Abstract:Gingival fibromatosis is a rare disease, especially its syndromic form. Here, we review the literatures on gingival fibromatosis and briefly summarize some characters on clinical, etiological, genetic and histopathological aspects. We also present a rare case of gingival fibromatosis with multiple unusual findings in a 21-year-old man. And we differentiate it from some well-known syndromes including gingival fibromatosis. Maybe it implies a new syndrome within the spectrum of those including gingival fibromatosis.
Solitary S-100-positive Congenital Granular Cell Tumor of the Tongue: a Case Report and Literature Review.

Abstract:Congenital granular cell tumors (CGCTs), which are rare benign lesions in newborns, locate predominantly on the anterior maxillary alveolar ridge of female neonates. However, this lesion rarely occurs on the tongue, and only 9 cases have been reported in the English literature. Of the 9 cases, which have been tested for S-100 protein, 4 were immunonegative to S-100 protein. In this present case, we reported a rare case of a 4-day-old Chinese girl with a CGCT on the anterior dorsum of the tongue. The lesion was excised under general anesthesia when the newborn was 3 months old. Histologically, pseudoepitheliomatous hyperplasia of the overlying mucosa was noted in this lesion. Immunohistochemical stains showed that the granular cells were diffusely strongly positive to S-100 protein, neuron-specific enolase, and vimentin. Depending on these findings, it was diagnosed as CGCT.
Gingival Plasma Cell Granuloma: A Case Report of Multiple Lesions

Abstract:Background: Plasma cell granuloma (PCG) is a rare benign pseudotumorous proliferation of unclear etiology that is mainly situated in the lungs. Gingival PCG is an even more peculiar lesion that usually occurs in middle-aged or elderly individuals and clinically manifests as a solitary entity. Case report: A 15-year-old male with no underlying medical conditions presented with multiple gingival masses in the right maxilla, which were initially thought to be epulis. The lesions were resected completely and the excisional biopsies sent for histological examination. Immunohistochemical (IHC) stain revealed dense polyclonal plasma cell infiltration with positive expression of both kappa and lambda light chains, confirming a diagnosis of gingival PCG. Subsequently, the affected gingiva healed uneventfully, with no sign of recurrence over 2 years of follow-up. Conclusions: The present report depicts an extremely unusual case of gingival PCG occurring in a juvenile with multiple lesions, which is worth attention in clinical pediatric dentistry. Excisional-biopsy and histological investigations are imperative for a confirmative diagnosis and to exclude potential aggressive conditions. Complete resection of lesions seems to be a valid treatment, while long-term clinical follow-up is still needed.

Abstract:Background: Malignant peripheral nerve sheath tumors (MPNST) are rare neoplasms, usually arising from peripheral nerves or showing a nerve sheath differentiation. Primary MPNSTs of the scalp is exceptionally rare, and only sporadic cases have been reported recently. Objectives: Report a rare case of giant malignant peripheral nerve sheath tumor (MPNST) beneath occipital scalp, and discuss how to-treat with this kind of tumor. Methods: Descriptive study of a rare case of giant peripheral nerve sheath tumors of occipital scalp without adjuvant treatment with nine months follow up. Results: In a 52-year-old man with MPNSTs beneath occipital scalp, the tumor was treated with complete surgical resection. Histological examination proved that the lesion was a scalp MPNST. The patient was followed up asymptomatic for the following nine months after surgical resection without adjuvant radiotherapy. Conclusion: MPNSTs beneath the occipital scalp should be treated individually, for those well-circumscribed MPNSTs without bone destruction or brain invasion (low-grade tumors), complete surgical resection with clear margins (if possible) is recommended. Otherwise, adjuvant postoperative radiotherapy is necessary.

