Abstract:Objective To investigate the current status of preoperative skin preparation in abdominal surgery in China,and put forward suggestions for improvement.Methods From April to May,2016,187 secondary and above hospitals in China were selected,questionnaires were used to investigate the methods for hair removal,skin disinfection on operative field,and application of surgical adhesive drape in abdominal surgery.Results A total of 187 hospitals in 14 provinces were investigated,of which 108 and 79 were tertiary and secondary hospitals respec-tively.87.70% of hospitals performed conventional preoperative hair removal in abdominal surgery,the percentage of conventional depilation in secondary hospitals was higher than tertiary hospitals(93.67% vs 83.33%,χ2 =4.520,P =0.033).85.98% of hospitals selected blade-shaving,7.93% and 23.78% applied chemical depilation and electric depilation respectively.Most hospitals (98.17%)performed skin preparation in the wards,mainly by ward nurses (98.17%),and conducted on the day of surgery (56.10%).86.63%,29.95%,and 3.74% of hospi-tals used iodophor,iodine plus alcohol,and chlorhexidine respectively for disinfecting skin on operative field. 92.51% of hospitals used skin incision/surgical adhesive drape (74.57% used selectively).Conclusion Most hos-pitals in China conventionally perform preoperative hair removal in abdominal surgery,and the primary method for depilation is blade-shaving,nearly half of the hospitals performed depilation on the day of surgery.Iodophor is the major skin antiseptic,surgical adhesive drape is widely used,but it is utilized in limited types of surgery.
Evidence-based practice of preoperative skin preparation in patients undergoing endovascular surgery

Abstract:Objective To explore the application effect of evidence-based nursing related to preoperative skin preparation in patients undergoing endovascular surgery.Methods Patients undergoing endovascular surgery were divided into two groups chronologically,totally 52 patients who admitted in May 2016 were regarded as a control group,and another 36 patients who admitted in October 2016 were involved as an observation group.The control group received skin preparation of shaving hair with a razor one day before surgery and bathing before surgery,while the observation group received skin preparation guided by best evidence.Results No surgical site infection occurred in the two groups.Two cases were detected skin damage caused by skin preparation in the control group,and none occurred in the observational group.The execution rate of preoperative hair removal at 30 minutes before picking up the patients to the operating room and shearing hair in the observational group were significantly higher than those in the control group (P＜0.01 for both).Conclusion Application of the best evidence for preoperative skin preparation in the patients undergoing endovascular surgery,can improve the standardized execution rate of preoperative skin preparation,and plays a role in decreasing surgical site infection.

Abstract:Scar, especially pathological scar, not only cause cosmetic concerns, but also affect the physiological function of related tissues or organs, leading to malformation and dysfunction, that will seriously affect the quality of patients’ life. Nowadays, there are many ways to prevent and improve scar. Local topical medicine such as silicone-based products and plant extracts, become more and more popular with patients because of convenience and reasonable price. In this review, the topical agents for the prevention and improvement of scar, and their therapeutic mechanisms were summarized, providing ideas for the treatment of scar with topical agents.
Influences of preoperative skin preparation methods on incision cicatrisation and infections

Abstract:Objective To find out the difference of preoperative shaving vs non-shaving and different time of preoperative skin preparation on incision infection rate.Methods Randomized block and cross-over design method was used.Operative pa-tients from 7 wards of 4 clinical departments were involved in the study following the different preoperative skin preparation methods at different time,i.e shaving vs non-shaving at the night before,or 2 hours before the surgery.Patients in each group showered and washed the operative area 2 times with soap.Results Totally 1505 patients were collected,included 364 pa-tients shaven at the night before the surgery,375 patients shaven 2 hours before surgery,374 patients cleaning without being shaven at the night before,392 patients cleaning without being shaven 2 hours before surgery.The results show that there were significant differences among of the incision cicatrisation status but no difference among the incision infections of the patients receiving different preoperative skin preparation methods.Conclusions Non-shaving skin preparation results in better incision cicatrisation and does not increase incision infections of the patients compared with shaving after cleaning the skin absolutely.Effective cleaning is the key point to preoperative skin preparation,and the skin preparation can be conducted either at the night or 2 hours before the surgery.

