Abstract:BACKGROUND: Hepatic venous pressure gradient (HVPG) is the gold standard for diagnosis of portal hypertension (PH), invasiveness and potential risks in the process of measurement limited its widespread use.AIM: To investigate the correlation of computed tomography (CT) perfusion parameters with HVPG in PH, and quantitatively assess the blood supply changes in liver and spleen parenchyma before and after transjugular intrahepatic portosystemic shunt (TIPS).METHODS: Twenty-four PH related gastrointestinal bleeding patients were recruited in this study, and all patients were performed perfusion CT before and after TIPS surgery within 2 wk. Quantitative parameters of CT perfusion, including liver blood volume (LBV), liver blood flow (LBF), hepatic arterial fraction (HAF), spleen blood volume (SBV) and spleen blood flow (SBF), were measured and compared before and after TIPS, and the quantitative parameters between clinically significant PH (CSPH) and non-CSPH (NCSPH) group were also compared. Then the correlation of CT perfusion parameters with HVPG were analyzed, with statistical significance as P < 0.05.RESULTS: For all 24 PH patients after TIPS, CT perfusion parameters demonstrated decreased LBV, increased HAF, SBV and SBF, with no statistical difference in LBF. Compared with NCSPH, CSPH showed higher HAF, with no difference in other CT perfusion parameters. HAF before TIPS showed positive correlation with HVPG (r = 0.530, P = 0.008), while no correlation was found in other CT perfusion parameters with HVPG and Child-Pugh scores.CONCLUSION: HAF, an index of CT perfusion, was positive correlation with HVPG, and higher in CSPH than NCSPH before TIPS. While increased HAF, SBF and SBV, and decreased LBV, were found after TIPS, which accommodates a potential non-invasive imaging tool for evaluation of PH.

Abstract:<h3 class="a-plus-plus">Abstract</h3> <h3 class="a-plus-plus">Objectives</h3> To correlate hepatic and splenic CT perfusion parameters with hepatic venous pressure gradient (HVPG) measurements in patients with cirrhosis. <h3 class="a-plus-plus">Methods</h3> Twenty-one patients with cirrhosis (males, 17; females, 4; mean ± SD age, 57 ± 7 years) underwent hepatic and splenic perfusion CT on a 320-detector row volume scanner as well as invasive measurement of HVPG. Different CT perfusion algorithms (maximum slope analysis and Patlak plot) were used to measure hepatic arterial flow (HAF), portal venous flow (PVF), hepatic perfusion index (HPI), splenic arterial flow (SAF), splenic blood volume (SBV) and splenic clearance (SCL). Hepatic and splenic perfusion parameters were correlated with HVPG, and sensitivity and specificity for detection of severe portal hypertension (≥12 mmHg) were calculated. <h3 class="a-plus-plus">Results</h3> The Spearman correlation coefficient was −0.53 (p < 0.05) between SAF and HVPG, and −0.68 (p < 0.01) between HVPG and SCL. Using a cut-off value of 125 ml/min/100 ml for SCL, sensitivity for detection of a HVPG of ≥12 mmHg was 94%, and specificity 100%. There was no significant correlation between hepatic perfusion parameters and HVPG. <h3 class="a-plus-plus">Conclusion</h3> CT perfusion in patients with cirrhosis showed a strong correlation between SCL and HVPG and may be used for detection of severe portal hypertension. <h3 class="a-plus-plus"> Key points </h3> • SAF and SCL are statistically significantly correlated with HVPG • SCL showed stronger correlation with HVPG than SAF • 125 ml/min/100 ml SCL-cut-off yielded 94 % sensitivity, 100 % specificity for severe PH • HAF, PVF and HPI showed no statistically significant correlation with HVPG

