The rapid air quality improvement driven by the clean air actions in China since 2013 provides an opportunity for a quasiexperiment to investigate the beneficial effect of PM2.5 reduction on kidney function. Methods. Based on two repeated nationwide surveys of the same population of 5115 adults in 2011 and 2015, we conducted a difference-in-difference study. Variations in long-term exposure to ambient PM2.5 were associated with changes in kidney function biomarkers, including estimated glomerular filtration rate by serum creatinine (GFRscr) or cystatin C (GFRcys), blood urea nitrogen (BUN), and uric acid (UA). Results. For a 10 μg/m3 reduction in PM2.5, a significant improvement was observed for multiple kidney functional biomarkers, including GFRscr, BUN and UA, with a change of 0.42 (95% confidence interval [CI]: 0.06, 0.78) mL/min/1.73m2, -0.38 (-0.64, -0.12) mg/dL, and -0.06 (-0.12, -0.00) mg/dL, respectively. A lower socioeconomic status, indicated by rural residence or low educational level, enhanced the adverse effect of PM2.5 on kidney function. Conclusions. These results support a significant nephrotoxicity of PM2.5 based on multiple serum biomarkers and indicate a beneficial effect of improved air quality on kidney function.

Effect modification indicated these associations were significantly stronger in urban areas compared with rural areas, in males compared with females, in participants aged <65 years compared with participants aged ≥65 years, and in participants without comorbid diseases compared with those with comorbidities.CONCLUSIONS:These findings regarding the relationship between long-term exposure to high ambient PM2.5 levels and CKD in the general Chinese population provide important evidence for policy makers and public health practices to reduce the CKD risk posed by this pollutant.

Abstract:Background: Chronic kidney disease (CKD) is one of the important global public health challenges. However, evidence regarding the associations between PM2.5 exposure over a broad range and CKD prevalence in the general population is extremely limited. Our aim is to explore the associations between long-term exposure to ambient PM2.5 and prevalence of CKD based on the China National Survey of CKD. Methods: A cross-sectional survey of a nationally representative sample of 47 204 Chinese adults recruited at 13 provinces from different provinces was conducted from September 2009 to September 2010. Annual exposure to PM2.5 prior to the survey date was estimated at each participant’s address using validated satellite-based spatiotemporal model with a 10km×10km resolution. Participants with estimated glomerular filtration rate (eGFR) <60 mL/min/1・73 m 2 or with albuminuria were defined as CKD. Generalized additive mixed effects models were used to estimate the associations, and the influences of the potential modifiers were also analyzed. Findings: Two-year mean PM2.5 concentration was 57・4 μg/m3, with a range from 31・3 to 87・5 μg/m3. A 10 μg/m3 increase in PM2.5 was positively associated with the prevalence of CKD with an odds ratio (OR) of 1・33 (95% confidence interval [CI]: 1・25, 1・41) and albuminuria with an OR of 1・47 (95% CI: 1・38-1・58). Stratified analyses indicated the associations were stronger in participants <65 years (vs. participants ≥65 years), in participants with normal weight (vs. overweight participants), and in participants without comorbid diseases (vs. those with comorbid diseases including diabetes, hypertension, and cardiovascular disease). Interpretation: This study showed a significant association between long-term PM2.5 exposure over a broad range and increased prevalence of CKD in the national representative general population. The findings will provide important evidence for policy markers and public health practices to reduce the disease burden of CKD related to ambient PM2.5 pollution. Long-term Exposure to Varying-Sized Particulate Matters and Kidney Disease in Middle-Aged and Elder Adults: A 8-Year Nationwide Cohort Study in China.

Abstract:Evidence for the causal relationship of particulate matters (PMs) exposure with kidney disease, especially PM1, PM1-2.5 and PM2.5-10, remained scarce among developing countries with severe pollution. We conducted a longitudinal cohort study involving 13,041 adults with free kidney disease from 150 Chinese counties. PMs concentrations were generated using a well-established satellite-based spatiotemporal model. And the time -varying Cox regression model along with stratified analyses were performed to determine the association and potential modifiers, respectively. We also calculated the population-attributable fraction to evaluate the burden of kidney disease attributable to PMs pollution. Between Jan 2011 and Dec 2018, 985 kidney disease incidents were identified with an incidence rate of 12.69 per 1000 person-years. Significant dose-response relationships were observed for all 5 kinds PMs. Specifically, an increased risk of kidney disease was associated with per 10 mu g/ m3 increment of PM1 (HR = 1.187, 95%CI: 1.114 to 1.265), PM1-2.5 (1.326, 1.212 to 1.452), PM2.5 (1.197, 1.139 to 1.258), PM2.5-10 (1.297, 1.240 to 1.357), and PM10 (1.137, 1.108 to 1.166). A mixture analysis method of weighted quantile regression model revealed that PM2.5-10 predominated the PMs mixture index (57.1 %), and followed with PM10 (26.4 %). Stratified analyses indicated the elder, overweight persons, smokers, respiratory patients and urban residents were more vulnerable to PMs pollution than their counterparts. Calculated population attributable fractions of kidney disease attributable to PMs pollution was 16.67-39.47 %. Higher PMs pollution was associated with the increased risk of kidney disease development in China. Acceleration of efforts to reduce PMs pollution was therefore urgently needed to alleviate kidney disease burden.