Abstract:Pregnancy gingivitis is an acute form of gingivitis that affects pregnant women, with a prevalence of 30%, possibly ranging up to 100%. Sometimes, pregnancy gingivitis shows a tendency toward a localized hyperplasia called gingival pyogenic granuloma. Pregnancy tumor is a benign gingival hyperplasia with the gingiva as the most commonly involved site, but rarely it involves almost the entire gingiva. A 22-year-old woman was referred to our clinic with a chief complaint of gingival swelling that had lasted for 2 days. The lesions progressed rapidly and extensively, and almost all the gingiva was involved a week later. Generalized erythema, edema, hyperplasia, a hemorrhagic tendency, and several typical hemangiomatous masses were noted. Pregnancy was denied by the patient at the first and second visits, but was confirmed 2 weeks after the primary visit. The patient was given oral hygiene instructions. She recovered well, and the mass gradually regressed and had disappeared completely at the end of 12 weeks of pregnancy, without recurrence. The gingival lesions were finally diagnosed as multiple gingival pregnancy tumors. The patient delivered a healthy infant. An extensive and rapid growth of gingival pregnancy tumors during the early first month of pregnancy is a rare occurrence that is not familiar to dentists, gynecologists, and obstetricians. Those practitioners engaged in oral medicine and periodontology, primary care obstetrics, and gynecology should be aware of such gingival lesions to avoid misdiagnosis and overtreatment.
Primary Malignant Myoepithelioma of the Left Maxillary Sinus: a Case Report.

Abstract:We describe the case of a 41-year-old woman who presented with a malignant myoepithelioma (MME) in her left maxillary sinus. Exploratory biopsy of the left maxillary sinus was performed and pathological examination demonstrated that the tumour was positive for calponin and cytokeratin 14, which are indicative of MME. Lateral rhinotomy and left total maxillectomy were undertaken and the patient received radiotherapy and chemotherapy post-surgery. Primary recurrence and metastasis to the left angle of the mandible occurred 9 months after the surgery. The patient died of cachexia 13 months after the surgery.

Abstract:Lymphatic malformation is a benign lesion, seldom affecting the gingiva. Gingival lesions are characterized by pebbly hyperplasia, occasional pain, and bleeding. The treatment for large and exceptional areas of involvement may face difficulties. Herein we report a rare case of gingival lymphatic malformation in a 10-year-old girl.

Malignant Peripheral Nerve Sheath Tumors in Oral Maxillofacial Region:Clinical Analysis of 11 Cases

Abstract:OBJECTIVE:To investigate the clinicopathologic features, treatment and prognosis of the malignant peripheral nerve sheath tumors (MPNST) in oral and maxillofacial region.METHODS:From 1987 to 2008, 11 patients underwent surgical treatment. We retrospectively reviewed clinical character, treatment and follow-up data and performed a literature review.RESULTS:The most common symptom of the patients was an enlarging mass. Five patients felt numb and/or pain. The immunohistochemical staining showed positive for SMA (5/11), S-100 (7/11), p53 (5/11), Ki-67 (11/11) and vimentin (11/11). The 2-year and 5-year survival rates of the patients were 30% and 9.5% respectively, and the average survival time was 33.9 months.CONCLUSION:MPNST is one of the most aggressive tumors with high risk of local recurrence and distant metastasis. Mulitimodal therapy including surgical resection and adjuvant radiotherapy is needed. Complete removal of the tumor is the main modality of treatment and a most important prognostic factor of MPNST.

Abstract:Peripheral primitive neuroectodermal tumor (pPNET) is a rare and highly malignant undifferentiated tumor, which presents in infants and young adults. pPNETs in the head and neck region are uncommon and have a varying incidence of occurrence. Peripheral PNETs of the maxilla and mandible are particularly rare. At present, only 16 cases of pPNET of the maxilla and 13 cases of pPNET of the mandible have been reported. The present study describes a case of pPNET of the maxilla in a 16-year-old male and a case of pPNET of the mandible in another 16-year-old male. The present study reports the radiological findings and the clinical courses of the two patients.

