Application value of fecal calprotectin by immunofluorescence assay in inflammatory bowel disease

Abstract:Objective To compared the immunofluorescence assay and ELISA assay for the assessment of fecal calprotectin (FC) and evaluated its application by immunofluorescence assay in inflammatory bowel disease. Methods 103 stool samples(77 cases of IBD and 26 controls)were detected by the immunofluorescence assay method and ELISA method. The sensitivity,specificity and correlation coefficients were analyzed. Stool samples of 122 patients with IBD(85 cases of CD and 37 cases of UC)undergoing colonoscopy were collected to evaluate the application of FC in IBD. These patients′clinical disease activities were assessed according to CDAI or UCAI. Endoscopic activity was assessed according to Mayo score , CDEIS or Rutgeerts′ score. Results Compared with ELISA method ,the sensitivity of the immunofluorescence assay was 92.30%,the specificity was 64.00%,the Spearman′s correlation coefficient rank was 0.815(95%CI:0.737~0.871,P<0.05). When FC was detected by immunofluorescence assay ,the FC levels of IBD patients in endoscopic inactivestage and endoscopic active stage were as follow:387.78(30.28~679.99)μg/g and 925.61(408.56~1634.00)μg/g(P<0.05). The prediction of"endoscopic active"with FC determined by immunofluorescence assay (cut?off=300.37 μg/g) presented a sensitivity of 76.60% and a specificity of 60.71%. Among 18 postoperative CD patients,72.22%(13/18) confirmed recurrence by endoscopy ,of which the median FC level was much higher (P<0.005). The prediction of"postoperative recurrence"exhibited a sensitivity of 76.92%,a specificity of 100%,and an AUC of 0.923 with a cut?off of 407.36 μg/g. Conclusions The immunofluorescence assay had a close correlation and a high sensitivity when compared with ELISA method.FC could reflect the disease activity of patients with IBD objectively and help to identify disease recurrence in postoperative CD patients early.
