Abstract:Purpose: Colorectal cancer (CRC) is one of the most frequent gastrointestinal malignancies in China, and the mortality is on the rise. The mechanism of CRC progression is not clear. Studies showed that inflammation was closely associated with cancer progression. A new type of CD4+ effector T cell that is different from Th1 and Th2 cells was identified. Interleukin-17 (IL-17) is its major effect cytokine that induces polymorphonuclear neutrophil (PMN) recruitment and involves in the inflammatory and autoimmune diseases. Recently, investigation on the relationship between IL-17 and cancer draws much attention of the researchers. This study was initiated to explore the expression of IL-17 in CRC and its potential influence on cancer progression. Methods: Seventy-eight patients diagnosed with CRC were recruited from Sun Yat-Sen Memorial Hospital. Thirty-six patients with colorectal adenoma were also recruited. From the 78 CRC patients, we randomly chose 40 cases and collected non-tumor tissues at 10cm from the edge of the resected tumor. Immunohistochemical staining was carried out using anti-IL-17 and anti-CD15 (targeting PMNs) antibody, respectively. Informed consent was obtained from all of the participants. The study was approved by the Ethics Committee of Sun Yat-Sen University. Results: IL-17 was positive in 67 of 78 CRC specimens, while only 22 of 36 adenoma and 22 of 40 non-tumor tissues showed positive IL-17 staining (P<0.01). The expression of IL-17 in CRC was correlated with Duke's staging and histological grading (P<0.01), respectively. However, it was not correlated with age, gender, tumor size, lymph node metastasis and distal metastasis (P>0.05). Both Kaplan-Meier analysis and Multivariate Cox proportional hazards regression analysis showed that the expression of IL-17 was able to independently predict the overall survival of CRC. Positive expression of IL-17 was the marker of favorable prognosis. The amount of PMNs infiltrated in the CRC specimens was 48.94±30.79 (per 400x view), whereas that in the adenoma was 6.83±3.01 and that in the non-tumor tissues was 6.28±2.72 (P<0.01). CRC specimens were divided into two groups by the median number of infiltrated PMNs (< 31/hp as low infiltrating group, >31/hp as high infiltrating group). Spearman's correlation analysis indicated that the amount of infiltrated PMNs was positively correlated with the expression of IL-17. However, Kaplan-Meier analysis showed that the amount of infiltrated PMNs did not affect the overall survival of CRC. Conclusion: IL-17 was the marker of favorable prognosis in CRC. The amount of infiltrated PMNs was positively correlated with the expression of IL-17. However, it did not affect the overall survival of CRC.
