Abstract:Objective This study aimed to analyze a comprehensive set of potential risk factors for obesity and overweight among Chinese children with a full range of ages and with wide geographical coverage. Methods In the Prevalence and Risk Factors for Obesity and Diabetes in Youth (PRODY) study (2017-2019), the authors analyzed 193,997 children aged 3 to 18 years from 11 provinces, autonomous regions, and municipalities that are geographically representative of China. All participants underwent physical examinations, and their caregivers completed questionnaires including dietary, lifestyle, familial, and perinatal information of participants. A multilevel multinomial logistic regression model was used to evaluate the potential risk factors. Results Among the actionable risk factors that were measured, higher consumption frequencies of animal offal (odds ratios [OR] for an additional time/day = 0.91, 95% CI: 0.88-0.95, same unit for OR below unless specified otherwise), dairy products (0.91, 95% CI: 0.88-0.94), freshwater products (0.94, 95% CI: 0.91-0.96), staple foods (0.94, 95% CI: 0.92-0.96), and coarse food grain (OR for every day vs. rarely = 0.92, 95% CI: 0.86-0.98) were associated with lower relative risk of obesity. However, higher restaurant-eating frequency (OR for >4 times/month vs. rarely = 1.21, 95% CI: 1.15-1.29) and longer screen-viewing duration (OR for >2 hours vs. <30 minutes = 1.16, 95% CI: 1.10-1.22) were associated with higher relative risk of obesity. Increased exercise frequency was associated with the lowest relative risk of obesity (OR for every day vs. rarely = 0.72, 95% CI: 0.68-0.77). Conclusions Changes in lifestyle and diet of Chinese children may help relieve their obesity burden.

Abstract:With the rapid development and democratization of the internet and smart phone industry, online food delivery services have become increasingly popular all over the globe, namely in China. One of the unfortunate drawbacks of these delivery services is that they mainly use single-use plastics as food packaging, therefore generating large amounts of disposable food containers to meet demand. Such plastic containers reach the end of their service life after a single meal, and are then discarded as plastic waste. The sheer amount of plastic food containers discarded in this manner exacerbates various environmental issues, including one that is invisible to the naked eye: microplastic pollution. This minireview summarizes the history of food delivery services in China, from orders made face-to-face to digital orders, as well as the consequences introduced by the tremendous amounts of plastic waste generated by the food delivery services. Microplastic pollution could be mitigated to a certain extent by improving the classification, handling and management of single-use plastic containers in China. Furthermore, additional studies focusing on microplastic pollution caused by food delivery services are needed, especially as the use of these services is on the rise worldwide.
An Indicator Measuring the Influence of the Online Public Food Environment: an Analytical Framework and Case Study

Abstract:The online public food environment (OPFE) has had a considerable impact on people's lifestyles over the past decade; however, research on its exposure is sparse. The results of the existing research on the impact of the food environment on human health are inconsistent. In response to the lack of food elements in the definition of the food environment and the lack of a clear method to assess the health attributes and the impact degree of the food environment, we proposed a new analytical framework based on the latest disease burden research, combining the characteristics of China's current food environment, from the perspective of environmental science. We redefined the food environment and proposed that food and its physical space are two core elements of the food environment. Accordingly, we extracted four domains of characteristics to describe the basic components of the food environment. Using the sales records, we designed an approach by referring to the standard process of environmental health indicators, including the health attributes and the impact degree of the food environment, to measure the OPFE of takeaway food outlets. Further, we conducted a case study and extracted three domains of characteristics for more than 18,000 effective takeaway meals from 812 takeaway food outlets located in 10 administrative subdivisions in the Haidian District and Xicheng District of Beijing Municipality. The results showed that more than 60% of single meals sold by takeaway food outlets were considered as healthy, and only 15% of takeaway food outlets sold healthy meals exclusively. Additionally, there were significant differences in health effects among different types of food environments, and high-risk areas of different types of food environments can be spatially identified. Compared with the counting method in the availability of food environment, the proposed new approach can depict food environment characteristics not only in the macro-scale like the counting method but also in the meal-scale. The indicators could be useful for large-scale and long-term monitoring of food environmental changes due to their simple calculation and design depending on the food delivery platform.

