Procalcitonin,C-reactive protein and interleukin-6 in differential diagnosis of gram-positive and gram-negative bacterial bloodstream infections

Abstract:OBJECTIVE To discuss the value of C‐reactive protein (CRP) ,procalcitonin (PCT ) and interleukin‐6 (IL‐6) in differential diagnosis of patients with bloodstream infections caused by gram‐positive and gram‐negative bacteria so as to better facilitate early clinical differential diagnosis .METHODS Totally 223 patients with blood‐stream infections whose two consecutive blood culture results were positive from Jan .2012 to Jan .2013 in the hos‐pital were selected ,including 102 patients with bloodstream infection caused by gram‐positive bacteria and 121 pa‐tients caused by gram‐negative bacteria .Another 100 healthy people were set as the control group .Blood samples were collected in the same day when blood culture samples were collected and the concentrations of CRP ,PCT and IL‐6 were tested in each group .The Mann‐Whitney U test was used to compare the differences in the three indica‐tors among the three groups ,using receiver operating characteristic curves (ROC curves) to analyze the role of in‐dicators in identification of gram‐positive and gram‐negative bacterial bloodstream infections .RESULTS Concentra‐tion of CRP ,PCT and IL‐6 in patients with bloodstream infections was significantly higher than the healthy con‐trol group (P<0 .05) ,the medians of CRP ,PCT and IL‐6 in the control group were 0 .50 mg/dl ,0 .09 ng/ml , 2 .10 pg/ml ,respectively ,which were 4 .70 mg/dl ,1 .37 ng/ml ,47 .58 pg/ml in the group with gram‐positive bacterial bloodstream infection ,and 7 .56 mg/dl ,3 .93 ng/ml ,276 .20 pg/ml in the group with gram‐negative bac‐terial bloodstream infection .There was no significant difference in the area under the ROC curves of CRP and PCT between the group with gram‐positive bacterial bloodstream infection (0 .889 and 0 .894) and the group with gram‐negative bacterial bloodstream infection (0 .963 and 9 .952) ,but the area under the ROC curve of IL‐6 in the group with gram‐negative bacterial infection (0 .967) was significantly higher than that of gram‐positive bacterial blood‐stream infection (0 .804) (P<0 .05) .CONCLUSION There was no difference in CRP and PCT between gram‐posi‐tive bacterial and gram‐negative bacterial bloodstream infections ,however ,the increase of IL‐6 in gram‐negative bacterial bloodstream infection was significantly higher than that in gram‐positive bacterial bloodstream infection .
