Abstract:Background: There is conflicting evidence as to whether the femoral head should be preserved or replaced in elderly patients with displaced intracapsular femoral neck fractures. In this article, we performed a systematic review and meta-analysis to compare the short- and long-term effectiveness of arthroplasty (AR) and internal fixation (IF). Methods: PubMed, Embase, and the Cochrane Library were searched systematically up to January 2016. All randomized controlled trials directly comparing the effectiveness of AR and IF for displaced intracapsular fracture were retrieved with no limitation on language or publication year. Results: In total, eight prospective randomized studies involving 2206 patients were included. The results of our study showed that patients in the AR group reported significantly lower complication (risk ratio: 0.56, 95% confidence interval [CI] = 0.38-0.80), re-operation (risk ratio: 0.17, 95% CI = 0.13-0.22), revision rates (risk ratio: 0.11, 95% CI: 0.08-0.16), and better function compared with their IF counterparts, and they were less likely to suffer postoperative pain. No statistically significant differences for the rates of mortality, infection, and/or deep vein thrombosis between AR and IF were found. Conclusions: Based on our analysis, we recommend that AR should be used as the primary treatment for displaced intracapsular femoral neck fractures in the elderly. However, IF may be appropriate for those who are very frail.

Abstract:Objective To compare the therapeutic effects between arthroplasty and internal fixation for displaced femoral neck fractures in elderly patients. Methods Medline( 1966 to September, 2009),EMbase (January, 1966 to September, 2009), Cochrane Library (Issue 1 2008), CBM (up to September, 2009), and CNKI (up to September, 2009) were searched with computer for randomized and quasi-randomized controlled trials on hip arthroplasty versus internal fixation for displaced femoral neck fractures in elderly patients, supplemented by hand searching. Meta-analysis was conducted using the Cochrane Collaboration's RevMan 4. 2. 8 software. Results A total of 328 published papers were retrieved, but only 18 randomized controlled trials involving 2561 patients met the criteria of the present study. The Meta-analysis revealed that hip arthroplasty showed greater advantages than internal fixation in terms of lower re-operation rates at 2 years (RR=0. 13,95% CI 0. 09 ～0. 17) and 5 years (RR =0. 11,95% CI 0. 06 ～0. 22) after operation,and lower major complications at 2 years ( RR = 0. 20, 95% CI 0. 15 ～ 0. 27) and 5 years ( RR = 0. 18,95%CI 0. 11 ～ 0. 30) after operation. However, there were no significant differences in the mortality at one month and 2 years between the 2 groups, with RR 95% CI being ( RR = 1.42,95% CI0. 89 ～2. 24), and ( RR =1. 01,95% CI 0. 86 ～ 1.18) respectively. Conclusion For elderly patients with displaced femoral neck fractures, arthroplasty may surpass internal fixation in terms of lower rates of re-operation and complications.
[Comparing Different Treatments for Femoral Neck Fracture of Displacement Type in the Elderly:A Meta Analysis].

Abstract:OBJECTIVE:To compare the effects and security between internal fixation and total hip arthroplasty for the patients in elderly with femoral neck fracture of displacement type through a meta analysis. METHODS:Studies on comparison between internal fixation and total hip arthroplasty for the patients in the elderly with femoral neck fracture of displacement type were identified from PubMed database,EMBase database, COCHRANE library, CMB database, CNKI database and MEDLINE database. Data analysis were performed using Revman 5.2.6(the Cochrane Collaboration). RESULTS:Six published randomized controlled trials including 627 patients were suitable for the review, 286 cases in internal fixation group and 341 cases in total hip arthroplasty group. The results of meta analysis indicated that statistically significant difference were observed between the two groups in the quality of life which was reflected by the Harris scale (RR = 0.82, 95%CI:0.72-0.93, P < 0.05) , the reoperation rate (RR = 5.81, 95%CI:3.09-10.95, P < 0.05) and the major complications rate (RR = 3.60, 95%CI:2.29-5.67, P < 0.05) postoperatively. There were no difference in the mortality at 1 year and 5 years postoperatively(P > 0.05). CONCLUSIONS:For the patients with femoral neck fracture of displacement type in the elderly, there is no statistical difference between two groups in the mortality postoperatively. The quality of life and the security of operation in internal fixation group is worse than the total hip arthroplasty group.