Abstract:BACKGROUND: Hepatic venous pressure gradient (HVPG) is the gold standard for diagnosis of portal hypertension (PH). However, its use can be limited because it is an invasive procedure. Therefore, it is necessary to explore a non-invasive method to assess PH.AIM: To investigate the correlation of computed tomography (CT) perfusion of the liver with HVPG and Child-Pugh score in hepatitis B virus (HBV)-related PH.METHODS: Twenty-eight patients (4 female, 24 male) with gastroesophageal variceal bleeding induced by HBV-related PH were recruited in our study. All patients received CT perfusion of the liver before transjugular intrahepatic portosystemic stent-shunt (TIPS) therapy. Quantitative parameters of CT perfusion of the liver, including liver blood flow (LBF), liver blood volume (LBV), hepatic artery fraction, splenic blood flow and splenic blood volume were measured. HVPG was recorded during TIPS therapy. Correlation of liver perfusion with Child-Pugh score and HVPG were analyzed, and the receiver operating characteristic curve was analyzed. Based on HVPG (> 12 mmHg vs ≤ 12 mmHg), patients were divided into moderate and severe groups, and all parameters were compared.RESULTS: Based on HVPG, 18 patients were classified into the moderate group and 10 patients were classified into the severe group. The Child-Pugh score, HVPG, LBF and LBV were significantly higher in the moderate group compared to the severe group (all P < 0.05). LBF and LBV were negatively associated with HVPG (r = -0.473, P < 0.05 and r = -0.503, P < 0.01, respectively), whereas splenic blood flow was positively associated with hepatic artery fraction (r = 0.434, P < 0.05). LBV was negatively correlated with Child-Pugh score. Child-Pugh score was not related to HVPG. Using a cutoff value of 17.85 mL/min/100 g for LBV, the sensitivity and specificity of HVPG ≥ 12 mmHg for diagnosis were 80% and 89%, respectively.CONCLUSION: LBV and LBF were negatively correlated with HVPG and Child-Pugh scores. CT perfusion imaging is a potential non-invasive quantitative predictor for PH in HBV-related liver cirrhosis.

Abstract:Objective To analyze the CT manifestation of abnormal hepatic perfusion, understanding its pathogenesis to improve the diagnosis of liver disease.Methods To analyze the cases show liver perfusion abnormalities with dynamic contrast-enhanced CT between June 2012 to June 2014, retrospectively. To analyze the relationship between the CT performance and primary disease.Results 38 cases of hepatic perfusion disorders, six cases for single, two cases for multiple; 29 cases associated with tumor, four cases with inflammation, three cases with portal vein thrombosis, one case with cirrhosis, and one case with unknown cause. CT findings showed high or low density abnormal enhancement in arterial phase and portal phase. mostly, isodense in the equilibrium phase. The case numbers of perfusion abnormalities shape shows like lobe or liver segment type, wedge or fan, sheet and banded were 3,18,16 and 1, respectively . Hepatic perfusion disorders has no mass effect, but has normal blood vessels crossed.Conclusion correct understanding of liver perfusion abnormalities is important in the diagnosis and differential diagnosis of liver disease.
Perfusion Homogeneity of Hepatic Parenchyma in Magnetic Resonance Imaging During Splenoportography

Abstract:OBJECTIVE:To evaluate the homogeneity of hepatic parenchyma enhancement in magnetic resonance imaging during splenoportography (MRSP) in the portal phase.METHODS:MRSP was performed in 16 patients suspected of space-occupying lesions in the livers. The signal intensity of non-lesion abnormal perfusion areas and that of peripheral parenchyma were measured in all the scanning phases including unenhanced phase, portographic phase, equilibrium phase, and delayed phase.RESULTS:Perfusion abnormalities were observed in 8 non-lesion foci in the portal phase in 4 cases of hepatic cirrhosis, most frequently appearing in triangle or fan-like shapes (7/8) with unpredictable locations. Hepatic cirrhosis patients had more non-lesion perfusion abnormalities than non-cirrhosis patients.CONCLUSIONS:The presence of non-lesion perfusion abnormalities in MRSP does not affect the diagnosis of the disease, and more homogeneous enhancement of the hepatic parenchyma in the portal phase can be achieved in MRSP than in CT during arterial portography and magnetic resonance imaging during arterial portography.

