Abstract:Objectives: To study the differential diagnostic value of Virtual touch tissue quantification (VTQ) in evaluating enlarged superficial lymph nodes compared with B-mode sonography, and also produce the best cutoff value for shear wave velocity (SWV). Methods: B-mode sonography and VTQ were evaluated in 176 patients with 203 superficial lymph nodes. The diagnostic criteria of B-mode ultrasonic images consisted of diameter, the short axis/longaxis (S/L) ratio, border, echogenicity, and hilum. For VTQ imaging, the transducer was gently touched the skin and maintained stability. The region of interest (ROI) was placed inside the cortex, the medulla and any degeneration within it was avoided as far as possible. 5 SWV values were measured at different sites and averaged to produce a mean value in each lymph node. All lymph nodes were confirmed by pathologic examination. Independent sample t test or Mann-Whitney U test were used for statistics of variance. Diagnostic efficacy were evaluated and compared. Results: The scores of B-mode sonography for malignant lymph nodes (7.55 +/- 1.29) were significantly higher than those of benign lymph nodes (5.96 +/- 0.80; Z = -8.25, P < 0.05). The SWV value was significantly higher in malignant lymph nodes (4.18 +/- 1.76 m/s) than in benign lymph nodes (2.06 +/- 0.78 m/s; F = 61.601, t =-10.335, P < 0.05), and the cutoff level was 2.525 m/s meanwhile. The SWV value of lymph node tuberculosis (3.03 +/- 0.47 m/s) was also higher than that of other benign lymph nodes (F = 1.849, t = 3.345, P < 0.05). The SWV value of metastatic lymph nodes (4.50 +/- 1.87 m/s) was significantly higher than that of primary lymphoma (3.37 +/- 1.16 m/s; F = 15.91, t = 3.295, P < 0.05), and the cutoff value was 4.125 m/s at the same time. The sensitivity, specificity, and accuracy of combined evaluation were all higher than those of B-mode sonography or VTQ alone. The area under the curve (AUC) of combined evaluation was significant higher than that of B-mode sonography (Z = 2.38, P < 0.05). Conclusions: VTQ has high sensitivity, specificity and accuracy in the differential diagnosis of superficial lymph nodes. The combination of B-mode sonography and VTQ improves the sensitivity and accuracy, and potentially helps to evaluate superficial lymph nodes.
