The Prognostic Value of Serum Homocysteine Level in Patients with Acute Myocardial Infarction after Primary Percutaneous Coronary Intervention

Abstract:Objective To investigate the influence of elevated serum homocysteine level on microvascular flow and survival in patients with acute ST-segment elevation myocardial infarction after primary percutaneous coronary intervention(PCI).Methods A total of 96 patients with STEMI underwent primary PCI were divided into 2 groups according to the serum homocysteine level after admission:group A:15μmol/L,group B:≥15μmol/L.TIMI flow and TMPG after PCI and cardiac function in the first week after PCI were assessed.Results Rates of 2-vessel or 3-vessel diseases were similar between two groups(48.1%vs 65.9%,P0.05).Rates of grade 3 TIMI flow after PCI were similar between two groups(90.4%vs 90.9%,P0.05).Rate of grade 0-2 of TIMI myocardial perfusion grade(TMPG)after PCI was higher in group B(21.2%vs 45.5%,P0.05).rate of grade 3 of TMPG after PCI was higher in group A(78.8%vs 54.5%,P0.05).LVEF was higher in group A than that of group B(56.6±8.9)%vs(49.0±8.1)%,P0.01.Conclusion Elevated serum homocysteine level in acute ST-elevation myocardial infarction patients treated with primary PCI is associated with reduced myocardial microvascular flow and cardiac function.