Abstract:Objective To study hemodynamics characteristics of hepatic metastases by using CT perfusion imaging.Methods CT perfusion scans were performed in 25 cases with hepatic metastatic.The perfusion parameters of tumor tissue and peripheral liver parenchyma were measured,including the values of hepatic blood flow(HBF),hepatic blood volume(HBV),mean transit time(MTT),permeablility(PS),hepatic arterial fraction(HAP),and calculate hepatic arterial perfusion(HAP) and portal vein perfusion(PVP).Meanwhile,the time-density curve(TDC) and perfusion parameters characteristics of hypervasular as well as hypovascular metastases were analyzed.Results In the hypervasular metastases,the TDC was fast risen and slowly fallen,the tumor tissue was hypertransfusion obviously in HBF images.HBF,PS,HAF and HAP were superior to peripheral liver parenchyma while HBV,MTT and PVP were inferior to peripheral liver parenchyma.All parameters were statistically significant.In the hypovascular metastases,the TDC was slowly risen and slowly fallen,the tumor issue was hypotransfusion obviously in HBF images.HBF,HBV,MTT and PVP were inferior to peripheral liver parenchyma while PS and HAF were superior to peripheral liver parenchyma,in which HBF,HBV,MTT,HAF and PVP were ststistically significant.Conclusion CT perfusion imaging,which can reflect hemodynamic traits of hepatic metastases,has a great bearing on guiding the qualitation diagnosis and differential diagnosis.

Abstract:Objective To evaluate the principle and the technique of CT perfusion and the changes of hepatic blood flow in patients with liver cirrhosis. Materials and Methods Dynamic single-section CT of liver was performed in 45 participants, including 27 patients with liver cirrhosis and 18 patients without liver disease (control subjects). In accordance with the Child-Pugh classification, 12 patients were classified as Child A, l0 with Child B, and 5 with Child C. CT scans sections were obtained at a single-level including the liver, spleen, aorta, and portal vein, the attenuation of these organs was measured to draw time density curves to calculate the liver CT perfusion parameters.Results (1)Hepatic artery perfusion, portal venous perfusion, total hepatic perfusion, hepatic perfusion index and mean transit time were decreased significantly compared with control group. (2)There were significant difference in partial liver perfusion paramrters according to the severity of liver cirrhosis. (3)Positive correlation was observed between splenic perfusion and portal venous perfusion. Conclusion (1)Dynamic single-section CT perfusion could quantify hepatic blood flow. (2)Hepatic perfusion parameters measured with CT were significantly altered in cirrhosis and correlated with the severity of liver cirrhosis.
Application of CT Perfusion Imaging on Transjugular Vein Intrahepatic Portal-Systemic Shunt

Abstract:Objective To evaluate the hepatic perfusion changes before and after transjugular vein intrahepatic portal-systemic shunt(TIPSS)by means of spiral CT perfusion imaging.Materials and Methods CT perfusion imaging was performed in fifteen patients with cirrhosis and portal hypertension 2 days before TIPSS and one week after TIPSS.The hepatic perfusion parameters before and after TIPSS were compared.Results Prior to TIPSS,portal venous perfusion(PVP),hepatic arterial perfusion(HAP),total liver perfusion(TLP),and hepatic arterial perfusion index(HPI) was(0.58±0.23) ml·min-1·ml-1,(0.14±0.13) ml·min-1·ml-1、(0.72±0.17) ml·min-1·ml-1,and(24.0±10.2)%,respectively.After TIPSS,PVP,HAP,TLP,and HPI was(0.15±0.04)ml·min-1·ml-1,(0.28±0.05) ml·min-1·ml-1,(0.43±0.07) ml·min-1·ml-1 and(64.1±13.9)%,respectively.There was significant difference in the hepatic function,blood ammonia and portal free pressure between pre-TIPSS and post-TIPSS.Conclusion Spiral CT perfusion imaging could reveal the hemodynamic changes of the liver before and after TIPSS.
A relative study of hepatic perfusion and portal vein pressure in rats with liver cirrhosis

Abstract:Objective To evaluate spiral CT perfusion in assessing portal vein pressure in rats with different stages of liver cirrhosis. Methods Seventeen rats with early stage of liver cirrhosis, 18 with intermediate stage, 12 with advanced stage, and 13 healthy rats as a control group were selected and recieved hepatic perfusion on a single- row spiral CT scanner. The parameters of hepatic perfusion were calculated using the deconvolution method. The portal vein pressure was measured by multi- physiographer. Results ① In study group, the PVP (portal venous perfusion) and THBP (total hepatic blood perfusion) were negatively correlated with FPP, while positively correlated with the HPI (hepatic perfusion index) and MTT (mean transit time). The FPP had a close relation with PVP. The equation, Y=20.671- 3.195X, could be conducted with linear regression analysis. ② According to the linear regression equation mentioned above, the FPP in 47 rats were 16.090± 2.150 cmH2O, which was highly correlated with the observed value16.108± 3.662 cmH2O ( r=0.823 P< 0.01) . Conclusion CT perfusion is a new non- invasive and efficient modality for assessment of the portal pressure in liver cirrhosis in various stages.
The Study of Hepatic Perfusion Disorders with Multi-Slice Spiral CT