Abstract:China is confronted with the challenge of increasing childhood obesity. Although interventions for childhood obesity have been developed, their effectiveness and implementation can vary considerably across nations. In this paper, we review and consolidate the evidence on childhood obesity intervention strategies aimed at multiple levels and components across life, in China. School-based interventions, primarily delivered with group-based dietary and physical education, have shown potential efficacy and can be enhanced by integrating food environment reform and family support. However, most interventional studies were done in eastern and urban areas of China, which highlights the need for attention in regions with insufficient health resources where policies and initiatives are less accessible. In addition, the sparse reporting on implementation design and delivery might hinder the scalability of potentially effective strategies. Amid rapid social and economic development over the past few decades, China faces unique challenges on urbanisation, physical inactivity, and nutritional transitions, necessitating an updated policy agenda for primary prevention of childhood obesity. Structural longitudinal evaluations of public initiatives led by the government and organisations along with inclusive and equitable interventions targeting children of different ages, urban–rural regions, and different ethnic and socioeconomic groups are warranted. Tackling childhood obesity in China needs a cohesive approach that integrates social, economic, cultural, and environmental strategies, combining school-based, family-based, and individual-based approaches, and concerted efforts from multiple sectors and entities within China.

Abstract:Rapid economic growth and urbanization have brought notable changes in China. This trend had dramatic impacts to food industry and diet-related behaviors, which leads to increasing prevalence of overweight and obesity as well as diet-related non-communicable diseases (NCDs). With a rapid development of internet and e-commerce market, another emerging trend that should not be overlooked is the change in food shopping habits induced by the popularity of online-to-offline (O2O) food delivery service in China. The O2O food market is a booming industry in China, the market scale of O2O food industry is increasing with remarkable speed. More than 1/5 of total population in China has already became the users of O2O food delivery market. Development of this new trend in food environment is inevitable. This game-changing trend brings great opportunities to improve food accessibility and availability but meanwhile poses inevitable challenges to the whole public health system and social environment in China. This paper offers a unique perspective of the opportunities and challenges that the new industry brings to food environment, health outcomes caused by related behavior change, and its broader influence on social environment.

New Online Market Connecting Chinese Consumers and Small Farms to Improve Food Safety and Environment

Abstract:In emerging economies where small farms are the main source of food supply, it is costly for the government to monitor and control food safety and production impact on the environment. However, the online food market can potentially give farmers stronger incentives to supply safer and more eco-friendly products, as they can access a national market where consumers are interested in healthy food, and they can differentiate their products by providing production information using videos and pictures. This research uses the choice experiment method to elicit farmers' preference for production practices and marketing channels in China where e-commerce and delivery businesses are fast-growing. Our main finding is that farmers perceive higher utility in selling safer and more eco-friendly products than conventional products when using e-commerce platforms, evidence of the online market's positive role in food safety enhancement. Our results also identify two types of farmers: traditional farmers and farmers open to the idea of online markets. Farmers who have higher education and live in villages with e-commerce service centers are more likely to be the latter.

