The Economic Impact of Chronic Disease in Rural China

Abstract:Introduction: China is facing significant health care challenges, not only with infectious diseases but also with the burden of chronic diseases. Particularly in rural China, its residents are facing serious problems in seeking health care services due to their limited healthcare resource and insufficient health insurance. The purpose of this study was to explore how chronic having diseases and other influencing factors such as gender, affect household income, and, health insurance in rural China. Methods: A cross-sectional survey was conducted among households in three counties from three provinces of China, namely Zhejiang, Hebei, and Sichuan Provinces. The survey (N = 1887) included 942 residents with chronic diseases and 945 residents without chronic diseases. The items of questionnaire include age, gender, education, family size, social economic status, occupation, work hours, type of chronic diseases, health utilization and cost, medical insurance and etc. Results: There were no significant differences between two groups of the residents with and without chronic diseases in the proportion of female, marriage status, family size, annual per capita income and expenditure, and the proportion of having New Cooperation Medical Insurance. The illiteracy rate of the residents with chronic diseases (42.5%) was higher than that of the residents without chronic diseases (24.5%). The proportion of having an outside work was higher in the group without chronic diseases than the one with chronic diseases. We found that the incidence of catastrophic health payments and poverty rate were higher in the group of the residents with chronic diseases than the group without chronic diseases, and higher for female than male, higher for uninsured residents than insured ones. We also found the strong positive association between chronic diseases and household income, especially for households with their heads suffering from chronic diseases. These positive relationships demonstrate that having a chronic disease is one of most important factors for the problem that Poverty Resulting from Sickness and the problem that Bring Residents back into Poverty. If we controlled the type of work, there was no significant difference in household income between the group with chronic diseases and the group without chronic diseases. Conclusions: Having a chronic disease has dramatically affected households' income in rural China, especial for the female with chronic diseases. The type of work is the intermediate outcome for chronic diseases affecting the household income. We believe that constructing New Rural Cooperative Medical Scheme could relieve some economic burdens to an extent for households in rural areas, but not completely, and multilevel health insurance schemes, which are available in urban areas, may need to be built in rural areas as well as.