Abstract:Abstract Neoplasms of the parotid glands constitute about 6% of head and neck tumours, the most common of which are: adenoma multiforme and Warthin’s tumor. Schwannoma is benign, encapsulated tumor of the nerve cells (lemocytes, Schwann cells), most often it occurs in the trunk, head, extremely rarely observed in the parotid gland. 9% Schwannomas derives from the facial nerve sheath, constituting from 0.5% -1.2% of all salivary gland tumors. The literature describes 80 cases of intraparotid Schwannoma. Malignant Schwannoma (MPNST) account for 5% to 10% of all soft tissue sarcomas. High-grade MPNST tumors are aggressive, with a tendency to relapse and metastasis. We present a case report of a 84-year-old female patient presented to the Head and Neck Oncology Clinic of the Medical University of Lodz, due to the painful tumor of the left parotid region. The tumor appeared six months before hospitalization. During the physical examination, there was a polycyclic tumor with reduced mobility, the skin on the tumor was slightly red, lymph nodes uninvolved, facial nerve function preserved. BACC revealed tumour tissue composed of sheets and spindle-shaped cells. The patient was qualified for surgical treatment. Under the general anesthesia the superficial lobe of the parotid gland along with the tumor was removed. After the procedure, no facial nerve palsy was found. The patient did well post-operatively and was discharged home on the 8th day after surgery. Immunohistochemical staining showed the tumour cells to be diffusely and strongly immunoreactive for S-100 protein and Ki67 40-50%. Strong and diffuse staining for S-100 protein were consistent with the malignant peripheral nerve sheath tumour (MPNST). The patient was referred for radiotherapy, due to the postoperative outcome and advanced age, no adjuvant treatment was proposed. The patient has been in observation for 1 year. No relapse was found in the follow-up studies.
[A midline neurofibroma, clinically manifested as a mandibular gingival hyperplasia in a seven-year-old girl whose mother suffers from neurofibromatosis]

Abstract:A seven-year-old girl was referred to the Department of Periodontology of the Semmelweis University with a symmetric bilateral, painless, non-inflammatory diffuse enlargement on the lingual aspects of her lower jaw. The family history revealed that her mother and elder sister had Recklinghausen's disease with typical characteristic dermatological signs and they are registered by the National NF Registry. Extraoral examination revealed an evident protrusion of the lips. Intraorally, the buccal gingiva around the upper and lower teeth appeared normal. The little patient had neither dermal nor oral mucous membrane signs characteristic of NF1. With the consent of her parents, the lingual firm mass of gingival enlargement was excised under local anesthesia and the removed tissue was histologically analyzed. The histology approved the clinical diagnosis as solitary neurofibroma. The postoperative healing was uneventful. Because of her very young age and the subtotal excision of the tumor, the patient has been regularly monitored. After one and a half years, recurrence or other sign of tumor regrowth have not been observed. The panoramic radiograph showed normal bone morphology and an age-related dental status with mixed dentition and undisturbed tooth eruption. The present case with a midline bilateral diffuse gingival overgrowth in the mouth of a 7-year-old girl without any characteristic dermatological signs in a family with genetically proven Recklinghausen's disease is very rare and unique in the literature. Malignant Nerve Sheath Tumour - A Case Report

Abstract:Malignant nerve sheath tumors are rare and aggressive soft tissue sarcomas. They contribute to 5-10% of all soft tissue sarcomas. They can be sporadic, occur in patients with NF1 (neurofibromatosis 1) or can occur after radiation therapy. A high rate of recurrence and hematogenous metastasis is seen in these patients. They are also associated with poor prognosis. A case of malignant nerve sheath tumor seen in a 44-year-old male with pre-existing NF1 is being discussed here due to the unique nature of the disease and its rarity.
A rare Encounter with Pediatric Malignant Peripheral Nerve Sheath Tumor in Nasal Sinuses and Orbit

Abstract:This case report explores a rare and aggressive Malignant Peripheral Nerve Sheath Tumor (MPNST) in a 7-year-old child affecting nasal sinuses, maxilla, and orbit, an exceptionally uncommon pediatric manifestation unrelated to Neurofibromatosis 1. The child presented with alarming symptoms-nasal obstruction, snoring, epistaxis, and difficulty swallowing-underscoring the case's urgency. Non-contrast computed tomography revealed an extensive mass infiltrating nasopharynx, nasal cavity, maxillary sinus, ethmoid sinuses, and orbit, causing destructive consequences. Histopathology confirmed a high-grade MPNST, marked by rapid growth and early metastasis, highlighting management challenges. The rarity of pediatric MPNST in the nasal cavity is discussed, emphasizing the need for a broad differential diagnosis. Treatment involves surgical resection and adjuvant chemoradiation with a grim prognosis due to diagnostic complexities and morphological mimicry in young patients.
Unusual Marginal Gingiva Squamous Cell Carcinoma Presenting As Localised Periodontitis — a Case Report