Abstract:Background: Geographical profiles of overweight and obesity in China are limited. We aimed to compare region-specific prevalences of overweight and obesity for Chinese children with a full range of ages and to describe the distribution of potential risk factors.Methods: We analyzed data from Prevalence and Risk factors for Obesity and Diabetes in Youth (PRODY) study, which was a cross-sectional survey of children aged 3-18 years from 11 provinces, autonomous regions, and municipalities that are geographically representative of western, southern, eastern, northern and central China. All participants underwent physical examinations, and their parents or caregivers completed online, self-administered questionnaires. We used multilevel logistic regression models to estimate prevalences of overweight and obesity and to assess related risk factors across sexes, age groups and regions. Findings: Between January 2017 and December 2019, 200,937 children in the PRODY study were included in our analysis. We found that children aged 8-13 years in northern China and children aged 3-5 years in western China were heavily burdened with overweight and obesity. In northern China the obesity prevalences peaked at 31·0% for boys and at 18·8% for girls, while in western China the prevalences of overweight and obesity peaked at 39·0% and 26·0%, respectively. Boys were vulnerable to overweight and obesity throughout all ages and regions (except western China). In addition, risk factors for overweight and obesity varied across ages, sexes and regions. Early-life factors (breast-feeding duration) and lifestyle factors (daily screen time, sleep time and exercise frequency) were the most important, modifiable factors for obesity control. Parental factors (parent’s overweight, socioeconomic characteristics and education status) had noteworthy impacts on their offspring’s overweight and obesity. Interpretation: The age- and sex-specific prevalences of overweight and obesity in Chinese children vary geographically; therefore, modifications of the major risk factors for the prevention and treatment of overweight and obesity should take age, sex and regional characteristics into consideration. Special attention should be paid to the most vulnerable children, and targeted interventions are urgently needed to mitigate the obesity burden in such a vast country. Cyber Food Swamps: Investigating the Impacts of Online-to-Offline Food Delivery Platforms on Healthy Food Choices

Abstract:Online-to-offline (O2O) food delivery platforms have substantially enriched the food choices of urban residents by allowing them to conveniently access farther food outlets. However, concerns about the healthiness of delivered food persist, especially because the impact of O2O food delivery platforms on users' healthy food choices remains unclear. This study leverages large-scale empirical data from a leading O2O delivery platform to comprehensively analyze online food choice behaviors and how they are influenced by the online exposure to fast food restaurants, i.e., online food environment. Our analyses reveal significant discrepancy in food preferences across demographic groups and city sizes, where male, low-income, and younger users and those located in larger cities more likely to order fast food via O2O platforms. Besides, we also perform a comparative analysis on the food exposure differences in online and offline environments, confirming that the extended service ranges of O2O platforms can create larger "cyber food swamps". Furthermore, regression analysis highlights that a higher ratio of fast food orders is associated with "cyber food swamps", areas characterized by a higher share of accessible fast food restaurants. A 10% increase in this share raises the probability of ordering fast food by 22.0%. Moreover, a quasi-natural experiment substantiates the long-term causal effect of online food environment changes on healthy food choices. Our findings underscore the need for O2O food delivery platforms to address the health implications of online food choice exposure, thereby informing efforts by various stakeholders to improve residents' dietary health.

Abstract:Community food environment is closely associated with residents’ health status, and it has received substantial research attention in recent years. However, such studies are sparse in China. This study aims to reveal the characteristics of food environment in China. A ground-truthing field census of food outlets was conducted in Xi Hu district, Hangzhou. A novel categorizing system was developed to better characterize food outlets in the study area. Geocoding and food environment analysis were performed by Arc-GIS software. A total of 9274 food outlets were identified, of these, 6161 (66.43%) were food service places and 3113 (33.57%) were food stores. Fast-food restaurants (2692) and convenience stores (1764) were the two most numerous outlet types. The density of food outlets was 29.72/km2 and 113.97/10,000 people. Availability of BMI-unhealthy food outlets was four times greater than that of BMI-healthy ones. Further, 41.86% of total food outlets provided food-delivery services; among them, fast-food restaurants were the highest (65.53%) providers. In the urban area, the average density of food outlets was 104/km2. While, it was only 29/km2 in the rural area. Availability of food outlets was excellent in our study area, though it showed regional imbalance. Food environment tends to be obesogenic and high prevalence of food delivery service might increase this trend. Thus, policy makers should guarantee the smooth development of catering industry in China, so as to ensure a balanced nutrition environment.

