Gd+-EOB-DTPA-enhanced MRI evaluation of colorectal liver metastases and chemotherapy-induced fatty liver

Abstract:Objective To evaluate chemotherapy-induced fatty liver and compare the image quality and detection rate of liver metastasis and between Gd+-EOB-DTPA-enhanced MRI and contrast-enhanced CT in colorectal malignancy.Methods 25 patients with liver metastases from colorectal cancer underwent Gd+-EOB-DTPA-enhanced MRI and contrast-enhanced CT.The image quality scores,sensitivity,specificity,positive and negative predictive values,and accuracy for lesion detection were compared.The T1 signal intensities of normal liver parenchyma in the hepatobiliary phase were compared with that of chemotherapy-induced fatty liver. Results 60 lesions were detected with Gd+-EOB-DTPA-enhanced MRI showed 60 hypointense lesions in the hepatobiliary phase whereas CT showed 40 lesions in the portal venous phase.57 lesions were proven to be metastases.The lesion conspicuity score (108),sensitivity(95%),specificity(95%),positive and negative predictive values(100%),and accuracy(95%)were better on Gd+-EOB-DTPA-enhanced MRI than that CT(62,63.2%,63.2%,36.8%,63.2%).The average T1 signal intensities of the fatty liver parenchyma after chemotherapy(236.9±23.45)were significantly lower than that of normal liver(371.9±48.69)in the hepatobiliary phase.Conclusion Gd+-EOB-DTPA-enhanced MRI displays colorectal liver metastases better than CT and allows assessment of liver function after chemotherapy.
