Association between phenotype and the S3-NIV Questionnaire in COPD patients on home mechanical ventilation

Abstract:Home mechanical ventilation (HMV) is increasingly used in severe Chronic Obstructive Pulmonary Disease (COPD) patients. Janssens et al proposed classifying COPD patients into two phenotypes: respiratory phenotype (RP) and systemic phenotype (SP), based on a formula including: body max index, lung function, diabetes, and sex. The SP strongly related to better prognosis. The S3-NIV questionnaire is a simple, valid tool to assess respiratory symptoms, sleep, and side effects in HMV patients, ranging from 0 to 10, 10 corresponding to the lowest impact of disease and treatment. The aim of this study was to evaluate the results of the S3-NIV in COPD patients according to their phenotype. A multicenter cross-sectional study was carried out. Phenotype was defined using the equation [Y = - 34.945 + 0.437 x BMI + 0.422 x FEV1 (%predicted) + 0.140 x FVC (%predicted) + 3.772 (if diabetes present) - 2.347 (if gender = male)]. If Y> 0 patient is classified as SP. Results of the 195 analyzed patients are presented on table (figure 1), being the majority classified as SP - 128 (65.6%). We found that SP patients had a significantly higher S3-NIV total score (7.04 vs 6.36, p = 0.010), and respiratory symptoms subscore (6.50 vs 5.50, p = 0.018). We conclude that there is a relationship between the S3-NIV questionnaire and the phenotype's characterization, reflecting a greater impact of the disease and/or treatment on the RP.
