Abstract:Objective:The purpose of this study was to compare the value of echocardiography with cardiovascular magnetic resonance imaging for the assessment of severity of pulmonary regurgitation in patients with repaired tetralogy of Fallot.Methods:We followed up 9 patients with TOF for 4～5 years after the repair using Doppler echocardiography and cardiac magnetic resonance imaging.Pulmonary regurgitation fraction(PRF) was assessed with cardiac MRI.Pressure half time(PHT),pulmonary regurgitation index(PRi),the ratio between pulmonary regurgitant jet width and pulmonary annulus diameter were measured with echocardiography.The relation of these data were compared by correlation-regression analysis.Results:The PRF was 28.8±5.5%(20.8-37.0%);PHT was 60.2±14.1msec(44.6-90.5msec);PRi was 0.7±0.13(0.54-0.93);The ratio between pulmonary regurgitant jet width and pulmonary annulus diameter was 53.3±10.5%(44.2-71.6%).PRF was negatively correlated with PHT(r=-0.51,P=0.16),negatively correlated with PRi(r=-0.69,P=0.04) and positively correlated with the ratio between pulmonary regurgitant jet width and pulmonary annulus diameter(r=0.71,P=0.03).The p value of the latter two had significant difference.Conclusion:Pulmonary regurgitation is common after the repair of tetralogy of Fallot.Severity of pulmonary regurgitation can be assessed noninvasively by cardiac magnetic resonance imaging and echocardio-graphy,with reasonable agreement between the two techniques.PRi is a convenient,accurate indicator for assessing the degree of pulmonary regurgitation.

Abstract:BACKGROUND:Although most patients with tetralogy of Fallot undergo radical repair during infancy and childhood, patients that remain undiagnosed and untreated until adulthood can still be treated. This study aimed to evaluate longterm outcomes of adult patients with tetralogy of Fallot who were treated surgically, and to determine the predictors of postoperative pulmonary regurgitation.METHODS:Fifty-six adult patients underwent complete surgical repair. Forty-three patients (76.8%) required a transannular patch. Systolic, diastolic, and mean pressure in the main pulmonary artery were measured after repair.RESULTS:The early mortality rate was 3.6%. The 16-year survival rate was (84.4 ± 11.5)%. Late echocardiography revealed 41 patients with transannular patch who had pulmonary regurgitation, consisting of mild pulmonary regurgitation in 28 patients, moderate in eight, and severe regurgitation in five patients. In addition, there was right ventricular outflow tract stenosis in nine patients, moderate/severe tricuspid valve regurgitation in seven, and residual ventricular septal defect in five. Logistic regression analysis demonstrated that the mean pulmonary pressure measured just after repair predicted late pulmonary regurgitation.CONCLUSIONS:The long-term survival of surgically treated adult patients with tetralogy of Fallot is acceptable. The mean pressure >20 mmHg in the main pulmonary artery measured right after surgical repair may be a feasible reference to time the reconstruction of the pulmonary valve.

Abstract:Objective To investigate the value of diagnosis and evaluation of MRI in tetralogy of Fallot(TOF)through retrospectively analyzing the MRI manifestation of TOF.Methods We collected 15cases of TOF that proved by operation and 20normal volunteers as references.We summarized the MRI manifestation of TOF,and songht the differences between TOF group and normal group.Results Ventricle defect,right outflow tract or pulmonary stenosis,right ventricle hypertrophy and aorta overriding were revealed well through MRI.The right outflow tract diameter was(9.47±2.80)mm,narrower than normal group(17.75±2.31)mm,the thickness(11.67±2.77)mm of the free wall of the right ventricle was thicker than normal group(3.50±0.83)mm,the ratio of left ventricle wall thickness(0.86)to left ventricle wall thickness was bigger than normal group(2.87),the aorta root and ascending aorta diameter was bigger than normal group,the differences were all signifant.The average ratio of aorta overriding was about 50.47%.Conclusion Not only diagnosing TOF accurately,MRI also had the value of evaluating right outflow tract,right ventricle hypertrophy and ascending aorta.