Abstract:Treatment of displaced femoral neck fractures includes internal fixation and arthroplasty. However, whether arthroplasty or internal fixation is the primary treatment for displaced femoral neck fractures in elderly patients remains a subject for debate. The literature contains conflicting evidence regarding rates of mortality, revision surgery, major postoperative complications, and function in elderly patients with displaced femoral neck fractures treated either by internal fixation or arthroplasty (either hemiarthroplasty or THA).We determined mortality, revision surgery rates, major surgical complications (which include infection, nonunion or early redisplacement, avascular necrosis, dislocation, loosening of the prosthesis, acetabular erosion, fracture below or around the implant, and other severe general complications such as deep vein thrombosis and pulmonary embolism), and function in patients treated with either internal fixation or arthroplasty for displaced femoral neck fractures in the elderly.We searched PubMed, Embase, and the Cochrane Library for randomized controlled trials (RCTs) comparing internal fixation and arthroplasty. We identified 20 RCTs with 4508 patients meeting all the criteria for eligibility. We performed a meta-analysis of the major complications, reoperations, function, pain, and mortality.Compared with internal fixation, arthroplasty reduced the risk of the major complications (95% CI, 0.21-0.54 for 1 year; 95% CI, 0.16-0.31 for 5 years) and the incidence of reoperation 1 to 5 years after surgery (95% CI, 0.15-0.34 for 1 year; 95% CI, 0.08-0.24 for 5 years), and provided better pain relief (95% CI, 0.34-0.72). Function was superior (RR = 0.59; 95% CI, 0.44-0.79) for patients treated with arthroplasty than for patients treated by internal fixation. However, mortality 1 to 3 years after surgery was similar (95% CI, 0.96-1.23, p = 0.20 for 1 year; 95% CI, 0.91-1.17, p = 0.63 for 3 years).Arthroplasty can reduce the risk of major complications and the incidence of reoperation compared with internal fixation, and provide better pain relief and function, but it does not reduce mortality.Level II, prognostic study. Abstract:Objective To compare the effects of arthroplasty with that of intenal xation for displaced femoral neck fractures in the elderly. Methods We searched for all randomized controlled trials and quasi-randomized controlled trials of hip arthroplasty versus internal xation for displaced femoral neck fractures in the elderly by electronically searching MEDLINE( 1966 to September, 2006),EMbase(1966 to September, 2006), Cochrane Library( Issue 4 2006), CBM( up to September, 2006) and CNKI (September, 2006) and handsearching grey literatures. The quality of the trials was assessed and meta-analyses were conducted using The Cochrane Collaboration’s RevMan 4.2.8 software. The quality of the trials was assessed. And meta-analysis was conducted by using RevMan4.2.8 software. Results A total of 228 papers were retrieved, but only 15 published randomized controlled trials involving a total of 2 254 patients were suitable for inclusionin the review. Based on the meta-analyses, hip arthroplasty showed obvious advantages over internal xation in terms of the incidences of major complications after 2 years (RR 0.15 ; 95%CI 0.09 to 0.23) and after 5 years (RR 0.18 ; 95%CI 0.11 to 0.30) as well as re-operation rate (RR0.12 ; 95%CI 0.08 to 0.18) after 2 years, and after 5 years (RR0.11 ; 95%CI 0.06 to 0.22), there was an advantage to performing hip asthroplasty. After one year, the mortality was the same in both groups with RR 1.05 and 95%CI 0.89 to 1.23. Conclusion There is an evidence base to support arthroplasty as a treatment for displaced femoral neck fractures in the elderly. Arthroplasty can not only decrease the rate of re-operation, but can also reduce the incidence of complications, with similar one-year mortality when compared to internal xation.

Abstract:We conducted an up-to-date meta-analysis of 20 eligible randomised controlled trials (RCTs) containing 3,109 patients to compare arthroplasty with internal fixation of displaced femoral neck fractures regarding the effect on clinical outcomes. Computerised databases were searched for RCTs published from January 1979 to May 2008. The results showed that compared to internal fixation arthroplasty led to significantly fewer surgical complications at two and five years postoperatively and reduced the incidence of reoperation at one, two and five years postoperatively ( P < 0.001). However, arthroplasty was associated with greater risk of deep wound infection, longer operating time and greater operative blood loss. Arthroplasty substantially increased the risk of reoperation following deep wound infection ( P < 0.05). For mortality, there was increased postoperative risk for arthroplasty compared with internal fixation, but there was no statistically significant difference between the two groups at the different follow-up times. For pain at one year postoperatively, the result showed no statistically significant difference.