How Foods and Beverages Are Promoted Online: A Content Analysis of the Digital Food Environment in China

Abstract:Digital platforms such as social media and e-commerce platforms have become a major space where foods and beverages (F&B) are promoted. Prior research has found that online, unhealthy F&B receive more presence than healthy F&B. This obesogenic food environment may increase the obesity rate. Therefore, it is critical to understand how healthy and unhealthy F&B are promoted online. A content analysis of 2906 posts related to F&B via five digital platforms was conducted in China, where the obesity rate has increased in recent years. Firstly, the results show that unhealthy F&B received more presence on digital platforms than healthy F&B. Secondly, healthy F&B posts tended to highlight the healthiness of the products, whereas unhealthy F&B posts leveraged a wide range of promotional strategies, specifically use cues, food cues, chewing sounds, sensory descriptions, friend cues, local cultural appeal, nostalgia appeal, price information, discount information, and trending hashtags or topics. Next, use cues, chewing sounds, sensory descriptions, family cues, and friend cues increased the quantity of audience feedback, whereas price information and using trending hashtags or topics lowered the quantity of audience feedback. Moreover, local cultural appeal and social proof exhibited the opposite impact on audience feedback. Finally, health benefit statements lowered audience feedback for healthy F&B posts, whereas brand visibility and purchase links inhibited audience feedback for unhealthy F&B posts. In addition to describing the digital food environment in China, the present research provides implications on how to promote healthy F&B. Particularly, we suggest that healthy F&B businesses and healthy eating campaigns should leverage the strategies unhealthy F&B use to receive more consumer attention, in order to increase their own products' public visibility and attractiveness.

Abstract:Prevention and control of childhood obesity in China is complex. Despite numerous existing policy endeavours, particularly Healthy China 2030, accompanied by multiministerial initiatives, childhood obesity persists and even exacerbates. In this paper, we review current national policies, assess progress of the existing system managing childhood weight, and identify implementation challenges. Leveraging insights from existing literature and guided by a PEDALS (ie, problem, evidence-based practice, determinants, actions, long term, and scalability) framework, we propose a strategy to refine and integrate current policies, interventions, and research into a policy framework, embedding evidence-based practices into practical solutions for childhood obesity prevention and control in China.

Abstract:Over the past four decades, China has witnessed an important nutritional transition, characterised by a heightened overnutrition burden among children. The country now has the largest population of children with obesity globally. In this paper, we review the epidemiology of childhood obesity in China, its determinants, and risk factors, with a particular focus on school-aged children. Evidence unveils substantial variations across age, gender, and region. We describe multilevel obesogenic determinants, including macro-level social, cultural, and environmental factors; meso-level factors related to schools and communities; and micro-level factors tied to families and individuals from the perinatal-infant stage to childhood and adolescence. The primary drivers of childhood obesity appear to be rooted in the broader macro-level social, economic, and technological environment; obesogenic factors, which have affected school, community, and family environments; and accelerated unhealthy behaviour uptake. Identifying and characterising the catalysts behind the rise in childhood obesity in China is imperative for the development of scalable, effective, and tailored prevention, control, and intervention strategies.
Hunger for Home Delivery: Cross-Sectional Analysis of the Nutritional Quality of Complete Menus on an Online Food Delivery Platform in Australia

Abstract:Online food delivery (OFD) platforms have changed how consumers purchase food prepared outside of home by capitalising on convenience and smartphone technology. Independent food outlets encompass a substantial proportion of partnering outlets, but their offerings’ nutritional quality is understudied. Little is also known as to how OFD platforms influence consumer choice. This study evaluated the nutritional quality and marketing attributes of offerings from independent takeaway outlets available on Sydney’s market-leading OFD platform (UberEats®). Complete menus and marketing attributes from 202 popular outlets were collected using web scraping. All 13841 menu items were classified into 38 food and beverage categories based on the Australian Dietary Guidelines. Of complete menus, 80.5% (11,139/13,841) were discretionary and 42.3% (5849/13,841) were discretionary cereal-based mixed meals, the largest of the 38 categories. Discretionary menu items were more likely to be categorised as most popular (OR: 2.5, 95% CI 1.9–3.2), accompanied by an image (OR: 1.3, 95% CI 1.2–1.5) and offered as a value bundle (OR: 6.5, 95% CI 4.8–8.9). Two of the three discretionary food categories were more expensive than their healthier Five Food Group counterparts (p < 0.02). The ubiquity of discretionary choices offered by independent takeaways and the marketing attributes employed by OFD platforms has implications for public health policy. Further research on the contribution of discretionary choices and marketing attributes to nutritional intakes is warranted.