Abstract:The concern with self-care has been growing, often among women, but the search for improved self-esteem also inflames men. Today there is a catalog of procedures, surgical and non-surgical, with variables of cost- effectiveness, technique and accessibility. Statistics show that from 2014 to 2016 there was a 390% increase in the demand for non-surgical aesthetic procedures in Brazil. The skin preparation is an essential step to ensure the quality of the result of the procedure performed. Before starting to apply any product, it is important to prepare the skin so that it is clean, hydrated, and protected. This includes removing impurities, toning, moisturizing, and applying sunscreen. Proper skin preparation helps improve product adhesion, prevents infection at the surgical site, and ensures that the skin looks healthy and radiant. The knowledge gained in this study is the facial cleansing system essential for cell regeneration, facial rejuvenation, oil control, skin whitening, clove and blackhead removal, improved uniformity, significant pigmentation and improved It was concluded that it showed significant benefits of local oxygenation, more youthfulness. It improves the finish of cosmetics used after cleansing and increases ease of removal. Based on free radicals, it delays premature aging of the skin and contributes to skin health and beauty. In summary, the goal is to present how skin preparation is a fundamental step in managing the procedure to be performed, taking care of the skin before, during and after, through the literature review.

Abstract:Bladder cancer is a common malignant tumor of the urinary system.The prognosis of patients with positive lymph nodes is worse than that of patients with negative lymph nodes.An accurate assessment of preoperative lymph node statushelps to make treatmentdecisions,such as the extent of pelvic lymphadenectomy and the use of neoadjuvant chemotherapy.Imaging examination and pathological examination are the primary methods used to assess the lymph node status of bladder cancer patients before surgery.However,these methods have low sensitivity and may lead to inaccuate staging of patients.We reviewed the research progress and made an outlook on the application of clinical diagnosis,imaging techniques,radiomics,and genomics in the preoperative evaluation of lymph node metastasis in bladder cancer patients at different stages.
Clinical effect of two different skin preparation methods on infant craniocerebral surgery

Abstract:Objective To compare the clinical effects of two different skin preparation methods for infant craniocerebral surgery. Methods Totally 120 infants who were going to receive craniocerebral surgery were divided into two groups by random number table, 60 cases in the observation group and 60 cases in the control group. The scalp of both groups was cleaned with moisturizing oil every day from 3 days before operation. On 1 day before operation, the observation group used electric shaver to shave off all hair on the head, and then rinsed with warm water. The control group was treated with skin preparation knife to shave all the hair under soap water lubrication and rinse with warm water. The skin injury rate, incision infection rate and pain score of the two groups were evaluated. Results The incidence of skin injury and incision infection were 0 and 1.7% (1/60) in the observation group, 18.3% (11/60) and 13.3% (8/60) in the control group, respectively. There were significant differences between the two groups ( χ2=12.110, 5.886, all P<0.01 or 0.05). The median score of pain in the observation group was 0 (Q1:0, Q3:0), while 1.5 (Q1:1, Q3:2) in the control group, and there were significant differences between the two groups (Z=3.286, P<0.01). Conclusion Electric shaver is superior to skin preparation knife in shaving hair of infants. It not only reduces the incidence of head skin injury, incision infection and pain in the process of skin preparation, but also reduces the incidence of incision infection after craniocerebral surgery in infants. It is worth popularizing.

Abstract:Objective To study the effect of skin preparation procedure without eyelashes cutting before intraocular surgery.Methods A total of 400 cases who underwent intraocular surgery were divided into two groups randomly from June to September,2009.The patients in the control group received routine preoperative procedure with eyelashes cutting,but the patients in the experimental group received skin preparation procedure without eyelashes cutting.The bacteria culture of specimens from palpebral margin and the conjunctival sac were performed at three time points(on the morning of surgery after conventional eye wash,immediately after preoperation disinfection and immediately after surgery,respectively).The eye irritation scores at days 1,7 and 14 after operation were compared.The incidence of complications of eyelashes cutting and ocular discomfort were also compared.Results The rates of positive bacterial culture did not differ significantly between the two groups at three time points(P>0.05).The experimental group got lower eye irritation score at day 1 after operation(P<0.05),but there were no significant differences in the scores at days 7 and 14 after operation between the two groups.Conclusion The skin preparation procedure without eyelashes cutting does not increase the risk of bacterial infection.

Abstract:OBJECTIVE To compare three surgical hand preparations for their effects on hand skin conditions.METHODS Three surgical hand antisepsis methods were selected to compare their immediate antimicrobial(efficacies) and effect on skin conditions,including 1% chlorhexidine gluconate(CHG)and 61% ethanol(pre-surgical) waterless,scrubless hand preparation,iodophor surgical scrub with sponge,and iodophor surgical scrub with brush.RESULTS All of the three methods could achieve satisfying immediate reduction in the normal(bacterial)(flora) of the hands.The alcohol-based surgical hand rub method was gentler to skin than the other two.(CONCLUSIONS) The alcohol-based surgical hand rub is recommended for surgical hand antisepsis.