Abstract:Background Femoral neck fractures are common fractures in the elderly. Common treatment options include internal fixation (IF) and hemiarthroplasty (HA). However, the clinical application of these two options is always controversial due to the potential clinical trauma, postoperative function, early complications, and other factors. Materials and methods Randomized controlled trials and cohort studies comparing screw fixation and hemiarthroplasty in elderly patients with displaced femoral neck fractures were extracted from databases such as PubMed, Web of Science, EMBASE, and Cochrane. The revised Jadad scale or NOS treatment evaluation form was used to evaluate the quality of the included studies. After extracting the data, the standard deviation of continuous data and the relative risk of binary data were used. The operation time, blood loss during operation, EQ-5D (EuroQol-5 Dimension) score, mortality rate, reoperation rate, and postoperative common complications were reviewed using Review Manager software (RevMan 5.3) were compared. Results There were 7 randomized controlled trials and 5 cohort studies. The results showed that the operation time, intraoperative blood loss, and short-term EQ-5D score of the internal fixation group were lower than those of the hemi-hip replacement group, but the reoperation rate was higher. There was no statistically significant difference in mortality and common complications such as deep vein thrombosis, pulmonary embolism, infection, and pressure sores during short-term follow-up. Conclusions In the treatment of elderly femoral neck fractures, the screw internal fixation group has shorter operation time and less intraoperative bleeding, and the perioperative advantage is more obvious. However, the hemi-hip replacement group had more advantages in postoperative functional scoring and reoperation.
Efficacy of hemiarthroplasty and internal fixation in the treatment of undisplaced femoral neck fractures in the elderly

Abstract:Objective:To investigate the clinical efficacy of hemiarthroplasty and internal fixation in the treatment of undisplaced femoral neck fractures in the elderly.Methods:Totally, 72 elderly patients with undisplaced femoral neck fractures who were hospitalized at the First Affiliated Hospital of Zhengzhou University for surgical treatment from June 2016 to June 2019 were grouped: 35 in the hemiarthroplasty group and 37 in the internal fixation group. The operative time, intraoperative bleeding, hospital stay, reoperation rate, and postoperative complications were compared between the two groups; the improvement of hip function was evaluated using the Harris score for both procedures. The χ2 test was used for comparison between qualitative data, and one-way ANOVA was used for comparison between different time points within the measurement data groups, and t-test was used for comparison between two groups at the same time point. Results:All 72 patients completed the surgery successfully, and the internal fixation group had significantly shorter operative time [(43.6±6.9) min], less bleeding volume [(59.4±13.2) ml], shorter and hospital stay [(10.2±2.1) d] than the hemiarthroplasty group [operative time (73.9±8.8) min ( t=-16.234, P<0.05), bleeding volume (296.1±19.5) s ( t=-60.643, P<0.05), and hospitalization time (14.9±1.6) d ( t=-10.645, P<0.05)], and the differences were statistically significant. The overall complication rate in the internal fixation group [40.5% (15/37)] was significantly higher than that in the hemiarthroplasty group [17.1% (6/35), χ2=4.766, P<0.05]. The reoperation rate in the hemiarthroplasty group [2.9% (1/35)] was significantly lower than that in the internal fixation group [21.6% (8/37), χ2=5.790, P<0.05]. At 6 months postoperatively, the rate of excellent hip Harris score in the hemiarthroplasty group [68.6% (24/35)] was higher than that in the internal fixation group [40.5% (15/37), χ2=5.692, P<0.05]. However, at other follow-up time points, the two groups had similar improvement in hip function. Conclusion:For elderly patients with undisplaced femoral neck fractures, patients in the internal fixation group have shorter operative time, less bleeding, and shorter hospital stays. However, hemiarthroplasty has the advantages of fewer total postoperative complications, lower reoperation rate, and better early functional recovery. In terms of long-term hip function recovery, they have similar efficacy. The findings suggest that hemiarthroplasty may be a better option for some elderly patients.
An Meta Analysis of Arthroplasty Compared with Internal Fixation for Unstable Intertroehanteric Hip Fractures of Aged Patients in China

