Abstract:Isolated traumatic dislocation of the distal and proximal interphalangeal joints of fingers is a common occurrence. However, simultaneous double dislocation of the interphalangeal joints in one finger is rare. Here we report one case of simultaneous dorsal dislocation of both interphalangeal joints in the little finger with a review of the literature. Isolated traumatic dislocation of the distal interphalangeal (DIP) joints and the proximal interphalangeal (PIP) joints of fingers is a common occurrence. However, simultaneous dislocation of the DIP and PIP joints in a single finger seems to be a rare event. Most of these dislocations are secondary to high energy sport injuries and may accompany with volar plate or collateral ligament injury. Routinely, these injuries are managed with closed reduction followed by immobilization for a period of 2o4 weeks. The handle bar of the motorcycle struck the tip of the little finger causing marked swelling and deformity. A physical examination revealed a stepladder deformity of the finger. There were no neurovascular and skin lesions. Radiographs confirmed dorsal dislocation of the middle phalanx on the proximal phalanx and in addition showed dorsal dislocation of the distal phalanx on the head of the middle phalanx (Figure A). Reduction was easily obtained with a gentle longitudinal traction. Following the reduction, a stress test was performed for each joint of the corresponding finger. No joint instability was observed. The little finger was immobilized for 3 weeks and active range of motion was then allowed (Figure B). The first case of simultaneous double interphalangeal dislocation was described by Bartels in 1874.1 Thereafter, cases of simultaneous DIP and PIP dislocation were reported, most of which were caused by high energy sport injuries.2 Here, we report for the first time, to our knowledge, a case of simultaneous double DIP and PIP dislocation caused by a motorcycle accident. A common sense is agreed by all authors regarding this injury.3 The mechanism of this injury is usually an acute hyperextension force to the tip of the finger that tears the volar capsule of the DIP joint and causes the first dorsal dislocation at the DIP joint. If the force is not dissipated by dislocation of the DIP joint, it is then transmitted to the exposed distal end of the middle phalanx, which results in disruption of the volar capsule of the PIP joint and then in a second dorsal dislocation at the proximal joint. Treatment of reduction should be performed as quickly as possible. Closed reduction with longitudinal traction is often the first choice with or without anesthesia. Open reduction is recommended if there is soft tissue interposition that prohibits the procedure, or a fracture that needs fixation, or ligament injury that needs repair. In most reported cases, full function of the injured finger is ultimately regained, although recovery can be slow. The demography and feature of these double dislocations could be drawn according to the literature review and our own case.4 First, the number of cases is predominantly greater in males than females, and the most prevalent cause of injury is a sport accident. Second, the right hand is more often involved than the left hand. Third, the finger most often injured is the little finger followed by the ring finger, middle finger, and the index finger. Only one case involving the thumb was reported by Hutchinson et al.5 In most cases, immobilization is recommended within a 2m4 week period. In our case, a splint was utilized to immobilize the little finger. Three weeks later, active motion was allowed to gain full function. The function outcome was satisfactory.
