Abstract:Purpose: Serum cystatin C (sCyC) has long been known as a reliable biomarker of renal injury. However, it remains to be tested whether sCyC is a reliable biomarker to predict renal recovery after continuous renal replacement therapy (CRRT). Methods: A retrospective analysis of patients admitted to a tertiary 18-bed intensive care unit from January 2008 to December 2011 was performed. Univariate and multivariate regression analyses were performed to test the independent predictors of renal recovery. The diagnostic value of sCyC in predicting renal recovery was assessed using a receiver operating characteristic curve (ROC). Results: Older age and higher sCyC were independent risk factors of renal nonrecovery (OR: 1.40 and 4.76, respectively). The area under the ROC of sCyC to predict renal recovery was 0.87 (95% CI: 0.82-0.92), with a sensitivity and specificity of 80.5% and 83.5% at the cutoff of 2.98 mg/l. Conclusion: A high sCyC level at the initiation of CRRT is associated with poor renal outcome.
Kidney Function in Patients With Neuromuscular Disease: Creatinine Versus Cystatin C

Abstract:Background: Accurate measurement of kidney function in patients with neuromuscular disorders is challenging. Cystatin C, a marker not influenced by skeletal muscle degradation, might be of clinical value in these patients. Methods: We consecutively enrolled 39 patients with neuromuscular disorders. We investigated the association of the eGFR, based on plasma creatinine and Cystatin C, with clinical and biochemical variables associated with kidney function, namely age and galectin-3. Results: Creatinine-based eGFR was 242 (±80) and Cystatin C-based eGFR was 110 (±23) mL/min/1.73 m2. Cystatin C-based eGFR was associated with age (β -0.63 p < 0.0001) and galectin-3 levels (β -0.43 p < 0.01), while creatinine-based eGFR was not (β -0.22 p = 0.20; β -0.28 p = 0.10). Sensitivity analyses in Duchenne and Becker patients revealed the same results: Cystatin C-based eGFR was associated with age (β -0.61 p < 0.01) and galectin-3 levels (β -0.43 p = 0.05), while creatinine-based eGFR was not (β -0.32 p = 0.13; β -0.34 p = 0.14). Conclusions: These data indicate that estimation of renal function in patients with neuromuscular disorders cannot reliably be achieved with creatinine, while Cystatin C appears a reasonable alternative. Since a large proportion of patients with neuromuscular disorders develops heart failure, and requires heart failure medication, adequate monitoring of renal function is warranted.

Abstract:Background and objective: Serum cystatin C (sCyC) has been associated with renal function, and it rises earlier than creatinine after renal injury. Methods: Medical charts of AKI patients that had been admitted to a tertiary 15-bed intensive care unit from January 2006 to January 2008 were reviewed. Renal recovery was defined as the return to pre-morbid renal function (sCr < 1.5 x pre-morbid sCr), or as an improvement in RIFLE classification. Univariate analyses with t test or Wilcoxon Rank-Sum test were performed to screen predictors of renal recovery, and multivariate Logistic regression analysis was performed to identify independent predictors of renal outcome. Diagnostic performance of sCyC was assessed. Results: Younger age and lower sCyC values were independent predictors of renal recovery (OR: 0.96 and 0.58, respectively); the area under the receiver operating characteristic curve (AU-ROC) of sCyC in predicting renal recovery was 0.66 (95% CI: 0.51 - 0.81), with the sensitivity and specificity of 57.69% and 86.79% at the cutoff of 3.13 mg/l. Conclusion: Elevated cystatin C at CRRT initiation predicts lack of renal recovery, but its diagnostic performance is suboptimal. Our study was limited by the small sample size and the lack of strict protocols on the initiation and cessation CRRT.
Correlation between cystatin C- and renal scan-determined glomerular filtration rate in children with spina bifida