Abstract:Objective:To compare the effects of arthroplasty with that of intenal fixation for unstable intertrochanteric hip fractures of aged patients in China.Methods:We searched for all randomized controlled trials of hip arthroplasty versus internal fixation for unstable intertrochanteric hip fractures of aged patients by electronically searching EMBASE,PubMed,Cochrane Library,CBM,VIPand CNKI.The quality of the trials was assessed and Meta—analyses were conducted using The Cochrane Collaboration's RevMan 4.2.10 software.Results:A total of 199 papers were retrieved,but only 10 published randomized controlled trials involving a total of 797 patients were suitable for inclusion.Statistically significant differences were observed in operation time,blood loss,blood transfusion,complication of general medical and implantation,time of bedside activities and mortality of post-operation after 1 year.There was an advantage to performing hip asthroplasty.But no significant difference was noted in patient excellency of the hip joint.Conclusion:There is an evidence base to support arthroplasty as a treatment for unstable intertrochanteric hip fractures of aged patients.But internal fixation can not be substituted by arthroplasty.Large-scale,long-term and appropriately designed RCTs are required.

Abstract:Objective To evaluate the clinical efficacy of internal fixation versus hemiarthroplasty in the treatment of unstable intertrochanteric fractures in the elderly. Methods A search was conducted in PubMed, Web of Science, Embase, and Cochrane Library databases up to April 2020. The present study compared internal fixation and hemiarthroplasty in the treatment of unstable intertrochanteric fractures in the elderly. RevMan5.3 software provided by the International Cochrane Group was used for the meta‐analysis. To compare the differences in the operation time, intraoperative bleeding, length of hospital stay, superficial infection, Harris hip score, mortality within 1 year, mortality within 2 years, reoperation, incidence of deep venous thrombosis (DVT), partial weight‐bearing time, non‐union, and implant‐related complications between an internal fixation group and an hemiarthroplasty group. Results A total of 1300 patients were included in nine studies. The results showed that the operation time ( MD = −18.09, 95% CI : −27.85–−8.34, P = 0.0003), intraoperative bleeding ( MD = −195.31, 95% CI : −244.8–−147.74, P < 0.0001), implant‐related complications ( MD = 3.83, 95% CI : 1.74–8.45, P = 0.0008), and partial weight‐bearing time ( MD = 17.21, 95% CI : 1.63–32.79, P = 0.03) have statistical significance. However, there is not statistical significance for the Harris hip joint function scale (HHS) ( MD = 5.60, 95% CI : −1.13–12.33, P = 0.10), DVT ( MD = 1.02, 95% CI : 0.45–2.27, P = 0.97), length of hospital stay ( MD = ‐1.08, 95% CI : −2.82–0.66, P = 0.22), superficial Infection ( OR = 0.92, 95% CI : 0.43–1.98, P = 0.89), mortality within 1 year ( OR = 0.95, 95% CI : 0.61–1.48, P = 0.81), mortality within 2 years ( OR = 0.93, 95% CI : 0.61–1.43, P = 0.75), reoperation ( MD = 1.80, 95% CI : 0.64–5.04, P = 0.26), and non‐union ( OR = 1.20, 95% CI : 0.48–3.03, P = 0.70). The result of the subgroup analysis showed no significant differences between the less than 2 years follow‐up and the 2 years or more follow‐up group. The only difference was for the Harris hip score: the internal fixation group was superior to the hemiarthroplasty group in the less than 2 years subgroup analysis, while there was no difference between the internal fixation group hemiarthroplasty group in the 2 years or more subgroup analysis. Conclusion Compared with the internal fixation group, those in the hemiarthroplasty group could carry out weight‐bearing training early and implant‐related complications were reduced, but it requires longer operation time and there is greater intraoperative blood loss. There is no difference in mortality, the incidence of DVT, non‐union, HHS, reoperation, length of hospital stay, and superficial infection. Hemiarthroplasty may be a better choice for unstable intertrochanteric fractures in the elderly.

