Influencing factors for nosocomial infections in patient with acute leukemia

Abstract:OBJECTIVE To analyze the clinical characteristics of the acute leukemia patients with nosocomial infec-tions ,observe the distribution of pathogens and the influencing factors for nosocomial infections ,and put forward targeted prevention measures so as to raise the survival rate of the patients .METHODS The patients with acute leukemia who were hospitalized the blood center of a three A hospital in Shandong province from Jan 2012 to Dec 2012 were recruited as the study objects .By means of the prospective targeted surveillance and self-designed ques-tionnaire ,the baseline data of the acute leukemia patients and the incidence of nosocomial infections were analyzed . RESULTS Of totally 333 cases of acute leukemia investigated ,the nosocomial infections occurred in 96 (104 case-times) cases with the infection rate of 28 .83% ( the case-time infection rate of 31 .23% ) ,among whom the pa-tients with upper respiratory tract infections accounted for 40 .38% , the patients with pulmonary infections 33 .65% .A total of 162 strains of pathogens have been isolated ,among which there were 89 (54 .94% ) strains of gram-positive bacteria ,67 (41 .36% ) strains of gram-negative bacteria ,and 6 (3 .70% ) strains of fungi .The non-conditional logistic regression analysis of the influencing factors for nosocomial infections indicated that the length of hospital stay ,community-acquired infection ,use of immunomodulatory agents ,chemotherapy during the hospi-tal stay ,and neutrophil count were the influencing factors for the nosocomial infections .CONCLUSION It is an ef-fective way to shorten the length of hospital as much as possible ,actively treat the community-acquired infection , appropriately increase the use of immunomodulatory agents ,and intensify the monitoring of the chemotherapy pa-tients to avoid the decrease of neutrophil counts so as to reduce the incidence of nosocomial infections in the pa-tients with acute leukemia .
