Risk factors of mortality in patients with bloodstream infections due to Acinetobacter baumannii

Abstract:OBJECTIVE To investigate the clinical manifestations,outcomes and the risk factors of mortality in patients with bloodstream infections due to Acinetobacter baumannii (ABA).METHODS A total of 148 patients with bloodstream infections due to ABA from Jan.2013 to Sep.2016 were retrospectively analyzed.The following data were collected:age,gender,underlying diseases,immunosuppressant therapy,duration of hospital stay and ICU stay before the onset of bacteremia,surgical procedure,mechanical ventilation,central venous catheters,prior antibiotic exposure,co-infection with other pathogens,temperature,levels of serum albumin and white blood cell,sequential organ failure assessment (SOFA) score,time to blood culture positivity,drug resistance,antimicrobial therapy and the outcomes of patients.The risk factors were analyzed by multivariable logistic regression analysis.RESULTS The mean survival time was 14.03 days,and the overall 14-day mortality was 35.8%.Among the 148 patients,the MDR-AB infections accounted for 123 cases(83.1%).Multivariate analysis showed that,SOFA scores≥ 9 and MDR-AB infection were independent risk factors of mortality.CONCLUSION The mortality of bloodstream infections due to AB is high.SOFA scores≥9 and MDR-AB infection are associated with poor prognosis.Early identification and effective therapy for high risk patients may improve the prognosis.
