Avascular Necrosis in Severe Acute Respiratory Syndrome: MR Imaging with Radionuclide Correlation

Abstract:Objective To evaluate the role of magnetic resonance imaging (MRI) and radionuclide (RN) in the early diagnosis of femoral head avascular necrosis (AVN) in severe acute respiratory syndrome (SARS) patients. Methods Thirty one SARS patients who were suspected of femoral head AVN underwent both MR and bone scan. All the MR and RN images were read independently by three observers who were blinded to the results of another imaging modality and the results of clinical records. The MR signal intensity and RN characteristics of all femoral head AVN were noted, and the receiver operating characteristic (ROC) method was used to compare the difference of diagnostic performance between the two techniques. Results AVN of femoral head was correctly diagnosed in 16/17 hips on MRI and RN, and normal femoral head was correctly diagnosed in 24/45 hips on MRI and in 11/45 hips on RN. The area under ROC curves for MR was larger than that for RN (0.985 vs 0.926), but the difference did not reach statistical significance. Conclusion MR imaging is more sensitive and accurate for the early diagnosis of femoral head AVN than RN in SARS patients.
