Abstract:www.PRSJournal.com 253 I is hard to imagine a busy plastic surgeon who has not struggled with treating seromas. These complications occur more frequently in obese patients and those in whom we perform large dissections, but also occasionally after an operation in which we think everything went perfectly. It is frustrating for both surgeons and patients to have to return for multiple aspirations, drain placements, and occasional operations to resolve seromas. Despite this frustration, there is actually very little that we know about the pathophysiology of seromas and the optimal way to treat them. We were excited to read the article by Janis and colleagues, who conducted a systematic review of postoperative seromas to categorize the available literature. This involved a comprehensive literature review of Level I and II studies published in English and aggregated a group of 7000 patients. The authors reviewed the various strategies for seroma treatment and prevention based on the level of evidence in the literature. They present the data both in a scientific fashion and in such a way that clinicians can easily understand their conclusions. This clearly will be a useful reference for plastic surgeons and residents to refer to when presented with these problems in practice. There are some limitations to this study, however, mostly because of the lack of well-designed studies published in the literature on this matter. Also, because of the small number of good studies, the authors were forced to aggregate and compare seromas from different areas of the body to achieve their conclusions. In our experience, there are differences between seromas developing in a breast implant case, an abdominoplasty case, and a face-lift case, making comparisons difficult. For instance, a typical implant-based breast reconstruction would include a silicone implant/expander and dermal matrix. Together with poorly vascularized mastectomy soft tissues, they form a complex that is difficult to heal, and ongoing fluid production is common and often precedes infections. Control of the fluid output in this setting is challenging and critical.1 The authors cite a study on talc used in the abdomen as being effective, yet our hope is that plastic surgeons would avoid the use of talc in a facial procedure. Also exciting are advances in surgical techniques that seem to have eliminated seroma formation. Typically, surgeons have turned to external drainage devices to prevent seromas, but these are often uncomfortable for our patients and provide a pathway for bacteria to enter the body. Eliminating the use of drains without the risk of seroma formation would be a very nice advance in plastic surgery. Many have already provided innovations in this area, and Level I studies of current breast reduction techniques show no benefit to the use of drains.2 Lipoabdominoplasty, where there is minimal undermining performed, and superficial dermolipectomy also allow surgeons to remove large amounts of skin and subcutaneous tissues with little risk of seroma formation.3 Leaving lymphatic pathways intact is a reasonable hypothesis to explore. Going forward, there are several basic science questions that would be important to answer. What exactly occurs to form a seroma? Why in some cases do they form and others they do not? What are the differences in how they form in different areas of the body, in different patients, and in different disease states? We congratulate the authors on performing a nice literature review of the topic of seromas and challenge our readers to continue to innovate so that we can prevent this annoying problem, possibly without the need for external drainage devices.
