Abstract:An improved method, where conventional otoscope investigation of human suspicious otitis media is combined with diffuse reflectance spectroscopy and gas in scattering media absorption spectroscopy (GASMAS) is being developed. Otitis media is one of the most common infectious diseases in children, whose Eustachian tube connecting the middle ear with the nasal cavity is more horizontal than for adults, which leads to impaired fluid drainage. At present, the use of an otoscope to visually observe possible changes in the tympanic membrane appearance is the main diagnostics method for otitis media. Inaccurate diagnosis related to similar symptoms, and the difficulty for small children to describe the condition experienced, frequently leads to over-prescription of antibiotics and alarming increase in bacterial resistance development. More accurate diagnostic methods are highly desirable. Diffuse reflectance spectroscopy is a non-invasive quantitative spectroscopic technique that enables to objectively quantify changes in the hemoglobin content of the tympanic membrane related to inflammation. If an infection is present, the ventilatory function of the Eustachian tube is frequently impaired and the middle-ear cavity will be filled with fluid. GASMAS, a non-invasive detection method, can non-invasively determine if gas is replaced by fluid in the middle-ear cavity.

Abstract:A noninvasive optical technique, which is based on a combination of reflectance spectroscopy and gas in scattering media absorption spectroscopy, is demonstrated. It has the potential to improve diagnostics of middle ear infections. An ear phantom prepared with a tissue cavity, which was covered with scattering material, was used for spectroscopic measurements. Diffuse reflectance spectra of the phantom eardrum were measured with a reflectance probe. The presence of oxygen and water vapor as well as gas exchange in the phantom cavity were studied with a specially designed fiber-optic probe for backscattering detection geometry. The results suggest that this method can be developed for improved clinical detection of middle ear infection.

Abstract:Otitis media (OM) is a common disease of the middle ear, affecting 80% of children before the age of three. The otoscope, a simple illuminated magnifier, is the standard clinical diagnostic tool to observe the middle ear. However, it has limited contrast to detect signs of infection, such as clearly identifying and characterizing middle ear fluid or biofilms that accumulate within the middle ear. Likewise, invasive sampling of every subject is not clinically indicated nor practical. Thus, collecting accurate noninvasive diagnostic factors is vital for clinicians to deliver a precise diagnosis and effective treatment regimen. To address this need, a combined benchtop Raman spectroscopy (RS) and optical coherence tomography (OCT) system was developed. Together, RS-OCT can non-invasively interrogate the structural and biochemical signatures of the middle ear under normal and infected conditions.In this paper, in vivo RS scans from pediatric clinical human subjects presenting with OM were evaluated in parallel with RS-OCT data of physiologically relevant in vitro ear models. Component-level characterization of a healthy tympanic membrane and malleus bone, as well as OM-related middle ear fluid, identified the optimal position within the ear for RS-OCT data collection. To address the design challenges in developing a system specific to clinical use, a prototype non-contact multimodal handheld probe was built and successfully tested in vitro . Design criteria have been developed to successfully address imaging constraints imposed by physiological characteristics of the ear and optical safety limits. Here, we present the pathway for translation of RS-OCT for non-invasive detection of OM.

Abstract:In order to evaluate if an ophthalmic instrument has exempt status it shall not exceed any of the emission limits, we designed a relatively simple and inexpensive optical radiation measurement system that can be used to evaluate if an ophthalmic instrument is below the emission limits specified in the standard. The system can complete one measure of the spectral irradiance (in W/(cm*cm*nm)) from 250nm similar to 1100nm in shortest time of 10us and the system is applicable to the instruments with continuous visible light output and pulsed light output.

Abstract:The mechanism of diagnosing and treating children's amblyopia was analyzed and a treating system was developed based on multimedia technology and children's psychology. The system was composed of diagnosis system, treatment system, expert system and recording system, with the functions of diagnosis, decision-making and treatment. The satisfying effect of the multimedia system was confirmed during the clinical application in many hospitals. The curing rate was up to 70% and the response rate was up to 93%. The results indicate that the system is a quick and simple way to objectively evaluate the treating effect with the advantage of cheap and woundless.