Abstract:Objective To evaluate the various patterns of hepatic perfusion disorders with multi-slice spiral CT and to emphasize the significance of recognition for them . Methods In a period of 1 year, the all CT images of 1568 patients received multi-slice spiral CT examination of upper abdomen were analyzed retrospectively. The CT findings, etiologies and mechanisms of different pattens of hepatic perfusion disorders were investigated. Results 103 cases were demonstrated to have hepatic perfusion disorders. 69 cases were associated with tumors and the other 34 cases were non-tumor associated. The etiologies of various hepatic perfusion disorders were classified as (1)arterioportal fistulae(41.7%), (2) portal venous inflow obstruction(23.3%), (3) inflammartory hyperemic changes(17.5%), (4) steal phenomenon by hypervascular tumors(6.8%), (5) hepatic venous outflow obstruction(4.9%), (6) aberrant hepatic blood supply (3.9%) and (7) uncertain causes(1.9%). There was significant correlation between the CT images of perfusion disorders in shape and distribution and the causes of them. Conclusion Multi-slice spiral CT provides a new effective technique for demonstrating and understanding various hepatic perfusion disorders. Familiarity with the spiral CT appearances of these perfusion disorders will result in more accurate evaluation for various pathologic or physiologic states reflected by them.

Abstract:Background: This study aimed to analyze the predictive value of multi-slice spiral computed tomography (CT) perfusion imaging for upper gastrointestinal bleeding in patients with cirrhotic portal hypertension. A total of 62 patients with cirrhotic portal hypertension and 28 healthy individuals were included. The results showed that multi-slice spiral CT perfusion imaging had a significant predictive value for upper gastrointestinal bleeding in patients with cirrhotic portal hypertension. The vascular area, number of vascular cross-sections, and gastric coronary vein diameter (GCVD) showed high predictive values, with the vascular area having the best predictive value. Aim: To investigate the predictive accuracy of multi-slice spiral CT perfusion imaging for upper gastrointestinal bleeding in patients with cirrhosis and portal hypertension. Methods: This study included 62 patients with cirrhotic portal hypertension (disease group) and 28 healthy individuals (control group). The disease group was further divided into two subgroups: Group A (n = 27, bleeding) and group B (n = 35, no bleeding). All patients underwent multi-slice spiral CT perfusion imaging at our hospital, and we compared various parameters such as liver blood flow, vein size, number of blood vessels, and blood vessel area between the two groups. We employed statistical analysis to identify factors associated with upper gastrointestinal bleeding and created a graph comparing the predictive value of different factors for bleeding. Results: We found no difference in hepatic artery (HAP) levels among the three groups (all P > 0.05). The portal vein levels in groups A and B were much lower than in the control group; group A was much lower than group B (all P < 0.05). The HAP perfusion index levels in groups A and B were much higher than in the control group; group A was much higher than group B (all P < 0.05). The portal vein diameter, splenic vein diameter, and GCVD levels in groups A and B were much higher than in the control group; those in group A were much higher than those in group B (all P < 0.05). The number of blood vessels and blood vessel area in groups A and B were much higher than in the control group; those in group A were much higher than those in group B (all P < 0.05). The statistical method showed a strong link between GCVD, number of blood vessels, blood vessel area, and upper gastrointestinal bleeding (odds ratio = 1.275, 1.346, 1.397, P < 0.05). The graph showed that GCVD, number of blood vessels, and blood vessel area could predict bleeding well, with blood vessel area having the best prediction power. Conclusion: That multi-slice spiral CT perfusion imaging can predict upper gastrointestinal bleeding well in patients with cirrhosis and high blood pressure in the portal vein. GCVD, number of blood vessels, and blood vessel area had high prediction power. The blood vessel area had the best prediction power, with an area under the curve of 0.831.
Assessment of Liver Fibrosis in Different Degree: Preliminary Study on Multi-Slice CT Perfusion Imaging

