Abstract:To the Editor: In human and other mammals, the zona pellucida (ZP) is a glycoprotein matrix surrounding oocyte, and is vital for oogenesis, fertilization, and preimplantation development. Humans have four genetic loci encoding ZP1, ZP2, ZP3, and ZP4, and they assemble to form the insoluble ZP. In contrast to mice, human ZP1 is essential for zona pellucida matrix formation, and its variants may lead to oocyte maturation defect and female infertility. Although the development of assisted reproductive technology has benefited millions of families in the past years, total fertilization failure is still a recurrent problem for 5% to 10% of routine in vitro fertilization (IVF) cycles, with defective sperm-zona binding and penetration being a major cause. We report novel ZP1 pathogenic variants identified in an infertile 32-year-old woman with 100% zona-free oocytes, who had undergone two failed IVF cycles 5 years before. Whole exome sequencing and variant analysis showed that the patient carried a single compound heterozygous mutation in ZP1 (NM_207341:c.A1100G:p.D367G and NM_207341:c.1215delG: p.L405fs), which was detected to be transmitted from her parents (Figure 1). Protein alignments showed that the two variants were highly conserved among different species and were predicted to be deleterious by software predictions (Figure 1). Three-dimensional structure of ZP1 protein was constructed by de novo modeling techniques. It can be seen from Figure 1 that the conserved region of ZP1 is mainly composed of ß-sheet (blue part), where the mutation residue Asp367 and L405fs are located in. Obviously, Asp367 mutating to glycine may disturb the surrounding hydrogen bonds, affecting the overall structure of the ZP1 conserved region. L405fs mutation directly leads to distinct change in the ZP1 protein structure, resulting in ZP domain incomplete and C-terminal absent. To further investigate the effects of ZP1 pathogenic variants on intracellular trafficking of the ZP proteins, wild-type and mutant (D367G) expression plasmids from human-encoding ZP1 fusion protein were transfected into Hela cells. Confocal fluorescent microscopy images showed that both ZP1 proteins showed diffuse staining in the cytoplasm, while mutant ZP1 protein showed increased concentration (Figure 1). The results indicate that D367G mutation in ZP1 protein might result in intracellular sequestration and aggregation of the ZP1 protein, leading to fertilization failure. The developmental potential of the zona-free oocytes is reported to be similar to the zona intact oocytes in terms of fertilization, culture, and pregnancy outcomes. In view of her previous abnormal fertilization experience, we treated the patient by intracytoplasmic sperm injection (ICSI). After two cycles of egg retrievals, one zona-free oocyte was obtained and it was inseminated successfully with ICSI. Monitored under the time-lapse imaging system, the fertilized zygote developed into 6BC embryo and was cryopreservation. After another failed egg retrieval, frozen-thawed embryo transfer was performed and the patient delivered a healthy baby by scheduled cesarean at 38 weeks of gestation. Our findings broaden the spectrum of genetic causes and pathogenic mechanisms of infertility and should facilitate diagnosis of oocyte-borne gamete interaction defects as well as promote therapeutic development.