Abstract:In this article, in order to meet the demand of diagnosis of respiration odor, a new style laboratorial system of artificial optical nose was introduced, which was used to explored the possibility of diagnosing disease through respiration odor. This system consisted of gas-sensitive optic fiber sensor array and signal processing system. Utilizing the characteristics of optic fiber sensor of combing with lots of sensitive polymer easily, responding rapidly and high-precisionly to the gas and obtaining dynamic information of olfaction easily, a method of computing chemistry was applied to achieve quantification of sensitive polymers and fluorescent reagents, and develop the optic fiber sensor array with different selectivities. Besides, an olfactory mechanism was adopted to the artificial nose, which can separate the component and concentration information of mixed gases from the responsive signal data of the optic fiber sensor array using OSN-OB-DLNN (olfactory sensory neuron-olfactory bulb-delay line neural network) arithmetic by simulating biological olfaction. Based on the experimental results, it can be concluded that this laboratorial system of optical artificial nose sensors has the characteristic of bionics and prospect of application.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objectives/Hypothesis</h3><p>To create a new strategy for monitoring pediatric otitis media (OM), we developed a brief, reliable, and objective method for automated classification using convolutional neural networks (CNNs) with images from otoscope.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Study Design</h3><p>Prospective study.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>An otoscopic image classifier for pediatric OM was built upon the idea of deep learning and transfer learning using the two most widely used CNN architectures named Xception and MobileNet‐V2. Otoscopic images, including acute otitis media (AOM), otitis media with effusion (OME), and normal ears were obtained from our institution. Among qualified otoendoscopic images, 10,703 images were used for training, and 1,500 images were used for testing. In addition, 102 images captured by smartphone with WI‐FI connected otoscope were used as a prospective test set to evaluate the model for home screening and monitoring.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>For all diagnoses combined in the test set, the Xception model and the MobileNet‐V2 model had similar overall accuracies of 97.45% (95% CI 96.81%–97.94%) and 95.72% (95% CI 95.12%–96.16%). The overall accuracies of two models with smartphone images were 90.66% (95% CI 90.21%–90.98%) and 88.56% (95% CI 87.86%–90.05%). The class activation map results showed that the extracted features of smartphone images were the same as those of otoendoscopic images.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>We have developed deep learning algorithms for the successfully automated classification of pediatric AOM and OME with otoscopic images. Abstract:SignificancePathologies within the tympanic membrane (TM) and middle ear (ME) can lead to hearing loss. Imaging tools available in the hearing clinic for diagnosis and management are limited to visual inspection using the classic otoscope. The otoscopic view is limited to the surface of the TM, especially in diseased ears where the TM is opaque. An integrated optical coherence tomography (OCT) otoscope can provide images of the interior of the TM and ME space as well as an otoscope image. This enables the clinicians to correlate the standard otoscopic view with OCT and then use the new information to improve the diagnostic accuracy and management.AimWe aim to develop an OCT otoscope that can easily be used in the hearing clinic and demonstrate the system in the hearing clinic, identifying relevant image features of various pathologies not apparent in the standard otoscopic view.ApproachWe developed a portable OCT otoscope device featuring an improved field of view and form-factor that can be operated solely by the clinician using an integrated foot pedal to control image acquisition. The device was used to image patients at a hearing clinic.ResultsThe field of view of the imaging system was improved to a 7.4 mm diameter, with lateral and axial resolutions of 38 μm and 33.4 μm, respectively. We developed algorithms to resample the images in Cartesian coordinates after collection in spherical polar coordinates and correct the image aberration. We imaged over 100 patients in the hearing clinic at USC Keck Hospital. Here, we identify some of the pathological features evident in the OCT images and highlight cases in which the OCT image provided clinically relevant information that was not available from traditional otoscopic imaging.ConclusionsThe developed OCT otoscope can readily fit into the hearing clinic workflow and provide new relevant information for diagnosing and managing TM and ME disease.

Abstract:OBJECTIVE: Conventional otoscopes and oto-endoscopes, which are used to examine the tympanic membrane (TM), do not provide tomographic information. Optical coherence tomography (OCT) non-invasively reveals the depth-resolved internal microstructure of the TM with very high spatial resolution. We designed this study to examine the TMs with middle ear diseases using a handheld otoscope employing 860 nm spectral domain (SD)-OCT, combined with video camera and to demonstrate the clinical applicability of this system.DESIGN: A total of 120 patients with otologic symptoms were enrolled. TM images were obtained using the handheld OCT-based otoscope (860 nm central wave length, 15 μm axial resolution, 15 μm lateral resolution, and 7 mm scanning range using relay lens). Both OCT and oto-endoscope images were compared according to the clinical characteristics such as perforation, retraction, and postoperative healing process.RESULTS: The objective grade about the thickness of perforation margins and the accurate information about the extent of TM retraction that was not distinguishable by oto-endoscopic exam could be identified using this system. The postoperative healing process of TMs could be also followed using the OCT device.CONCLUSION: These analyses from the surgeon-oriented perspective suggest another useful application of the handheld OCT device.

