Abstract:Objective It is to manage the patients with chronic heart failure (CHF) by multiple self-test parameters such as weight, 6-minute walk test (6-MWT), resting heart rate (RHR) and plasma brain natriuretic peptide (BNP), and to assess its effect on patient quality of life and prognosis. Methods A total of 207 patients with CHF with NYHA cardiac function classification Ⅱ-Ⅳ were selected and treated with diuretics, β-blockers, and angiotensin conversion enzyme inhibitor/angiotensin Ⅱ receptor antagonist (AEEI/ARB).When leaving hospital after the disease was relieved, the patients were randomly divided into the follow-up management group and the control group according to the discharge date. Continuous follow-up for 12 months. The weight, serum creatinine (SCr), plasma BNP levels, left ventricular ejection fraction (LVEF), 6-MWT, early morning RHR, and Minnesota Quality of Life Score (LQSM) were monitored and followed up. Results After one year of follow-up, 3 patients in the management group were withdrawn from follow-up management, and 10 patients in the control group withdrew from follow-up.After one year of follow-up, the body weight, BNP, RHR and LQSM of the management group were lower than those of the control group, LVEF and 6-MWT were higher than that in the control group (P<0.05), and there was no significant difference in SCr between the two groups (P>0.05).The proportion of primary cardiac endpoint (MACE) events, readmission, and death in the management were lower than those in the control group during the follow-up period (P<0.05). Conclusion The combination of multiple self-test parameters for the management of CHF can significantly improve the cardiac function and quality of life of CHF patients, reduce mortality and rehospitalization, and have significant medical and social effects.
