Abstract:Aim: To observe the changes of serum interleukin-8, monocyte chemoattractant protein-1and interleukin-10 in patients with obstructive sleep apnea hypopnea syndrome (OSAHS), and to explore the significance. Methods: Thirty-five patients with obstructive sleep apnea hypopnea syndrome confirmed by polysomnography, who hospitalized in Department of Respiratory Medicine, First Hospital of Lanzhou University from December 2002 to December 2003, were selected. Ten with moderate and severe OSAHS of these patients had been treated by nasal continuous positive air pressure for over half a year, were selected as treatment group aged (51 ±13) years old. The included patients all knew the fact of the treatment program and agreed. Twenty-five healthy subjects in this hospital were recruited as the control group during the same period, and agreed to take part in the experiment. The apnea hypopnea index and lowest blood oxygen saturation were detected by the Polywin polysomnography. The serum levels of interleukin-8, interleukin-10 and monocyte chemoattractant protein-1 were measured by enzyme linked immune sandwich assay. The patients with moderate and severe OSAHS were treated with continuous positive airway. The pressure of nasal continuous positive airway was 0.588-1.176 kPa for 6-8 hours every day. The t-test was performed among the comparison of groups, and the paired t-test was conducted among the comparison within the groups. The correlation check among indexes was done by linear correlation analysis. Results: Before treatment the number of people kept the upper number, and after treatment 10 patients with moderate and severe OSAHS and 25 healthy people were involved in the result analysis. 1 The serum interleukin-8 and monocyte chemoattractant protein-1 levels in patients with OSAHS (35 cases) were significantly higher than those in the control group (t=5.498, 9.626, P < 0.01), and the level of serum interleukin-10 was lower obviously than that in the control group (t=-4.667,P < 0.01). 2 The levels of serum interleukin-8 and monocyte chemoattractant protein-1 in patients with OSAHS had significantly positive correlation with apnea hypopnea index (r=0.671, 0.742, P < 0.01), and had negative correlation with lowest blood oxygen saturation (r=-0.442, -0.531, P < -0.01).The level of serum interleukin-10 had significantly negative correlation with apnea hypopnea index (r=-0.688, P < 0.01), and had positive correlation with lowest blood oxygen saturation (r=0.454, P < 0.01).3 The serum interleukin-10 and interleukin-8 had remarkably negative correlation with monocyte chemoatractant protein-1 (r=0.509, -0.666, P < 0.01), and the interleukin-8 had obviously positive correlation with monocyte chemoattractant protein-1 (r=0.706,P < 0.01). 4 The levels of serum in-terleukin-8 and monocyte chemoattractant protein-1 in patients of treat-ment group (10 cases)after half year treatment were lower significantly than those before treatment (t=4.896, 5.609, P < 0.01).The level of in-terleukin-10 was significantly higher than that before treatment (t=-4.625, P < 0.01). Conclusion: 1 The hypoxia induces the increase of serum interleukin-8 and monocyte chemoattractant protein-1, and the decrease of interleukin-10 in patients with OSAHS, which may join the occurrence of cardiovascular disease. 2 The nasal continuous positive airway treatment may reach the effect of preventing the occurrence of cardiovascular lesion in patients with OSAHS by means of relieving ischemia, decreasing the production of interleukin-8 and monocyte chemoattractant protein-1 and increasing the production of interleukin-10.