Abstract:Objective To evaluate the role of multi-slice CT (MSCT) perfusion in early diagnosis of liver fibrosis. Methods Thirty-three subjects underwent CT perfusion of the liver. Among whom, 11 subjects were volunteers without hepatic disease and the other 22 subjects were pathologically confirmed with liver fibrosis who were further divided into slight (n= 10) and severe (n=12)liver fibrosis according to the lshak system. Parameters of CT perfusion were measured and compared among three groups. Results The mean hepatic arterial fraction in controls, light and severe fibrosis tended to increase with the severity of liver fibrosis[(18. 49 ± 9. 69) %, (19. 92 ± 6.01) % and (21.31±7.47)% ,respectively], and the mean mean transit time tended to decrease with the severity of liver fibrosis [(13.80 ± 2. 60) s, (12.35 ± 1.31) s and (12.19 ± 3.33) s, there was no significant difference in all parameters between any two groups (P>0.05). Conclusions Quantitative measurement of hepatic blood supply can be obtained by CT perfusion. Some parameters will be helpful in staging fibrosis to a certain extent. But its clinical usefulness for the evaluation of the early diagnosis may not be affirmed yet.
Quantitative assessment of portal hypertension and its effect on the peripheral organs by CT plain scanning

Abstract:Objective To investigate the value of CT plain scanning in the quantitative assessment of portal hypertension and its effect on the peripheral organs.Methods A respective analysis was made on the CT scan data of 86 people,of whom 45 were patients with portal hypertension (the portal hypertension group) and 41 were normal adults (the control group).Close observation was made on the CT values of the portal right and left branch junction,the abdominal aorta in the same scanning layer,the pancreas,the double kidneys and the spleen of the 2 groups.The patients in the portal hypertension group were classified in accordance with liver function,then,measurements and statistical analyses were made in the CT values of the above corresponding parts.Results The CT values of the portal right and left branch junction,the abdominal aorta in the same scanning layer,the pancreas,the double kidneys and the spleen were respectively (36.97 ± 5.81),(41.06 ± 7.22),(42.64 ± 6.07),(33.18 ± 4.20) and (45.29 ± 4.60) HU,while those of the control group were respectively (43.92 ± 5.71),(47.19 ± 5.80),(50.06 ± 7.80),(36.12 ± 4.50) and (52.38 ± 4.95) HU.The CT value of the above corresponding parts regarding liver function classification (A,B and C) were respectively (39.57 ±(38.67 ±7.35),(37.33 ±4.09),(33.46 ±3.97),(28.23 ±4.13),(48.66 ±4.38),(45.03 ±4.43) and(40.12 ±5.32) HU.Variance analysis showed that there were statistical differences in the CT values of the portal vein,abdominal aorta,pancreas,double kidneys antd spleen between the portal hypertension group and the normal group (P ＜ 0.05).There was also statistical significance in the above corresponding parts regarding liver function classification (A,B and C).Conclusion In case of portal hypertension,the CT valtues of the portal vein,the abdominal aorta,the pancreas,the double kidneys and the spleen would decrease,andt the bigher the liver function classification,the lower the CT values,irndlicating that portal hypertension had certain effects on the tuction of peripheral organs and hemodynamics of aorta.