Abstract:consent for participation in this study, and the institutional To the Editor: Nowadays, most infants with tetralogy of Fallot (TOF) survive the initial surgical repair; however, ethics committee approved the review of medical records. they continue to experience residual hemodynamic and physiologic abnormalities in the follow-up. Pulmonary regurgitation (PR) is still recognized as the most common complication that determines late outcomes. PR may result in right ventricular (RV) dilatation and, ultimately, cardiac dysfunction. Indicators of deterioration of clinical status such as impaired exercise tolerance, ventricular arrhythmia, and sudden cardiac death have all been associated with chronic PR. The degree of PR is one of the most important factors determining RV dysfunction; thus, pulmonary valve replacement becomes necessary to retain lower RV volumes, increase the RV systolic function, and lower the mortality rates. Cardiac magnetic resonance (CMR) imaging is now recommended as the optimal method to use in the follow-up of patients with TOF. It has been proven to be more accurate than transthoracic echocardiography for the evaluation of RV function due to the irregular RV cavity. Indexed PR volume (PRVi) and PR fraction (PRF) are the two main markers to be measured; however, the optimal method for PR quantification remains controversial. Previous studies have recommended the use of PRVi and hypothesized that this marker is better than PRF in reflecting RV preload. However, PRVi is not widely used in clinical practice and has not been the diagnostic standard for PR classification. Accordingly, our study aimed to discover the optimal index for the assessment of the degree of PR.

Abstract:Objective To assess pulmonary regurgitation in repaired tetralogy of fallot (TOF) with cardic MRI (c-MRI) and to analyze the factors related with severe pulmonary regurgitation (PR). Methods Fifty-eight patients who underwent TOF repair from Jan. 1996 to Jun. 2007 were subjected to c-MRI examination for PR fraction (PRF). All cases were divided into two groups according to PRF < 35% and PRF≥35%. SPSS11.5 software was used to analyze the relationship between PRF and the factors including operation age, gender, weight, the procedure of RVOT reconstruction, preoperative NAKATA index, preoperative McGoon index, duration of CPB, aorta clamping, mechanical support, ICU stay and followup, current NAKATA index, current MeGoon index, pulmonary annular index (PAI). Results PRF was (25.9 ±15.2) %. Univariate analysis revealed that the procedure of RVOT reconstruction (P < 0. 05 ), current NAKATA index ( P < 0.01 ), current McGoon index ( P < 0. 01 ), PAl ( P < 0. 05 ) had statistically significant difference. Binary Logistic analysis indicated that current NAKATA index ( P <0. 05 ) and PAI ( P < 0. 01 )were independent factors related with severe PR. Conclusion Severe PR may be correlated with the procedure of transannular enlarging RVOT, currently hypoplastic pulmonary arteries and high PAI.
Assessment of regional and global right ventricular systolic function in children with repaired tetralogy of Fallot

Abstract:Background: Echo assessment of right ventricular function is difficult due to its complex geometry and regional differences in wall motion. In patients with repaired tetralogy of Fallot, cardiac MRI is the gold standard for assessment of right ventricular function. There is scant data regarding the correlation of echo measures of right ventricular function with cardiac MRI right ventricular function in the paediatric population. Methods: Echocardiographic measures of regional and global right ventricular function were reviewed in 56 patients with repaired tetralogy of Fallot and 27 patients with structurally normal hearts. These measures were compared to global right ventricular ejection fraction by cardiac MRI. Results: Despite having the same right ventricular ejection fraction by cardiac MRI as normal controls, patients with repaired tetralogy of Fallot had significantly worse global and regional echocardiographic measures of right ventricular function. Right ventricular fractional area change and fractional shortening of the right ventricular outflow tract correlated best with global right ventricular function by MRI. Incorporation of these measures into a regression model allowed accurate prediction of global right ventricular function by cardiac MRI. Conclusion: Children with repaired tetralogy of Fallot have abnormal regional and global right ventricular function compared to patients with normal hearts. Despite the presence of patch material, the right ventricular outflow tract significantly contributes to global right ventricular function by cardiac MRI. When incorporated into a model that includes regional and global echocardiographic measures of right ventricular function, right ventricular outflow tract fractional shortening helps predict global right ventricular function by MRI.