Abstract:We report on the relationships between serum cystatin C level, glomerular filtration rate (GFR) estimated from a cystatin C-based prediction equation (that of Filler and Lepage), GFR calculated by the Schwartz formula and technetium 99m-diethylene triamine penta-acetic acid ((99)Tc-DTPA)-determined GFR in 28 children with spina bifida. All children underwent measurement of height, weight, serum cystatin C level, and serum creatinine level at the time of their renal scan. The relationship between variables was assessed by Pearson correlation. Pearson correlation for the relationship between (99)Tc-DTPA GFR and GFR calculated by the cystatin C-based equation was significant and higher than that of the relationship between (99)Tc-DTPA GFR and GFR calculated by the Schwartz equation, which was not statistically significant. The correlation for Filler GFR was 0.42 (P = 0.03) and for Schwartz GFR was 0.21 (P = 0.28). Although we use renal scan determination of GFR as the best measure, and a creatinine-based formula as the most practical measure, perhaps a formula such as that published by Filler and Lepage, which is not dependent on anthropometric data, might be a more useful (and accurate) tool for establishing GFR in children with spina bifida.

Abstract:Background: Cystatin C (CysC) has been proposed as a filtration marker for the early detection of acute kidney injury (AKI); however, a wide range of its predictive accuracy has been reported.Study Design: Meta-analysis of diagnostic test studies.Setting & Population: Various clinical settings of AKI, including patients after cardiac surgery, pediatric patients, and critically ill patients.Selection Criteria: Computerized search of PubMed, Current Contents, CINAHL, and EMBASE from inception until November 15, 2010, was performed to identify potentially relevant articles. Inclusion criteria were studies investigating the diagnostic accuracy of CysC level to predict AKI. There were no language restrictions in the search.Index Tests: Increasing or increased serum CysC level or urinary CysC excretion.Reference Tests: The outcome was the development of AKI, primarily based on serum creatinine level (definition varied across studies).Results: We analyzed data from 19 studies and 11 countries involving 3,336 patients. Of these studies, 13 could be included in the meta-analysis. Across all settings, the diagnostic OR for serum CysC level to predict AKI was 23.5 (95% CI, 14.2-38.9), with sensitivity and specificity of 0.84 and 0.82, respectively. The area under the receiver operating characteristic curve (AUROC) of serum CysC level to predict AKI was 0.96 (95% CI, 0.95-0.97). Subgroup analysis showed that serum CysC was of diagnostic value when measured early (within 24 hours after renal insult or intensive care unit admission). For the diagnostic value of urinary CysC excretion, the diagnostic OR was 2.60 (95% CI, 2.01-3.35), with sensitivity and specificity of 0.52 and 0.70, respectively. The AUROC of urinary CysC excretion to predict AKI was 0.64 (95% CI, 0.62-0.66).Limitations: Variation in criteria for definitions of index and reference tests, absence of measured glomerular filtration rate in most studies.Conclusion: Serum CysC appears to be a good biomarker in the prediction of AKI, whereas urinary CysC excretion has only moderate diagnostic value. Abstract:Cystatin C has been shown to be a reliable and accurate marker of kidney function across diverse populations. The 2012 Kidney Disease Improving Global Outcomes (KDIGO) guidelines recommended using cystatin C to confirm the diagnosis of chronic kidney disease (CKD) determined by creatinine-based estimated glomerular filtration rate (eGFR) and to estimate kidney function when accurate eGFR estimates are needed for clinical decision-making. In the efforts to remove race from eGFR calculations in the United States, the National Kidney Foundation (NKF) and American Society of Nephrology (ASN) Joint Task Force recommended increasing availability and clinical adoption of cystatin C to assess kidney function. This review summarizes the key advantages and limitations of cystatin C use in clinical practice. Our goals were to review and discuss the literature on cystatin C; understand the evidence behind the recommendations for its use as a marker of kidney function to diagnose CKD and risk stratify patients for adverse outcomes; discuss the challenges of its use in clinical practice; and guide clinicians on its interpretation.