Abstract:BACKGROUND:Chronic kidney disease (CKD) is a serious global public health challenge, but there is limited information on the connection between air pollution and risk of CKD. OBJECTIVE:The aim of this study was to investigate the association between long-term exposure to particulate matter (PM) with an aerodynamic diameter of less than [Formula: see text] ([Formula: see text]) and the development of CKD in a large cohort. METHODS:A total of 100,629 nonCKD Taiwanese residents age 20 y or above were included in this study between 2001 and 2014. Ambient [Formula: see text] concentration was estimated at each participant's address using a satellite-based spatiotemporal model. Incident CKD cases were identified by an estimated glomerular filtration rate (eGFR) of less than [Formula: see text]. We collected information on a wide range of potential confounders/modifiers during the medical examinations. Cox proportional hazard regression was applied to calculate hazard ratios (HRs). RESULTS:During the follow-up, 4,046 incident CKD cases were identified, and the incidence rate was 6.24 per 1,000 person-years. In contrast with participants with the first quintile exposure of [Formula: see text], participants with the fourth and fifth quintiles exposure of [Formula: see text] had increased risk of CKD development, adjusting for age, sex, educational level, smoking, drinking, body mass index, systolic blood pressure, fasting glucose, total cholesterol, and self-reported heart disease or stroke, with an HR [95% confidence interval (CI)] of 1.11 (1.02, 1.22) and 1.15 (1.05, 1.26), respectively. A significant concentration-response trend was observed ([Formula: see text]). Every [Formula: see text] increment in the [Formula: see text] concentration was associated with a 6% higher risk of developing CKD (HR: 1.06, 95% CI: 1.02, 1.10). Sensitivity and stratified analyses yielded similar results. CONCLUSIONS:Long-term exposure to ambient [Formula: see text] was associated with an increased risk of CKD development. Abstract:Particulate of diameter ≤ 1 µm (PM1) presents a novel risk factor of adverse health effects. Nevertheless, the association of PM1 with the risk of chronic kidney disease (CKD) in the general population is not well understood, particularly in regions with high PM1 levels like China. Based on a nationwide representative survey involving 47,204 adults and multi-source ambient air pollution inversion data, the present study evaluated the association of PM1 with CKD prevalence in China. The two-year average PM1, particulate of diameter ≤ 2.5 µm (PM2.5), and PM1-2.5 values were accessed using a satellite-based random forest approach. CKD was defined as estimated glomerular filtration rate < 60 ml/min/1.73 m2 or albuminuria. The results suggested that a 10 μg/m3 rise in PM1 was related to a higher CKD risk (odds ratio [OR], 1.13; 95% confidence interval [CI] 1.08-1.18) and albuminuria (OR, 1.11; 95% CI, 1.05-1.17). The association between PM1 and CKD was more evident among urban populations, older adults, and those without comorbidities such as diabetes or hypertension. Every 1% increase in the PM1/PM2.5 ratio was related to the prevalence of CKD (OR, 1.03; 95% CI, 1.03-1.04), but no significant relationship was found for PM1-2.5. In conclusion, the present study demonstrated long-term exposure to PM1 was associated with an increased risk of CKD in the general population and PM1 might play a leading role in the observed relationship of PM2.5 with the risk of CKD. These findings provide crucial evidence for developing air pollution control strategies to reduce the burden of CKD.