Abstract:Purpose: To prospectively assess, with magnetic resonance (MR) imaging, right ventricular (RV) diastolic function after repair of tetralogy of Fallot (TOF) at rest and during pharmacologic stress and to study relationship between main pulmonary artery end-diastolic forward flow (EDFF) (indicative of restrictive RV physiology) and clinical status. Materials and methods: Institutional medical ethics committee approval and patient or parent informed consent were obtained. Patients with TOF corrected through the transatrial-transpulmonary approach underwent MR imaging at rest and during dobutamine stress and maximal exercise testing. Two-dimensional (2D) cine volumetric data were acquired. Flow measurements were performed with a standard 2D flow-sensitized sequence. MR imaging flow curves for tricuspid and pulmonary valves were combined into RV time-volume change curves, from which indexes of RV filling were derived. Patient results were compared with published data in control subjects. Student t tests, Mann-Whitney U tests, analysis of covariance, and paired and one-sample t tests were used. Results: Thirty-six patients (mean age at repair, 0.9 year +/- 0.5 [standard deviation]; median age at study inclusion, 17 years [range, 7-23 years]; 26 male and 10 female patients) were included. Abnormalities in RV filling included impaired relaxation (prolonged deceleration time, P = .002; smaller early filling fraction, P = .02) in the entire group compared with published data in healthy control subjects and signs of restriction to RV filling (smaller atrial filling fraction and higher early filling/atrial filling peak ratio, P < .05 for both) in patients with EDFF (n = 24) compared with patients without EDFF (n = 12). Stress response was abnormal in patients with EDFF, who developed impaired RV relaxation not appreciated at rest. Patients with EDFF had more severe pulmonary regurgitation (P < .05) and poorer exercise performance (P < .001). Conclusion: In patients with TOF corrected with currently widely accepted surgical strategies, pulmonary artery EDFF relates to worse clinical state at mid- to long-term follow-up. Dobutamine stress imaging may unmask abnormalities in RV diastolic filling not appreciated with rest imaging alone.

Abstract:Studies of larger patient groups for systematic assessment of the anatomical accuracy of magnetic resonance imaging (MRI) for partial anomalous pulmonary venous drainage (PAPVD) have been performed so far only in adults. This study was undertaken to evaluate whether MRI can precisely depict pulmonary venous anatomy in infants and young children. Data on 26 children under 10 years old that underwent MRI over the past 2 years for suspected PAPVD were assessed. The MRI protocol included shunt quantification by velocity-encoded cine as well as morphological and functional assessment by multislice multiphase and contrast-enhanced MR techniques. MRI was performed in the compliant patient in breath-hold (n = 8; age range, 4.6–9.5 years) and in the noncompliant patient in conscious-sedation free breathing (n = 18; age range, 0.4 to 7.5 years). In 22 patients, PAPVD was diagnosed with MRI and confirmed during surgery. In four patients with large atrial septal defects not accessible to percutaneous closure, normal pulmonary venous return was demonstrated by MRI and confirmed during surgery. MRI under conscious sedation accurately specifies the anatomy of pulmonary veins in infants and small children. Therefore, we suggest performing MRI in patients with inconclusive transthoracic echocardiographic results in the preoperative assessment of PAPVD.

Abstract:Abstract Mitral regurgitation (MR) is a common valvular heart disease and is the second most frequent indication for heart valve surgery in Western countries. Echocardiography is the recommended first-line test for the assessment of valvular heart disease, but cardiovascular magnetic resonance imaging (CMR) provides complementary information, especially for assessing MR severity and to plan the timing of intervention. As new CMR techniques for the assessment of MR have arisen, standardizing CMR protocols for research and clinical studies has become important in order to optimize diagnostic utility and support the wider use of CMR for the clinical assessment of MR. In this Consensus Statement, we provide a detailed description of the current evidence on the use of CMR for MR assessment, highlight its current clinical utility, and recommend a standardized CMR protocol and report for MR assessment.

Abstract:OBJECTIVE: To assess relations between right ventricular pressure measured with a high fidelity transducer tipped catheter and the characteristics of tricuspid regurgitation recorded with Doppler echocardiography.DESIGN: A prospective non-randomised study of patients with severe pulmonary hypertension referred for consideration of lung transplantation.SETTING: A tertiary referral centre for cardiac and pulmonary disease, with facilities for invasive and non-invasive investigation, and assessment for heart and heart-lung transplantation.PATIENTS: 10 patients with severe pulmonary hypertension being considered for lung transplantation.ENDPOINTS: Peak right ventricular, pulmonary artery, and right atrial pressures; peak positive and negative right ventricular dP/dt; peak Doppler right ventricular-right atrial pressure drop; Doppler derived peak positive and negative right ventricular dP/dt; and time intervals of Q to peak right ventricular pressure and to peak positive and negative right ventricular dP/dt.RESULTS: The mean (SD) pulmonary artery systolic pressure was 109 (29) mm Hg. The peak Doppler right ventricular-right atrial pressure drop underestimated peak right ventricular pressure by 38 (21) mm Hg, and by 21 (18) mm Hg when the Doppler value was added to the measured right atrial pressure (P values < 0.05). This discrepancy was greater for higher pulmonary artery pressures. The timing of peak right ventricular pressure differed, with the Doppler value consistently shorter (mean difference 16 ms, P < 0.05). Values of peak positive and negative right ventricular dP/dt and the time intervals Q-peak positive right ventricular dP/dt and pulmonary closure to the end of the pressure pulse differed between the two techniques in individual patients, but not in a consistent or predictable way.CONCLUSIONS: Doppler echocardiography significantly underestimates the peak right ventricular pressure and the time interval to peak right ventricular pressure in pulmonary hypertension, particularly when severe. These differences may be related to orifice geometry. Digitisation of Doppler records of tricuspid regurgitation provides useful semiquantitative estimates of absolute values and timing of peak positive and negative right ventricular dP/dt. Clinically significant differences may exist, however, and must be considered in individual patients.