Abstract:This study was designed to evaluate the usefulness of serum cystatin C (CysC) as a marker of renal function in 83 patients with diabetic nephropathy, considering multiple factors including tubular function and body mass index. Serum CysC was assayed using a particle-enhanced nephelometric immunoassay and the glomerular filtration rate (GFR) was obtained by measuring the plasma disappearance of the isotope (99m)Tc-diethylenetriamine penta-acetic acid. By comparing the correlation of CysC and serum creatinine (SCr) with GFR, it was concluded that CysC may be a better indicator of GFR than SCr in diabetic patients, in both the early hypertransfusion stage and in the late renal dysfunction stage. CysC showed a slightly higher sensitivity for renal function evaluation than SCr in patients with renal tubular dysfunction and moderate to severe proteinuria. In addition, CysC was not affected by the metabolic index. Thus, CysC may serve as an ideal endogenous marker of GFR in patients with diabetic nephropathy.

Abstract:Objective To discuss the significance of serum cystatin C ( SCysC ) for glomerular filtration rate (GFR). Methods To assay the level of SCysC, serum creatinine (Scr) and 24 h creatinine clearance (Ccr) in 43 patients with nephropathy and 60 healthy subjects. Results SCysC , Scr and Ccr were highly correlated with GFR detected by 51Cr-EDTA (P 0. 01). SCysC was highly correlated with Scr and Ccr (P 0. 01). There was no obvious difference between the area of SCysC, Scr and Ccr under the ROC curve (P 0. 05). The level of SCysC was obviously higher than Scr in nephropathy patients with normal GFR (P 0. 05). Conclusion SCysC is a more sensitive marker for assessment of GFR in patients with renal diseases .
Evaluation of Renal Function in Patients with Diabetic Nephropathy Using Cystatin C

Abstract:Objective:To evaluate the Cystatin C(Cys-C)as a marker of renal function in patients with diabetic nephropathy(DN).Methodology:Eight three patients with DN were enrolled in this study.Serum Cys-C was tested by immunoturbidimetry,and serum creatinine(SCr)was measured at the same day.Radionuclide 99mTc-DTPA was used to evaluate glomerular filtration rate(ECT-GFR).The tubular markers(NAG enzyme,retinol binding protein,urine osmotic pressure),24-hour urine proteinuria excretion,serum albumin,and uric acid were also measured.The correlation between Cys-C,SCr,MDRD-GFR and ECT-GFR was evaluated in the different renal stages.Further more,the correlation of Cys-C and GFR was detected in different level of tubular function,proteinuria,serum albumin,uric acid and BMI.Receiver operator characteristic curve(ROC curve)of Cys-C and SCr was made by ECT-GFR as standard,and calculated the area under the curve(AUC).Results:(1)In the whole study population:the Pearson correlation coefficient of Cys-C,SCr and ECT-GFR was-0.740,-0.663,respectively(P0.01).Cys-C correlated with GFR better than SCr.When GFR was less than 60ml/min,the correlation coefficient of Cys-C,MDRD-GFR,SCr and GFR was-0.798,0.760,-0.716,respectively(P0.01).Cys-C correlated with GFR better than SCr or MDRD formula.When GFR was more than 90 ml/min,the correlation coefficient of Cys-C,SCr and GFR was-0.561,-0.465,respectively(P0.05).Cys-C was also correlated with GFR better than SCr.(2)In the patients with tubular impairment:when NAG enzyme was more than 20 u/g·cr,the correlation coefficient of Cys-C,SCr and GFR was-0.660,-0.595,respectively(P0.01);when RBP was more than 2 mg/L,the correlation coefficient was-0.672,-0.635,respectively(P0.01);and when Urine osmolarity was less than 600 mOsm/kg·H_2O,the correlation coefficient was-0.696,-0.663,respectively(P0.01).Cys-C was alsocorrelated with GFR better than SCr when tubular damage occurred.(3)When proteinuria was more than 1 g/24hr,the correlation coefficient of Cys-C,SCr and GFR was-0.704,0.649,respectively(P0.01).When the concentration of serum albumin was less than 35g/L,the correlation coefficient of Cys-C,SCr and GFR was-0.635,0.6581,respectively(P0.01).Cys-C was also correlated with GFR better than SCr in cases with heavy proteinuria.(4)In different levels of serum uric acid or BMI,the correlation was not affected.In the ROC,the AUC of Cys-C and SCr was 0.915 vs 0.902,respectively(P0.01)(GFR 90ml/min as cut off value),Cys-C was more sensitive and specific.Conclusion:Cys-C may be correlated with GFR better than SCr in type 2 diabetic patients with different renal stages,including early hypertransfusion period or overt renal dysfunction state.In patients with renal tubular dysfunction or medium to massive urine protein,renal function could be evaluated by Cys-C better.Cys-C is not affected by metabolic index.Thus,Cys-C could be an ideal endogenous marker of GFR in DN patients.