Abstract:Previous research has implicated PM2.5 as a potential environmental risk factor for CKD, but little is known about the associations between its components and CKD. We conducted a nationwide cross-sectional study using the updated air pollution data in the nationwide population (N = 2,938,653). Using generalized additive models, we assessed the association between long-term exposure to PM2.5 and its components (i.e., black carbon [BC], organic matter [OM], nitrate [NO3-], ammonium [NH4+], sulfate [SO42-]), and CKD prevalence. The air pollution data was estimated using high-resolution and high-quality spatiotemporal datasets of ground-level air pollutants in China. Besides, we adopted a novel quantile-based g-computation approach to assess the effect of a mixture of PM2.5 constituents on CKD prevalence. The average concentration of PM2.5 was 78.67 ± 22.5 μg/m3, which far exceeded WHO AQG. In the fully adjusted generalized additive model, at a 10 km × 10 km spatial resolution, the ORs per IQR increase in previous 1-year average PM2.5 exposures was 1.380 (95%CI: 1.345-1.415), for NH4+ was 1.094 (95%CI: 1.062-1.126), for BC was 1.604 (95%CI: 1.563-1.646), for NO3- was 1.094 (95%CI: 1.060-1.130), for SO42- was 1.239 (95%CI: 1.208-1.272), and for the OM was 1.387 (95%CI: 1.354-1.421), respectively. Subgroup analysis showed females, younger, and healthier were more vulnerable to this effect. In the further exploration of the joint effect of PM2.5 compositions (OR 1.234 [95%CI 1.222-1.246]) per quartile increase in all 5 PM2.5 components, we found that PM2.5SO42- contributed the most. These findings provide important evidence for the positive relationship between long-term exposure to PM2.5 and its chemical constituents and CKD prevalence in a Chinese health check-up population, and identified PM2.5SO42- has the highest contribution to this relationship. This study provides clinical and public health guidance for reducing specific air particle exposure for those at risk of CKD.

Abstract:Background: Chronic kidney disease (CKD) is one of the important global public health challenges. However, evidence regarding the associations between PM2.5 exposure over a broad range and CKD prevalence in the general population is extremely limited. Our aim is to explore the associations between long-term exposure to ambient PM2.5 and prevalence of CKD based on the China National Survey of CKD. Methods: A cross-sectional survey of a nationally representative sample of 47 204 Chinese adults recruited at 13 provinces from different provinces was conducted from September 2009 to September 2010. Annual exposure to PM2.5 prior to the survey date was estimated at each participant’s address using validated satellite-based spatiotemporal model with a 10km×10km resolution. Participants with estimated glomerular filtration rate (eGFR) <60 mL/min/1・73 m 2 or with albuminuria were defined as CKD. Generalized additive mixed effects models were used to estimate the associations, and the influences of the potential modifiers were also analyzed. Findings: Two-year mean PM2.5 concentration was 57・4 μg/m3, with a range from 31・3 to 87・5 μg/m3. A 10 μg/m3 increase in PM2.5 was positively associated with the prevalence of CKD with an odds ratio (OR) of 1・33 (95% confidence interval [CI]: 1・25, 1・41) and albuminuria with an OR of 1・47 (95% CI: 1・38-1・58). Stratified analyses indicated the associations were stronger in participants <65 years (vs. participants ≥65 years), in participants with normal weight (vs. overweight participants), and in participants without comorbid diseases (vs. those with comorbid diseases including diabetes, hypertension, and cardiovascular disease). Interpretation: This study showed a significant association between long-term PM2.5 exposure over a broad range and increased prevalence of CKD in the national representative general population. The findings will provide important evidence for policy markers and public health practices to reduce the disease burden of CKD related to ambient PM2.5 pollution. Abstract:We investigated the association of long-term exposure to atmospheric PM2.5 with non-accidental and cause-specific mortality in Yinzhou, China. From July 2015 to January 2018, a total of 29,564 individuals aged ≥ 40 years in Yinzhou were recruited for a prospective cohort study. We used the Cox proportional-hazards model to analyze the relationship of the 2-year average concentration of PM2.5 prior to the baseline with non-accidental and cause-specific mortality. The median PM2.5 concentration was 36.51 μg/m3 (range: 25.57–45.40 μg/m3). In model 4, the hazard ratios per 10 μg/m3 increment in PM2.5 were 1.25 (95%CI: 1.04–1.50) for non-accidental mortality and 1.38 (95%CI:1.02–1.86) for cardiovascular disease mortality. We observed no associations between PM2.5 and deaths from respiratory disease or cancer. In the subgroup analysis, interactions were observed between PM2.5 and age, as well as preventive measures on hazy days. The observed association between long-term exposure to atmospheric PM2.5 at a relatively moderate concentration and the risk of non-accidental and cardiovascular disease mortality among middle-aged and elderly Chinese adults could provide evidence for government decision-makers to revise environmental policies towards a more stringent standard.
[Association between exposure to PM_(2.5) and chronic kidney disease in a population with metabolic syndrome]

