The Value of Coagulation Tests in Predicting the Risk of Hemorrhage in Major Oral and Maxillofacial Operations

Abstract:Objective: To evaluate the value of coagulation tests in predicting the risk of hemorrhage in major oral and maxillofacial operations. Methods: The platelet count (PC). prothrombin time (PT) and activated partial thromboplastin (aPTT) of 387 patients undergoing major oral and maxillofacial operations from 12/2002 to 12/2003 were reviewed. 12 patients with abnormal results were screened out as study group. For each of these patients,another 24 control patients undergoing the same surgery and matched for age and gender were identified. Comparison was made regarding the incidence of perioperative bleeding correlated clinical affairs blood loss amamt, blood tramsfusion amount between two groups. Results: The overall incidence of abnormality was 3. 10% , the incidence of abnormal PC was 2. 07% and the incidence of abnormal PT and aPTT was 1.34%. Only 5 patients (1. 29% ,5/387) with thrombocytopenia or prolonged PT and aPTT who had no clinical symptoms, signs and history were found. Although study group patients had more probability to delay the operations or change the protocol of operation (66.7% vs. 0% ,P = 0. 000016) and use of biological hemostatics (58.3% vs. 8. 3 % , P = 0. 0025), the incidence of other clinical affairs was not different significantly. Conclusion: Routine preoperative coagulation tests of patients undergoing major oral and maxillofacial operations can not predict the risk of perioperative hemorrage. But for abnormal patients with clinical symptoms,signs and history,the results of tests can indicate the degree of coagulation abnomahty and be of certain value to predict the probability of bleeding complications.