Abstract:This review examines the reliability of cystatin C as a biomarker for kidney function in paediatric populations. Chronic kidney disease (CKD) affects a significant number of children globally, leading to severe health complications such as anaemia, hypertension, and growth disorders. Traditionally, kidney function has been assessed using the estimated glomerular filtration rate derived from serum creatinine, though this method is flawed due to variability in muscle mass, age, gender, and diet. Cystatin C offers an alternative as it is less influenced by these factors. Evidence from various studies indicates that cystatin C provides a more accurate assessment of kidney function, especially in neonates and children with urinary tract malformations. Additionally, it is more reliable in early detection of acute kidney injury in paediatric intensive care units. Despite its potential, cystatin C is not yet widely adopted in clinical guidelines, primarily due to a lack of large-scale paediatric studies. Nonetheless, existing research supports its utility in providing a consistent and precise measure of kidney function across different paediatric age groups, suggesting that it could enhance early diagnosis and management of CKD in children if more extensive validation studies are conducted.
[Practical markers of renal function in cystinosis patients]

Abstract:Background: Cystinosis is an autossomic recessive systemic disease that leads to renal insufficiency early in life unless cysteamine be started early. Unfortunately, even in this situation the patients will develop chronic renal disease with need of renal replacement therapy about second decade of life. Therefore, the renal function evaluation is essential to these patients. The aim of this study was to evaluate cystatin C, serum creatinine and creatinine clearance estimated by stature (Schwartz Formula) in cystinosis patients, with different degrees of renal function, and to correlate these parameters. Methods: We studied cystinosis patients, aged lower than 18 years, with different degrees of renal function, classified according to KDOQI in Chronic Kidney Disease stage 1 to 4. No patient was under renal replacement therapy. In these patients we evaluate the serum creatinine, cystatin C and creatinine clearance according to Schwartz Formula. Results: We analyzed 103 blood samples of 26 patients. We detected a significant statistical correlation between serum creatinine and cystatin C (r = 0.81, p < 0.0001), cystatin C and creatinine clearance estimated by stature (r = -0.84, p < 0.0001) and between serum creatinine and creatinine clearance estimated by stature (r = -0.97, p < 0.0001). Conclusions: The expensive measurement of cystatin C showed no advantage in relation to serum creatinine and creatinine clearance according to Schwartz Formula in cystinosis patients to estimate the glomerular filtration rate. This is the first report checking the value of serum creatinine, creatinine clearance estimated by stature and cystatin C in cystinosis patients.