[Food environment and its relation to diet behavior and obesity in China]

Abstract:Objective: In this study, a systematic review was made on scientific evidence regarding the impact of food environment on diet-related behavior and obesity in China. Methods: Search on related keywords and references were conducted from four electronic databases including the Cochrane Library, PubMed, Web of Science, and CNKI. Results: Data showed that good availability, accessibility of neighborhood food outlets had increased the diversity of food, including the intakes of total calories, protein, fat, and carbohydrate; however, the consumption of healthy foods such as fruit and vegetables was reducing among the Chinese residents. The effects of neighborhood food environment on obesity remained inconclusive. A large number of fast food restaurants around schools might be responsible for the increase of risk on obesity among students. Regulations set on the school vicinity food stalls might reduce the intake of sugary beverages, snacks and fast food among students. Conclusion: Building a healthy food environment is warranted to nudge the Chinese people towards a healthier diet pattern. However, more evidence is needed to support the evidence in building a healthy food environment.

Abstract:A modern diet with high energy density is harmful to health and negatively affects the environment. Eating out is particularly prevalent among young adults and has steadily increased in many countries over the last few decades. This study aims to assess the impact of nutrition education interventions on social media on cognitive and behavioral changes in the diet of university students. A cluster-randomized trial with an intervention group (n = 853) and a no-intervention control group (n = 846) was conducted in Chongqing. A two-month educational intervention was conducted through social media (WeChat official account, Sina Weblog, and Tencent Weibo) with participants of the intervention group. Self-reported eating-out-related knowledge and behaviors were assessed at baseline and six and eight months after randomization. Multilevel mixed-effects logistic regression and linear regression analyses accessed the outcomes between the intervention and control groups. The proportion of participants in the high-level knowledge and attitude group in the intervention group increased by 13% and 8.5% compared with the control group (p < 0.05). The effects of the intervention on the outcomes significantly changed over time, as demonstrated by the achieved statistical significance for knowledge and attitude score (p for group and time interaction <0.001) in the interaction between time and randomization groups. The high-frequency eating away from home/school at weekends decreased from 29.6% to 27.3% in the intervention group. In the control group, the high-frequency eating away from home/school at weekends in the increase was increased (26.3%–27.2%). A total of 73.6% of respondents in the intervention group reported that they consciously reduced their frequency of eating away from home/school. The possibility exists for narrowing the perception and behavior disparity gap through the effective use of social media, such as WeChat and Microblog, which could be a sustainable platform to provide health education or new food policies to promote healthy eating and reduce food waste among university students.

Quantitative assessment of environmental exposure of delivery men in Wuhan

Abstract:The take out industry is rapidly developing in China, but the environmental exposure of delivery men is neglected. "Ele.Me," an online food ordering platform, is one of the two largest catering online-to-offline platforms in China. From January 2018 to April 2018, data from 16,528 take out shops in Wuhan were crawled by using Python. The data included store name, average delivery time, delivery price, region, and store type. With the use of Baidu map's API batch reverse address resolution, the map locations of the take out shops were identified. Network service area was utilized to obtain the activity space of delivery men along the Wuhan traffic road network. Combined with the data on sulfur dioxide (SO2), nitrogen dioxide (NO2), carbon monoxide (CO), ozone (O3), PM2.5, PM10, and noise provided by the Wuhan Environmental Protection Bureau, the spatial interpolation, regional analysis, superposition analysis, and other geographic information system spatial analysis methods were used to study the air and noise pollution within the activity space of delivery men in various take out shops. Results revealed the unbalanced environment faced by Wuhan delivery men. The exposure to NO2, CO, and PM2.5 was mainly concentrated in the central city, whereas the exposure to SO2, O3, and PM10 was mainly noted far from the city. Noise pollution was mainly observed in the districts of Hannan, Caidian, and Dongxihu; and the most serious pollution was recorded in Shamao Street. The weighted comprehensive pollution index model was used to calculate the contribution weight of each evaluation factor to obtain the results of environmental exposure to air pollution. The influence of noise was further considered to obtain the consequences of comprehensive environmental exposure to pollution. The highly polluted regions were those in the central part of the city. Pollution was mainly concentrated in the third loop, whereas Zhucheng Street in the Xinzhou District provided the best environment. The results of this study can serve as a reference for the physical health risk assessment of delivery men.
The Neighborhood Food Environment and the Onset of Child-Hood Obesity: A Retrospective Time-Trend Study in a Mid-sized City in China