Abstract:Background: Gingival squamous cell carcinoma (SCC) is a malignant neoplasm, which presents 10% of diagnosed intraoral carcinoma. Due to proximity to dentition, it can be misdiagnosed as tooth-related inflammatory conditions, like periodontitis or endodontic-related diseases. In this study we report a case of marginal gingival SCC that mimicked a periodontal diseases in its early presentation. Objectives: A 64-year-old male complained about the toothache over lower left posterior area for one month. Clinical examination revealed deep pockets over tooth 46 47 lingual area with teeth mobility grade I but without gingiva swelling or pus discharge. After three months of periodontal therapy, tooth 47 was extracted but found the wound was unhealed. An incisional biopsy was obtained from the lesion. Methods: The diagnosis of SCC was made and we arrange the examination for the tumour staging. After discussed with the patient, the operation was done with supraomohyoid neck dissection of the regional lymph nodes, tumour wide excision with segmental mandibulectomy and fibula flap reconstruction. Postoperative concurrent chemoradiotherapy was also done. There was no sign of recurrence or metastasis six months postoperatively. Conclusion: Marginal gingival SCC easily mimics localised periodontal disease and can be misdiagnosed. In this instance dentists play an important role in early detection of the disease and should aware that lesions that do not respond normally to routine therapy should be biopsied.
Nonsyndromic Gingival Fibromatosis: A Rare Case Report

Abstract:Hereditary gingival fibromatosis (HGF) is an uncommon gingival disease of attached gingiva, which is manifested as localized or generalized form. The HGF inheritance is transmitted through both autosomal dominant and recessive modes. Here, we are discussing a rare case report of an 8-year-old child with gingival fibromatosis in mixed dentition, which caused damage to his speech, mastication, and esthetics and led to significant change in his facial profile. The patient noticed that the gingival enlargement was simultaneous with deciduous dentition eruption and gradually covered entire dentition. Gingival enlargement covered all teeth anteriorly and posteriorly and only occlusal surfaces were visible. The enlarged tissue was resected by the external bevel gingivectomy under general anesthesia arch wise. The postoperative healing was satisfactory, uneventful, and there was significant change in patient's esthetics. Patient has been kept on regular recall visits. Abstract:Malignant peripheral nerve sheath tumors (MPNSTs) of the cervix are rare, particularly in patients with neurofibromatosis type 1 (NF1). This report describes a cervical MPNST in an 18‐year‐old patient with no history of sexual activity, abnormal vaginal discharge, and prolonged menstruation. She had more than six café‐au‐lait spots on her body since birth and was diagnosed with NF1 at 2 years of age. Positron emission tomography‐computed tomography revealed a large pelvic mass and lung and bone metastases. Biopsy confirmed MPNST. Immunohistochemical staining showed diffuse positivity for CD10, approximately 30% positivity for cyclin D1, partial positivity for α‐SMA, desmin, and MyoD1, and negativity for myogenin, S‐100, and SOX‐10. A cancer gene panel identified several genetic abnormalities, but none were actionable mutations. Despite systemic chemotherapy, the tumor progressed rapidly, and the patient died 8 weeks post‐admission. Early diagnosis of MPNST is crucial. In patients with NF1, even mild symptoms can indicate MPNST.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Introduction: Malignant peripheral nerve sheath tumors (MPNSTs) are rare tumors that develop from peripheral nerve sheath cells and they account for approximately 5% to 10% of all soft-tissue sarcomas. MPNSTs in the head and neck region represent approximately 2% to 6% of all head and neck sarcomas and account for 12% to 19% of all MPNSTs, and the infratemporal fossa is a rare site for MPNSTs. MPNSTs originating from the trigeminal nerve are extremely rare. Case presentation: A 19-year-old female presented with a 6-month history of left-sided facial pain and paresthesia on the same side. On examination, there was left-sided facial paresthesia at the third trigeminal nerve (V3) areas; computed tomography scanning and magnetic resonance imaging showed an infratemporal lesion and surgical resection was done. Histological examination and the immunostaining finding showed high-grade MPNST. Conclusion: MPNSTs in the head and neck region may manifest with nonspecific symptoms. The diagnosis often requires a combination of clinical, pathological, and immunohistochemistry studies. Treatment involves total surgical resection with adjuvant radiotherapy.