Abstract:Abstract Background and Aims Serum creatinine has been widely utilized to estimate glomerular filtration rate (eGFR), although various biomarkers have been developed to address its limitations. Among these biomarkers, the clinical use of Cystatin C is increasingly gaining recognition. However, there is a lack of evaluation regarding the utility of cystatin C in predicting the risk of postoperative acute kidney injury (PO-AKI) in non-cardiac surgeries. Method A retrospective cohort study was conducted on patients who underwent non-cardiac surgeries more than 1 hour from five departments (general surgery, neurosurgery, obstetrics and gynecology [OBGY], orthopedics, and urosurgery) at Chung-Ang University Hospital between 2018 and 2020. Patients with missing baseline information, including creatinine and cystatin C, and those without follow-up serum creatinine levels were excluded. PO-AKI was defined as KDIGO-AKI criteria occurring within 7 days after surgery. The estimated glomerular filtration rate (eGFR) was calculated from 2012 and 2021 chronic kidney disease-epidemiology collaboration (CKD-EPI) equations. Logistic regression was used to develop a prediction model, and C-statistics and the Delong test were used to compare the performance. Baseline comorbidities (diabetes mellitus [DM] and hypertension), laboratory findings (hypoalbuminemia [serum albumin <3.5 g/dL], anemia [hemoglobin <12 g/dL for female or <13 g/dL for male], and hyponatremia [sodium <135 mEq/L]), medication usage within 90 days of surgery (nonsteroidal anti-inflammatory drug and renin-angiotensin-system [RAS] blocker), and operation information (Anesthesia type, emergency operation, and operation duration) were adjusted. Results Overall, 339 patients were enrolled, with 49 patients (14.5%) developed PO-AKI. The median age was 73.0 [64.0; 80.0]; eGFR-Cr 86.4 [65.5; 99.1] mL/min/1.73 m2, eGFR-Cystatin C 70.0 [45.2; 92.7] mL/min/1.73 m2; eGFR-Cr/Cystatin C 77.2 [54.5; 100.5] mL/min/1.73 m2. Patients who developed AKI had a higher prevalence of diabetes (AKI 49.0%, no AKI 28.3%; p-value 0.006) and prescription of RAS blockers (AKI 63.3%, no AKI 34.5%; p-value <0.001), longer operation duration (AKI 63.3%, no AKI 34.5%; p-value <0.001), and lower eGFR of all types (AKI: eGFR-Cr 69.8, eGFR-Cystatin C 48.0, eGFR-Cr/Cystatin C 57.2; no-AKI: eGFR-Cr 88.0, eGFR-Cystatin C 74.0, eGFR-Cr/Cystatin C 80.5 mL/min/1.73 m2; p-values <0.001). No statistical differences were observed in other laboratory findings between the two groups (Table 1). The model with eGFR-Cystatin C demonstrated the highest area under the curve (AUC) among the three models (eGFR-Cr: AUC 0.78, 95% confidential interval [CI] 0.702‒0.857; eGFR-Cystatin C: AUC 0.81, 95% CI 0.735‒0.877; eGFR-Cr/Cystatin C: AUC 0.80, 95% CI 0.724‒0.871). Only the eGFR-Cystatin C-based model showed significant improvement in prediction compared to the eGFR-Cr-based model in the Delong test (eGFR-Cystatin C vs eGFR-Cr: p-value 0.033; eGFR-Cr/Cystatin C vs eGFR-Cr: p-value 0.055, Fig. 1). Conclusion Our findings suggest the potential usefulness of cystatin C in predicting the risk of PO-AKI non-cardiac surgery in addition to conventional PO-AKI risk factors. Further studies including larger and external cohorts, are needed for validation.