Abstract:BACKGROUND:There is limited evidence for the evaluation of the efficacy and safety of the hemiarthroplasty versus screw fixation in elderly patients with the displaced femoral neck fractures. Our current investigation aimed at assessing the complications, functional outcome, and revision rate of the patients (over 65 years old) who received internal fixation or hemiarthroplasty via a same senior surgeon.METHODS:A retrospective study was conducted on elderly patients with displaced femoral neck fractures from May 2014 to February 2018. The current study was carried out at our hospital and it was approved through our institutional review committee of West China Hospital. Inclusion criteria were as follows: the patients were 65 years or older, this is the anesthesia grade. The higher grade of the patients,the greater risk of surgery. level I-III, and the patients with displaced intracapsular fractures of the femoral neck, with the radiographic and clinical follow-up of 12 months or more. The major outcome was the revision rate between the 2 groups. And the secondary outcomes contained the life quality and functional outcome detected via utilizing the interview-administered and self-administered questionnaires, length of hospital stay, surgery time, and hip-related complications (such as hip dislocation, loosening or breakage of implant, wound problems, infection, osteolysis, neurovascular injury, and bone nonunion).RESULTS:It was assumed that hemiarthroplasty would result in fewer revisions or complications and better functional scores in comparison with internal fixation technique.

Abstract:Displaced intracapsular femoral neck (AO type 31 B2/3) fractures have various treatments, including internal fixation (IF), unipolar uncemented hemiarthroplasty (HA), bipolar uncemented HA, unipolar cemented HA, bipolar cemented HA, uncemented total hip replacement (THR), and cemented THR. Systematic literature retrieval was performed from the databases to compare them in a network meta-analysis. Forty studies (85 arms) containing 6141 patients were included. Overall, our network meta-analysis rank the orders of 7 procedures in reoperation, mortality, dislocation and infection, which indicates that IF may provide the highest reoperation incidence, unipolar cemented HA may provide the lowest reoperation incidence; uncemented THR contributes the highest dislocation incidence; and bipolar uncemented HA provides the lowest infection incidence. No differences in mortality were observed among the treatments. This conclusion is indirect; higher-quality direct comparisons are required.
Hip Arthroplasty Versus Internal Fixation for Treatment of Femoral Neck Fractures in the Elderly in China: a Meta-Analysis

Abstract:Objective To compare the clinical effects of internal fixation (IF) versus hip arthroplasty (HA) for the treatment of femoral neck fractures in the elderly (≥60 years of age) in China.Methods We electronically searched for all clinical trials in China comparing IF versus HA for the treatment of displaced femoral neck fractures in the elderly (≥60 years of age) in PubMed (2000 to 2012),OVID MEDLINE (2000 to 2012),SinoMed (2000 to 2012),Wanfang data (2000 to 2012),and China Knowledge Resource Integrated Database (2000 to 2012).A meta-analysis was conducted to assess postoperative one-year mortality,general complications,major complications,reoperation,and functional recovery between IF and HA.Quality of the trials was assessed.The meta-analysis was conducted using Stata 10.0 software.Results Altogether 24 eligible trials (4 randomized controlled trials and 20 observational studies) involving a total of 2738 patients were included in the present meta-analysis.The results showed no significant difference in postoperative one-year mortality [OR =0.97,95％ CI(0.44,2.11),P =0.929] between IF and HA.Patients undergoing HA had significantly lower rates of general complications [OR =0.48,95％ CI(0.31,0.74),P =0.001],major complications [OR =0.35,95％ CI(0.27,0.44),P ＜ 0.001] and reoperation [OR =0.24,95％ CI(0.18,0.33),P ＜ 0.001],but significantly better functional recovery of the hip joint than patients undergoing IF [OR=2.15,95％ CI (1.73,2.67),P ＜0.001].Conclusion HAmaybea better treatment than IF for elderly Chinese patients with displaced femoral neck fracture.