Mitral Valve Regurgitation: Assessment with Dual Source Computed Tomography.

Abstract:To evaluate diagnostic accuracy of dual source computed tomography (DSCT) for evaluation of mitral valve regurgitation (MR) with transthoracic echocardiography (TTE). We evaluated a total of 60 patients who underwent both DSCT and TTE. According to Doppler echocardiography, we divided the patients into two groups: 40 with MR and 20 without MR. We assessed the presence and severity of MR on the basis of systolic regurgitant orifice size on DSCT images and compared the results with two-dimensional Doppler echocardiography. We graded the morphology of the mitral apparatus of the 40 patients with MR using Real-time three-dimensional echocardiography( RT-3DE) and DSCT. Features such as calcification, valve thickening, valve prolapse, flail leaflet movement and local limitation of stretching on DSCT images were used to diagnose each patient and the results were compared with RT-3DE findings. Diagnosis of MR by DSCT was in good agreement with traditional two-dimensional Doppler echocardiography (Kappa = 0.883, P <; 0.01); for the severity of MR, the correlation coefficient between the two methods was 0.94. DSCT was able to diagnose five different types of pathologic morphology with no statistically significant difference to RT-3DE. DSCT can provide quick, multi-angle, multiphase observation of the mitral valve. Also DSCT can accurately assess the severity of regurgitation and pathologic morphology in MR disease.
Reliability of Doppler-based Measurement of Pulmonary Vascular Resistance in Congenital Heart Disease with Non-restrictive Defect

Abstract:Objective To explore the reliability of echocardiography in measuring the pulmonary vascular resistance (PVR) in congenital heart disease with non-restrictive defect.Methods 19 patients with congenital non-restrictive tricuspid valve defect (13 non-restrictive ventricular septal defect (VSD) and 6 non-restrictive patent ductus arteriosus) were enrolled in the study,and the other 19 patients with small defect were selected in the control group.The Doppler echocardiography was used to measure the peak velocity of tricuspid regurgitation and the forward blood flow spectrum and time-velocity integral of the right ventricular outflow tract (TVIRVOT).PVR was calculated according to the first formula PVRecho=TRV/TVIRVOT+0.16 and the second formula PVRecho2=TRV2/TVIRVOT*5.19-0.4.Meanwhile,the right cardiac catheterization (RHC) was performed within three days after the echocardiography examination of all the patients.The correlation was analyzed between the results obtained.Results Two direction shunt appeared in most of the non-restrictive VSD.The PVR calculated by the second formula was positively correlated with that by RHC.The correlation abate for the patients with enlarged RVOT.In patent ductus arteriosus with non-restrictive shunt,there is no significant correlation between the PVR measured by echocardiography and that by catheterization.Conclusion The Doppler echocardiography is reliable in measuring the congenital heart disease with non-restrictive VSD and especially helpful in measuring PVR of suspected Eisenmenger syndrome with bidirectional shunt.Therefore,it may become a useful and cost-effective tool to evaluate PVR without invasion and keep track of PVR changes.
Qualitative echocardiographic assessment of aortic valve regurgitation with quantitative cardiac magnetic resonance: a comparative study