Abstract:Kidney function as part of metabolic changes could be associated with amyotrophic lateral-sclerosis (ALS). We investigated the associations between estimated chronic kidney disease (CKD), based on the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) cystatin C equation, and the risk at onset and prognostic value of CKD for ALS. Between October 2010 and June 2014, 362 ALS cases (59.4% men, mean age 65.7 years) and 681 controls (59.5% men, means age 66.3 years) were included in a population-based case-control study based on the ALS registry Swabia in Southern Germany. All ALS cases were followed-up (median 89.7 months), 317 died. Serum samples were measured for cystatin C to estimate the glomerular filtration rate (eGFR) according to the CKD-EPI equation. Information on covariates were assessed by an interview-based standardized questionnaire. Conditional logistic regression models were applied to calculate odds ratios (OR) for risk of ALS associated with eGFR/CKD stages. Time-to-death associated with renal parameters at baseline was assessed in ALS cases only. ALS cases were characterized by lower body mass index, slightly lower smoking prevalence, more intense occupational work and lower education than controls. Median serum cystatin-C based eGFR concentrations were lower in ALS cases than in controls (54.0 vs. 59.5 mL/min pro 1.73 m2). The prevalence of CKD stage ≥ 3 was slightly higher in ALS cases than in controls (14.1 vs. 11.0%). In the adjusted models, CKD stage 2 (OR 1.82, 95% CI 1.32, 2.52) and stage 3 (OR 2.34, 95% CI 1.38, 3.96) were associated with increased ALS risk. In this cohort of ALS cases, eGFR and CKD stage ≥ 3 (HR 0.94; 95% CI 0.64, 1.38) were not associated with prognosis. In this case-control study, higher CKD stages were associated with increased ALS risk, while in the prospective cohort of ALS cases, no indication of an association of CysC-based CKD on mortality was seen. In addition, our work strengthens the importance to evaluate renal function using a marker independent of muscle mass in ALS patients.
Markers of renal function at admission and mortality in hip fracture patients - a single center prospective observational study

Abstract:Plasma cystatin C and shrunken pore syndrome (SPS) are associated with increased mortality in older adults. The objective was to assess the association between these markers of kidney function at admission and mortality in hip fracture patients. Hip fracture patients presenting at Lund University Hospital were eligible for inclusion. Cox regression was used to assess association between plasma cystatin C, creatinine, cystatin C- or creatinine-based estimations of glomerular filtration rate (eGFRCYS and eGFRCREA), or SPS (defined as eGFRCYS/eGFRCREA < 0.7) and mortality during one year follow up. Improvement in discrimination relative to the Nottingham Hip fracture score was assessed by Receiver Operational Characteristics (ROC) analysis and calculation of Net Reclassification Index (NRI). 996 patients were included in the study. Cystatin C, creatinine, eGFRCYS and eGFRCREA were associated with one-year mortality in both unadjusted and adjusted analyses. The association with mortality was stronger for cystatin C and for eGFRCYS than for creatinine and eGFRCREA. Patients with SPS had doubled mortality compared with patients without SPS (43.7 and 20.2%, respectively, p < .001). Hazard ratio for SPS in the adjusted analysis was 1.66 (95%CI; 1.16–2.39, p = .006). None of the markers improved discrimination compared to the Nottingham Hip Fracture Score using ROC analysis whereas eGFRCYS and eGFRCREA improved NRI. Our conclusion is that plasma concentrations of creatinine or cystatin C, eGFRCYS or eGFRCREA or SPS at admission in hip fracture patients are associated with mortality when known risk factors are accounted for. Identification of high risk patients may be improved by eGFRCYS or eGFRCREA.

Serum cystatin C as a predictor for cardiovascular events in end-stage renal disease patients at the initiation of dialysis

Abstract:Background: There has been no study to investigate whether cystatin C could predict cardiovascular events in incident dialysis patients. We aimed to delineate the role of serum cystatin C and cystatin C-based estimated glomerular filtration rate (eGFR(cysC)) for prediction of cardiovascular events. Methods: This study included 66 end-stage renal disease patients who survived for >3 months after the start of dialysis, and serum cystain C levels were measured at the point of dialysis initiation. Results: Serum cystatin C was correlated with blood urea nitrogen (r = 0.537, p < 0.001), serum creatinine (r = 0.480, p < 0.001) and smoking (r = 0.284, p = 0.021). Cystatin C was inversely correlated with age (r = -0.316, p = 0.01) and eGFR(Cr) by modification of diet in renal disease (r = -0.533, p < 0.001). Kaplan-Meier analysis for cardiovascular events revealed that patients in the group with lower cystatin C levels (<4.14 mg/L) had a better event-free survival rate than patients in the group with higher cystatin C levels (≥4.14 mg/L) (p = 0.039). In univariate analysis, cystatin C (hazard ratio (HR) 2.62, p = 0.011) and eGFR(cysC) (HR 0.64, p = 0.004) were significant factors for the prediction of cardiovascular events. After multivariate adjustment, serum cystatin C and eGFR(cysC) were independent determinants of cardiovascular events (HR 3.952, p = 0.001 and HR 0.640, p = 0.004, respectively). Conclusion: Serum cystatin C might be an independent marker of cardiovascular events in incident dialysis patients. Furthermore, eGFR(cysC) based on measured serum cystatin C could have a new role in predicting cardiovascular events beyond the estimation of true GFR.