Abstract:Objective To compare the clinical efficacy of internal fixation versus hemiarthroplasty in the treatment of undisplaced femoral neck fracture in the aged patients.Methods From September 2009 to May 2014,63 consecutive patients of over 75 years old were treated surgically for undisplaced femoral neck fractures (Garden types Ⅰ and Ⅱ) at our department.Closed reduction and internal fixation with cannulated nails was adopted in 26 cases,including 1 1 males and 15 females (mean age,83.9 ±4.9 years).Femoral head arthroplasty was adopted in 37 cases,including 10 males and 27 females (mean age,81.6 ±3.4 years).The 2 groups were compared in terms of operation time,blood transfusion,major complications,reoperation rate,postoperative mortality,postoperative Visual Analog Scale (VAS) and Walking Ability Index (WAI) at different time points,and EQ-5D health states at the final follow-ups.Results The 63 patients were followed up for 12 to 68 months (average,39.9 months).The operation time (30.7 ± 16.2 min),hospitalization time (7.5 ± 3.4 d),transfusion number (1),hospitalization expenses (18,183 ± 4,820 Yuan) in the internal fixation group were significantly less than those in the hemiarthroplasty group (54.9 ± 19.2 min,13.7 ±3.0 d,12,and 30,843 ±7,866 Yuan,respectively) (P ＜ 0.05).There were no significant differences between the 2 groups in reoperation rate,postoperative mortality,or postoperative WAI at the final follow-ups (P ＞ 0.05).The postoperative VAS scores at 3 and 6 months and the final follow-up,and the WAI scores at 3 and 6 months in the hemiarthroplasty group were significantly lower than those in the internal fixation group while the EQ-5D scores in the hemiarthroplasty group were significantly higher than in the internal fixation group (P ＜ 0.05).Conclusions Internal fixation is simple,easy,and less invasive for the aged patients with undisplaced femoral neck fracture while hemiarthroplasty may result in less pain and better quality of life.The appropriate operation should be determined according to the systemic health status of a given patient and his or her demand for quality of life as well.

Abstract:The relative effects of internal fixation strategies for intertrochanteric fracture after operation remain uncertain. We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) to address this important issue. We searched PubMed, EMBASE and CENTRAL for RCTs that compared different internal fixation implants in patients with intertrochanteric fracture at 6-month follow-up or longer. We ultimately included 43 trials enrolling 6911 patients; most trials were small in sample sizes and events. Their risk of bias was generally unclear due to insufficient reporting. Because of these, no statistically significant differences were present from most of the comparisons across all the outcomes, and no definitive conclusions can be made. However, a number of trials compared two commonly used internal fixation strategies, gamma nail (GN) and sliding hip screw (SHS). There is good evidence suggesting that, compared to SHS, GN may increase the risk of cut out (OR = 1.87, 95% CI, 1.08 to 3.21), re-operation (OR = 1.61, 95% CI, 1.02 to 2.53), intra-operative (OR = 3.14, 95% CI, 1.34 to 7.35) and later fractures (OR = 3.67, 95% CI, 1.37 to 9.83). Future randomized trials or observational studies that are carefully designed and conducted are warranted to establish the effects of alternative internal fixation strategies for intertrochanteric fracture.

Abstract:Objective To evaluate the clinical outcome of the internal fixation and arthroplasty for the displaced femoral neck fractures in the middle-aged and elderly patients.Methods All the 80 middle-aged and elderly patients with displaced femoral neck fractures who had been treated from January 2006 to June 2009 were reviewed.Of them,25 cases received internal fixation(Group A),27 femoral head arthroplasty(FHA,Group B) and the other 28 total hip arthroplasty (THA,Group C).Comparisons were made among the 3 groups in hospitalization,operating time,intraoperative bleeding,postoperative complications,and functional recovery.Results The operating time and blood loss in Group C is the most than that in Group A and Group B.Group A have the most postoperative complication,Group B have the least Harris score,Group C have the more pain score than Group B,Group C have the most function score and Group B have the least.Statistical analysis showed that there is a significant difference in operating time,intraoperative bleeding,postoperative complications and postoperative Harris score,and no significant difference in the hospitalization,walking score and motion range.Conclusion Internal fixation can be applied to patients who are relatively young and active,have good quality of bone and can tolerate ambulation lately;THA is suitable for patients with the displaced femoral neck fractures of the elderly those who are active,have good general condition or have relatively longer life expectancy or preexistent hip diseases.FHA is suitable for patients who have a poor general condition or have serious medical diseases but can tolerate general surgery in preoperative assessment.
Comparison of three different procedures in the treatment of senile femoral neck fracture