Abstract:This study examined the correlation of echocardiography (ECHO) and Cardiac Magnetic Resonance (CMR) in the assessment of aortic valve regurgitation (AR) in children and young adults with congenital heart disease. We hypothesized that qualitative ECHO assessment correlates insufficiently with quantitative CMR data and compared subjective ECHO evaluations with objective measurement of regurgitant fractions (RF) by CMR. Patients who had both ECHO and CMR assessments of AR within 60 days of each other were included. The qualitative ECHO assessment (mild, moderate, severe) of AR and left ventricular dimension at end diastole were recorded. RF was quantified by CMR using phase-contrast velocity mapping. Repeat ECHO review and grading of AR was performed by a blinded single reader in a randomly chosen subgroup of patients. In 43 patients studied, statistical significance was observed in the CMR-RF between mild and moderate, and between mild and severe ECHO grades. There was significant overlap of objective RF between subjective grades. Mild ECHO AR corresponded to an RF (%) of 0-29, moderate 1-40, and severe 5-58. Overlap was more significant at moderate and severe grades. Results were similar in the group in whom a single reader interpreted the ECHO assessment. In conclusion, results derived from a real-life multiple-reader ECHO laboratory showed inconsistencies in ECHO grading of AR, with a wide range of objectively measured RF within a given ECHO grade. ECHO is less reliable in identifying more severe AR, often overestimating severity. Quantitative CMR is a potentially useful supplement to ECHO for management decisions and assessments of medical and surgical therapies in children and young adults with AR.

Abstract:Atrial fibrosis can be estimated noninvasively by magnetic resonance imaging (MRI) using late gadolinium enhancement (LGE), but diastolic dysfunction is clinically assessed by transthoracic echocardiography (TTE), and rarely by MRI. This study aimed to evaluate well-established diastolic parameters using MRI, and validate them with TTE and left ventricular (LV) filling pressures, and to study the relationship between left atrial (LA) remodeling and parameters of diastolic function. The study retrospectively included 105 patients (53 +/- 16years, 39 females) who underwent 3D LGE MRI between 2012 and 2016. Medical charts were reviewed for the echocardiographic diastolic parameters E, A, and e' by TTE, and pressure catheterizations. E and A were measured from in-plane phase-contrast cardiac MRI images, and e' by feature-tracking, and validated with TTE. Interobserver and intraobserver variability was examined. Furthermore, LA volumes, function, and atrial LGE was correlated with diastolic parameters. Evaluation of e' in MRI had strong agreement with TTE (r=0.75, P < 0.0001), and low interobserver and intraobserver variability. E and A by TTE showed strong agreement to MRI (r=0.77, P=0.001; r=0.73, P=0.003, for E and A, respectively). Agreement between E/e' by TTE and MRI was strong (r=0.85, P=0.0004), and E/e' by TTE correlated moderately to invasive pressures (r=0.59, P=0.03). There was a strong relationship between LA LGE and pulmonary capillary wedge pressure (r=0.81, P=0.01). In conclusion, diastolic parameters can be measured with good reproducibility by cardiovascular MRI. LA LGE exhibited a strong relationship with pulmonary capillary wedge pressure, an indicator of diastolic function.
Application of magnetic resonance imaging in follow-up after surgery for tetralogy of Fallot

Abstract:Objective To investigate the application of magnetic resonance imaging (MRI) in the follow-up after surgery for tetralogy of Fallot (TOF). Methods Seventy-one patients were examined with MRI during follow up after surgery for TOF, and the scanning sequences included fast imaging employing steady-state acquisition (FIESTA), phase-contrast magnetic resonance imaging (PCMR) and contrast enhanced magnetic resonance angiography (CE-MRA). Results Branch pulmonary artery stenosis was detected by CE-MRA in 44 of 71 (62.0%) patients, including left pulmonary artery stenosis (n = 29, 40.8%), right pulmonary artery stenosis (n = 4, 5.6%) and both left and right pulmonary artery stenosis (n = 11, 15.5%). With FIESTA sequence, right ventricular end-diastolic volume was measured at (115.28 ± 15.18) mL/m2. Varied amount of pulmonary regurgitation (42. 12% ± 16.30%) occurred in all 38 cases with PCMR. Meanwhile, serious pulmonary regurgitation was observed in 14 cases (36.8%), with pulmonary artery regurgitant fraction >40%. In patients without branch pulmonary artery stenosis, regurgitant fraction of the left pulmonary artery (57.70% ± 21.99%) was significantly higher than that of the right pulmonary artery (30.32% ± 13.44%) (P <0.01). Delayed onset of the tricuspid valve flow was observed by PCMR in 18 (54.5%) of 33 patients. However, there was no significant difference in the pulmonary artery regurgitant fraction, right ventricular ejection fraction and right ventricular end-diastolic volume index between patients with and without delayed onset of the tricuspid valve flow (P > 0.05). Conclusion Left pulmonary stenosis, pulmonary regurgitation and right ventricular enlargement are common complications after surgery for TOF. MRI can provide adequate information on residual anatomical problems and functional evaluation, which makes it ideally suited to the evaluation of patients after surgery for TOF.
Value of cardiac MRI in evaluation of left atrial function in the postoperative tetralogy of Fallot and pulmonary stenosis patients