Abstract:Background and objectiveThe individual dosing of drugs that are mainly eliminated unchanged in the urine is made possible by assessing renal function. Most of the methods used are based on serum creatinine (SCr) levels. Cystatin C(CysC) has been proposed as an alternative endogenous marker of the glomerular filtration rate (GFR). Carboplatin is one of the drugs for which elimination is most dependent on the GFR. A prospective clinical trial including 45 patients was conducted to assess the value of serum CysC as a predictor of carboplatin clearance (CL).MethodsThe patients were receiving carboplatin as part of established protocols. Carboplatin was administered as a daily 60-minute infusion at doses ranging from 290 to 1700mg. A population pharmacokinetic analysis was performed using the nonlinear mixed effect modelling NONMEM program according to a two-compartment pharmacokinetic model.ResultsData from 30 patients were used to test the relationships between carboplatin CL and morphological, biological and demographic covariates previously proposed for prediction of the GFR. The interindividual variability of carboplatin CL decreased from 31% (no covariate) to 14% by taking into account five covariates (SCr, CysC, bodyweight [BW], age and sex). Prospective evaluation of these relationships using the data from the other 15 patients confirmed that the best equation to predict carboplatin CL was based on these five covariates, with a mean absolute percentage error of 13% as an assessment of precision. NONMEM analysis of the whole dataset (n = 45 patients) was performed. The best covariate equation corresponding to the overall analysis was: CL (mL/min) = 110 · (SCr/75)−0.512 · (CysC/1.0)−0.327 · (BW/65)0.474 · (age/ 56)−0.387 · 0.854sex, with SCr in μmol/L, CysC in mg/L, BW in kilograms, age in years and sex = 0 if male and 1 if female. To put the value of CysC as an endogenous marker of the GFR into perspective, covariate equations without SCr were also evaluated; a better prediction was obtained by considering CysC together with age and BW (interindividual variability of 16.6% vs 23.3% for CysC alone).ConclusionCysC is a marker of drug elimination that is at least as good as SCr for predicting carboplatin CL. The model based on five covariates was superior to those based on only four covariates (with BW, age and sex combined with either SCr or CysC), indicating that CysC and SCr are not completely redundant to each other. Further pharmacokinetic evaluation is needed to determine whether SCr or CysC is the better marker of renal elimination of other drugs.

Abstract:Purpose of review In this report, we summarize why the availability of cystatin C is important across a variety of clinical scenarios, the recent literature on when, why and in whom cystatin C testing should be considered, and how nephrologists can take practical steps to incorporate cystatin C testing into their practice. Recent findings Large intra-individual discrepancies between estimated glomerular filtration rate by creatinine (eGFRcr) and estimated glomerular filtration rate by creatinine eGFRcys (known as eGFRdiff) are observed in at least 1 in 4 people. These differences are seen more commonly among more vulnerable individuals: older adults, females, non-White individuals and those living with multiple medical conditions. A large eGFRdiff, where eGFRcys is lower than eGFRcr, is associated with a plethora of adverse outcomes, including medication-associated adverse events, acute kidney injury, cardiovascular disease, kidney failure and all-cause mortality. Among studies that have measured GFR, eGFRcr-cys usually provides the most accurate estimation of kidney function compared to mGFR, including among participants with large discrepancies between eGFRcr and eGFRcys. Summary Cystatin C improves sensitivity and specificity of chronic kidney disease diagnosis, improves detection of harmful acute and chronic changes in kidney function, improves precision of treatment eligibility and safety, and may reduce healthcare inequalities. Better education, curiosity, and motivation among nephrologists could substantially improve the availability and utilization of cystatin C.

