Abstract:Objective To propose the interventions with the prospetive study on nosocomial infection in ICU and the guideline of ICU infection control for decreasing the incidence of nosocomial infection in ICU.Methods Patients in ICU from Jan.2007 to Dec.2008 were enrolled in the prospective study.The antimicrobial sensitivity of isolated organisms was tested.The results were input to Excels for statistical analysis.Results Four Hundred and thirteen nosocomial infection cases were determined and the patients over 60 years was 86.0%.The incidence of nosocomial infection in ICU and the nosocomial infection incidence per ICU day were 13.3% and 23.89‰ respectively.The lower respiratory tract infection incidence was 62.46%.The nosocomial infection incidence associated with urinary catheter,central line and mechanical ventilator-related instruments were 8.21‰,6.81‰ and 30.26‰,respectively.A total of 206 streams of organisms were isolated in our survey and the gram-positive bacterium was 37.4%.The detected strain number in order was Staphylococcus aureus,Enterococcus faecium,Staphylococcus epidermidis and Enterococcus faecalis from high to low.The gram-negative bacterium accounted for 62.6% and the detected strain number in order was aeruginosa,Acinetobacter,Escherichia coli and Klebsiella pneumoniae from high to low.Most of the organisms were drug-resistant.MRSA,ESBL (+)Escherichia coli,ESBL (+) Klebsiella pneumoniae accounting for 87.8 %,50% and 29.4%,respectively.54.5 % of the 22 strains of Escherichia coli were resistant to Ceftriaxone.28.6% pseudomonas aeruginosa strains were resistant to Ciprofloxacin and 11.4 % of them were of multidrug-resistance.25.8 % of the 31 strains of Acinetobacter baumannii were resistant to imipenem and 22.6% of them were of pan-drug resistance.Conclusion According to the analysis of the risk factors of nosocomial infection in ICU,the risk factors were age,basis illness,invading operation and environment,etc.
