Risk factors associated with persistent lesion in patients with positive margins after conization for HSIL: A study of 200 cases

Abstract:Objective To explore the risk, relevant factors and management strategies of persistent lesion in patients with positive margins after conization for high-grade squamous intraepithelial lesion (HSIL).Methods A total of 200 patients with positive margins after conization for HSIL or cervical intraepithelial neoplasia (CIN) 2,3 treated in Peking University First Hospital between January 2013 and January 2016 were collected and clinical data including age, lesion grade after conization, area of positive margin and liquid-based thin-preparation cytology test (TCT), human papilloma virus (HPV) detection, colposcopy or endocervical curettage (ECC) in six month of follow-up was analyzed retrospectively.Risk, related factors and management strategies for persistent existence of above lesion after conization for HSIL were explored.Results Age of 200 patients with positive margin after cervical conization ranged from 22 to 68 with an average age of 39.92±9.12 years.Only 2 cases in patients below 35 years showed lesion persistence, accounting for 3.39% (2/59), while among patients at and above 35 years of age, 27 cases showed lesion persistence, accounting for 19.15% (27/141), and difference was statistically significant between two groups (χ2=8.332,P=0.004).Twenty-five cases among patients with HSIL/CIN3+ showed lesion persistence after conization, accounting for 18.94% (25/132).In patients with HSIL/CIN2, 4 cases were found with lesion persistence, accounting for 5.88% (4/68), and difference was statistically significant (χ2=6.172,P=0.018).Three cases in patients with positive outer margin showed lesion persistence, accounting for 4.84% (3/62), and 26 cases in patients with positive inner margin showed lesion persistence, accounting for 18.84% (26/138), and difference in lesion persistence between patients with different positive margin area had statistical significance (χ2=6.765,P=0.009).Logistic regression analysis on patient''s age, lesion grade after conization and positive margin area revealed that lesion grade was independent risk factor of HSIL+ lesion persistence after conization (OR=3.158,95%,P=0.009).Conclusion Attention should be paid to patients with positive margins after conization, particularly to those with HSIL/CIN3+ grade after conization, and intensive follow-up is needed.