Abstract:Abstract Background Neurogenic bladder due to myelomeningocele (MMC) is a significant risk factor for chronic kidney disease in children. Cystatin C (CysC) is a more accurate GFR marker than creatinine as it is unaffected by muscle mass but may be influenced by fat mass and BMI. This study evaluates: (1) GFR measurement accuracy using CysC and creatinine in MMC-related neurogenic bladder, (2) the relationship between body composition via bioelectrical impedance analysis (BIA) and renal parameters, and (3) the use of BIA for non-invasive GFR and body composition assessment. Methods Forty children (median age 9.96 years) underwent serum creatinine, CysC testing, and BIA measurements. We assessed age, sex, spinal lesion level, anthropometric measurements, BMI, and activity using Hoffer’s scale. GFR was calculated using five creatinine-based formulas, three CysC-based, and three combining CysC and creatinine, including BIA GFR as an alternative approach. Results Creatinine-based GFR estimates were significantly higher than CysC-based GFR. Although only 30% of MMC patients met the traditional BMI criteria for overweight/obesity, 62.5% were obese based on BIA-measured body fat percentage. Significant differences were found in CysC and CysC-based GFR equations within BMI and fat mass groups. Positive correlations were observed between CysC and body weight, BMI percentiles, body fat mass and fat-to-muscle ratio. Muscle mass positively correlated with creatinine. Conclusions BIA-determined fat mass percentage is a more sensitive obesity indicator than BMI in MMC patients. CysC levels and CysC-based GFR equations are influenced by body fat mass, requiring consideration of adiposity to avoid misestimating renal impairment. Graphical abstract

Abstract:Estimated glomerular filtration rate (eGFR) based on serum creatinine (sCr) improves early after left ventricular assist device (LVAD) implantation but subsequently declines. Although sCr is a commonly accepted clinical standard, cystatin C (CysC) has shown superiority in assessment of renal function in disease states characterized by muscle wasting. Among patients with an LVAD, we aimed to (1) longitudinally compare CysC-eGFR and sCr-eGFR, (2) assess their predictive value for early postoperative outcomes, and (3) investigate mechanisms which might explain potential discrepancies. A prospective cohort (n=116) with CysC and sCr concurrently measured at serial time points, and a retrospective cohort (n=91) with chest computed tomography performed within 40 days post-LVAD were studied. In the prospective cohort, the primary end point was a composite of in-hospital mortality, renal replacement therapy, or severe right ventricular failure. In the retrospective cohort, muscle mass was estimated using pectoralis muscle area indexed to body surface area (pectoralis muscle index). In the prospective cohort, sCr-eGFR significantly improved early post-LVAD and subsequently declined, whereas CysC-eGFR remained stable. CysC-eGFR but not sCr-eGFR predicted the primary end point: odds ratio per 5 mL/(min·1.73 m 2 ) decrease 1.16 (1.02–1.31) versus 0.99 (0.94–1.05). In retrospective cohort, for every 5 days post-LVAD, a 6% decrease in pectoralis muscle index was observed (95% CI, 2%–9%, P =0.003). After adjusting for time on LVAD, for every 1 cm 2 /m 2 decrease in pectoralis muscle index, there was a 4% decrease in 30-day post-LVAD sCr (95% CI, 1%–6%, P =0.004). Initial improvement in sCr-eGFR is likely due to muscle wasting following LVAD surgery. CysC may improve assessment of renal function and prediction of early postoperative outcomes in patients with an LVAD.