Atypical cellular neurothekeoma of the oral mucosa: A rare case report and literature review

Abstract:Cellular neurothekeoma (CN) is a benign dermal neoplasm that most often affects the head and neck region and rarely occurs in the oral mucosa. We report a rare case of CN with atypical features on the floor of the mouth and summarize the reported cases of oral CN in English‐language literature. A 62‐year‐old woman presented with a 6‐month history of a painless mass on the floor of the mouth. Histopathological analysis of the excised specimen revealed a proliferation of neoplastic cells with oval to spindle morphology arranged in a vaguely nested and multinodular architecture separated by scarce hyaline collagen within a predominantly myxoid‐rich stroma. The tumor cells were positive for NSE, and CD63 (NKI/C3), and negative for S100 protein, CD34, and SMA. Thus, the final diagnosis was CN. In addition, we summarized all clinicopathological data on oral CNs reported in the English‐language literature. Nineteen cases were reviewed. Among them, only one case affected the floor of the mouth of a young girl, in contrast to the present case that occurred in an elderly woman. CN is particularly rare in this location and may be a diagnostic challenge for oral pathologists due to its rarity and morphological similarity with other lesions.

Granular Cell Tumor of the Hypoglossal Nerve Presenting with Tongue Atrophy and Deviation: A Case Report

Abstract:Granular cell tumors (GCT) are rare tumors of Schwann cell origin which were more often found in subcutaneous locations than in relation to major nerves. We describe a GCT that occurred within the hypoglossal nerve located in the left submandibular area of a 32-year-old man who initially presented with a muscular atrophy and deviation of the tongue. We also review the literature related to the differential diagnoses of such a submandibular tumor, with particular emphasis on the origin of these possibilities. In this case, the lesion was observed within the hypoglossal nerve in the submandibular area. The hypoglossal nerve was transected above and below the mass in order to achieve an adequate resection. The pathology demonstrated polygonal cells with diffuse eosinophilic granular cytoplasm and positive for CD68 and S-100 which established the diagnosis of GCT. This is the first patient, to our knowledge, with a GCT arising from the hypoglossal nerve.
Peripheral calcifying odontogenic cyst in maxillary anterior gingiva: A case report

Abstract:Background: Calcifying odontogenic cysts (Gorlin cysts) most commonly present centrally and have only rarely been reported in peripheral locations. The purpose of this report is to describe a new case of peripheral calcifying odontogenic cyst (PCOC) occurring in the anterior maxillary gingiva and to review the management and differential diagnosis of such a lesion. Methods: A 37-year-old female presented with a long-standing submucosal nodule on the gingiva between the maxillary central incisors, with asymptomatic growth over the last three years. Following an initial incisional biopsy, a diagnosis of PCOC was established. To exclude the possibility of a central process, a corresponding small field of view cone beam CT scan was obtained and the patient returned for a 6 mm excisional biopsy to the depth of the periosteum. Results: Results of these additional assessments supported the original diagnosis of PCOC. Following uneventful healing of the second biopsy, no recurrence or other clinical findings were noted at 1-year follow-up. Conclusion: While rare, the peripheral variant of calcifying odontogenic cyst, and other peripheral counterparts to recognized central cysts and tumors, should be considered in a differential diagnosis for a benign gingival nodule. Gingival tissue should be submitted for histologic evaluation to ensure a neoplastic process is not present. Key points: Various lesions may present on the gingiva as a "bump"; these can represent common clinical entities, such as pyogenic granuloma, peripheral ossifying fibroma, peripheral giant cell granuloma, and fibroma, or more rare conditions that may not be adequately considered in the differential diagnosis. A rarely documented case of peripheral calcifying odontogenic cyst (PCOC; Gorlin cyst) on the maxillary anterior gingiva of an adult female is reported here and compared with the few other similar PCOC cases in the literature. A biopsy of gingival lesions is always necessary to establish the correct diagnosis and provide the appropriate treatment. Plain language summary: Several different lesions can appear on the gingiva (gums). Some are quite common, and some are rare. This report documents the occurrence of a new case of calcifying odontogenic cyst (Gorlin cyst), a type of cyst that has been rarely found outside the jawbone, presenting as a "bump" on the gingiva between the maxillary central incisor teeth of an adult female. Because of the patient history, a peripheral calcifying odontogenic cyst (PCOC) was not initially suspected. Following a biopsy, a PCOC diagnosis was given. The possibility of a lesion within the bone was then excluded by an X-ray (cone beam CT) scan examination. A second, more extensive biopsy confirmed the diagnosis and the removal of the lesion. The patient had no complications or recurrence for the following 12 months. This case highlights the need to always biopsy lesions presenting on the gums to obtain a proper diagnosis and provide the correct treatment.
