Clinical distribution and drug resistance genes of carbapenem-resistant Enterobacteriaceae strains in a hospital of Ningxia

Abstract:OBJECTIVE To investigate the clinical distribution,drug resistance rates and drug resistance mechanisms of the carbapenem-resistant Enterobacteriaceae (CRE) strains so as to provide guidance for reasonable clinical use of antibiotics.METHODS All of the CRE strains that were isolated from Jan 2014 to Dec 2016 were taken for statistics by using WHONET5.6 software,the drug susceptibility testing was performed by of MIC method,the carbapenemases were detected by means of modified Hodge test and EDTA synergy test,and the drug resistance genes of carbapenemases were detected and sequenced by using PCR.RESULTS A total of 70 strains of CRE were isolated,28 of which were Enterobacter cloacae,and 23 were Klebsiella pneumoniae;most of the strains were isolated from department of hepatobiliary surgery.The drug resistance rates to penicillins and cephalosporins were more than 90%.The positive rates of the modified Hodge test and EDTA synergy test were 77.1% and 87.1%,respectively.The positive rate of the drug resistance gene NDM-1 was the highest (75.7 %),the positive rates of the drug resistance genes KPC-2 and IMP-4 were 17.1% and 11.4%,respectively.Totally 3 strains carried with two drug resistance genotypes at the same time,while the drug resistance genes GES-4,VIM-2 and OXA-48 were not detected.CONCLUSION The NDM-1 is dominant among the drug resistance genes of carbapenemases.It is necessary for the hospital to strengthen the surveillance of drug resistance of the strains and reasonably use antibiotics so as to prevent the outbreak of infection epidemic.