Abstract:Objective: To investigate the association between long-term fine particulate matter(PM_(2.5)) exposure and the risk of chronic kidney disease(CKD) in people with abnormal metabolism syndrome(MS) components. Methods: Based on health checkup data from a hospital in Beijing, a retrospective cohort study was used to collect annual checkup data from 2013-2019. A questionnaire was used to obtain information on demographic characteristics and lifestyle habits. We measured blood pressure, height, weight, waist circumference, concentrations of triglycerides(TG), fasting glucose, and high-density lipoprotein cholesterol(HDL-C). Longitude and latitude were also extracted from the addresses of the study subjects for pollutant exposure data estimation. Logistic regression models were used to explore the estimated effect of long-term PM_(2.5) exposure on the risk of CKD prevalence in people with abnormal MS components. Two-pollutant and multi-pollutant models were developed to test the stability of these result. Subgroup analysis was conducted based on age, the presence of MS, individual MS component abnormalities, and dual-component MS abnormalities. Results: The study included 1540 study subjects with abnormal MS components at baseline, 206 with CKD during the study period. The association between long-term PM_(2.5) exposure and increased risk of CKD in people with abnormal MS fractions was statistically significant, with a 2.26-fold increase in risk of CKD for every 10 μg/m~3 increase in PM_(2.5) exposure(OR=3.26, 95% CI 2.72-3.90). The result in the dual-pollutant models and multi-pollutant models suggested that the association between long-term PM_(2.5) exposure and increased risk of CKD in people with abnormal MS fractions remained stable after controlling for contemporaneous confounding by other air pollutants. The result of subgroup analysis revealed that individuals aged 45 or older, without MS, with TG<1.7 mmol/L, HDL-C≥1.04 mmol/L, without hypertension, and with central obesity and high blood sugar had a stronger association between PM_(2.5) exposure and CKD-related health effects. Conclusion: Long-term exposure to PM_(2.5) may increase the risk of CKD in people with abnormal MS components. More attention should be paid to middle-aged and elderly people aged ≥45 years, people with central obesity and hyperglycemia.

Abstract:Background Accumulating studies have linked ambient particulate matter (PM) pollution to impaired kidney function, but the results are still inconsistent and the effects of particulate matter with an aerodynamic diameter ≤1 μm (PM 1 ) on the kidney remain less clear. Objective This study aimed to investigate the association of long-term exposure to ambient PM 1 , particulate matter with an aerodynamic diameter ≤2.5 μm (PM 2.5 ), and particulate matter with an aerodynamic diameter ≤10 μm (PM 10 ) with kidney function in older adults. Methods In this cross-sectional study, we investigated 199,635 adults who were ≥65 years and underwent physical examinations in 695 community health service centers in Shenzhen, China from 2018 to 2019. An estimated glomerular filtration rate (eGFR) of each subject was derived by the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation. Chronic kidney disease (CKD) was defined as eGFR <60 ml/min/1.73 m 2 . Proteinuria was assessed by a urine dipstick test on freshly voided urine. A validated 10 km × 10 km grid dataset was used to assess long-term residential exposure to PM 1 , PM 2.5 , and PM 10 for each subject. Linear regression models and logistic regression models were implemented to quantify the association of exposure to PM with eGFR level, CKD, and proteinuria. Results Each 10 μg/m 3 increase of exposure to PM 1 , PM 2.5 , and PM 10 was associated with a 0.9%, 2.7%, and 1.0% decrease in eGFR level, an 18%, 36%, and 17% increase in odds of CKD, and a 15%, 11%, and 10% increase in odds of proteinuria, respectively (all p < 0.05). The associations of exposure to PM with the eGFR level and CKD were stronger among women and subjects who were aged <75 years, never smoked, and never drank alcohol. Conclusions Long-term exposure to ambient PM 1 , PM 2.5 , and PM 10 was significantly associated with kidney function impairment in older adults.