Abstract:To explore the feasibility of noninvasive assessment of portal hypertension (PHT) by ultrasonography. A total of 18 PHT patients who were planning to undergo splenectomy combiend with pericardial devascularization received ultrasonography one week before surgery (PHT group). In addition, 20 healthy volunteers were enrolled as the controls (control group). The conventional two-dimentional (2D) ultrasound, Doppler ultrasound and contrast-enhanced ultrasound (CEUS) parameters were compared between these two groups. For patients in the PHT group, the portal vein pressure was measured using portal vein puncture, and its relationship with the above parameters were further analyzed. The 2D ultrasound, Doppler ultrasound and CEUS parameters showed significant differences between the PHT group and control group (p < 0.01). Three ultrasonographic features including area under the time-intensity curve (TIC) of portal venous phase/hepatic arterial phase (Qp/Qa), intensity of portal venous phase/hepatic arterial phase (Ip/Ia), and base-to-peak ascending slope (βp/βa) were negatively correlated with the portal vein pressure (correlation coefficient: -0.701, -0.625, and -0.494, respectively). In this study, Qp/Qa and Ip/Ia have reduced significantly in PHT group compared with that in the control group (p <0.01), suggesting that in PHT patients, intensity and proportion of portal vein perfusion declined; while intensity and proportion of hepatic artery perfusion gradually rise due to buffering effect, thus gradually reversed the proportion of blood supplying through portal vein and hepatic arteries. Meanwhile, the decline of βp/βa, TTP, and RT confirmed that the increased flow velocity of the hepatic artery and decreased flow velocity of portal vein led to increased slope of TIC curve of the ascending branch of hepatic artery segment and decreased slope in portal vein segment, moreover; meanwhile, the increased overall slope of curve caused an advanced time to peak. In PHT group, Qp/Qa, Ip / Ia, and βp/βa were negatively correlated with the portal vein pressure, with correlation coefficients of -0.701, -0.625 and -0.494, respectively, indicating that these three ultrasound contrast parameters not only objectively reflected the pathophysiological process and the hemodynamic characteristics of chronic liver diseases but also showed a good correlation with portal pressure, therefore it was expected for clinical use as a new method for noninvasive assessment of portal vein pressure in PTH patients.

Abstract:Objective To investigate the effect of spleen spontaneous shunt on portal vein hemodynamics in patients with liver cirrhosis with Doppler ultrasound.Methods Eighty-seven patients were divided into chronic hepatitis group,liver cirrhosis group and control group.Liver function of cirrhosis patients was classified into Child A and Child B/C according to Child-Pugh categorization criteria.Hemodynamic parameters of hepatic artery,portal vein,superior mesenteric artery/vein and spleen artery/vein were examined on resting condition.Then the blood shunt ratio of spleen vein and superior mesenteric vein with portal vein,as well as hepatic circulation index(HCI)were calculated.The relationship between spleen blood shunt and HCI was analyzed.Results Portal vein blood flow was not significantly different among groups.Spleen vein blood flow increased in cirrhosis group,which was significantly different to that of control group and chronic hepatitis group.The spleen shunt ratio of cirrhosis group was greater than that of control group and chronic hepatitis group,as well as the Child B/C and Child A in cirrhosis group;the ratio of spleen vein flow to portal vein flow and spleen vein flow to superior mesenteric vein flow increased,but the ratio of superior mesenteric vein flow to portal vein flow decreased with the liver function decreased in cirrhosis group.There was non-linear negative correlation between HCI and the spleen shunt ratio.Conclusion Spleen spontaneous shunt and splenic hyperdynamic circulation play an important role in liver perfusion.Detecting V spv /V pv ratio with Doppler ultrasound in patients with liver cirrhosis is helpful in assessing liver function reserve.

Abstract:ObjectivesThe clinical utility of contrast‐enhanced sonography in portal hypertension remains unclear. We explored the feasibility of using contrast‐enhanced sonography for noninvasive assessment of portal venous pressure.MethodsTwenty healthy individuals (control group; 9 men; mean age, 46.4 years) and 18 patients with portal hypertension (15 men; mean age, 46.2 years) were enrolled in this study. The portal hypertension group included patients who underwent splenectomy and pericardial blood vessel disarticulation at our hospital from October 2010 to March 2011. One week before surgery, patients with portal hypertension underwent preoperative liver contrast‐enhanced sonography. Two‐dimensional, Doppler, and contrast‐enhanced sonographic parameters were compared between the groups. Portal venous pressure was measured intraoperatively by portal vein puncture in the portal hypertension group, and its relationship with the other parameters was analyzed.ResultsThe 2‐dimensional, Doppler, and contrast‐enhanced sonographic parameters differed between the groups (P < .01). Portal venous pressure was inversely correlated with the area under the portal vein/hepatic artery time‐intensity curve ratio (Qp/Qa), portal vein/hepatic artery strength ratio (Ip/Ia), and portal vein/hepatic artery wash‐in perfusion slope ratio (βp/βa), with correlation coefficients of −0.701, −0.625, and −0.494, respectively.ConclusionsMeasurement of the liver contrast‐enhanced sonographic parameters Qp/Qa, Ip/Ia, and βp/βa could be used as a new quantitative method for noninvasively assessing portal venous pressure.
Evaluation of CT perfusion imaging for the intrahepatic portal-systemic vein shunts on liver ischemia/ reperfusion injury

