Abstract:Critical care medicine began in 1982 in China and is still in a phase of rapid development. Despite the inadequate resources compared with those of developed countries, critical care medicine has been recognized as a specialty by the government and by other specialties. National critical care societies are dedicated to promoting professional education and the academic improvement of critical care medicine in China. Although critical care services and clinical research are still underdeveloped due to scarce resources, great improvement is expected in the near future. HISTORY OF CRITICAL CARE MEDICINE IN CHINA During the past three decades, critical care medicine (CCM) has developed rapidly with the development of life support techniques. Before 1982, critical care was initially practiced in an isolation area (rescue room) within the general ward or in some postoperative recovery rooms. The first real intensive care unit (ICU) in China, was set up by Dr. CHEN De-chang, in the Peking Union Medical College Hospital in 1982.1,2 Since 1988, the rapid expansion of CCM in China has been due to the Regulation of Hospital Accreditation and Management issued by the Ministry of Health, which required the establishment of an ICU as a prerequisite for accreditation as a tertiary hospital.1,2 Many physicians, such as anesthesiologists, surgeons, internists and pediatricians, returned to practice as intensivists. With the development of critical care medicine and an increasing number of intensivists, a critical care society was established by the Chinese Medical Association (The Chinese Society of Critical Care Medicine, Chinese Medical Association; CSCCM-CMA) in 2005. The first promoters of the CSCCM-CMA were Prof. LIU Dawei (the first president, Peking Union Medical College Hospital), Prof. QIU Hai-bo (the second president, Nanjing Zhongda Hospital, Southeast University), TANG Yao-qing (Shanghai Ruijin Hospital) and XIAO Zhenglun (Guangzhou Medical College). The establishment of the CSCCM-CMA indicated that critical care was recognized as a specialty by other specialties. Three years later, critical care medicine was officially recognized as a primary clinical discipline (Disciplinary code: 320.58) by the National Standards Committee in 2008 and a Grade 1 medical subject (such as internal medicine and surgery) of clinical medicine by the Ministry of Health in 2009. Thus, critical care medicine became a primary specialty in China. The quality and safety of critical care were required by the Ministry of Health during a review of tertiary hospitals in 2012. CRITICAL CARE RESOURCES AND SERVICES Critical care resources (including the number of ICUs, intensivists, nurses, and relevant facilitie) were unclear before the first census on critical care resources in China, which was performed by the CSCCM-CMA in April 2011. Based on the data from the census, ICUs have been established in more than half of the tertiary hospitals in the interior of China. Among all ICUs, most are closed and general ICUs. In the interior of China, ICU beds number approximately 3.23±2.74 per 100 000 population. This figure is lower than that of eight countries in North America and Western Europe, where it ranges from 3.9 to 24.6 ICU beds per 100 000 population.3 It has been estimated that the total ICUs beds in the interior of China can serve approximately 3.8 million patients per year. This figure compares with 4.0 million in the United States. Taking the baseline population of more than 1.3 billion into account, the problem of ICU bed shortage is prominent in China. In addition to the lack of ICU beds, physicians and nurses are in even shorter supply in these ICUs. Only 20% of these ICUs reach a physician-to-bed ratio of more than 0.8, and less than 30% of ICUs reach a nurse-to-bed ratio of more than 2.5. Modern monitoring and treatment techniques are increasingly popular. Central line insertion and hemodynamic monitoring are performed in most ICUs, especially general ICUs. However, intracranial pressure monitoring, bedside ultrasound, fibrobronchoscopy and gastrointestinal endoscopy are not so popular, especially in specialized ICUs. Despite the advantage of enteral nutrition, wellknown to intensivists, the percentage of critically ill patients who receive only parenteral nutrition is much higher in China (33%) than in Western countries (5.4%).4 The hospital mortality of acute respiratory distress syndrom(ARDS) patients in China looks slightly higher than in Western countries,5-8 however, the hospital mortality rate of severe sepsis patients was comparable.9,10 The lack of critical care resources might be the main reason for the gap in the mortality rate between China and Western countries. Large-scale observational studies on the case mix in Chinese ICUs are still needed to clarify the incidence and mortality of each disease. CRITICAL CARE MEDICINE AS A PRIMARY SPECIALTY In the early stage, physicians of other relevant specialties, such as anesthesiology, surgery and internal medicine, who were familiar with necessary techniques, disease entities and urgent treatments, were assigned to work in ICUs as intensivists. It was real a hard time for intensivists before they were recognized and respected by other specialties, who often still desire to treat critically ill patients. The public health care crises in China since 2003 have provided intensivists with an opportunity to demonstrate their knowledge and skills. During epidemics of severe acute respiratory syndrome in 2003 and the avian influenza outbreak, as well as the Wenchuan Earthquake in 2008 and Yushu Earthquake in 2010, intensivists were often selected by the government to be involved very early in crisis management. Their ability in patient management and policymaking were well demonstrated during daily work and recognized by the general public and healthcare authorities. As a result, critical care medicine was officially recognized as a primary clinical discipline (Disciplinary code: 320.58) by the National Standards Committee in 2008 and as a specialty of clinical medicine by the Ministry of Health in 2009. Subsequently, ICUs could be registered as Department of CCM with the local bureau of