Abstract:Background: Limited studies have examined associations between air pollutants exposure and renal function, especially in China, with the most extensive chronic kidney disease (CKD) disease burden worldwide. Objectives: This study examines associations between long-term exposure to ambient PM2.5, NO2, CO, O-3, SO2 and renal function. Methods: We included 80,225 participants aged 30-79 years from the baseline data of the China Multi-Ethnic Cohort (CMEC) study. Three-year average concentrations of PM2.5, NO2, CO, O-3, and SO2 were estimated using satellite-based spatiotemporal models. Renal function is determined by the estimated glomerular filtration rate (eGFR) using Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation. After adjusting for covariates, generalized propensity scores (GPS) weighting regression was used to estimate associations between ambient air pollutants and renal function. Results: An increase of 0.1 mg/m(3) CO (OR [odds ratio] =1.20 95% CI [confidence interval], 1.05-1.37) was positively associated with CKD. An increase of 1 mu g/m(3) in SO2 (1.07, 1.00-1.14) concentration was positively associated with CKD. An increase of 10 mu g/m(3) in PM2.5 (1.17, 0.99-1.38), NO2 (1.12, 0.83-1.51) and O-3 (1.10, 0.81-1.50) concentration was not associated with CKD. These effects are stronger in those younger than 65, smoking and with low BMI. Conclusions: In this study, we found that long-term exposure to ambient CO and SO2 were positively associated with CKD. Gaseous pollutants should also arouse the concern of relevant departments.
Ambient Air Particulate Matter and Hospital Admissions for Chronic Kidney Disease in China: A Nationwide Case-Crossover Study

Abstract:Incident chronic kidney disease (CKD) may be accelerated (could be indicated by repeated admissions to the hospital) by environmental triggers such as ambient particulate matter (PM). Additionally, hospital admission is a sensitive proxy reflecting the disease burden. However, the association of PM exposure with hospital admissions for CKD is still unknown, let alone the excess risks (ERs) in hospital admissions for CKD due to high PM level exposure. In this study, a two-stage time-stratified case-crossover study was conducted to investigate the association of ambient air PM exposure with hospital admission for CKD in 282 Chinese cities of prefecture-level or above during 2013-2017. City-specific associations of single and cumulative 0-3 days lagged exposure to fine particulate matter (PM2.5) and inhalable particles (PM10) with hospital admissions for total CKD and its subtypes were evaluated by the conditional logistic regression model, then were pooled using the random-effect model. A total of 3,490,416 hospital admissions for CKD were identified. We found that per interquartile range (IQR) increment in PM2.5 at lag02 and per IQR increment in PM10 at lag03 were associated with increases of 2.36% (95%CI: 1.58%, 3.14%) and 2.87% (95% CI:1.91%, 3.85%) in hospital admissions for total CKD, respectively. Compared to control concentrations (PM2.5: 35 μg/m3; PM10: 50 μg/m3), the largest ERs in hospital admissions for total CKD were 2.63% (95% CI: 2.15%-3.11%) and 4.45% (95% CI: 3.85%-5.06%) in association with exposure to heavily excessive PM2.5 (≥75 μg/m3) and PM10 (≥150 μg/m3), respectively. Moreover, the attributable fractions (AFs) for CKD admissions were 2.83% for PM2.5 and 3.46% for PM10 during the study period. These findings suggested that exposure to PM2.5 and PM10 is associated with substantially increased risk and burden of CKD admissions.

Abstract:Evidence of associations between ambient fine particulate matter (PM 2.5 ) and risks of decline of kidney function and hyperuricemia is limited. We aimed to investigate the associations between long-term exposure to PM 2.5 with decline of kidney function and hyperuricemia in China. We conducted a two-stage study based on China Health and Retirement Longitudinal Study (CHARLS) from 2011 to 2015. Cox proportional hazard regression models and restricted cubic splines were used to evaluate the associations of PM 2.5 with risks of decline of kidney function and hyperuricemia. Latent class trajectory models (LCTM) were used to identify trajectories of PM 2.5 from 2011 to 2015 in the sensitivity analysis. A total of 9760 participants were included in baseline analysis, and 5902 participants were in follow-up analysis. PM 2.5 was associated with the risks of decline of kidney function [hazard ratio (HR): 2.14; 95% confidence interval (CI): (1.03, 4.44)] and hyperuricemia [HR 1.40 (95% CI: 1.10, 1.79)] in the second quartile group versus the lowest quartile group of PM 2.5 . We also observed nonlinear relationships between PM 2.5 and the risks of the decline of kidney function and hyperuricemia ( P non-linear < 0.001). In sensitivity analysis, four trajectory groups were identified. "Maintaining a high PM 2.5 " [odds ratio (OR): 2.20; 95%CI: (1.78, 2.73)] and "moderately high starting PM 2.5 then steadily decreased" [OR (95%CI): 5.15 (1.55, 16.13)] were associated with hyperuricemia risk, using "low starting PM 2.5 then steadily decreased" trajectory as reference. In conclusion, improved air quality is essential for prevention of decline of kidney function and hyperuricemia.
