Pathological features and outcomes of IgA nephropathy with hematuria and/or minimal pro- teinuria

Abstract:Objective To investigate the pathological features and outcomes of IgA nephropathy patients with simple hematuria and minimal proteinuria .Methods One hundred and thirty-seven IgA nephropathy patients with mild asymptomatic proteinuria and /or hematuria from January 2006 to December 2013 in our hospital were collected .We analyzed the renal biopsies according to the Oxford classification criteria.Results Of the patients,55 cases (40.14%) were male patients.The onset ages were (36.63±10.83 ) years old.Their renal pathological changes included 40 cases with mesangial hypercellularity (29.19%),30 with segmental glomerulosclero-sis (30.65%),39 with endocapillary hypercellularity (28.46%) and 6 with tubular atrophy/interstitial fibrosis.Eighty-seven patients (63.50%) were treated with angiotensin converting enzyme inhibitors (ACEI) or rennin angiotensin blockers (ARB).The median fol-low-up period was (45.51±13.94)months.Serum creatinine level was increased in 7 patients (5.1%) when compared to basic value before biopsy.There were 99 patients (72.26%) without complete clinical remission .The logistic analysis showed that mesangial prolif-eration and increased proteinuria were independent risk factors for patients with non -complete remission .Conclusion Severe renal his-tological injury may be observed in some IgA nephropathy patients with hematuria and /or minimal proteinuria .Therefore,renal biopsy is necessary for the patients with mild asymptomatic proteinuria and /or hematuria .