public health, and ICU physicians had the choice to be registered as intensivists rather than remaining registered under their primary specialty. The specialist training of physicians and nurses in critical care medicine has become increasingly popular in China. After graduation from medical school, residents can choose critical care medicine as their specialty and be intensivists after 5 or 6 years of specialist training in critical care. In contrast, in USA, after finishing a 3- or 4- year fellowship training program in internal medicine, anesthesia, general surgery, or emergency medicine, junior residents who are interested in critical care can choose to become intensivists if they complete the remaining 1 or 2 years of specialist training in critical care. For the past several years, the national critical care societies have been dedicated to promoting specialist training. The CSCCM-CMA successfully organizes the Chinese Critical Care Certified Course (5c). More than 7000 intensivists have passed this course, which includes 4 days of lectures, five workshops and a final examination on knowledge and skills. The Chinese Society of Critical Care Medicine, which is affiliated with the Chinese Association of Pathophysiology (CSCCM-CAP), also organizes The Basic Assessment and Support Intensive Care course and promotes the course in China. Since 2009, a national board positional title exam for critical care medicine has been available, which suggests that professional education in China has a national standard for intensivists. However, the number of trainees is limited compared with the large number of intensivists in China. Professional education courses should be expanded in the future, especially long-term specialist education. With respect to the specialist training of critical care registered nurses, the critical care nurse certification program (Provincial Nursing Association or the China Nursing Association) was established in 2003. It usually includes one month of lectures and one month of clinical practice in the Department of CCM. We expect that the lack of accredited critical care training programs will no longer be a major obstacle for improving professional education in China. With the development of critical care medicine and growth of national critical care societies, the subdivision of a subspecialty has become increasingly important and necessary. The thematic conferences of critical respirology and critical nephrology were held on August 31st, 2012, and January 25th, 2013, respectively, which marked the rapid development of subspecialties under critical care medicine. PROTOCOLS AND GUIDELINES With regard to the unbalanced development of critical care medicine between coastal areas and inland areas, the lack of national protocols and guidelines for critically ill patients is believed to be a major obstacle for improving critical care medicine in China. To solve this problem, the CSCCM-CMA began to enact clinical practice guidelines and clinical technical protocols in 2006. After 5 years of hard work, 13 guidelines have been published and are continuously updated. To standardize the clinical practice of intensivists, Clinical Practice Guideline “Department of critical care medicine”, and Clinical technical operation specification “Department of critical care medicine” were published. Additionally, the Chinese Critical Care Certified Course was dedicated to improving basic knowledge and standardized treatment in critical care medicine. A new Clinical Information System, which includes all the guidelines and protocols for critical care, was established and applied in the ICU of Nanjing Zhongda Hospital, Peking Union Medical College Hospital, and Shanghai Ruijin Hospital. ACADEMIC PROMOTION As a main platform for promoting the academic improvement of critical care medicine in China, the CSCCM-CMA organizes a 3-day annual national congress (biennial from 2005), and the number of attendees has increased from 1000 in 2005 to more than 6500 people in 2012, including physicians, nurses, medical students and company representatives. Since 2010, a book on critical care medicine (Yearbook of Critical Care Medicine in China) has been edited and published by the CSCCM-CMA annually. CSCCM-CAP also has been actively working to promote the academic improvement of critical care medicine by hosting international congresses. However, promoting several short-term congresses or symposia is not enough. Professional education and academic communication should be generalized to the Department of CCM in local hospitals. CLINICAL RESEARCH Critical care research in China is in its infancy. Increasing numbers of research studies are being published in peer-reviewed international medical journals; however, most study results have been published in national medical journals in the Chinese language. In addition to inadequate training and experience in clinical research, inadequate funding and language barriers are two of the main obstacles. In 2010, the CSCCM-CMA established a funding platform named the Chinese Intensive Medicine Exploration (CHIME) to attract sponsors to provide funding for valuable clinical studies. In 2011, the Ministry of Health established a special fund for scientific public health research in critical care medicine, which will greatly promote the development of critical multiplecenter research in China. Additionally, more intensivists have become motivated to perform clinical research and become involved in national or international multicenter studies during recent years. With respect to the large number of critically ill patients in China, there is a great potential for future improvement in clinical research. CONCLUSIONS In summary, after 30 years of hard work, critical care medicine has developed rapidly and has been recognized as a specialty by the government as well as other specialties. However, scarce resources are still a major obstacle for better critical care services. National critical care societies have worked together to establish clinical practice standards and professional education programs. Critical care research is beginning to evolve, which represents a great potential for future improvement.