Structure-constrained deep feature fusion for chronic otitis media and cholesteatoma identification

Abstract:Chronic suppurative otitis media (CSOM) and middle ear cholesteatoma (MEC) were two most common chronic middle ear disease(MED) clinically. Accurate differential diagnosis between these two diseases is of high clinical importance given the difference in etiologies, lesion manifestations and treatments. The high-resolution computed tomography (CT) scanning of the temporal bone presents a better view of auditory structures, which is currently regarded as the first-line diagnostic imaging modality in the case of MED. In this paper, we first used a region-of-interest (ROI) network to find the area of the middle ear in the entire temporal bone CT image and segment it to a size of 100*100 pixels. Then, we used a structure-constrained deep feature fusion algorithm to convert different characteristic features of the middle ear in three groups as suppurative otitis media (CSOM), middle ear cholesteatoma (MEC) and normal patches. To fuse structure information, we introduced a graph isomorphism network that implements a feature vector from neighbourhoods and the coordinate distance between vertices. Finally, we construct a classifier named the "otitis media, cholesteatoma and normal identification classifier" (OMCNIC). The experimental results achieved by the graph isomorphism network revealed a 96.36% accuracy in all CSOM and MEC classifications. The experimental results indicate that our structure-constrained deep feature fusion algorithm can quickly and effectively classify CSOM and MEC. It will help otologist in the selection of the most appropriate treatment, and the complications can also be reduced.
Comparison of Diagnostic Methods of Secretory Otitis Media in Children

Abstract:Objective To investigate the diagnostic value of endotoscopy,tympanometry,pure tone audiometry (PTA) and high resolution temporal bone CT in children with secretory otitis media (SOM).Methods The study comprised 414 ears in 207 children with adenoid hypertrophy.Endotoscopy,tympanometry,PTA and high resolution temporal bone CT were conducted to evaluate the function of middle ear.Based on the imaging findings of tympanic cavity fluid by high resolution temporal bone CT,the diagnostic accordance rate of endotoscopy,tympanograms and PTA for SOM were studied.Results The diagnostic accordance rate of endotoscopy,tympanometry and PTA for SOM were 98.79％(409/414),98.07％ (406/414) and 88.41％ (366/414),respectively.The sensitivity of the three examinations were 99.20％(124/125),100％ (125/125),63.20％ (79/125),respectively.The specificity of the three examinations were 98.62％ (285/289),97.23％ (281/289),99.31％ (287/289),respectively.The diagnostic accordance rate of endotoscopy combined with tympanometry for SOM was significantly higher than any single examination or any combined examinations.Conclusion Endotoscopy combined with tympanometry shows a great value in diagnosing SOM,which is accurate and safe.
Deep Learning in Automated Region Proposal and Diagnosis of Chronic Otitis Media Based on Computed Tomography

Abstract:Objectives: The purpose of this study was to develop a deep-learning framework for the diagnosis of chronic otitis media (COM) based on temporal bone computed tomography (CT) scans. Design: A total of 562 COM patients with 672 temporal bone CT scans of both ears were included. The final dataset consisted of 1147 ears, and each of them was assigned with a ground truth label from one of the 3 conditions: normal, chronic suppurative otitis media, and cholesteatoma. A random selection of 85% dataset (n = 975) was used for training and validation. The framework contained two deep-learning networks with distinct functions: a region proposal network for extracting regions of interest from 2-dimensional CT slices; and a classification network for diagnosis of COM based on the extracted regions. The performance of this framework was evaluated on the remaining 15% dataset (n = 172) and compared with that of 6 clinical experts who read the same CT images only. The panel included 2 otologists, 3 otolaryngologists, and 1 radiologist. Results: The area under the receiver operating characteristic curve of the artificial intelligence model in classifying COM versus normal was 0.92, with sensitivity (83.3%) and specificity (91.4%) exceeding the averages of clinical experts (81.1% and 88.8%, respectively). In a 3-class classification task, this network had higher overall accuracy (76.7% versus 73.8%), higher recall rates in identifying chronic suppurative otitis media (75% versus 70%) and cholesteatoma (76% versus 53%) cases, and superior consistency in duplicated cases (100% versus 81%) compared with clinical experts. Conclusions: This article presented a deep-learning framework that automatically extracted the region of interest from two-dimensional temporal bone CT slices and made diagnosis of COM. The performance of this model was comparable and, in some cases, superior to that of clinical experts. These results implied a promising prospect for clinical application of artificial intelligence in the diagnosis of COM based on CT images.