Abstract:Objective To compare internal fixation and arthroplasty in curative effects for the femoral neck fractures in the aged population. Methods All the 182 old patients with femoral neck fracture who had been treated from January 1999 to August 2007 in our department were reviewed. Of them, 42 cases received internal fixation (Group A), 64 femoral head arthroplasty (Group B) and 76 total hip joint replacement (Group C) . The 3 groups were similar in age, fracture classification and preoperative health condition. Their average follow-up was 38 (25 to 65) months. Comparisons were made between the 3 groups in hospitalization, operating duration, intraoperative bleeding, ambulation time, postoperative complications, secondary surgery, and functional recovery. Results Group A had less operating duration and blood loss, a healing rate of 90.5% (38/42), a rate of avascular femoral head necrosis of 23.8% (10/42), and a re-operation rate of 26. 2% (11/42). In Groups B and C, the re-operation rate was 7.8% (5 cases) and 5.3% (4 cases) respectively. The good-to-excellent rates of functional recovery were 85.9% (55/64), 89.5% (68/76) and 61.9% (26/42) respectively in Groups B, C, and A. In operating duration, blood loss, ambulation time, complications, secondary surgery and functional recovery, there was a significant difference (P<0.05) between internal fixation and arthroplasty, but there was no significant difference in hospitalization (P>0.05). Conclusions In treatment of femoral neck fracture of Garden Ⅰ and Ⅱ in patients older than 60 years, internal fixation may be the first choice, and arthroplasty may be suitable for patients older than 65 years or those with fractures of Garden Ⅲ and Ⅳ.

Abstract:Objective To compare the clinical efficacy of extramedullary fixation system,intramedullary fixation system and hip arthroplasty in treatment of intertrochanteric fractures in the elderly.Methods The retrospective study involved 184 patients treated between January 2007 and January 2011,which were assigned to dynamic hip screw (DHS) group (n =99),locking plate group (n =19 ),intramedullary fixation group (n =25) and joint replacement group (n =41 ).There were 80 males and 104 females,at age range of 60-99 years,mean (72.4 ± 3.9) years.According to the modified Evan classification,the fractures included type Ⅰ A in 20 patients,type Ⅰ B in 21,type Ⅱ in 75 and type Ⅲ in 68.The preoperative conditions,postoperative weight-bearing time,fracture heating time,hip function and complications were comparatively studied among four groups. Results The patients were followed up for 7-22 months (mean 12 months).The four groups showed no significant differences regarding the gender,age,fracture types and medical complications.The postoperative weight-bearing time of the joint replacement group was significantly shorter than that of the other three groups,with statistical significance.The fracture healing time was insignificantly different among the four groups.The postoperative complications were found in 24 patients (13％),including 11 patients in the DHS group,six in the locking plate group,five in the intramedullary fixation group and two in the joint replacement group.Harris score was the highest in the joint replacement group,with statistical significance in comparison with the other three groups. Conclusions Intramedullary fixation should be the treatment of choice for the patients with unstable,or severe osteoporosis fractures and with intolerance of joint replacement.Artificial bipolar femoral head replacement is suitable for the elderly patients with unstable or severe osteoporosis fractures,capability of walk or tolerance of operation.

Abstract:OBJECTIVE To investigate the prognosis for prospective study of the elderly with displaced femoral neck fractures by internal fixation and joint replacement. METHODS The clinical data of 70 cases diagnosed for the elderly femoral neck frac- tures with comparable were randomly divided into two groups. A group with 38 cases was named for the arthroplasty group , us- ing arthroplasty for treatment. Another group with 32 cases named for the internal fixation group , namely the use of internal fixa- tion of femoral neck fracture. The blood loss, operative time, hospital stay and postoperative complications, postoperative deaths for two years, needed surgery were observed and recorded for a comparative analysis. RESULTS 70 patients were suc- cessfully completed the surgery. The surgery time, blood loss in the internal fixation group was significantly less than the tradi- tional group ( P ﹤ 0.05) , there was a statistically significant difference. The length of stay in the internal fixation group was longer than joint replacement. The postoperative complications, reoperation, and the number of death two years after surgery in the joint replacement group was significantly less than the internal fixation group (P﹤0.05), the difference was with statistically significance. CONCLUSION Both surgical methods have their advantages, joint replacement surgery has fewer complications, reoperation rate is low, and rate of deaths is low. Compared with it, the internal fixation has the advantage of less blood loss, small and cheap trauma.
