Abstract:Objective To investigate the effects of total knee arthroplasty in late phase rheumatoid arthritis(RA) with stiff or ankylosed knees.Methods A retrospective study of total knee arthroplasties was carried out in 16 rheumatoid ankylosed or stiff knees in flexion,and three rheumatoid ankylosed knees in extension,which were done by the senior author between 1986-2006.ResultsIn the group of 11 patients with ankylosed knees in flexion,follow-up time ranged from one to 19 years(average 8.4 years).The average postoperative range of motion was 72.5°(45-100°),and the average extension lag was 7.8°(0-15°).The average knee score was 78.5(70-89) postoperative,47.6 more than the preoperative.One patient with rheumatoid arthritis had a supracondylar fracture during extension exercise in postoperative rehabilitation,and two patients had late-phase infection,which needed to be revised.Three patients had transient palsy of the common peroneal nerve but restored to normal state in half a year after operation.The average follow-up time of the three knees ankylosed in extension was 38.8 months(ranging from six months to nine years).Radiographs revealed no infection,loosening or displacement,no heterotrophic ossification or stiffness.The mean value of pre-operative ROM was 7.4°(0-15°),but post-operative ROM was 90°(75-100°),so the overall improvement in ROM was 82.6°.The mean KSS score improved from 43(20-85) to 79.5(77-89) and the functional score improved from 22.9(0-55) to 85.4(75-90).There was 5° extension lag in one knee because of patella tendon avulsion.There was one patient with skin edge necrosis,whose wound had healed at discharge by decreasing the exercise intensity and frequency.There was another knee with superficial wound infection at early-stage after the operation,which resolved after debridement,tension-decreasing suture and sensitive antibiotics.Conclusions Although TKA in an ankylosed knee demands high techniques and has a high rate of complications,it is reliable for achieving pain relief,increasing range of motion,maintaining stability,and improving walking ability in patients with stiff and ankylosed knees in flexion or extension by careful selection of patients,meticulous surgical technique and aggressive rehabilitation.