Abstract:Abstract Background The application of antiseptic skin agents prior to incision minimizes the rate of surgical site infection. Despite their ubiquity, the optimal skin preparation agent remains uncertain. A retrospective economic analysis was conducted to complement the results from the NEWSkin Prep trial which prospectively compared three preparation agents. Methods A cost and cost-effectiveness analysis was performed from a healthcare service perspective to compare chlorhexidine with 70% ethanol, and aqueous povidone-iodine, against povidone-iodine with 70% ethanol. Resource use estimates accounted for hospital admissions, readmissions associated with surgical site infection, outpatient and general practitioner attendances, visits from community nurses and therapeutic consumables. The measure of effectiveness comprised the net difference in number of patients with surgical site infections per 1000 patients. Costs were compared using a two-sample Welch's t-test. Deterministic and probabilistic sensitivity analyses were performed to evaluate the incremental cost-effectiveness ratio. Results The null hypothesis that the mean costs for the trial arms were significantly different was not rejected (Welch's t-test P value: 0.771 for chlorhexidine with 70% ethanol against povidone-iodine with 70% ethanol; and 0.955 for aqueous povidone-iodine against povidone-iodine with 70% ethanol). Based on bootstrap averages, the chlorhexidine with 70% ethanol intervention generated 8.0 fewer surgical site infections per 1000 patients and net cost savings of €151,698 (Euros) per 1000 patients compared with povidone-iodine with 70% ethanol, and aqueous povidone-iodine produced a net cost saving of €37,494 per 1000 patients but generated an additional 11.6 surgical site infections per 1000 patients compared with povidone-iodine with 70% ethanol. The comparison of chlorhexidine with 70% ethanol to povidone-iodine with 70% ethanol was sensitive to the inclusion of cost outliers, while the comparison of aqueous povidone-iodine to povidone-iodine with 70% ethanol was sensitive to the estimated cost per surgical site infection. Conclusion Based on the outcomes from the NEWSkin Prep study, this economic analysis found no definitive evidence in favour of any one of the study comparators. Future model-based economic analyses of alternative skin preparations should critically address the quality of evidence and integrate the results from the NEWSkin Prep study.

Abstract:High quality bowel preparation is a necessary part of preoperative preparation for colorectal surgery and one of the keys to the success of surgery, which directly affects the quality of intraoperative procedures and postoperative recovery of patients. Conventional intestinal preparation mainly includes three aspects: preoperative dietary control, intestinal cleansing and prophylactic use of antibiotics. With the development of evidence-based medicine, the concepts and methods of bowel preparation have also changed. Long fasting is no longer advocated before surgery, and the traditional mechanical bowel preparation are also challenged. This article summarizes the application and research progress of different intestinal preparation methods before colorectal surgery, aiming to provide reference for clinical work of colorectal surgeons.

Abstract:Abstract Preoperative skin preparation is an effective method to prevent surgical site infections (SSI). Alcoholic chlorhexidine (CHG) and povidone iodine (PV-I) are the most widely used antiseptic agents. This meta-analysis aims to determine their efficacy in reducing natural bacterial skin flora in clean orthopedic surgery. A systematic search was conducted through current literature up to June 2021 to identify clinical randomized trials that compared the efficacy of alcoholic chlorhexidine and povidone iodine in reducing bacterial skin colonization after preoperative skin preparation. A meta-analysis was conducted. Of 235 screened articles, 8 randomized controlled trials were included. The results of the meta-analysis demonstrate a significantly lower positive culture rate in the chlorhexidine group than in the povidone iodine group (RR = 0.53, 95% Cl: 0.32–0.88). The present data show the superiority of chlorhexidine in reducing the normal bacterial flora compared to povidone iodine in clean orthopedic surgery.

Abstract:Liver cancer is one of the most common cancers in China. In recent years, liver cancer tends to be treated with comprehensive therapies, including surgery, ablation, interventional embolization, radiotherapy, chemotherapy, targeted therapy, immunotherapy, and liver transplantation. At present, the low surgical resectionrate is one of the main factors affecting the prognosis of liver cancer patients. Preoperative neoadjuvant therapy or conversion therapy for liver cancer can maximize the rate of surgical resection and improve the prognosis. With the rapid development of radiotherapy and immunotherapy in the comprehensive treatment of liver cancer, it has been gradually confirmed that the unique effects of preoperative radiotherapy and immune therapy for liver cancer can improve the prognosis of the patients. Therefore, this paper reviewed the research progress in the preoperative radiotherapy and immunotherapy for liver cancer by searching relevant literature and reports at home and abroad.