Abstract:Sinusitis is a very common disease and improved diagnostic tools are desirable also in view of reducing over-prescription of antibiotics. A non-intrusive optical technique called GASMAS (GAs in Scattering Media Absorption Spectroscopy), which has a true potential of being developed into an important complement to other means of detection, was utilized in this work. Water vapor in the frontal sinuses, related to the free gas volume, was studied at around 937 nm in healthy volunteers. The results show a good stability of the GASMAS signals over extended times for the frontal sinuses for all volunteers, showing promising applicability to detect anomalies due to sinusitis. Measurements were also performed following the application of a decongestion spray. No noticeable signal change was observed, which is consistent with the fact that the water vapor concentration is given by the temperature only, and is not influenced by changes in cavity ventilation. Evaluated GASMAS data recorded on 6 consecutive days show signal stability for the left and right frontal sinus in one of the test volunteers.

Abstract:The electrochemical method is commonly applied in diagnosing and monitoring diseases. When used in conjunction with optical disease detection systems, it can broaden the detection range, lower the limit of detection and create mutual confirmation. Utilizing the electrochemical-optical dual-mode detection method for disease diagnosis is non-invasive, low-cost, easy-to-use, efficient and accurate compared with the traditional single-mode method. This paper reviews the reports of electrochemistry and optical methods combined in recent years, such as colorimetry, fluorescence, Surface Enhanced Raman Spectroscopy (SERS), surface plasmon resonance (SPR) and spectroelectrochemistry. We cover researches on several electrochemical-optical dual-mode detection methods for identifying disease markers, including biochemistry, cancer, infection, cardiovascular and cerebrovascular disease, and neurological diseases as well. These studies demonstrate the feasibility of electrochemical-optical dual-mode detection and its broad application prospects in the field of disease detection.

Abstract:Wideband Absorbance Immittance (WAI) has been available for more than a decade, however its clinical use still faces the challenges of limited understanding and poor interpretation of WAI results. This study aimed to develop Machine Learning (ML) tools to identify the WAI absorbance characteristics across different frequency-pressure regions in the normal middle ear and ears with otitis media with effusion (OME) to enable diagnosis of middle ear conditions automatically. Data analysis including pre-processing of the WAI data, statistical analysis and classification model development, together with key regions extraction from the 2D frequency-pressure WAI images are conducted in this study. Our experimental results show that ML tools appear to hold great potential for the automated diagnosis of middle ear diseases from WAI data. The identified key regions in the WAI provide guidance to practitioners to better understand and interpret WAI data and offer the prospect of quick and accurate diagnostic decisions.

Abstract:Background Otitis media (OM) is the infection and inflammation of the mucous membrane covering the Eustachian with the airy cavities of the middle ear and temporal bone. OM is also one of the most common ailments. In clinical practice, the diagnosis of OM is carried out by visual inspection of otoscope images. This vulnerable process is subjective and error-prone. Methods In this study, a novel computer-aided decision support model based on the convolutional neural network (CNN) has been developed. To improve the generalized ability of the proposed model, a combination of the channel and spatial model (CBAM), residual blocks, and hypercolumn technique is embedded into the proposed model. All experiments were performed on an open-access tympanic membrane dataset that consists of 956 otoscopes images collected into five classes. Results The proposed model yielded satisfactory classification achievement. The model ensured an overall accuracy of 98.26%, sensitivity of 97.68%, and specificity of 99.30%. The proposed model produced rather superior results compared to the pre-trained CNNs such as AlexNet, VGG-Nets, GoogLeNet, and ResNets. Consequently, this study points out that the CNN model equipped with the advanced image processing techniques is useful for OM diagnosis. The proposed model may help to field specialists in achieving objective and repeatable results, decreasing misdiagnosis rate, and supporting the decision-making processes.

