Relationship between T lymphocyte counts and severity of pneumonia

Abstract:OBJECTIVE To explore the relationship between the CD4 + T lymphocytes counts and the severity of pneumonia and understand its correlation with the prognosis .METHODS The clinical data of 100 pneumonia patients who were hospitalized from Dec 2012 to Dec 2013 were retrospectively analyzed ,then the patients were divided into the no less than 200 /mm3 group and the less than 200 /mm3 group according to the CD4 + T lymphocytes counts ,the severity of the pneumonia was evaluated ;the incidence of shock ,mechanical ventilation , length of ICU stay ,and mortality were compared between the two groups ;the CD4 + T lymphocytes counts were compared between the patients aged no less than 65 years old and the patients aged less than 65 years old ,and the severity of pneumonia of the two age groups of patients was evaluated .RESULTS The CURB-65 score and PSI score of the patients with the CD4 + T lymphocytes counts less than 200/mm3 were higher than those of the patients with the CD4 + T lymphocytes counts no less than 200/mm3 ,there was statistically significant difference (P<0 .05) .The incidence of shock ,rate of mechanical ventilation ,and mortality were higher in the patients with the CD4 + T lymphocytes counts less than 200/mm3 than in the patients with the CD4 + T lymphocytes counts no less than 200 /mm3 ,and the length of ICU stay of the patients with the CD4 + T lymphocytes counts less than 200/mm3 was longer than that of the patients with the CD4 + T lymphocytes counts no less than 200 /mm3 .There was no statistically significant difference in the CD4 + T lymphocytes counts between the patients aged no less than 65 years old and the patients aged less than 65 years old .As compared with the two age groups of patients ,the mortality of the patients with the CD4 + T lymphocytes counts less than 200 /mm3 was higher than that of the patients with the CD4 + T lymphocytes counts no less than 200 /mm3 (P<0 .05) .CONCLUSIONS In the early stage of onset of pneumonia ,it is more severe in the patients with the reduced CD4 + T lymphocytes counts ,and the prognosis is poorer ,indicating that the determination of CD4 + T lymphocytes counts in early stage of pneumonia has the effect on prediction of the severity of disease as well as the prognosis .