Abstract:Nowadays, obesity and its associated chronic diseases have become a steadily growing public health problem, spreading from the older to younger age groups. Studies have contended that the built environment, particularly the food environment and walkability, may contribute to the prevalence of childhood obesity. In Asian countries which are characterized by rapid urbanization, high population density and oriental diets, little is known about how such urban built environment affects the onset of childhood obesity. This study juxtaposes the effect of food environment, walkability, and outdoor activity spaces at the neighborhood level upon childhood body weight in a mid-sized city in China. This observational study utilizes a retrospective time-trend study design to examine the associations between neighborhood built environment and children's body weight in Zhanjiang City, a mid-sized city in Guangdong Province, China. Robust multiple linear and logistic regression models were used to estimate associations between the built environments and child BMI and weight status (i.e., overweight/obesity and obesity only). This study finds that: (1) Western-style fast food and Chinese-style fast food have divergent impacts on childhood body weight. At neighborhood level, while increased exposure to Western-style fast food may increase child BMI and the risk of overweight and obesity, increased exposure to Chinese-style fast food, on the contrary, may reduce child BMI and the risk of overweight and obesity, indicating a positive health impact of Chinese-style fast food. (2) However, the positive health impacts brought about by Chinese-style fast food, walkable environments and accessible traditional fruit/vegetable markets have gradually disappeared in recent years. This study is among the first to simultaneously consider the divergent and changing impact of food environment upon childhood body weight in urban China. The findings provide important implications for healthy city design and the management of food retail industry in addressing the obesity epidemic in younger generations living in Asian cities. As prominent differences exist in food culture between Asian and Western cities, more attention should be paid to healthy food environment in future studies and related urban planning strategies formulation.

Abstract:This paper offers a unique perspective about the development of the online-to-offline (O2O) food delivery industry from 2017 to 2019 in China. This study demonstrates the latest transformation and improvements of the O2O market that address some common problems in the early stages of the development of this raising industry in China. New strategies and regulations from the O2O platforms, food providers, and national and local governments are discussed. In our view, the mission of the O2O industry in general has shifted from pursuing enormous quantity to ensuring high quality. China's O2O food delivery industry warrants further attention and studies as it grows and develops into the future. We suggest future studies to work on its economic, behavioral, and health impacts on population level as it encompasses both great risks and rewards.

Abstract:The increasing popularity of online food delivery (OFD) has led to drastic changes in conventional foodscapes. There is yet a limited understanding regarding if the new form of food ordering and delivering through third-party platforms improve the equity of food accessibility or exacerbate existing, if not generate new, equity issues. In this paper, we attempt to answer this question based on a case study of Nanjing, China. We first adapt the widely used spatial accessibility measure, enhanced two-step floating catchment area (E2SFCA), to quantify food accessibility that accounts for both onsite and online food services. We then examine if and how OFD changes the spatial and social disparities in food accessibility with different transport modes. The results show that OFD improves the equity of food accessibility in general but exacerbate existing spatial disparities between urban and rural communities. Socially disadvantaged groups remain vulnerable to food procurement opportunities. The empirical findings have important implications on developing hybrid onsite-online strategies for food service provisions.