Antibiotics are the most widely utilized treatment for otitis media, however, they can cause drug resistance and harmful side effects. In this study, we reported an antibacterial nanocomposite in combination with sonodynamic therapy that consists of herbal antibacterial agents such as Curcumin (CUR) and Tanshinone IIA (TSIIA), as well as Chitosan (CS), for the treatment of acute otitis media. CUR/TSIIA/CS nanocomposite (NC) with ultrasonic irradiation demonstrated that it could eliminate Staphylococcus aureus . In vivo experiments revealed that NC-mediated sonodynamic therapy had excellent antibacterial and anti-inflammatory activity, displaying a consistent performance comparable to ofloxacin. The therapeutic efficiency was attributed to capturing bacteria through spider-web-like effect and destroying bacteria through the reactive oxygen species generated under ultrasonic irradiation. Significantly, NC did not induce bacterial resistance and showed good biocompatibility. This study provides a novel strategy to develop an ultrasound-assisted nanocomposite with an enhanced antibacterial effect. Further, it unlocks new doors for the substitute of antibiotics to combat otitis media by establishing efficient therapeutic systems.

Smartphone-Enabled versus Conventional Otoscopy in Detecting Middle Ear Disease: A Meta-Analysis

Abstract:Traditional otoscopy has some limitations, including poor visualization and inadequate time for evaluation in suboptimal environments. Smartphone-enabled otoscopy may improve examination quality and serve as a potential diagnostic tool for middle ear diseases using a telemedicine approach. The main objectives are to compare the correctness of smartphone-enabled otoscopy and traditional otoscopy and to evaluate the diagnostic confidence of the examiner via meta-analysis. From inception through 20 January 2022, the Cochrane Library, PubMed, EMBASE, Web of Science, and Scopus databases were searched. Studies comparing smartphone-enabled otoscopy with traditional otoscopy regarding the outcome of interest were eligible. The relative risk (RR) for the rate of correctness in diagnosing ear conditions and the standardized mean difference (SMD) in diagnostic confidence were extracted. Sensitivity analysis and trial sequential analyses (TSAs) were conducted to further examine the pooled results. Study quality was evaluated by using the revised Cochrane risk of bias tool 2. Consequently, a total of 1840 examinees were divided into the smartphone-enabled otoscopy group and the traditional otoscopy group. Overall, the pooled result showed that smartphone-enabled otoscopy was associated with higher correctness than traditional otoscopy (RR, 1.26; 95% CI, 1.06 to 1.51; p = 0.01; I2 = 70.0%). Consistently significant associations were also observed in the analysis after excluding the simulation study (RR, 1.10; 95% CI, 1.00 to 1.21; p = 0.04; I2 = 0%) and normal ear conditions (RR, 1.18; 95% CI, 1.01 to 1.40; p = 0.04; I2 = 65.0%). For the confidence of examiners using both otoscopy methods, the pooled result was nonsignificant between the smartphone-enabled otoscopy and traditional otoscopy groups (SMD, 0.08; 95% CI, -0.24 to 0.40; p = 0.61; I2 = 16.3%). In conclusion, smartphone-enabled otoscopy was associated with a higher rate of correctness in the detection of middle ear diseases, and in patients with otologic complaints, the use of smartphone-enabled otoscopy may be considered. More large-scale studies should be performed to consolidate the results.

Abstract:Tissue oximetry parameter detection based on near-infrared spectroscopy has the advantages of being rapid,real-time,and non-invasive,and has been widely used in clinics with important application values.Near-infrared spectroscopic tissue oximetry requires a combination of different wavelengths of near-infrared light to calculate the concentration of oxyhaemoglobin and reduced haemoglobin,but the combination of wavelengths currently used to detect oximetry information is not uniform.The wavelength selection of the light source is directly related to the accuracy of the extraction of tissue blood oxygenation parameters.In order to improve the measurement accuracy,a wavelength selection method based on condition number was established in this study and experimentally verified by tissue mimicry and in vivo experiments.The experimental results show that when the absorption and scattering coefficients of tissue mimics are changed,the error of total haemoglobin concentration calculated by the smallest conditioned number group is the smallest,and the error calculated by the largest conditioned number group is the largest.In the in vivo experiment,when the collected spectral signals were denoised,the smallest conditioned number group calculated oxygen saturation with small error and the largest conditioned number group inverted oxygen saturation with the largest error,which is in accordance with the underlying theory of conditioned numbers.Therefore,it is feasible to optimize and select the wavelength using the conditional number method,which improves the tolerance of error in the calculation of optical parameters into physiological parameters,and provides a methodological basis for the rational selection of wavelength for tissue blood oxygen detection.
