Pathologic and Magnetic Resonance Imaging Characters of Eosinophilic Fasciitis

Abstract:Objective To investigate the pathologic characteristics and the magnetic resonance imaging (MRI) features of eosinophilic fasciitis(EF),and to determine the clinical value of these two diagnostic procedures in patients with EF.Methods The clinical manifestations,laboratory examinations,myofascial MRI,and pathological findings of the EF patients,admitted to Peking Union Medical College Hospital from February 1 995 to February 201 3,were retrospectively analyzed.Results Totally 1 4 EF patients were collected,in which 9 were males and 5 were females.The average age on diagnosis was (41 ±1 6)years old,and the average disease duration from the initial presentation to diagnosis confirmation was (1 1 ±9 )months. Laboratory tests showed that 1 3 cases (92.9%)had eosinophilia,and 1 0 cases (71 .4%)had positive antinuclear antibody. The myofascial MRI showed thickening and enhancement of muscular fascia. Myofascial biopsy were performed in 1 2 cases and the pathologic features included chronic inflammation, fibrinoid degeneration,and lymphocytes and plasma cell infiltration,while mild eosinophil infiltration were only found in 3 cases.The majority of cases was treated with glucocorticosteroid and immunosuppressants and achieved long-term remission during a 0.5 ~6.0 years follow-up.Conclusions EF is a clinically uncommon connective tissue disease with the characteristic of eosinophilia,specific pathologic findings and MRI manifestation.Positive ANA is notuncommon.The prognosis is generally benign.