Abstract:Objective To investigate the application of CT perfusion imaging (CTPI) for the intrahepatic portal sys temic vein shunts on liver ischemia/reperfusion injury. Methods Twelve adult dogs were injected with Lipo-PGE1 (1 μg/kg) via saphenous. CTPI and EMBF were performed before medication 1 h and after medication 5 min at laparotomy. The corre- lation of PVP using the two methods was analyzed. Twenty-one adult dogs were divided into three groups randomly: control group, IV group and SMA group, each group includes seven dogs. Animals were occluded porta hepatis 45 min and reper- fused 60 min to establish I/R injury models. The dosage of Lipo-PGE1 was 1 μg/kg and rate 0.05 μg/(kg·min) in IV and SMA group; 0.9% sodium chloride of 2 mL/kg was used in the control group. The administration was conducted before porta occlusion 5 min and after reperfusion 60 min, respectively. CTPI, free portal pressure measurement (FPP) and pathological sampling were performed in succession and compared before first administration 30 min and after second administration 30 min. Results PVP by CTPI and EMBF was similar and highly correlated (r=0.87). PVP obviously increased after taking Lipo-PGE1 via vein. The PVP [mL/(min·mL)] in control group, IV group and SMA group were 0.22±0.06, 0.26±0.06 and 0.38±0.11 by CTPI post-I/R and 0.29±0.08, 0.30±0.07 and 0.41±0.11 by EMBF, respectively. There was a significant differ- ence in group comparison between two methods (t=-9.410, P=0.000), (t=-11.711, P=0.000), (t=-9.370, P=0.000). The dif- ference of PVP between two methods were 31%, 25%, 8% in three groups, respectively. FPP increased obviously after I/R than that in control group, but there were no significant differences between SMA and IV groups. Microscopically, the struc- ture of liver was evidently vague, hepatocytes were cloudy swelling, erythrocytes stasis in hepatic sinusoid, and neutrophilic granulocyte predominately infiltrated portal area in control group. The liver injury was moderately improved in IV group and markedly improved in SMA group. Conclusion CT perfusion imaging can quantitatively estimate the alterations of intra- hepatic portal systemic shunts on ischemia/reperfusion injury following the treatment.

Abstract:Background: Early diagnosis of portal hypertension is imperative for timely treatment to reduce the mortality rate. However, there is still no adequate method to noninvasively and accurately assess the portal hypertension in routine clinical practice. Purpose: We aimed to evaluate the accuracy of parameters measured using dual energy spectral computed tomography (LightSpeed CT750 HD) in assessing portal venous pressure in patients with liver cirrhosis. Study: Forty-five patients with liver cirrhosis who underwent percutaneous transhepatic portal vein puncture as part of their treatment for liver disease were enrolled in this study. Measurement of direct portal venous pressure was performed preoperatively. All patients underwent dual energy spectral computed tomography within 3 days before their operations. Results: The iodine concentrations of portal vein and hepatic parenchyma during the portal venous phase and the alanine aminotransferase level were found to be independently correlated with the direct portal venous pressure according to stepwise multivariate linear regression analysis (P<0.001, 0.004, and 0.024, respectively). In a receiver operating characteristic analysis, the area under the receiver operating characteristic of iodine concentrations of the portal vein (ICPV) for identifying clinically significant portal hypertension (>= 10 mm Hg) was significantly higher than that of iodine concentrations of hepatic parenchyma (ICliver) and the alanine aminotransferase level (0.944, 0.825, and 0.301, respectively). The threshold ICPV of 58.27 yielded a sensitivity of 93.8%, specificity of 69.2%, positive predictive value of 88.2%, and negative predictive value of 81.8%, respectively. Conclusions: ICPV values may be a useful tool in noninvasively assessing the portal venous pressure and identifying clinically significant portal hypertension in liver cirrhosis.
