, continuous clonal evolution to better adapt to the action of chemotherapy. Most chemotherapeutic drugs can only eliminate LSC-derived clones, reducing the number of leukemic cells in the BM to a normal range in order to achieve complete remission (CR). LSCs hidden in the BM niche can hardly be targeted or eradicated, leading to drug resistance and AML relapse. Understanding the relationship between LSCs, the BMM, and the generation and evolution laws of LSCs can facilitate the development of effective therapeutic targets and increase the efficiency of LSCs elimination in AML.

Abstract:Abstract Although clonal hematopoiesis (CH) can precede the development of acute myeloid leukemia (AML), it can also persist after achieving remission. Long-term clonal dynamics and clinical implications of persistent CH are not well understood. Here, we studied the prevalence, dynamics, and clinical implications of postremission CH in 164 AML patients who attained complete remission after induction chemotherapies. Postremission CH was identified in 79 (48%) patients. Postremission CH persisted long term in 91% of the trackable patients despite treatment with various types of consolidation and maintenance therapies. Postremission CH was eradicated in 20 out of 21 (95%) patients who underwent allogeneic stem cell transplant. Although patients with postremission CH as a group had comparable hematopoiesis with those without it, patients with persistent TET2 mutations showed significant neutropenia long term. Postremission CH had little impact on relapse risk, nonrelapse mortality, and incidence of atherosclerotic cardiovascular disease, although the clinical impact of post-CR CH was heterogeneous among different mutations. These data suggest that although residual clonal hematopoietic stem cells are generally resistant to consolidation and maintenance therapies, they retain the ability to maintain normal hematopoiesis and have little impact on clinical outcomes. Larger study is needed to dissect the gene-specific heterogeneity.