Abstract:Objective:To explore the value of cardiac MRI (CMRI) in evaluating left atrial function in patients with postoperative tetralogy of Fallot (rTOF) and postoperative pulmonary stenosis (rPS).Methods:Totally 67 pediatric patients (49 with rTOF, 18 with rPS) with preserved left ventricular ejection fraction (EF) were recruited between January 2019 and October 2021 in Shanghai Children′s Medical Center, School of Medicine, Shanghai Jiao Tong University. Thirty-three healthy volunteers, matched in gender and age, were included as controls from July 2017 to August 2018. Left atrial EF, strain and strain rate of three phases (reservoir, conduit and pump), left atrial volume (maximum volume index, minimum volume index and pre-atrial contraction volume index) were measured with corresponding cardiac function analysis software. Then, the differences in these parameters were analyzed between the three groups by ANOVA or Kruskal-Wallis test with post hoc comparison and Bonferroni correction.Results:Compared with controls, patients with rTOF had lower reservoir function parameters (EF, strain and strain rate), conduit EF, conduit strain, and left atrial maximum volume index ( P<0.05), but higher pump EF ( P<0.05). In patients with rPS, only the reservoir strain rate decreased compared with controls ( P<0.05), and the remaining data showed no significant difference ( P>0.05). The reservoir and conduit EF and strain in patients with rPS were higher than those in patients with rTOF ( P<0.05). Conclusions:In patients with rTOF and rPS, left atrial function has changed despite the preservation of left ventricular EF, which may be an early marker of left ventricular diastolic dysfunction. In children with rTOF, left atrial reservoir and conduit functions decreased while the pump function increased. The reservoir and pump functions in rPS were better than those in rTOF. In addition, CMRI can detect left atrial dysfunction early before it enlarged.

Abstract:Background: Four-dimensional flow cardiac magnetic resonance (4D flow CMR) is an emerging non-invasive imaging technology that can be used to quantify mitral regurgitation (MR) volume. Current methods of quantification have demonstrated limitations in accurate analysis, particularly in difficult cases such as complex congenital heart disease. 4D flow CMR methods aim to circumvent these limitations and allow accurate quantification of MR volume even in complex cases. This systematic review aims to summarize the available literature on 4D flow CMR MR quantification methods and examine their ability to accurately classify MR severity. Methods: Structured searches were carried out on Medline and EMBASE in December 2018 to identify suitable research outcome studies. The titles and abstracts were screened for relevance, with a third adjudicator utilized when study suitability was uncertain. Results: Seven studies met the eligibility criteria and were included in the systematic review. The most widely used 4D flow MRI method was retrospective valve tracking (RVT) which was examined in five papers. The key finding of these papers was that RVT is a reliable and accurate method of regurgitant volume quantification. Conclusions: MR quantification through 4D flow MRI is both feasible and accurate. The evidence gathered suggests that for MR assessment, 4D flow MRI is potentially as accurate and reliable to echocardiography and may be complementary to this technique. Further work on MR quantification 4D flow image analysis is needed to determine the most accurate analysis technique and to demonstrate 4D flow MRI as a predictor of clinical outcome. Abstract:Mitral regurgitation (MR), a primary cause of valvular disease in adults, affects millions and is growing due to an ageing population. Cardiovascular magnetic resonance (CMR) has emerged as an essential tool, offering insights into valvular and myocardial pathology when compared to the primary imaging modality, echocardiography. This review highlights CMR's superiority in high-resolution volumetric assessment and tissue characterization, including also advanced techniques like late gadolinium enhancement imaging, parametric mapping, feature tracking and 4D flow analysis. These techniques provide a deeper understanding of MR's pathophysiology and its effect on cardiac chambers, enabling CMR to surpass echocardiography in predicting hard clinical outcomes and left ventricular (LV) remodelling post mitral valve surgery. Despite its advantages, CMR's application faces limitations like cost, lack of standardization, and susceptibility to arrhythmia artifacts. Nonetheless, as technological advancements continue and new evidence emerges, CMR's role in MR assessment is set to expand, offering a more nuanced and personalized approach to cardiac care. This review emphasizes the need for further research and standardized protocols to maximize CMR's potential in MR management.
