Distribution of pathogens and changes of cytokines in peripheral blood of patients after abdominal surgery

Abstract:OBJECTIVE To investigate the infection status of patients with abdominal surgery and further analyze the distribution of the bacteria and the changes of cytokines in peripheral blood.METHODS A total of 643 cases of abdominal surgery patients during the clinical treatment from Dec.2013 to Dec.2015 were chosen.The incidence of infection after laparoscopic surgery was statistically analyzed.The risk factors of infections after laparoscopic surgery were analyzed by univariate analysis and multivariate regression analysis,and the pathogens causing infections were detected.The peripheral blood cytokines levels of IL-8,IL-1β,IL-6,IL 10,TNF-α and IL-12p70 in non-infected and infected patients were observed and compared.RESULTS Totally 64 cases were infected in the 643 cases of laparoscopic surgery patients,and the infection rate was 9.95%.Logistic analysis showed that lung disease,hypoproteinemia,operation time,body mass index,diabetes and gastrointestinal function recovery time were independent risk factors for infection after laparoscopic surgery.Totally 81 strains of pathogens were detected in the 64 cases of pathogenic bacteria,the gram-negative bacteria,gram-positive bacteria and fungi accounted for 66.67%,28.40% and 4.94%,respectively.The levels of IL 8,IL-1β,IL-6,IL-10,TNF-α and IL-12p70 in the infection group were significantly higher than those in the non-infection group (P<0.05).CONCLUSION Postoperative infection of abdominal surgery patients should be paid attention in clinical treatment.The pathogens causing postoperative infection are mainly gram-negative bacteria,operation time,body mass index,hospitalization time,diabetes mellitus,lung disease,low protein blood and gastrointestinal function recovery time are the risk factors leading to postoperative infections,and the peripheral blood cytokines levels of the infected patients will be significantly increased.
